2.Clinical characteristics and countermeasures of osteoporotic vertebral fractures treated with percutaneous vertebroplasty

Objective To explore clinical the characteristics and countermeasures of osteoporotic vertebral fractures treated with percutaneous vertebroplasty. Methods From March 2005 to March 2008,64 patients (totally 86 vertebra) with osteoporotic vertebral fractures undergoing percutaneous vertebroplasty were retrospectively analyzed. The level of fracture was between T5 to L5. The symptomatic fractured vertebrae were determined with combination of regional pain, X-rays and MR images. The postoperative effect were evaluated by the visual analog score (VAS) and Oswestry disability index (ODI). Results All the patients were followed up 12 - 23 ( 13.40 ± 1.03 ) months. The VAS decreased from (8.3 ± 1.2) scores to bdfore surgery (2.3 ± 1.4) scores after surgery and to (2.5 ± 1.1 ) scores at the last follow-up time(P < 0.01 ). The ODI decreased from (62.0 ± 2.3 ) scores before surgery to (23.1 ± 2.5 ) scores after surgery and to ( 26.3 ±2.4) scores at the last follow-up time (P <0.01). The back pain of all patients were relieved to different degrees postoperatively. No severe complications including pulmonary embolism occurred. Conclusions The key point is to find the symptomatic fractured vertebrae not only based on preoperative X-rays but also the MR images, especially fat-compressing MR images. Only when the fractured vertebrae demonstrate low intensity on T1WI MR image and high intensity on T2WI or fat-compressing MR image can be considered new fractures. The injection should reach the new fractured part and if necessary the bilateral approaches should be considered and the injection can be separated for several steps in multi-segments fracture. PMMA should be injected after the bone cements are solid enough so as not to develop complications such as pulmonary embolism.
3.Effect of Herba Cichorii Extracts in Reducing Serum Uric Acid and Blood Lipid

s:Objective To study the effect of Cichorii Extract(CE)N2 on quail with hyperuricemia and hypertriglyceridemia.Methods Quail models were established by feeding of yeast.Effect of CE N2 on biochemical parameters such as serum uric acid,triglyceride,nitric oxide(NO),xanthine oxidase(XOD),etc.were observed dynamically.Results As compared with the control group,CE N2 in three different dosages decreased serum uric acid,triglyceride and NO;CE N2 in high and low dosage could also lower the serum activity of XOD.Conclusion CE N2 can lower serum uric acid and triglycercide levels in quails with hyperuricemia and hypertriglyceridemia.
4.High Performance Liquid Chromatograph Fingerprint of Liqi Xiaoying Tablets

Objective To establish high performance liquid chromatograph ( HPLC ) fingerprint of liqi xiaoying tablets,and to provide reference for quality control of the herbal medication. Methods The chromatography conditions consisted of Aichrom Bond-AQ C18(250 mmí4. 6 mm,5 μm) column with mobile phase of acetonitrile-0. 1% phosphoric acid ( gradient elution) , column temperature of 30 ℃, flow rate of 1 mL · min-1 , injection volume of 20 μL, and UV detection wavelength of 226 nm. Results HPLC fingerprint was established with 13 common peaks,4 of which were identified. The similarity of the HPLC fingerprints of liqi xiaoying tablets from 10 batches was greater than 0. 95. Conclusion The method is accurate, reliable, and can reflect complete information for the quality of liqi xiaoying tablets.

Objective Systematic evaluation of cerebrolysin in the treatment of neonatal hypoxic -ischemic encephalopathy.Methods Computer search Cochrane Library, EMBase, PubMed, Chinese Journal Full-text Database (CNKI), Chinese Biomedical Literature Database (CBM), Wanfang Database ( each database retrieval time from its creation to May 2016 ) About cerebrolysin Randomized controlled clinical trial of neonatal hypoxic -ischemic encephalopathy.According to the inclusion and exclusion criteria of the literature, the methodological quality of the included research was evaluated and the data were extracted.Metadata was analyzed by RevMan 5.3 software.Results A total of 41 RCT patients were enrolled, with a total of 3695 patients, the experimental group was 1932 cases, the control group was 1763 cases.Meta analysis showed, the efficacy of cerebrolysin in the treatment of neonatal hypoxic-ischemic encephalopathy was significantly higher than that in the control group [OR=3.85, 95% CI (3.15,4.70), P<0.000001], for the impact of clinical symptoms, the number of primitive reflexes of neonatal hypoxic-ischemic encephalopathy in the treatment group was significantly higher than that in the control group [MD=-1.22, 95% CI ( -2.06,-0.38), P=0.004], muscle tension recovery days[MD=-1.69, 95% CI ( -1.80,-1.58), P<0.00001], conscious state recovery days[MD=-1.32, 95% CI ( -2.25,-0.40), P=0.005], seizure recovery days [MD=-2.36, 95% CI ( -3.70, -1.03), P =0.0005], and other indicators were shorter than the control group, the difference between the experimental group and the control group was statistically significant.Conclusion Cerebrolysin havean effective in treating HIE.
6.Proposal on the cultivation of hearing aids fitting practitioner in China

7.The blood and sputum levels of leukotriene in children with respiratory syncytial viral bronchiolitis

