Objective To evaluate the effectiveness of a insulin regimen for the management of stress hyperglycemia induced by head injury.Methods Eighty patients (64 male, 16 female) admitted to the hospital within 24h after head injury were enrolled in the study. The age ranged from 16-75 yr. The patients were divided into 3 groups according to the numeric values of Glasgow Coma Scale (GCS) : group Ⅰ 0, the step Ⅲ was taken to maintain blood glucose at normal level using formula Ⅱ : the maintenance dose of insulin = (last blood glucose ? 18) / 150 U?h-1 .Results The blood glucose levels before anesthesia were (12 ? 7) mmol (group Ⅰ ), (9 ? 3) mmol (group Ⅱ ) and (8 ? 5) mmol (group Ⅲ ) . 2.Study on Preparation Process of Sodium Copper Porphyrin of Herba Ephedrae and Its Character

Objective: To study the comprehensive utilization of Herba Ephedrae resources during the extraction process of ephedrin, that is the extracting of porphyrin compound.Methods: The process conditions for preparing sodium copper porphyrin were optimized by the two preparation processes and twice orthogonal tests. Results: Herba Ephedrae was powdered, steeped and boiled in alcohol, saponified, then extracted with xylene. The ephedrin was extracted from the upper layer of xylene extraction solution and sodium copper porphyrin was obtained from the lower layer by copper substituton, acidulation and alkalization. Conclusion: The optimum preparation process for producing sodium copper porphrin of Herba Ephedrae which is superior to porphyrin in the stability and colour is ascertained.
3.Video-mediastinoscopy: Report of 128 cases

Objective To study the application of video-mediastinoscopy in the preoperative staging of lung cancer and the diagnosis and treatment of mediastinal tumors or malignant pleural effusion.Methods Video-mediastinoscopy was performed in 128 cases under general anesthesia and single lumen endotracheal intubation,including 48 cases of cervical mediastinoscopy,33 cases of parasternal mediastinoscopy,and 47 cases of intercostal mediastinoscopy.Results The diagnosis was clarified under video-mediastinoscopy in 125 cases,including 38 cases of adenocarcinoma,33 cases of metastatic squamous cell carcinoma,9 cases of tuberculosis,8 cases of lymph node inflammation,7 cases of small cell carcinoma,6 cases of squamous cell carcinoma of the thymus,5 cases of non-Hodgkin's lymphoma,4 cases of neuroblastoma,4 cases of thymoma,3 cases of pleural mesothelioma,2 cases of Hodgkin's lymphoma,2 cases of neurilemmoma,1 case of sarcoidosis,1 case of hyperplasia of thymus gland,1 case of carcinoid,and 1 case of primitive neuroectodermal tumor.In 1 case,the mediastinal lymph nodes were diagnosed as reactive hyperplasia by video-mediastinoscopy,and then were confirmed as squamous cell carcinoma by postoperative pathological examination.Two cases were confirmatively diagnosed as having lung cancer in the left inferior lobe by brochoscopy, and video-mediastinoscopy revealed the metastasis to right paratracheal lymph nodes.Complications included 1 case of pneumothorax,1 case of hemorrhage,2 cases of recurrent laryngeal nerve paralysis,and 2 cases of superficial incision infection. Conclusions Video-mediastinoscopy is not only an important examination for preoperative staging of lung cancer and diagnosis of mediastinal diseases,but also a surgical treatment for mediastinal tumors and malignant pleural effusion.
4.Video-mediastinoscopic biopsy and treatment for pleural effusion

