1.A Study of Relativity between TCM Syndrome and Clinic Pathology on 1010 Patients with IgA Nephropathy

Objective To explore the TCM syndrome of IgAN, and formulate standard scheme on differentiation of symptoms and sings. Methods Adopted epidemiological investigation of TCM syndrome, and analyzed the relativity of clinic and pathology with the TCM syndrome. Results Two schemes of TCM showed that the qiyinliangxu syndrome was primary pattern of IgAN. The TCM syndrome showed changes of single syndrome (qiyinliangxu) → bideminy syndrome (qiyinliangxu accompanied shenluoyubi or fengshineirao)→ trigeminy syndrome (qiyinliangxu accompanied shenluoyubi and fengshineirao) with the aggravation of IgAN.Conclusions The New Scheme was more predominant in differentiation of symptoms and sings than The Old Scheme, and The New Scheme was more easy to operat and master. It was worthy to spread and utilize The New Scheme in clinic.
2.Cloning and identification of partial positive regulatory sequence of Na+/H+ exchanger-1 gene from human lung cancer cells

Objective To clone the partial positive regulatory fragment of Na+/H+ exchanger-1 (NHE-1) gene from human lung cancer cells. Methods After BamHⅠ and EcoRⅠ cut sites were added to the 5' ends of the upstream and downstream primers respectively, the partial positive regulatory sequence of NHE-1 gene was cloned with the length of 170 bp from genomic DNA of lung cancer cell line A549 cells with PCR method. The cloned fragment was ligated to plasmid pUC18. Finally, the constructed recombinant was identified with enzyme cut, PCR and DNA sequencing. Results The cloned fragment was about 170 bp in size and successfully ligated to pUC18 with identifiation of double enzyme cut and PCR. DNA sequencing approved that the fragment cloned was objective one with 168 bp in length. Compared with the reported sequence, two t were lost. Conclusion The positive regulatory fragment of NHE-1 gene from human lung cancer cells was successfully cloned.
3.Role of bcl-2 and bax expression in apoptosis of pulmonary artery smooth muscle cells of rats induced by NHE-1 inhibition in vitro

AIM:To investigate the role of bcl-2 and bax expression in apoptosis of pulmonary artery smooth muscle cells(PASMC) of rats induced by Na +/H + exchanger isoform-1(NHE-1) inhibition in vitro. METHODS:Intracellular Ca 2+ concentration ([Ca 2+ ]i) was measured using Fura-2/AM. The expression of bcl-2 and bax mRNA in cells were detected by sqRT-PCR, and the expression of Bcl-2 and Bax protein were examined immunohistochemically. RESULTS:The expression of bax mRNA and Bax protein and [Ca 2+ ]i in cells transfected with NHE-1 ribozyme gene increased significantly compared with those of cells transfected with pLXSN and nontransfected control. Meanwhile, the expression of bcl-2 mRNA and Bcl-2 protein in cells transfected with NHE-1 ribozyme gene decreased significantly. CONCLUSION:Apoptosis of pulmonary artery smooth muscle cells induced by inhibiting NHE-1 may have relevance to the increase in [Ca 2+ ]i and bax expression and the decrease in bcl-2 expression.
4.Protective effect of adiponectin and its receptors in diabetic nephropathy rats