Objective To investigate the effects of leukotriene in children with respiratory syncytial (RSV)viral bronchiolitis.Methods The blood and sputum levels of Ieukotriene were detected in 33 cases diagnosed RSV bronchiolitis and 12 cases which were diagnosed pneumonia without RSV infection.Thirty-three cases of bronchiolitis were devided into mild-moderate group(n =22)and severe group(n =11)according to the lowell score.Results The blood and sputum levels of leukotriene in mild-moderate group,severe group,and pneumonia group were(76.96 ± 28.19)pg/ml,(103.53 ± 16.85)pg/ml,(18.14.± 7.49)pg/ml;(31.83 ± 19.14)pg/ml,(67.11 ± 15.11)pg/ml,(6.81 ± 2.90)pg/ml in acute period,and(36.04 ±16.38)pg/ml,(52.27 ± 17.03)pg/m l,(18.14 ± 7.49)pg/ml of serum in recovery period.There were significant differences among three groups(F =48.09,P < 0.001 ; F =15.50,P < 0.001 ; F =44.43,P <0.001).After treatment,the blood levels of leukotriene were significantly decreased,but were still higher than that of pneumonia group(P < 0.05).Conclusion The blood and sputum levels of leukotriene increase in children with RSV bronchiolitis,which is related with the severity of bronchiolitis.
8.Clinicopathologic features, diagnosis and treatment of pancreatic mucinons noncystic adenocarcinoma

Objective To explore the clinicopathological features, diagnosis and therapy for pancreatic mutinous noncystic adenocareinoma(MNAC). Methods Eleven MNAC cases treated in our hospital were studied retrospectively. The clinical and pathologic data were reviewed. Results Of the 11 patients, 6 were male and 5 were female. The age ranged from 47 to 76 years with average of 60. 5 years.The main complains included upper abdominal discomfort in 8 cases, abdominal mass in 1 case and jaundice in 2 cases. CA199 serum test was positive in all patients and 5-fold high above normal value in 6 cases. The preoperative radiography of MNAC was usually not specific. Tumors located in the head in 5 cases and in the body and tail of the pancreas in 6 cases. The diameter of the tumor was 4 - 11 cm with average of 6. 5 cm.All cases underwent operation, with radical resection in 2 cases. Liver or abdominal metastasis was found in 5 cases. Postoperative immunohistochemistry showed that MUC-1 was negative in all patients. MUC-2,PCNA and Ki-67 were positive in all patients. All 9 cases that were followed-up died within 5 -22 months,with an average survival time of 10 months. Conclusions The diagnosis of MNAC depends on pathology. Its prognosis is poor.
9.Influence of head anteflexion on airway sealing pressure during intermittent positive pressure ventilation with ProSeal laryngeal mask airway with an esophageal vent

Objective To evaluate the influence of head anteflexion on airway sealing pressure during intermittent positive pressure ventilation(IPPV) with ProSeal laryngeal mask airway (PLMA) with an esophageal vent.Methods Fifty ASA Ⅰ or Ⅱ patients (20 males and 30 females), aged 18-51 ye are, weighing 50-70 kg and scheduled for elective plastic surgery under general anesthesia, were enrolled in this study. Anesthesia was induced with fentanyl 2 μg/kg, propofol 2 μg/kg and vecuromium 0.1 mg/kg. PLMA with an esophageal vent was inserted at 2 min after intravenous vecuronium injection.The airway sealing pressure, the anatomic position of the cuff and the efficacy of positive pressure ventilation were checked in the neutral and anteflexed head positions with the cuff deflated and inflated to an intracuff pressure of 60 cm H2 O, respectively.Results The lungs were better ventilated in the head anteflexion position than in the head neutral position whether the cuff was deflated or inflated. There was no significant difference in the volume of air required to achieve an intracuff pressure of 60 cm H2O between the two head positions ( P> 0.05). The airway seating pressure increased from (27 ± 6) cm H2O in the head neutral position to (33 ± 6) cm H2O in the head anteflexion position, with no significant difference between them ( P> 0.05). The expired tidal volume and the peak inspiratory pressure during IPPV were (496 ± 81 ) ml and (14.3 ± 1.9) cm H2O respectively in the head neutral position and (496 ± 81 ) ml and ( 14.5 ± 2.1 )cm H2O respectively in the head anteflexion position.Conclusion Head anteflexion can significantly improve airway sealing but does not affect the anatomic position of the cuff.Appropriate head anteflexion is a simple and effective way to improve IPPV when the airway sealing pressure is inadequate in the head neutral position.
10.Analysis of clinical characteristics of sigmoid sinus diverticulum

OBJECTIVE To analyze the clinical features of 8 patients with sigmoid sinus diverticulum and to improve the level of diagnosis and treatment. METHODS The tinnitus patients were asked in detail about the history, and performed a series of routine hearing tests (pure tone audiometry, acoustic impedance audiometry, otoacoustic emission inspection and brainstem auditory evoked potential (BAEP) etc.) and temporal bone HRCT scan. Eight cases were diagnosed with sigmoid sinus diverticulum, in which six cases undertook a surgery to reconstruct the sinus wall while the other 2 cases chose conservative treatment. RESULTS The diagnosis of 6 cases was proved to be correct by surgery, and the pulsatile tinnitus disappeared after reconstructing the sinus wall. No one recurred after following up from 2 to 12 months. Two cases of conservative treatment had no obvious improvement in symptoms. CONCLUSION Clinical manifestation, hearing examination, temporal bone CT/HRCT and Cranial CTA/CTV contributed to the diagnosis of sigmoid sinus diverticulum. Sinus wall reconstruction is a better approach.
