1.Isolation of differently expressed cDNAs from the hippcampus of rats with vascular dementia by RNA fingerprinting

Objective To isolate differently expressed cDNAs associated with vascular dementia from hippocampus of rats. Methods The creation of vascular dementia model of rat was by 4 vessel occlusion method. The learning and memory of vascular dementia(VaD) rats were examined by Morris water maze. RNA fingerprinting was used to analyse the differently expressed cDNAs in the hippocampus of normal aging and VaD rats. At the same time, cDNAs expressed differently during the progression of VaD were isolated. Results Thirty two candidate cDNA fragments were isolated by RNA fingerprinting. The two cDNA fragments specifically expressed in VaD rats were analysed by Northern blot. Homology analysis through BLAST revealed that these two were novel genes and were given numbers of BG937392 and BG937393 in the gene bank. Conclusions RNA fingerprinting is a simple and quick method for isolating differently expressed genes. These two new gene fragments may contribute significantly to the process of VaD as pathogenic genes or protective genes.
2.Preliminary study on mobile platform-oriented echocardiogram data visualization for the evaluation of left ventricular diastolic function

Objective To research and develop the mobile platform-oriented echocardiogram data visualization technology for the evaluation of left ventricular diastolic function,and investigate its application value.Methods First,diagnostic algorithms for the evaluation of left ventricular diastolic function (based on recommendations from ASE & EAE guidelines) were programmed into a mobile application software package,using Adobe Flex builder 4.0 as a basic tool for data visualization.Second,the program was installed in the Apple iPad4 tablet computer,then debugged and improved according to feedback information from practical applications.Results The echocardiogram data visualization for the estimation of left ventricular filling pressures in patients with depressed ejection fraction(EF) or normal EF and grading diastolic function can be implemented in mobile platform device,and simultaneously with emergency bedside echocardiography.The evaluation process can be visually presented by using the graphics to show the algorithm framework,the animation to express the analysis step,the axis to indicate the direction of information,and the color to differentiate the degree of the diastolic dysfunction.Conclusions The mobile platform-oriented echocardiogram data visualization technology makes the evaluation of left ventricular diastolic function more convenient,efficient and fast,and shows its potential to become an important adjunct to evaluation of cardiac function in the context of mobile health.
3.Enternal nutrition support of Nutrison fibre in neurosurgical coma patients

Objectives:To discuss clinical significance of enternal nutrition of Nutrison fibre in neurosurgical coma patients. Methods:The study group(20 cases,GCS≤5) was fed gastrically with Nutrison fibre by a nasal feeding tube, and the control group(20 cases,GCS≤5) was supported with common fluid diet.In all cases,triceps skin fold,blood sugar,albumin,total lymphocyte count,complications and nitrogen balance were observed. Results:There were not significant differences between the study group and the control group in nutritional status,but the rate of complication in study group was obviously lower than that in control group. Conclutions:Enteral nutrition with Nutrison fibre can decrease the complications in nurosurgical coma patients and improve the quality of life.
4.Application of emergency nursing management before disasters in great medical succors

Objective To supply the fastest and most effective first aid for patients with the least medical workers and within the shortest time after the occurrence of emergent public health accidents by application of emergency management. Methods We carried out retrospective invetigations about 55 great medical succors from the year of 2004 to 2006.Results Application of emergency nursing management played a pivotal role in these 55 great medical succors. The medical workers handled all the emergencies without confusion and a large number of patients got first aid in time. Conclusion It could facilitated the first aid for emergent public health accidents by application of emergency nursing management.
5.Clinical efficacy of micro-tympanoplastic operation with osteo retention on treating cholesteatomatous otitismedis