Objective To investigate the protective effect and possible mechanism of adiponectin (ADPN) and its receptors in diabetic nephropathy (DN). Methods A total of 64 Sprague-Dawley female rats were equally divided into diabetes mellitus (DM) group with streptozotocin(STZ) 60 mg/kg injection, and normal control (NC) group with the same dose of citric acid buffer randomly. At the end of 2, 6, 10 and 12 weeks, weight, fast blood glucose, 24-hour urinary albumin excretion, serum creatinine and serum fast blood insulin were measured. The level of adiponectin in urine and serum was examined by ELISA. The pathological change of kidney tissue was studied by periodic acid-Schiff's staining (PAS) and the expression of adiponectin receptor 1 (adipoR1), adiponectin receptor 2 (adipoR2) was determined by immunohistochemistry.The NRK-52E cells were cultured with 5 mmol/L glucose (control group), 30 mmol/L glucose (high glucose group), 30 mmol/L glucose+different concentrations of adiponectin (1 mg/L, 5 mg/L, 10 mg/L, ADPN groups), respectively. The mRNA expression of monocyte chemoattractant protein 1 (MCP-1) was measured by RT-PCR after 12 h. Results (1)Both in urine and serum, the levels of ADPN in DM group were higher than that in NC group after 6 weeks (P<0.01) and increased gradually during the whole experiment. (2)Compared with NC group, the expressions of AdipoR1 and AdipoR2 in kidney tissues were elevated gradually in DM group. There was a significant positive correlation of ADPN with AdipoR1 and AdipoR2 in DM group(r=0.666, P<0.01;r= 0.684, P<0.01). (3)The expression of MCP-1 increased in high glucose group, but decreased in 1 mg/L, 5 mg/L, 10 mg/L adiponectin group. Conclusions The level of ADPN in urine and serum is positively correlated with its receptors and elevates with the process of diabetic kidney disease. High level of serum adiponectin plays a protective role through the direct action of AdipoR and the alleviation of MCP-1 expression in renal tubules.

6.Clinical analysis of posttransplant lymphoproliferative disorder in kidney transplant recipients and hematopoietic stem cell transplant recipients summary

Objective To provide theoretic rationales and clinical experience for post-transplant lymphoproliferative disorder (PTLD ) by comparing the characteristics of PTLD in kidney and hematopoietic stem cell transplant recipients and reviewing the relevant literature reports .Methods Twenty-seven adult PTLD patients from 2000 to 2017 were retrospectively reviewed .There were 11 kidney transplant recipients (KT group) and 16 hematopoietic stem cell transplant recipients (HSCT group) .Clinical characteristics and outcomes were analyzed between two groups .Cox's proportional hazard model was utilized for evaluating the prognostic factors .Results The incidence of PTLD for KT and HSCT groups were 0 .5 % and 1 .1 % respectively .PTLD patients of KT group had a later onset than that of HSCT group (105 .1 vs 3 .1 months , P<0 .01) .Also Epstein-Barr virus was less frequently detected in KT group (36 .4 % vs 81 .3 % , P< 0 .05) .The 5-year overall survival was (46 .8% ± 10 .5% ) .According to Cox analysis ,application of antithymocyte globulin (ATG) and high ECOG scores were risk factors for a poor prognosis of PTLD .Conclusions Most cases of KT-PTLD have a late onset . In contrast , HSCT-PTLD has an earlier onset and a higher incidence of EBV infectious .And application of ATG and high ECOG scores are poor prognosis factors of PTLD .
7. Neuropathologic findings in intractable epilepsy: a clinicopathologic analysis of 822 cases

Objective:
To investigate the clinicopathologic characteristics of intractable epilepsy.
Methods:
Based on the classification criteria proposed by the International League Against Epilepsy (ILAE), a retrospective analysis of the pathological characteristics was done in 822 patients who underwent epilepsy surgery in Sanbo Brain Hospital, Capital Medical University, from June 2008 to December 2012.
Results:
The mean age of epilepsy onset was 9.9 years, mean duration of epilepsy was 11.9 years. Complex partial seizures were the main presenting features. Histopathological study showed 33 cases (4.01%) with mild forms of cortical malformations, 690 cases (83.94%) with focal cortical dysplasia (FCD) and 99 cases with others (including 39 pure hippocampal sclerosis, 20 cystosclerosis, 19 Sturge-Weber syndrome, 8 tuberous sclerosis complex, 6 without significant pathological changes, 5 gyral malformations and 2 hamartoma). Among the 690 FCD cases, 106 were FCD typeⅠ, 91 were FCD typeⅡ and 493 were FCDⅢ(Ⅲa: 160, Ⅲb: 106, Ⅲc: 26 and Ⅲd: 201).
Conclusions
FCDⅢd is the most common histopathological subtype causing intractable epilepsy, mainly due to focal hypoxia/ischemia in the perinatal period, which results in scarring of local brain tissue; this is followed by other isolated forms of FCD (FCDⅠand FCDⅡ), and then FCD Ⅲa and FCD Ⅲb. The reason to distinguish isolated forms of FCD (types Ⅰ and Ⅱ) from FCD Ⅲ and to subclassify FCD Ⅲ is to allow better definition of cortical dyslamination. Therefore, the pathogenic factors of intractable epilepsy can be grouped in greater details, and facilitate the diagnosis and potential curative treatment of intractable epilepsy.
8.Influence of Yiqi Huoxue Tongluo Fang on JNK signal pathway and inflammatory factors in mice with diabetic peripheral neuropathy