Objective To investigate the application of video-mediastinoscopic biopsy and treatment for pleural effusion. Methods Thirty-two patients with pleural effusion received mediastinoscopic biopsy or treatment. The patients were maintained at a lateral decubitus. A 2 cm incision was made along the midaxillary line for introducing the mediastinoscope. The pleural fluid was drawn out and exploration with biopsy was conducted under mediastinoscope. For 25 patients with malignant hydrothorax, talcum powder was applied for pleurodesis. Results The operation time was 30~70 min (mean, 42 min). A confirmative diagnosis was obtained by video-mediastinoscopy in all the 32 patients, including adencarcinoma in 22 patients, poorly differentiated squamous cell carcinoma in 2, invasive thymoma in 1, tuberculosis in 5, and inflammation in 2. All the operations were successful and the patients recovered smoothly without severe postoperative complications. Conclusions Video-mediastinoscopy is one of effective methods for biopsy and treatment of pleural effusion.
5.COMBINED TRANSPLANTATION OF FETAL PANCREAS AND THYMUS FOR THE TREATMENT OF DIABETES MELLITUS I: A REPORT OF ONE-YEAR OBSERVATION

Combined transplantation of fetal pancreas and thymus was performed in 10 patients suffering from severe diabetes I. They were closely followed for over one year, and it was found that among them 7 showed spectacular clinical improvement, including 2 with total discontinuation of insulin, and 5 with average reduction of insulin by 82%. In 3 patients, the dosage of insulin was reduced by an average of 56.9%. The average reduction of insulin for the 10 patients in 2 months, 6 months and one year after the transplantation was 68.3%, 74.5%, and 78.4%, respectively. The effect of warm ischemia of different durations on the donor tissues was studied with electron microscopy. The significance of combined transplantation and the relationship of the techniques of procuring the donor tissues and transplantation is discussed.
6.Accuracy of coronary calcium measurements with low dose 64-slice multi-detector row CT

Objective To compare the low- and standard-dose protocols in the evaluation of coronary artery calcification (CAC) scores and in image quality,and to assess the accuracy of low-dose prospective ECG-gated helical CT in CAC measurements.Methods Forty-three patients with CAC were scanned twice using a tube current setting of 100 and 55 mA retrospectively.CAC scores (Agatston) of different branches of coronary artery were recorded.The mean and SD of the CT attenuation values in regions of interest in the aorta were measured,and the value (mean + 2 × SD) were obtained.Results Calcification plaques were found in 105 of the 172 vessels in the 43 subjects.The radiation dose of the 50 mAs group was (0.72 ± 0.04) mSy,significantly lower by 0.6 mSv than that of the 100 mAs group [(1.32 ± 0.08) mSv,P < 0.05].The total calcification score of the 55 mAs group was significantly positively correlated with the routine dose group after performing the square root transformation (r =0.998).A signficant correlation existed in vessel-based calcification between these 2 groups (r= 0.997).Conclusions Low-dose prospective ECG-gated 64-slice CT yields high accuracy on CAC scoring while maintaining the image quality needed to measure CAC.
7.The role of JNK in the hydrogen treatment for intestinal barrier dysfunction in severe septic mice

Objective To investigate the role of JNK in intestinal barrier dysfunction in severe septic mice treated by hydrogen. Methods Eighty male ICR mice were randomly divided into four groups (n=20 each):sham operation group, hydrogen control group, sepsis group and hydrogen treatment group. Severe sepsis rat model was reproduced by cecal ligation and puncture (CLP). Laparotomy without CLP was performed in sham operation group and hydrogen control group. The mice in hydrogen control group and hydrogen treatment group received 1-hour inhalation of 2%hydrogen at 1 hour and 6 hours after sham operation or CLP, respectively. Ten mice of each group were selected at 20 h after CLP operation and were gavaged with fluorescein-isothiocyanate-conjugated dextran (FITC-dextran). Blood samples were obtained by cardiac puncture to measure the serum concentration of FITC-dextran 4 h after treatment with FITC-dextran . Ten mice in each group were sacrificed at 24 h after CLP operation. The colony-forming unit (CFU) numbers in the peritoneal lavage fluid were counted. The middle intestinal tissues were obtained for the measurement of tumor necrosis factor alpha (TNF-α), interleukin (IL)-1βand high mobility group box 1(HMGB1) by ELISA. The level of phosphorylated JNK (p-JNK) and the expression of tight junction protein ZO-1 and Occludin were detected by Western blot assay. The intestinal pathological changes and epithelial ultrastructure changes were observed by light microscope and transmission electron microscope (TEM). Results There was no statistical significance in clinical variables between sham operation group and hydrogen control group. Compared with sham operation group, the serum FITC-dextran concentration, the CFU numbers in the peritoneal lavage fluid, the levels of TNF-α, IL-1βand HMGB1 in intestine, and the expression of p-JNK were significantly increased, the expression of ZO-1 and Occludin were down-regulated in sepsis group(P < 0.05). There was a significant intestinal pathological injury along with epithelial ultrastrcture injury in sepsis group. Compared with sepsis group, the serum FITC-dextran concentration, the CFU numbers in the peritoneal lavage fluid, the levels of intestinal TNF-α, IL-1β and HMGB1, and the expression of p-JNK were significantly decreased, the expression of ZO-1 and Occludin were up-regulated in hydrogen treatment group(P < 0.05), and the pathological and ultrastructure damage was significantly reduced. Conclusion Hydrogen can decrease levels of proinflammatory factors and up-regulate the expression of tight junction to improve intestinal barrier dysfunction caused by severe sepsis, which is related with the inhibition of JNK signaling pathway.

