2.Observation of the effect of enalapril folic acid in the treatment of H type hypertension

Objective To observe the efficacy of enalapril folic acid in the treatment of H type hypertension. Methods 196 H type hypertension patients with carotid atherosclerosis were randomly divided into the enalapril folic acid tablets group(leaf group 99 cases)and the control group(enalapril group 97 cases),respectively,enalapril folic acid tablets and enalapril,once a day.Continuous administration of 10 months,before and after treatment,the blood pressure control level,plasma homocysteine levels and carotid intima media thickness were compared between the two groups.Results ,Before and after treatment,the systolic blood pressure in the leaf group were (159.51 ±8.55)mmHg, (138.68 ±7.82)mmHg,and the diastolic blood pressure were (98.11 ±7.44)mmHg,(82.98 ±5.22)mmHg.In the enalapril group,before and after treatment,the systolic blood pressure were (160.10 ±9.12)mmHg,(139.47 ± 7.99)mmHg,and the diastolic blood pressure were (97.88 ±7.62)mmHg,(83.12 ±5.57)mmHg.Blood pressure before and after treatment in the two groups were significantly different(all P <0.05),there were no significant differ-ence between the two groups(all P >0.05).In the leaf group,before and after treatment,the plasma homocysteine concentrations were (16.89 ±5.62)μmol/L,(10.37 ±3.06)μmol/L,there was significant difference(P <0.05), and in the enalapril group before and after treatment,the plasma homocysteine concentrations were (17.11 ± 6.33)μmol/L,(16.88 ±6.48)μmol/L,there was no significant difference(P >0.05).In the leaf group,before and after treatment the carotid artery intima media thickness were (1.28 ±0.33)mm,(1.08 ±0.36)mm,there was sig-nificant difference(P <0.05),and in the enalapril group before and after treatment the carotid artery intima media thickness were (1.29 ±0.38)mm,(1.31 ±0.43)mm,there was no significant difference(P >0.05).Conclusion Enalapril folic acid tablets in treatment of H type hypertension has significant efficacy,it can reduce blood pressure and plasma homocysteine levels and delay the progression of atherosclerosis.
3.Quantitative assessment of lung cancer tumor vascular parameters by using high speed perfusion CT: influence of tumor region of interest

Objective To prospectively determine whether position and size of tumor region of interest (ROI) influence estimates of lung cancer vascular parameters at 256-slice CT perfusion study.Methods After institutional review board approval and informed consent,16 men and 11 women with lung cancer underwent 70-second CT perfusion study. Perfusion, blood volume, peak enhancement weredetermined for 60 or 120 mm2 circular ROIs placed at the tumor edge and center and around (outlining) visible tumor. ROI analysis was repeated twice by two observers in the same way to get the average values.The correlation between the measurements and microvascular density counts was determined in 19 patients with operation.Measurements were compared by using Pair-Samples t test,a difference with P≤0.05 was significant.Results Perfusion,blood volume,peak enhancement measurements were substantially higher at the edge than at the center for both 60 and 120 mm2 ROI. For 60 and 120 mm2 ROI,median of the three measurements were (M=15.40,9.50 ml · min- 1 · 100 ml - 1,M =35.60,25.40 ml · 100 g-1,and M=16.47,10.20 HU),respectively,at the edge versus(M =14.80,8.52 ml · min- 1 · 100 ml- 1,M =33.50,23.90 ml · 100 g- 1,and M =15.64,9.40 HU),respectively,at the center(t value was 10.95,9.80,7.06,2.93,7.74 and 10.09,respectively,all P < 0.05 ). Measurements varied substantially depending on the ROI size,values for the ROI for outlined tumor were intermediate between those at the tumor edge and center.Perfusion(M=12.51 ml.min- 1 · 100 ml-1),blood volume(M=29.31 ml.100 g-1),peakenhancement (M =12.93 HU) for the ROI outlining tumor were intermediate between those at the tumor edge and center.There was a good correlation between perfusion,blood volume,peak enhancement for the ROI outlining tumor and microvascular density[ ( 19.43 ± 8.78 ) vessels/0.74 mm2 ],respectively ( r value 0.732,0.590 and 0.544,all P < 0.05 ).Conclusions Position and size of tumor ROI substantially influence ultimate perfusion values.ROI for outlined entire tumor is more reliable for perfusion measurements and more appropriate clinically than use of arbitrarily determined smaller ROI.
4.Accuracy of coronary calcium measurements with low dose 64-slice multi-detector row CT