ObjectiveTo study the influence of Yiqi Huoxue Tongluo Fang (Qi-Replenishing Blood-Activating Collateral-freeing Decoction) on signal pathway of c-jun N-terminal kinase (JNK) and inflammatory factors in mice with diabetic peripheral neuropathy (db/db mice),and discuss its mechanism in protecting peripheral nerve.Methods The db/db mice (n =40) were randomly divided into 5 groups:model group,lipoic acid group,high-dose Yiqi Huoxue Tongluo Fang group (6.4 g/kg,high-dose group),mid-dose Yiqi Huoxue Tongluo Fang group (3.2 g/kg,mid-dose group) and low-dose Yiqi Huoxue Tongluo Fang group (1.6 g/kg,low-dose group),and other db/m mice (n =8) were chosen into normal group.All groups were intragastrically given corresponding drugs once a day for 8 weeks.After the last administration of drugs,the plantar thermal response time and motor nerve conduction velocity (MNCV) were detected.The levels of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) were detected by using enzyme-linked immunosorbent assay (ELISA).The protein expressions of JNK and phosphorylase JNK (p-JNK) in dorsal root ganglion (DRG) were detected by using Western blot method.Results Compared with normal group,MNCV was slower,plantar thermal response time was longer,levels of IL-1β and TNF-α increased,and protein expressions of JNK and pJNK are stronger in model group (P < 0.01 or P < 0.05).Compared with model group,Yiqi Huoxue Tongluo Fang promoted MNCV,shortened plantar thermal response time,decreased levels of IL-1 β and TNF-α and down-regulated protein expressions of JNK and p-JNK to some extent (P < 0.01 or P <0.05).ConclusionYiqi Huoxue Tongluo Fang can inhibit the abnormal activation of JNK signal pathway and relieve inflammatory reaction to protect peripheral nerve in db/db mice.
9.The effects of electroacupuncture at and points of stomach on gastric motility, the NMDA of vagus nerve dorsal nucleus and serum NO expression in functional dyspepsia rats.

OBJECTIVETo research the central molecular mechanism of gastric motility in functional dyspepsia (FD) rats treated with electroacupuncture (EA) at and points of stomach.
METHODSA total of 30 SD rats were randomized into a blank group, a model group, a Zhongwan+Weishu group, a Weishu group and a Zhongwan group, 6 rats in each group. FD rats were established by moderate clipping tail infuriation and irregular feeding except in the blank group. EA was used at "Zhongwan"(CV 12),"Weishu"(BL 21), and"Zhongwan"(CV 12) +"Weishu"(BL 21) in the corresponding groups for 7 days, once a day, and 20 min a time. No intervention was used in the blank and model groups. Grabbing and fixation were applied in the model group. Gastric antrum motion range and frequency were recorded by gastrointestinal pressure transducer. The expression of subunit NR1 of N-methyl-D-aspartate recepter (NMDAR) in dorsal motor nucleus of the vagus (DMV) was determined by Western blotting. The content of serum nitric oxide (NO) was measured by ELISA.
RESULTSCompared with the blank group, the gastric antrum motion range and NR1 in the DMV decreased and the serum NO content increased in the model group (all <0.05). Compared with the model group, the gastric antrum motion range and NR1 in the DMV increased and the serum NO content decreased in the three EA groups (all <0.05). Compared with the Zhongwan and Weishu groups, the gastric antrum motion range and NR1 in the DMV increased in the Zhongwan + Weishu group (all <0.05). Compared with Zhongwan + Weishu and Zhongwan groups, the expression of NO in the Weishu group decreased (both <0.05). The gastric antrum motion frequency among the 5 groups had no statistical significance (all >0.05).
CONCLUSIONEA at the and points can regulate the gastric motility in FD rats which may be by modulating the activity of NMDAR in the central DMV region, thus regulating the serum NO content.
