1.Characteristics and clinical significance of diffusing capacity in the patients with lung cancer.

BACKGROUNDTo explore the characteristics and clinical significance of diffusing capacity in the patients with lung cancer.
METHODSThe pulmonary diffusing capacity for carbon monoxide (DLCO) was measured with the rebreathing method in 138 patients with primary lung cancer, and 86 were performed pulmonary resection.
RESULTSThere was no significant difference in DLCOrb and DLCOrbc between the groups with different histological type and degree of ventilation impairment and general type (P > 0.05). DLCOrb/VA mildly decreased in the patients whose lung ventilation function was normal. DLCOc/VA decreased in the patients with restrictive ventilation dysfunction (P < 0.01), and DLCOrb/VA decreased in the patients with obstructive and mixed ventilation dysfuncion (P < 0.05). DLCOc/VA decreased in the patients with light and obvious lung dysfunction (P < 0.05). DLCOc/VA in the patients with central lung cancer was lower than that in the peripheral ones (P < 0.01). DLCOc/VA and D LCOrb in the group with postoperative respiratory failure were lower than that in the group without respiratory failure (P < 0.05). When DLCOc/VA of less than 80% and DLCOrb/VA of less than 70% were used to predict the postoperative respiratory failure, the correct ratios of dignosis and the diagnostic indexes were high.
CONCLUSIONSThe diffusing capacity decreases in the patients with lung cancer, and the main manifestation is the abnormal DLCOrb/VA and DLCOc/VA. The general type of lung cancer and the degree of pulmonary dysfunction may influence the diffusing function of the patients. DLCOc/VA of less than 80% and DLCOrb/VA of less than 70% should be selected for predicting postoperative respiratory failure.
2.Impacts of Zishenyutai Pills on the Ovarian Tissue Oxidative Damage and Nrf2 Protein Expression in Rats Exposed by Electromagnetic Radiation

OBJECTIVE:To study the impacts of Zishenyutai pills on the ovarian tissue oxidative damage and Nrf2 protein ex-pression in rats exposed by electromagnetic radiation (EMR). METHODS:Accordiny to their weight,30 SD rats were randomly divided into normal group,radiated group and radiated-treated group with 10 rats in each group. Normal group were not radiated, and other 2 groups were exposed by 900 MHz radiation for 4 h every day;radiated-treated group was given Zishenyutai pills sus-pension [0.25 g intragstrically(medicinal materials)/ml] after radiation;normal group and radiated group were given constant vol-ume of normal saline intragastrically for consecutive 20 d. The rats were killed when they were at the estrum. The Nrf2 proteins ex-pression of ovary were detected by immunohistochemical method,the content of ovary MDA(malondialdehyde),and activity of to-tal superoxide dismutase (T-SOD) and superoxide dismutase (GSH-Px) were detected by biochemical method. RESULTS:Com-pared with the rats in normal group,the Nrf2 protein expression was higher in the rats of the radiated group;the content of ovary MDA was higher and activity of ovary T-SOD and GSH-Px were lower significantly(P<0.05). While compared with the rats in ra-diated group,the Nrf2 protein expression didn’t obviously change in the rats of the radiated-treated group(P>0.05),the content of ovary MDA was lower and activity of ovary T-SOD and GSH-Px were higher significantly (P<0.05). CONCLUSIONS:Zish-enyutai pills can prevent and cure ovary tissue oxidative injury induced by 900 MHz radiation,but have no effect on the expression of Nrf2 protein.
3.Investigation of maxillary preprosthetic situations after oral tumor treatment.

In order to investigate the maxillary preprosthetic situation after oral tumor treatment and/or reconstructive surgery, based on the review of case history and the clinical records 47 cases were analyzed after oral tumor treatment and/or reconstruction, including residual maxillary bone, intermaxillary relationships, defection of maxilla and oral situation after radiation therapy. The results showed that the residual maxillary bone was useful for implantation in the front alveolar bone and zygoma area. The maxillary preprosthetic situation after tumor treatment and/or reconstructive surgery was difficult due to maxillary resection, intermaxillary relationships, unsuitable soft and hard tissue transfer and the irradiation. It is suggested that the maxillary preprosthetic situation after oral tumor treatment is getting worse not only due to maxillary resection and/or irradiation, but surprisingly also due to mandibular resection and/or irradiation.

