1.Advances in radiation therapy for early stage breast cancer

Radiation therapy plays an important role in the multidisciplinary treatment of early stage breast cancer. While breast conservative treatment has become an established strategy, recent clinical investigations focus mainly on the possibility of modification to the standard practice, including the significance of tumor bed boost after whole breast irradiation, the criteria for low-risk patients not to receive adjuvant radiotherapy after breast conservative surgery and the feasibility of partial breast irradiation. Post-mastectomy radiotherapy to the chest wall and regional lymph nodes in high-risk patients has been proved to reduce the local-regional recurrence by 2/3. However, only a few trials support the improvement of survival, and significant controversy exists concerning the role of post-mastectomy radiotherapy in intermediate-risk patients. Current review summarizes the advances and controversies stated above for the treatment of early stage breast cancer.
2.The plasma protein C, platelet alpha-granule membrane glycoprotein and diabetic nephropathy

To investigate the influence of abnormal blood coagulating state on development of diabetic nephropathy. Methods Using radioimmunoassay and bivanlent antibody sandwich technic, the levels of plasma protein C (PC), platelet alpha-granule membrane glycoprotein (GMP-140), and factor Ⅷ: C and vWF were detected in 65 NIDDM patients and 15 normal controls. Results (1)PC level in microalbuminuric patients(DMⅡ) was significantly lower than that in normal controls and in normoalbuminuric patients (DMⅠ ), the lowest was in clinical proteinuric patients (DMⅢ); PC levels correlated negatively with urinary albumin exeretion rates(UAER). (2)The GMP-140 level was higher signifi-antly in DMn than in normal controls and in DMⅢ ; and the highest was in DMⅠ; the GMP-140 levels positively correlated with UAER. (3)There were negative correlations between PC and GMP-140、Ⅷ C-vWF. Conclusion A hyper-coagulable state occurs early in diabetic nephropathy as valuable indexes in reflecting the microangiopathy in diabetes.
3.Advances of transforming growth factor-β inhibitors.

Transforming growth factor-β is an important cytokine with various bioactivities, including embryonic development, wound healing, chemotaxis and cell cycle regulation. Epithelial-mesenchymal transition (EMT) is the main pathway of tumor cell to obtain the ability of invasion and metastasis. The TGF-β is the key factor known to induce EMT in cancer cells and plays an important role in the process. In recent years, some progress has been obtained. Some TGF-β inhibitors have approved in the market or in clinical trials. TGF-β inhibitors can play an important role on the treatment of tumors, glaucoma, liver and kidney fibrosis disease and scar repair. Novel TGF-β inhibitors reported in recent years were reviewed in this article.
4.Location and expression of galactose β-1,4-glycosidic bonds in keloid tissue

Objective To observe the expression and location of galactose β-1,4-glycosidic bonds (Gal β-1,4-GlcNAc) in keloid tissue,and to investigate the role of glycoprotein galactosylation in the formation of keloid.Methods This study included 10 keloid tissue specimens,7 hyperplastic scar tissue specimens,and 6 normal skin specimens.Lectin blot analysis was performed to measure the glycosylation level of glycoproteins,saturated picric acid-Sirius red staining followed by polarization microscopy to observe the type,expression and distribution of collagens in these specimens.The type Ⅰ/type Ⅲ collagen ratio was calculated.Immunofluorescence-based histochemistry was carried out by using Ricinus communis agglutinin I to analyze the expression and location of Gal β-1,4-GlcNAc in these skin samples,and double immunofluorescent staining to observe the colocalization of Gal β-1,4-GlcNAc and type Ⅰ procollagen α1.Results Compared with the normal skin tissue,the keloid tissue showed a significant increase in the expression of Gal β-1,4-GlcNAc in glycoproteins with a relative molecular mass of 30 000 to 40 000.Polarization microscopy revealed that there was a considerable expression of type Ⅰ collagen fibers,which amounted to (71.53 ± 4.03)% in all the collagen fibers.The type Ⅰ/type Ⅲ collagen ratio was significantly higher in keloid tissue specimens than in normal tissue specimens (2.56 ± 0.53 vs.0.91 ± 0.11,P <0.05).Fluorescence microscopy showed that Gal β-1,4-GlcNAc was uniformly distributed in the membrane and cytoplasm of fibroblasts in keloid tissue,and the expression intensity of Gal β-1,4-GlcNAc in keloid tissue was notably stronger than that in normal skin tissue.There was a colocalization between Gal β-1,4-GlcNAc and type Ⅰ procollagen α1 in keloid tissue.Conclusions The expression of Gal β-1,4-GlcNAc,which is mainly observed in fibroblasts,is upregulated in keloid tissue,suggesting that Gal β-1,4-GlcNAc may be involved in the modulation of factors responsible for excessive fibre formation during the repair process of keloid.

