BACKGROUND:The distal forearm buckle fracture in children has no secondary displacement risk, and the literatures have reported that the fracture can be treated with external fixators of plaster cast, palm side plate, splint, brace and bandage, and obtain good effect. But different external fixators have different effects on the functional recovery and viability.
OBJECTIVE:To assess the clinical efficacy and safety of splint versus plaster cast for the treatment of distal forearm buckle fracture in Children.
METHODS: The Medline database, Embase database, Cochrane Library, CNKI database and CBM database were searched from the database establishment to April 2011 with the computer;the relative conference papers were hand searched;the Important Clinical Trial Register was searched online without limitation of language. Randomized control ed trials and quasi-randomized control trials were selected for quality evaluation, and the Meta-analysis was performed with Revman 5.1. RESULTS AND CONCLUSION:Two published trials including a total of 314 patients were included, and one randomized control ed trial was evaluated as B scale, another one quasi-randomized control trial was evaluated as C scale. The results demonstrate that no fracture, un-union or re-fracture occurred after treated with splint and plaster case external fixator, and there were no significant differences in pain scores as wel as the daily activities of writing, drawing, self-eating and washing;while the early bathing ability of the patients in the splint group was earlier than that in the plaster cast group, and had no significant difference in the advanced bathing ability. The regular exercise participation in the splint group was better than that in the plaster cast group. There were no significant differences in the incidence of adverse events between two groups. The results indicate that the pain score of the patients with distal forearm buckle fracture in children do not improved after treated with splint and plaster cast, but splint fixation is better than plaster cast fixation in maintaining the batching and regular exercise participation ability with good safety. But the wel-designed and implement large sample and multi-center randomized control ed trials are needed for validation.

Objective To investigate the application value of 6 kinds of tumor markers serum neuron-specific enolase(NSE),cy-tokeratin 19 fragment(Cyfra21-1),carbohydrate antigen 19-9(CA19-9),carbohydrate antigen 12-5(CA12-5),squamous cell carcino-ma antigen(SCC-Ag),carcino embryonic antigen(CEA)in the diagnosis and differential diagnosis of lung cancer.Methods Serum NSE,Cyfra21-1,CA19-9,CA12-5,SCC-Ag and CEA levels were detected in 140 cases of lung cancer,57 cases of benign lung disea-ses and 45 individuals with healthy physical examination as control group by electrochemiluminescence immunoassay.The values of these 6 kinds of tumor marker in the diagnosis and the differential diagnosis of lung cancer were observed.Results The levels of CA12-5,CA19-9 and CEA in the lung cancer group were significantly higher than those in the benign lung diseases group and the healthy physical examination group(P <0.05 ),the sensitivity and accuracy in the combined detection of the six kinds of tumor marker in diagnosing lung cancer were 95.2% and 89.7% respectively,which was superior to those in single index detection.Con-clusion The tumor makers need to be jointly detected,which can increase the sensitivity and accuracy for diagnosing lung cancer and conduces to the differential diagnosis of early lung cancer.

Macrophages are a diverse phenotype that has important functions in inherent and adaptive immunity. Macrophage ac-tivation has two distinct states of polarization:the classically activated (M1) and the alternatively activated (M2) macrophage pheno-types in different micro-environments. M2 macrophages exert an immuno-regulatory activity that is associated with the induction of Th2 responses. The macrophages can infiltrate in the tumor micro-environment, and is also known as tumor-associated macrophages (TAMs). TAMs show the M2 macrophage phenotypes that have been associated with tumor neo-vascularization, infiltration, and metas-tasis. This review aims to investigate the importance of the immuno-regulatory activity of TAM in the carcinogenesis of gastric cancers.

OBJECTIVE To compare the effectiveness of two laboratory detections methods of Epstein-Barr virns (EBV). METHODS Immuno-fluorescence technique was applied to detect the serum EBV-specific antibody and EBV-DNA was identified by fluorescence quantitative polymerase chain reaction (FQ-PCR) in 124 children infected with EBV. RESULTS The positive rates of VCA-IgM,VCA-IgG,EA-IgG,EBNA-1-IgG and EB-DNA in various samples of 124 cases were 0,89.5%,23.4%,49.2% and 30.3%,respectively,with 21 cases (16.9%) showing simultaneously positive in EB-DNA and EBNA-1-IgG. CONCLUSIONS The two laboratory detections of EBV share their advantages and disadvantages. FQ-PCR is a method for time-saving,accurate and sensitive detection of EBV-DNA,showing clinical significance in the diagnosis of EBV-associated diseases. It's worthy of clinic application.

Objective To investigate the treatment effect and nursing of intravitreous injection of Avastin combined with laser photocoagulation in macular edema caused by vein occlusion.Methods 38 patients(38 macular edema eyes)who had macular edema caused by retinal vein occlusion and accepted intravitreous injection of Avastin combined with laser photocoagulation were selected.Relevant nursing was given before and after the clinical treatment.The visual acuity and retinal thickness were observed and recorded.Results Visual acuity was improved in 36 eyes(94.7％),no change occurred in 2 eyes(5.3％).The thickness of macular was statistically significant compared with before treatment and 2,4,6 weeks after treatment.Conclusions Intravitreous injection of avastin combined with laser photocoagulation is effective on macular edema caused by vein occlusion.Successful treatment is guaranteed by psychological nursing,treatment guidance and health guidance.