Objective To compare the low- and standard-dose protocols in the evaluation of coronary artery calcification (CAC) scores and in image quality,and to assess the accuracy of low-dose prospective ECG-gated helical CT in CAC measurements.Methods Forty-three patients with CAC were scanned twice using a tube current setting of 100 and 55 mA retrospectively.CAC scores (Agatston) of different branches of coronary artery were recorded.The mean and SD of the CT attenuation values in regions of interest in the aorta were measured,and the value (mean + 2 × SD) were obtained.Results Calcification plaques were found in 105 of the 172 vessels in the 43 subjects.The radiation dose of the 50 mAs group was (0.72 ± 0.04) mSy,significantly lower by 0.6 mSv than that of the 100 mAs group [(1.32 ± 0.08) mSv,P < 0.05].The total calcification score of the 55 mAs group was significantly positively correlated with the routine dose group after performing the square root transformation (r =0.998).A signficant correlation existed in vessel-based calcification between these 2 groups (r= 0.997).Conclusions Low-dose prospective ECG-gated 64-slice CT yields high accuracy on CAC scoring while maintaining the image quality needed to measure CAC.
5.Comparison of therapeutic effects of transureteroscope holmium laser lithotripsy and retroperitoneal laparo-scopic ureterolithotomy for unilateral upper ureteral calculi

Objective To compare the therapeutic effects of transureteroscope holmium laser lithotripsy ( URL) and retroperitoneal laparoscopic ureterolithotomy ( RLU) for unilateral upper ureteral calculi and evaluate the selection of indications and clinical value of URL and RLU .Methods 37 patients were enrolled into the study . 14 patients received URL ,23 patients received RUL .Double-J stent was routinely indwelled in both groups .The oper-ation time,hospitalization time,clearance rate of the stone and incidence of postoperative complications .Results The stone clearance rate after postoperation of RLU group was 100%, which was significantly higher than that of URL group 64.4%(χ2 =4.15,P<0.05).The mean operation time in RLU group was(132.0 ±7.8)min,which was sig-nificantly longer than URL group(46.5 ±7.2)min(t=6.23,P<0.05).The mean intraoperative blood loss of RLU group was(38.0 ±4.6)mL,which was significantly higher than URL group (10.0 ±3.2)mL(t=5.20,P<0.05). The postoperative complication rate of RLU group was 8.7%,which was significantly lower than URL group 21.5%(χ2 =4.06,P<0.05).There was no significant difference in hospitalization time and stone clearance rate between the two groups(P>0.05).Two cases converted to operation in URL group ,while only 1 case converted to operation in RLU group.Conclusion Laparoscopic ureterolithotomy has a higher stone clearance rate and shorter operation time compared with ureteroscopic lithotripsy .Laparoscopic ureterolithotomy is a safe and effective therapeutic method for unilateral upper ureteral stones .

; Objective; To observe the preventive and therapeutic effects of Jieduquyuziyin methods in treating corticosteroid osteoporosis in systemic lupus erythematosus (SLE) and investigate into its mechanism. Method; 147 cases of SLE were randomly divided into the integrated traditional Chinese and western medicine group (79 cases) and the western medicine group (68 cases). There 21 cases which were diagnosed primarily among all 147 cases. 30 healthy cases were taken as the normal control group. The western medicine group (68 cases) was treated mainly with prednisone only, while the integrated traditional Chinese and western medicine group (79 cases) with Jieduquyuyiyin methods on the basis of normal western medicine. Both groups were treated for 3 months as a course. 16 cases in the western medicine group and 25 cases in the integrated traditional Chinese and western medicine group were kept on being treated and observed for 2 years. The lever of serum Ca, P, ALP, BGP, IPTH, 25-(OH) D3 and urinary Ca, P and BMD were checked and observed. 8.Reverse less invasive stabilization system for treatment of femoral intertrochanteric fracture

