The methylation of CpG island in promoter region of E-cad gene which usually leads to inactivation of E-cad gene is closely related to the occurrence, development, invasion, metastasis and prognosis of a variety of gastrointestinal cancer, such as esophageal carcinoma, gastric carcinoma, hepatocellular carcinoma,cholangiocellular carcinoma, carcinoma of gallbladder, pancreatic carcinoma and colorectal cancer. Further study would probably guide the early diagnosis, gene therapy, prophylaxis and predicting recurrence of tumor metastasis of gastrointestinal cancer.

Objective To evaluate the curative effect of laparoscopic Palomo operation compared with open inguinal varicocelectomy in children with varicocele, and testis development after treatment. Methods Data of 63 boys with varicocele were reviewed. The control group(n = 29) was underwent open inguinal varicocelectomy,the lap-aroscopy group(n =34) was underwent laparoscopic Palomo operation. The operation time,hospital stay,recurrence rate,and complications rate were compared between two groups. The boys in larparoscopy group were examined the diameter of spermatic vein and testicular volume before and after operation by ultrasound. All data were analyzed by SPSS 13.0 software. Results The hospital stays and recurrence rates and complications rates were significantly different between two groups (P < 0.05 ) , but the operation time in two groups had no statisticd difference (P > 0. 05 ). The diameters of spermatic vein in larparoscopy group were decreased significantly after operation (P < 0.01). The blood supply for testis didnt decrease after operation,the operative lateral testicular volume grew faster than the contra lateral,and reached almost the same volume. Conclusions Laparoscopic Paloma varicoelectomy was safe and simple, brings a better effect for children with varicocele than open inguinal varicocelectomy does. The operative lateral testis would a postoperative compensatory growth,no testis atrophy or hypogonadism occurs.

Objective:To determine and evaluate the antimicrobial effectiveness of 10% chloral hydrate solution. Methods:The determination of antimicrobial effectiveness of 10% chloral hydrate solution from two different manufacturers was carried out according to Chinese Pharmacopoeia (2015 edition). Results:The antimicrobial effectiveness of 10% chloral hydrate solution met the require-ments of Chinese Pharmacopoeia (2015 edition). Conclusion:10% Chloral hydrate solution shows adequate antimicrobial effective-ness.

AIM　To investigate the effect of 4-aminopyridine (4-AP) on ion channels of myocytes. METHODS　L-type calcium channel and sodium channel currents were recorded in guinea pig single ventricular myocyte using whole-cell patch-clamp techniques. RESULTS　4-AP, 0.1, 0.5 and 1.0 mmol*L-1 were shown to inhibit L-type calcium channel currents (ICa,L) and sodium channel currents (INa) concentration-dependently. The percentage of inhibition were (11.6±1.7)%, (37.5±8.3)% and (54.5±6.9)% (P<0.01) respectively for ICa,L, and (22.1±14.3)% (P<0.05), (39.4±8.8)% and (62.3±6.8)% (P<0.01) respectively for INa. 4-AP 0.5 mmol*L-1 shifted the I-V curves of ICa,L and INa upwardly. CONCLUSION　4-AP blocked L-type calcium channel and sodium channels in guinea-pig ventricular myocytes concentration-dependently.

Objective To explore clinical features and antimicrobial resistance of lower respiratory tract infection (LRTI)with Streptococcus pneumoniae (S .pneumoniae)isolated from children in Nanjing.Methods Clinical data of children with confirmed S .pneumoniae LRTI through sputum culture at a children’s hospital in Nanjing between July 2013 and June 2014 were analyzed retrospectively,S .pneumoniae strains were performed antimicrobial susceptibility testing through K-B method and minimum inhibitory concentration (MIC)testing.Results Among 197 children with S .pneumoniae infection,72.59% were <3 years old,63.96% occurred in autumn and winter, 57.87% had elevated leukocyte count,cough and fever were the most common clinical symptoms,complications of digestive and circulatory system were also common.The resistance rates of S .pneumoniae to azithromycin, penicillin,and erythromycin were 94.92%, 92.98%, and 88.83% respectively; the sensitivity rates to vancomycin,chloramphenicol,meropenem,ceftriaxone,and ofloxacin were all > 90%,vancomycin was up to 98.98%.Conclusion The percentage of S .pneumoniae LRTI is high in children < 3 years old,most occur in autumn and winter,resistance rates to azithromycin,penicillin,and erythromycin are all high,antimicrobial agents should be selected for the treatment of infection according to antimicrobial susceptibility testing.

Objective To explore the relationship between expression of muc-1 gene in non-small cell lung cancer (NSCLC) tissues and lymph node metastasis and its clinical significance.Methods The expression of muc-1 gene in 36 specimens of NSCLC tissues and 10 normal lung tissues were determined by RT-PCR.Results muc-1 gene was positively expressed in 19 of 36 specimens of NSCLC tissues,the positive expression rate was 52.7%.The expression of muc-1 mRNA in NSCLC tissues was related to metastasis of lymph node(?2 =6.733,P0.05).The positive expression rate of muc-1 gene was higher in NSCLC tissues with lymph node metastasis(N0 10%,N1 58.82%,N2 88.9%).Conclusion The lymph node metastasis rate in NSCLC patients with muc-1 gene positive expression is higher,the detection of muc-1 gene expression can guide the prognosis and therapy of NSCLC patients after operation.