Objective To investigate the D-in patients with severe trauma in two dimers (D-D),fibrinogen (Fbg) and platelet parameters (PLT,MPV,PDW) level induced by postoperative deep vein thrombosis (DVT) in the formation of value in clinical diagnosis.Methods Eighty cases of healthy people as control group,were compared before and after operation and level and 321 severe traumatic patients were measured before surgery,after 1 days and 3 days of D-D,Fbg and platelet parameters,trauma were divided into fracture,traumatic brain injury,multiple injuries in three groups.Results Different trauma patients with D-D,Fbg concentration and platelet index compared with the control group increased obviously significant (P ＜ 0.05),especially the brain trauma and multiple injuries in two group increased significantly (P ＜0.01),the positive rate of D-D was 22.8％,35.2％,40.9％.Postoperative 1D in addition to brain injury group,trauma group D-D levels compared with those before operation were significantly increased with significant (P ＜ 0.05),other indicators compared with preoperative changes little,the positive rate of D-D increased slightly,but without statistical significance.Levels of Fbg 3D after the trauma group decreased significantly,D-D increased significantly,decreased the number of PLT,MPV,PDW value increases,the value of preoperative has the remarkable significance (P ＜ 0.01 or P ＜ 0.05).The positive rate of D-D was increased up to 41％,59.2％,62.3％,compared with those before operation by chi-square test was significant (P ＜0.01).Conclusion The patients with severe trauma patients after two D-dimer concentration should be alert to the occurrence of DVT,contribute to the clinically as soon as possible to take measures to avoid,there is an important clinical significance in the treatment and prognosis of trauma patients.

Objective To develop the adolescent online game experience scale(AOGES) and tests its reliability and validity,so as to explore the component construct of the adolescents' online game experience.Methods Interviews and open questionnaire investigation as well as the existing scales were used to form the earlier version of the measurement,and the formation of test version were based on the expert group discussion,A total of 785 middle school students completed the AOGES,factor analysis method were used to form the final questionnaire.60 students participated in the retest 3 weeks later.The Online Game Addiction Scale and APIUS were used as criterions.Results The exploratory factor analysis indicated that the AOGES was composed of 40 items and six-factors; The range of factor loading was between 0.40-0.89,and explained 60.42％ of the total variance.The cortfirmatory factor analyses indicated that the six-factor model had good fitting indices (x2/df =2.83,GFI =0.93,TLI =0.92,IFI =0.95,CFI=0.95,RMSEA=0.069).The scores of the AOGES were positively correlated with the scores of the Online Game Addiction Scale and APIUS (r=0.12-0.60,P＜0.05).The Cronbach oα of the AOGES was 0.96 and six subscales were between 0.76-0.95.The test-retest reliabilities of the AOGES was 0.81 and six subscales were between 0.51-0.86.Conclusion It suggests that the Adolescent Online Game Experience Scale may be a sixdimension model which has good reliability and validity.The questionnaire may be suitable for measuring the adolescents' online game experience in China.

【Objective】 To observe the therapeutic effect of focus-locating scalp circum-needling for ischemic stroke (IS) at acute stage. 【Methods】 Sixty-one cases of IS were randomized to two groups: group A (n=31) was treated with acupuncture on homolateral scalp region projected by CT-located focus; group B (n = 30) was treated with acupuncture on conventional scalp regions which were in charge of motor and sensory function. The cases were treated with acupuncture once a day for 30 days. The changes of symptoms and signs and the plasma contents of norepinephrine (NE) and epinephrine (E) were observed before and after treatment. 【Results】 Twenty cases were cured, 10 markedly effective, 1 effective and 0 ineffective in group A; 11 cases were cured, 15 markedly effective, 4 effective and 0 ineffective in group B. Objective To analyze the value of resting state BOLD-fMRI in the early diagnosis of brain injury after nasopharyngeal carcinoma radiotherapy.Methods Four groups of nasopharyngeal carcinoma patients,including before radiotherapy group (G0),0-3 months after radiotherapy group (G1), 3-6 months after radiotherapy group (G2) and 6-9 months after radiotherapy group (G3),were underwent a resting state BOLD-fMRI scan, and Matlab software DPARSF tool was used to analyze and postprocess the data based on G1-G0,G2-G0,G3-G0,G2-G1,G3-G2 and G3-G1.Results Compared with the G0 group,the activity of bilateral hippocampus and temporal lobes was decreased in G1 and G2 groups,and the decreased degree in G2 group was smaller than that in G1 group.Compared with G1 group,brain metabolic activity increased in G2 and G3 group,but did not return to the initial level before radiotherapy.Conclusion Abnormal changes of resting state BOLD-fMRI have an important clinical significance in early diagnosis of radiation-induced brain injury after nasopharyngeal carcinoma radiotherapy.

Objective To analyze the diagnostic value of diffusion tensor imaging (DTI) for early radiation-induced brain injury.Methods We searched CNKI and PubMed and included articles according to the PRISMA statement.FA value, λ||, and λ丄 were pooled, and Forest plot generation, cumulative analysis, sensitivity analysis, and regression analysis were performed.We also used the funnel plot to analyze publication bias.Results A total 11 studies were included for meta-analysis.The results of FA value heterogeneity test were as follows:P=0.000, I2=93.6% in China;P=0.004, I2=82.2% in other countries and regions;P=0.000, I2=91.5% in all regions.The regression analysis of time-dose dependence and study design showed significant and non-significant associations, respectively, with the heterogeneity of these studies (adjusted P=0.026, P=0.005;adjusted P=0.859, P=0.908).The results of λ||and λ丄 heterogeneity tests were as follows:P=0.908, I2=0.0%;P=0.100, I2=56.6%.There was a sign of publication bias by Egger's test (coefficient:-6.26, 95%CI:-9.31 to-3.20, P=0.001) and Begg's test (P=0.004).Conclusions DTI-derived metrics are sensitive and reliable in the diagnosis of early radiation-induced brain injury.