Objective To evaluate the clinic effect of reverse less invasive stabilization system (LISS) in treatment of femoral intertrochanteric fracture.Methods The study enrolled 22 cases of femoral intertrochanteric fractures treated with reverse LISS from January 2007 to January 2011.Twelve out of the cases were males and 10 were females,with age ranging from 44 to 86 years (mean,72.5 years).Left fracture occurred in 8 cases and right in 14 cases.Causes of injury included fall on the ground in 15 cases,vehicle accidents in 4 cases,and fall from height in 3 cases.There were 15 cases of type Ⅲ and 9 of type Ⅳ according to Evans-Jensen classification and all fractures were closed injuries.Time from injury to operation was 3-14 days (mean,5.3 days).Results Intraoperative hemorrhage was (130.5 ± 60) ml and operation time was (55 ± 15) min.All cases were followed up for 12-30 months (mean,14.8 months).Fracture healing time was 10-27 weeks (mean,13.4 weeks).Harris hip score was excellent in 11 cases,good in 8,fair in 2,and poor in 1,with excellent and good rate of 86%.Two cases felt greater trochanter pain at follow-up ; one presented with plate breakage and malunion at 6 months postoperatively,but may need no further treatment; the rest healed without complications of surgical site infection,varus deformity of the hip,implant breakage,fracture redisplacement,screw drawing pullout or cutout.Conclusion Reverse LISS plating is an effective treatment for femoral intertrochanteric fractures,but the procedure can not assure 100% success.
9.Treatment of intertrochanteric femoral fractures with minimally invasive dynamic hip screws

Objective To evaluate effects of minimally invasive dynamic hip screw(DHS)in treatment of intertrochanteric femoral fractures.Methods The study involved 98 patients with intertrochanteric femoral fractures treated by closed reduction and C-arm fluoroscopy guided minimally invasive DHS from January 2004 to January 2010.According to AO classification,there were 38 patients with type A1 fractures and 60 with type A2 fractures.According to Evans classification,there were nine patients with typeⅠ?fractures,29 with type Ⅱ,36 with typeⅢ?and 24 with type V.Intraoperative blood loss,operation time and incision length were recorded.Results The intraoperative hemorrhage,operation time and incision length were average 250 ml(range,150-450 ml),54.3 minutes(range,45-70 minutes),and 5.2 cm(range,4-7 cm),respectively.All the patients were followed up for 12-38 months(mean 16.8 months).Fracture healing time was 10-14 weeks(average 11.5 weeks).According to Zuekerman functional scoring for hip joint,the results were excellent in 61 patients,good in 30,fair in four and poor in three,with excellence rate of 92.9%.Varus deformity of hip occurred in four patients.No patient presented surgical site infection,implant failure or displacement of fractures.Conclusion Minimally invasive DHS is an effective means in treating intertrochanteric femoral fractures,but the key point of successful treatment is to strictly grasp the correct operative procedures.
10.Effect of MAPK signal transduction pathway on the expression of TNF-? in alveolar macrophage of chronic bronchitis rats and the target of TAL

Aim To investigate the effect of MAPK signal transduction pathway on the expression of TNF-? in AM(alveolar macrophage) of CB(chronic bronchitis) rats,meanwhile,to initially discuss the possible regulated target of TAL(triterpene acids of loquat.leaf),to explain the mechanism about anti-inflammatory action of TAL on CB rats.Methods The depurated AM was incubated with the special inhibitors of MAPK signal transduction pathway respectively(ERK,p38 or JNK) and the expression of TNF-? was observed by RT-PCR and Western blot.Using factorial design,TNF-? level in AM culture supernatant was detected by ELISA.Results The special inhibitors of ERK(PD98059) and p38(SB203580) could change the expression of TNF-? in AM of CB rats apparently.Factorial analysis shows there is statistical significance between TAL,PD98059/SB203580 and mixture of them.Conclusions TAL reducing the expression of TNF-? may be related with the inhibition of ERK and p38 signal transduction pathway.It may be one of the mechanisms by which TAL has the preventive and therapeutic effects on chronic bronchitis.