Objective To discuss with micro-tympanoplastic operation with osteo retention on treating cholesteatomatous otitismedis. Methods Choose 33 cases cholesteatomatous otitis medis patients who toke the tympanoplastic with osteo retention operation,then analyze the postulate and method of the operation, curative effect and complication after operation. Results Follow up with those patients by 12-72 months, and when 3-4 months after operation we endoscopiced the ears, could see that 43 cases of patients ear mastoid process and tympanic sinus were covered with epithelium, those mastoid cavity were well epithelial metaplasia;and the middle ear was separate from mastoid cavity; 39 cases patient of transplant eardrums are heal up (39/43, 90.7%). 2 cases have perforation of the tympanic membrane toke place after 4 months and 6 months with the operation, and it will heal up by anti-inflammatory drug and laser burning, 1 case had tympanic sinus hyperplasia with granulation tissue, and also had grassery juice; after taken anti-inflammatory drug and change bandage for 15 weeks, the symptom disappear. 1 case had grassery juice of operation cavity, after taking anti-inflammatory drug and physiotherapy, the symptom disappear in 24 weeks. After operation the pure tone audiometry shows that air conduction raise (16.8±6.8) dB HL, hearing raise up to:79.1%(34/43). Conclusion Base on right indication of operation choose, taken combined therapy of ambi-operation period, focus of infection are thoroughly cleaning up. Tympanoplastic with osteo retention are treating well with patients of cholesteatomatous otitismedis.
6.Moxibustion on Telomerase Activity in Aging Rat

Objective: To investigate the effect of moxibustion on telomerase activity and genes expression in tissues of senescent rats. Methods: Subacute aging rats model were established by injection with D-gal solution. Points Shenshu (BL 23) were treated with moxibustion in treatment group, contrasting with a model group and a normal group. Enzyme-Linked Immunosorbent Assays(ELISA) was used for the level oftelomerase activity in liver tissues, and In Situ Hybridization(ISH) was used for the condition of expression of telomerase genes in liver tissues. Results: The level of telomerase activity in the aging model group was obviously lower than that in normal control group (P<0.01), the level in moxibustion group was obviously higher than that in model group(P<0.05). In comparison with normal rats, the positive-expressed areas and photodensity of telomerase genes in aging model group were all significantly lower (P<0.01,P<0.01), and the positive-expressed areas in moxibustion group was significantly higher than that in model group (P<0.01). Conclusions: Moxibustion could regulate telomerase activity of senile rats, hence delaying aging.
7.Construction and application of computer-aided echocardiographic evaluation system for left ventricular diastolic function

ObjectiveTo build the computer-aided diagnosis system of left ventricular diastolic function for echocardiography,to make the evaluation process of left ventricular diastolic function for echocardiography more convenient,efficient,standardized and to obtain more accurate,practical,objective and reliable diagnostic results.MethodsFirst,the system was developed by the Macromedia Flash Professional referred to the newest guidelines and suggestions published by the American Society of Echocardiography(ASE) in 2009.Second,the program of left ventricular diastolic function in the computer- aided diagnosis system was designed,debugged and improved according to feedback information frompractical applications.ResultsThe diagnostic system can work simultaneously with echocardiography.If assessment parameters of the newest guidelines and suggestions,published by the American Society of Echocardiography in 2009,are imported to the system,then it can give the diagnosis of the change about the left ventricle filling pressures and score the left ventricular diastolic function quickly and automatically.The system will export standard report with both pictures and text.ConclusionsAccording to our experiment,the computer-aided diagnosis system is able to make the evaluation process of left ventricular diastolic function much more standardized and simplified,which guarantee the objectivity and reliability of the evaluation results.Consequently,it will be able to apply to clinical practice in the future.
8.Survey on prevalence rate of healthcare-associated infection in a hospital in three years

Objective To investigate the prevalence rate of healthcare-associated infection(HAI)in a hospital,so as to provide reference for making HAI control measures.Methods The cross-sectional survey on HAI was carried out among all hospitalized patients on May 26,2010,December 12,2012 and December 4,2013,respectively,sur-veyed data were analyzed.Results The prevalence rate was 6.66%(n =116),6.67%(n =113)and 6.33%(n =120)in 2010,2012 and 2013 respectively,and case rate was 7.29%(n=127),7.38%(n=125)and 6.97%(n=132) respectively,intensive care unit(ICU )had the highest infection rate,internal medicine ICU was up to 71 .43%. The main infection site was lower respiratory tract(44.53%),followed by surgical site infection (9.11 %)and uri-nary tract infection (9.11 %).The isolation rate of gram-negative bacteria,gram-positive bacteria and fungi was 60.81 %,20.38% and 18.81 % respectively.Usage rate of antimicrobial agents in three years was 32.95%, 29.87% and 25.59% respectively (χ2 = 13.16,P <0.01 ).Conclusion Prevalence rate of HAI in this hospital is high ,the main pathogen is gram-negative bacteria,the main infection site is lower respiratory tract ,antimicrobial use decreased year by year.Monitor on high risk departments,main sites and pathogens should be intensified.
9.Effects of TotalPanax Japonicus Extract onD-galactose Induced Aging Mice