The paper elaborates on the design thinking of the outpatient intelligent medicine delivery system under the two parallel Hospital Information Systems (HIS) of the Central Hospital of Wuhan and implements the distribution of medicine delivery windows based on the multi-channel payment methods of the old HIS outpatient system and the preplanned and real-time intelligent medicine delivery based on the new HIS pharmacy system.Using the integrated platform of Ensemble and services of Web Service,it realizes the data interaction among multiple systems.The system operates steadily and the new HIS is well connected with the old.Thus,the efficiency can be enhanced and the quality can be ensured.
6.A quantitative index system for casualty forecasting in refugees living in border areas

Objective:To investigate the priorities of each factor influencing the casualties in refugees living in border areas, so as to provide evidence for casualty forecasting in the refugees. Methods: We summarized the factors affecting the refugee casualty in the border area through searching and reviewing the related literatures. The identified factors were classified into 3 levels and the quantitative index system was established by using Delphi method, i.e. expert consulting method. The names and the meanings of each index were revised according to experts’ suggestions after 3 rounds of consulting. The weights of each index were determined by analytic hierarchy process (AHP) and comparing-reordering method. Results: A 3-level quantitative system was successfully constructed, which consisted of 4 first level indices (including natural factors, social factors, medical factors, and war factors), 12 second level indices, and 37 third level indices; the weights of all indices were determined. Conclusion: The result of our study can be used in predicting refugee casualty and provide a reference for the medical service of refugees living in the border areas.
7.Prognostic predictors for breast cancer patients with brain metastasis after rad iotherapy

However in the Cox mu ltivariate analysis, only KPS, numbers of BM, extracranial systemic metastasis a nd chemotherapy were significant prognostic factors. Just as those of RPA subgro ups, the median survival from the start of WBRT was 3, 9, 16 months in 3 PI subg roups respectively.Conclusions:Overall survival in BM from breast cancer remains p oor. KPS, numbers of BM, extracranial systemic metastasis and chemotherapy were independent prognostic factors. RPA classification can predict the prognosis of patients with BM from breast cancer, so can the PI model.
8.Effects of mesenchymal stem cells intravenous transplantation on the glial cell line derived neurotrophic factor after the spinal cord injury of rats

Objective To observe the effects of mesenchymal stem cells (MSCs)injection from tail-vein on the expression of glial cell line derived neurotrophic factor (GDNF) after the spinal cord injury (SCI) of rats.MethodMSCs were cultured from the thighbone of adult wistar rats.The T10 level spinal cord injury was execute by the modify-Allen's device. The MSCs labeled by bromodeoxyuridine were transplanted into the vein of tail by injection immediately after spinal cord injury. Adult Wistar rats were randomly divided into three groups: SCI cured with MSCs transplantation (group A), SCI received normal saline(group B),and control group (group C). Then MSCs were detected and the expressions of GDNF of the lesion and neighbor areas were examined by Reversetranscription polymerase chain reaction (RT-PCR) and immunohistochemistry.ResultImmunohistochemistry: MSCs labeled by bromodeoxyuridine could be detected in the injuried spinal cord after transplantation. It was located at the nucleolus of MSCs. The cells survived and migrated rostrallyand caudally from the injection sites. With the time passed, the number of MSCs labeled with bromodeoxyuridine was decreased. RT-PCR: The expression of Group A was increased on the followed day, and the expression of Group B was increased on the 1st day but decrease from the 5th day then on.compared with group B, transplantation of MSCs significantly enhanced the expression of GDNF mRNA than group A on the 1st, 3rd, 5th day after cell transplantation(P
9.Study on Curricula-setting in Pre-job Training of Graduates from Local Medical Universities as New Recruits

10.Clinical trial of analgesic effects of buprenorphine sublingual tablet in dental treatment

Objective:To assess the analgesic effects and side effects of buprenorphine sublingual tablet in dental treatment.Methods:500 cases for dental treatment were selected.Before management of teeth,all the patients took 0.2 mg of buprenorphine sublingual tablet.The VAS score,Kuttner face score and side effects after dental treatment were recorded.Results:The VAS scores of the patients underwent preventive,operative and prosthetic treatments were 3.99?2.34,2.72?1.67 and 2.78?1.55,the Kuttner scores of those 1.98?1.23,2.66?1.04 and 2.03?0.93,respectively.Side effect was not observed in all the patients.Conclusion:Buprenorphine sublingual tablet can relieve the pain in dental treatment.