9.Extraction of Ephedrine and Prophyrin Copper Sodium of Ephedra

Objective: To obtain ephedrine with porphyrin copper sodium from Herba Ephedrae. Methods: Application of ethanol to obtain ephedrine and porphyrin copper sodium from ephedra. The spoinfy reaction of porphyrin will take place, the integral ephedrine will turn into free ephedrine (dissolved in ether), when the liquor of ethanol of ethanol of ephedra are heated with NaOH. Using dimethylponyle to extract, the liquor will into two phases, one's (including sponify porphyrin) is water phase, the other's (including free ephedrine) is ether phase. Results: The yield proportion of ephedrine is 0.72%, and porphyrin copper sodium is 1.8%.Conclusion: It is practicable to extract porphyrin copper sodium with ephedrine from Herba Ephedrae.
10.Effects of propofol on invasion and migration of glioma cells in rats and the role of ADAR2-AMPA receptor GluR2 pathway

Objective To evaluate the effects of propofol on the invasion and migration of glioma cells in the rats and the role of adenosine deaminase acting on RNA 2 (ADAR2)-α-amino-3-hydroxy-5-methylisoxazole-4-propionic acid (AMPA) receptor subunit glutamate 2 (GluR2) pathway.Methods C6 glioma cells were subcuhured and randomly divided into 4 groups (n =24 each) using a random number table:control group (group C);propofol group (group P);negative siRNA transfection + propofol group (group NP);ADAR2-siRNA transfection + propofol group (group AP).The cells were cultured in the common culture medium in group C.In NP and AP groups,negative siRNA and ADAR2-siRNA were transfected into the cells,respectively,and 48 h later the other procedures were similar to those previously described in group P.Propofol with the final concentration of 1.2 μg/ml was added,the cells were cultured for 6 h and then were cultured in the common culture medium for another 18 h in group P.The cells were selected to detect the cell viability by MTT colorimetric assay.The invasion of cells was determined by Transwell invasion assay,and the invaded cells were counted.The migration of cells was determined by cell scratch test,and cell migration rates were calculated.The expression of ADAR2 in the nucleus of cells and GluR2 in the cytomembrane was detected by Western blot.Results Compared with group C,the cell viability,the number of invaded cells and cell migration rates were significantly decreased,and the expression of ADAR2 in the nucleus of cells and GluR2 in the cytomembrane was significantly up-regulated in P and NP groups (P<0.05).Compared with group P,the cell viability,the number of invaded cells and cell migration rates were significantly increased,and the expression of ADAR2 in the nucleus of cells and GluR2 in the cytomembrane was significantly down-regulated in group AP (P<0.05).Compared with group NP,the cell viability,the number of invaded cells and cell migration rates were significantly increased,and the expression of ADAR2 in the nucleus of cells and GluR2 in the cytomembrane was significantly downregulated in group AP (P<0.05).Conclusion Propofol can inhibit the invasion and migration of glioma cells in the rats,and the mechanism is associated with activation of ADAR2-AMPA receptor GluR2 pathway.