Objective To research the protective effects of totalPanax Japonicus extract on learning memory, antioxidation, and anti-apoptosis of aging mice,and explore the mechanism. Methods Male mice were randomly divided into normal group, model group, Vitamin E (VE) group, Panax Japonicus extract low and high dose group. Except for the normal group, the other groups were injected withD-gal on the back of the neck subcutaneously to establish aging model. Normal group and model group were given a gavage with saline and each treatment group was given a gavage with totalPanax Japonicus extract and VE once a day for 7 weeks after the aging model established. All mice were be measured their learning and memory ability in the eighth week. After the test, the morphological changes of CA1 neurons were observed by HE stain. SOD, GSH-Px, MDA levels in brain tissue were measured by biochemical method, the expressions of Bcl-2 and Bax mRNA were evaluated by RT-PCR.Results Mice inPanax Japonicus extract low and high dose group could spend less time in searching for the platform, improve the learning and memory ability. TotalPanax Japonicus extract increased the activity of SOD and GSH-Px, while decreased the content of MDA. In addition, it could increase the expression level of Bcl-2 mRNA and reduce the expression level of Bax mRNA as well.Conclusion TotalPanax Japonicus extract has anti-aging effect.
10.Clinical efficacy of neoadjuvant chemotherapy combined with radical gastrectomy for advanced gastric cancer

Objective To investigate the clinical efficacy of neoadjuvant chemotherapy combined with radical gastrectomy for advanced gastric cancer.Methods The retrospective cross-sectional study was conducted.The clinicopathological data of 73 patients who underwent neoadjuvant chemotherapy combined with radical gastrectomy for advanced gastric cancer at the First Affiliated Hospital of Zhejiang University between June 2004 and December 2009 were collected.Neoadjuvant chemotherapy regimens included XELOX and FOLFOX.Patients received radical gastrectomy within 2 weeks after the completion of the last cycle of neoadjuvant chemotherapy and then continued to undergo postoperative neoadjuvant chemotherapy.Observation indicators:(1) adverse event of neoadjuvant chemotherapy;(2) surgical and postoperative situations;(3) follow-up situations.Follow-up using outpatient examination and telephone interview was performed to detect survival of patients up to December 2014.Measurement data with skewed distribution were described as M (range).Overall survival time was from the beginning of treatment to death or end of follow-up (patients with loss to follow-up).Progression-free survival time was from the beginning of treatment to tumor progression,recurrence and metastasis or death.The survival curve was drawn by the Kaplan-Meier method.Results (1) Adverse event of neoadjuvant chemotherapy:of 73 patients,38 received XELOX regimens and 35 received FOLFOX regimens,with a median cycle of 3 (range,1-7 cycles).There were 55 adverse events during neoadjuvant chemotherapy,including 47 with grade 1-2 and 8 with grade 3-4.(2) Surgical and postoperative situations:all the 73 patients underwent successful D2 radical gastrectomy for gastric cancer,including 40 receiving total gastrectomy,31 receiving distal gastrectomy,1 receiving total gastrectomy with transverse colon resection and 1 receiving distal gastrectomy with cholecystectomy.Of 73 patients,10 with postoperative complications were improved by conservative treatment,including 3 with pleural effusion,2 with peritoneal effusion,2 with anastomotic bleeding,2 with cholecystitis and 1 with lympha fistula.No patient received reoperations or died within 30 days postoperatively.Pathological TNM staging:22 patients were detected in stage Ⅰ-Ⅱ,45 in stage Ⅲ,4 in stage Ⅳ and 2 in stage T0N1M0.Three patients (in stage T0N0M0) had complete remission.Forty-three patients underwent postoperative chemotherapy.(3) Followup:all the 73 patients were followed up for 8-125 months,with a median time of 51 months.The median survival time,5-year overall survival rate and 5-year disease-free survival rate of 73 patients were 52 months,41.1% and 34.2%,respectively.Conclusion XELOX and FOLFOX regimens of neoadjuvant chemotherapy combined with radical gastrectomy for advanced gastric cancer are safe and effective.