Objective To study the changes of CDH1 gene promoter CpG island methylation and its clinical significance in patients with esophagus and stomach double primary carcinoma(ESDC).Methods The expression of CDH1 gene methylation in cancerous tissues and adjacent cancerous tissues in 18 cases of ESDC were detected using methylation-specific PCR method. Results Eighteen patients were endoscopically diagnosed as ESDC between Jan. 2007 and Sep. 2009 in the 4th Hospital Affiliated to Hebei Medical University. The positive methylation of CDH1 gene in tissues of esophageal squamous cell carcinoma (ESCC)and adjacent cancer were 66.7% and 33. 3%, respectively, with significant difference (χ2= 4. 167, P = 0. 031). Whereas the positive methylation of CDH1 gene in tissues of gastric carcinoma (GA) and adjacent cancer were 77.8% and 44.4%, respectively, without statistical difference (χ2=1.786, P= 0. 180). There was no significant difference (P=0. 500) in positive rate of CDH1 gene methylation between ESCC tissues and GA tissues in same individual with ESDC. For 18 patients with ESDC, consistent change of CDH1 methylation in tissues of two kinds of cancers was found in 16 patients with a total agreement of 88.9 % (positive agreement of 66.7 % and negative agreement of 22. 2%). Statistical analysis showed a significant correlation between two groups (P = 0. 005). Conclusion In patients with ESDC, there is a high consistency of CDH1 methylation change, between ESCC and GA,which suggests that two kinds of cancer may have similar risk factors and molecular mechanisms.

Objective To study the effects of ?2-adrenergic receptor gene Arg16Gly polymorphism on blood lipid and apolipoprotein ratio and its role in blood lipid and apolipoprotein ratio mediated by high carbohydrate/low fat (HC/LF) diet in healthy young persons.Methods Fifty-six healthy young volunteers had regular diet for 7 d followed by HC/LF diet for 6 d.Twelve-hour fasting venous blood samples were collected on days 1,8 and 14 to measure blood lipid and apolipoprotein (apo) AI and B100 levels,and to calculate ratios of TG/HDL-C,log (TG/HDL-C),TC/HDL-C,LDL-C/HDL-C and apoAI/apoB100.DNA was isolated from genome.Arg16Gly polymorphism was analyzed by PCR-RFLP.Results No significant difference was found in the baseline lipid and apolipoprotein ratio in subjects with AA genotype and G carriers before and after regular or HC/LF diet.The ratios of TG/HDL-C (P=0.017),log (TG/HDL-C) (P=0.031),and apoAI/apoB100 (P=0.006) were significantly higher,while those of TC/HDL-C (P=0.001) and LDL-C/HDL-C (P

Objective To evaluate the efficacy of mirror therapy for stroke patients with hemiplegia.Methods Eighty stroke patients with hemiplegia were randomly divided into study group and control group with 40 cases in each group.Conventional rehabilitation training was given to patients in both groups;patients in study group were given additional mirror therapy for 24 weeks.The Fugl-Meyer Assessment (FMA) scale was used to assess the motor function,Functional Independence Measurement (FIM) scale to assess the activity of daily living,Modified Barthel Index to assess the ability of daily living,Brunnstrom Stage Scale to evaluate the states of the upper extremities,and the modified Ashworth Scale to evaluate muscle spasticity before and after 24-week treatment.Results Mter 24 weeks of treatment,there were significant differences in FMA score [(44.08 + 2.44) vs.(40.53 +2.11),t =7.07,P＜0.05,FIM score [(71.08 + 5.37) vs.(64.18 + 4.47),t =7.20,P ＜ 0.05],modified Barthel index score [(59.75+7.84) vs.(54.25 +8.88),t =3.03,P＜0.05 and Brunnstrom stage (Z=-3.000,P＜ 0.05) between the study group and the control group.However,there was no significant difference in modified Ashworth score between the two groups of patients (Z =-1.732,P ＞ 0.05).Conclusion Mirror therapy can improve upper limb and hand function of the stroke patients with hemiplegia and enhance their ability of daily life activities;however,it has no significant effect to improve the limb spasm.

Objective To assess the efficacy and safety of monoclonal antibody rituximab combined with cyclophosphamide (CTX) in the treatment of refractory and recurrent autoimmune hemolytic anemia.Methods Seven cases with refractory and recurrent autoimmune hemolytic anemia ( including 1 case of Evans syndrome) were recruited during January,2007 to December,2010.Treatment regimens were as follows:rituximab:375 mg/m2,1 time/week,2-6 courses; CTX:1 g,1/10 d,2-7 courses; combined with intravenous immunoglobulin (IVIG) 5 g,1 time/week,given 1 day after rituximab administration.The efficacy and safety of this regimen were assessed during follow-up.Results All the patients showed good responses (7/7).Six patients achieved complete remission (6/7) and one achieved partial remission ( 1/7 ).Average follow-up time for the patients was 27 months.All patients remained in remission during the 12-month follow-up visits.Two patients showed elevated indirect bilirubin and increased reticulocyte counts within 24 months.One patient achieved complete remission after additional rituximab therapy,and another patient remained partial remission after cyclosporine therapy.At the time of 36-month follow-up visit,the patient relapsed and was retreated with 3 courses of rituximab combined with CTX and eventually achieved partial remission.All patients tolerated the treatment well with few mild side effects.Conclusions Rituximab combined with CTX is effective and relatively safe in patients with refractory and recurrent autoimmune hemolytic anemia.Additional treatment to relapse patients about 12-24 months after drug withdrawal continues to be effective.
