1.Bone marrow fat in the regulation of bone metabolism and its pathological role in osteoporosis

Recently,more and more researches focus on the relationship between bone marrow fat and osteoporosis.From a pathological point of view, osteoporosis is traditionally regarded as the dysfunction of osteoblastic bone formation and osteoclastic bone resorption.Actually, bone marrow adipose tissue that constitutes a large proportion of bone marrow cells is indispensable for the balance of bone metabolism.Bone marrow fat, which is abundant in yellow bone marrow, increases gradually with aging.Bone marrow adipocytes and osteoblasts differentiate from a common progenitor, bone marrow mesenchymal stem cells(BMSCs).It is generally acknowledged that they are reciprocal repellent in the process of BMSC differentiation.Exploring the association between bone marrow adipocytes and osteoporosis is critical for better understanding its role in the pathogenesis and the future treatment of osteoporosis.In this review, we summary the recent progress in the association between bone marrow adipocytes and osteoporosis.
2.Epidemiology investigation of nonalcoholic fatty liver disease in Yichang professional crowd and its correlation with the metabolic syndrome

Objective To study the prevalence rate of nonalcoholic fatty liver disease (NAFLD)and its correlation with the metabolic syndrome among professional crowd in Yichang city ,and analyze the characteristic of prevalence of NAFLD .Methods Physical check‐up and liver ultrasonography were done and fasting blood GLU ,TG ,HDL‐C ,UA ,CRP were measured for sampling survey professional people in Yichang city .We sampled 6 450 people in 15 company ,including 3 284 men ,3 166 women(20 to 70 years old) ,the results were analyzed .Results The NAFLD prevalence rate of Yichang professional crowd was 21 .71% ,28 .68% in male and 14 .47% in female respectively(P<0 .01) .The prevalence rate in male was higher than that of female before 60 years old . NAFLD prevalence rate in women showed a trend of increasing along with the age growth ,the incidence rate come up to 31 .31%when women were over 60 years old .The highest prevalence rate of MS related components in NAFLD group were obesity (69 .98% )、high blood TG level(61 .10% ) .Conclusion Male before 60 years old and female over 60 years old of Yichang profes‐sional crowd belong to NAFLD high‐risk groups ,the group should be focused on as regular monitoring ,prevention and interven‐tion .NAFLD prevalence rate significantly increased in people with MS .The most important factors of suffering from NAFLD are o‐besity ,high blood TG level .
3.Three-dimensional finite element analysis on computer aided reconstruction of hip capsular ligament and its biomechanical significance

Objective To discuss biomechanical mechanism of reconstructed joint capsule ligament stabilizing hip joint by means of three-dimensional finite element analysis.Methods A finite element model of total hip arthroplasty(THA)including ischiofemoral ligament reconstruction was construtted by using finite element analysis software Unigraphics NX 2.0 and SolidWorks 2006 to simulate seated leg crossing and obtain peak resisting moment and range of motion prior to impingement.Results The current form of finite element model was characterized by large deformation multi-body contact,large interfacial sliding and high elasticity and could clearly reflect real anatomy and biomechanical behavior of ischiofemoral ligaments.Compared with model with only metal,model of ischiofemoral ligament reconstruction could reduce the peak polyethylene stress at the impingement site and at the head egress site by typically 17%and 31%respectively,increase peak resisting moment by nearly 57%and provide 2.29-fold stability.Conclusions As a discrete structure within the posterior capsule of the hip joint,the ischiofemoral ligament may be the most important contributor to the mechanical integrity of the posterior stability structure.The joint capsule ligament must be reconstructed in hip arthroplasty.
4.Predicative value of thrombelastogram in aSAH with delayed cerebral ischemia

Objective To investigate the clinical value of thrombelastogram (TEG) in prediction for the cause of delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (aSAH). Methods In this study, there were 30 patients with DCI (group DCI) and 45 patients without DCI (group No-DCI). TEG was performed in all the patients at post-bleeding day 1, 5, 10 and 15 after aSAH(PBDn, n=1,5,10,15). The changes of reaction time (R value), coagulation time (K value),coagulation angle (α value),maximum thrombus consistency (MA value) and coagulation index (CI value) were examined at different time points after aSAH. Results Thirty of 75 aSAH patients developed DCI and the incidence of DCI was 40 percent. According to linear mixed model, both MA value and CI value were significantly statistical different at different time points within each group (P<0.05,for all) as well as between No-DCI group and DCI group.MA value and CI were significantly statistical different at same time point (P<0.05,for all). The results of logistic regression analysis showed that modified Fisher levelⅢ-Ⅳ,△MA5-1(OR=1.124,P=0.024,95% CI=1.015~1.244), PBD5 MA>70(OR=5.605,P=0.011,95% CI=1.464~21.457)were the independent risk factors of DCI.By using ROC curve to define a threshold for prediction of the occurrence of DCI,the rate of DCI was significantly increased when △MA5-1>3.05.Conclusion aSAH Patients, especially those with DCI have severe hypercoagulation. The MA value in PBD5 has an important predictive value for DCI.
5.Role of mesenchymal stem cells and their exosomes in the treatment of drug-induced liver injury

The incidence rate of drug-induced liver injury (DILI) is increasing year by year with unknown mechanisms, and the treatment methods for DILI mainly include drugs, liver support systems, and liver transplantation, all of which have certain limitations. Therefore, the search for safer and more effective treatment methods has become a research hotspot at present. Studies have shown that mesenchymal stem cells and their exosomes can alleviate liver injury by reducing liver inflammation, promoting hepatocyte proliferation and regeneration, inhibiting the apoptosis of hepatocytes, improving oxidative stress, and regulating immunity. This article briefly reviews the role of mesenchymal stem cells and their exosomes in the treatment of DILI, so as to provide a reference for further research.
6.Research advances in the mechanism of action of nucleotide-binding oligomerization domain-like receptor protein 3 inflammasome in autoimmune hepatitis

Autoimmune hepatitis (AIH) is chronic hepatitis caused by the attack of live cells by the immune system, and at present, the pathogenesis of AIH remains unclear. Inflammasomes are important components of innate immunity and are involved in a variety of pathophysiological processes. Studies have shown that inflammatory response associated with nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) plays an important role in the pathogenesis of AIH, which mainly mediates the release of proinflammatory factors and pyroptosis, thereby participating in the pathophysiological process of AIH. Therefore, the development and progression of AIH can be delayed by inhibiting the activation of NLRP3 inflammasomes, which provides new ideas for the prevention and treatment of AIH.
7.Mechanism of action of follicular helper T cells in autoimmune hepatitis

Autoimmune hepatitis (AIH) is a type of chronic hepatitis caused by the attack of hepatocytes by the autoimmune system, and with the prolongation of disease course, it may gradually progress to liver cirrhosis and even hepatocellular carcinoma. Although great achievements have been made in the understanding and treatment of AIH, its etiology and pathogenesis still remain unclear. T cells play a crucial role in the development and progression of AIH, and by focusing on follicular helper T cells, this article elaborates on the research advances in follicular helper T cells in AIH, in order to provide new ideas and strategies for the clinical treatment of AIH.