4.Investigation on schistosomiasis japonica infection and preventive knowledge of immigration population in Jinshan District,Shanghai

Objective To investigate the schistosomiasis japonica infection and preventive knowledge of immigration popula-tion in Jinshan District,Shanghai so as to provide the evidence for improving the control work. Methods The immigration popula-tion from schistosomiasis endemic provinces and local people were screened by serum IHA screening for schistosome infection, and the positives were further examined by Kato-Katz technique and the miracidium hatching method. The schistosomiasis preven-tive knowledge and behaviors were investigated by questionnaires. Results A total of 579 were investigated with 348 immigration people and 231 local people,and among them,the male accounted for 72.0%and female accounted for 28.0%. In the immigration population,the main age group was 20-29(56.0%),and 56.6%of the immigration had an education level at senior middle school or higher. The most proportion of immigration was from Anhui Province(36.2%). There were 4 IHA positive persons(1.15%)in the immigration,and 1 was positive in the stool test,and the schistosome infection rate was 0.29%. There were no positives in the local people. The schistosomiasis preventive knowledge of the immigration was generally poor,and some people’s behaviors of see doctor and medication were incorrect. Conclusion There are schistosome infected persons among the immigration population from schistosomiasis endemic provinces,and their schistosomiasis prevention knowledge was poor,thus there is a schistosomiasistransmission risk.
5.Research Progress of Gd-EOB-DTPA Enhanced MRI in Evaluating Liver Function in Liver Cirrhosis

Liver function can reflect the severity of cirrhosis,and has important clinical value for disease monitoring,efficacy evaluation and surgical risk prediction.At present,the methods commonly used in clinical evaluation of liver function include blood biochemical examination,drug excretion test,comprehensive scoring system and routine imaging examination.All these methods have their limitations.Gd-EOB-DTPA is a hepatocellular specific contrast agent,which is taken up and excreted by liver cells and can directly reflect liver function.Therefore,it is expected to achieve non-invasive segental quantitative evaluation of liver function through enhanced MRI of Gd-EOB-DTPA,which is the focus of current research.In this paper,the methods of Gd-EOB-DTPA enhanced MRI for liver function assessment were reviewed,and the feasibility of Gd-EOB-DTPA in liver function assessment was discussed.
6.Observation on the characteristics and influencing factors of cardio-pulmonary function in patients with lung cancer.

BACKGROUNDTo explore the characteristics of exercise cardio-pulmonary function and its possible influencing factors in patients with lung cancer.
METHODSThe pulmonary function, ECG and exercise cardio-pulmonary function were measured in 198 patients with lung cancer and 20 healthy controls.
RESULTS1. Compared with healthy group, VO₂%P, VO₂/kg, AT, VO₂/HR%, VE and VT/VC significantly decreased in lung cancer patients with normal resting pulmonary ventilation, however, BR remarkably increased (P < 0.05 or P < 0.01). 2. In patients with normal resting pulmonary ventilation, there was no significant difference of exercise cardio-pulmonary function between the central and peripheral lung cancer groups. 3. The exercise cardio-pulmonary function was closely related to the TNM stages (P < 0.05 or P < 0.01). 4. W%, VO₂%P , AT and VO₂/HR% in patients with great vessel invasion were remarkably lower than those without great vessel invasion (P < 0.05 or P < 0.01).
CONCLUSIONSThe results suggest that exercise ventilation is impaired in lung cancer patients with normal resting ventilation. And the decrease of exercise cardio-pulmonary function may be related to TNM stage and to great vessel involvement.
7.Evaluation of cardiopulmonary exercise test on predicting post-operative respiratory failure in patients with lung cancer.

BACKGROUNDTo evaluate the clinical significance of predicting post-operative respiratory failure in patients with lung cancer using cardiopulmonary exercise test (CPET).
METHODSBefore operation, 260 patients with lung cancer underwent CPET with incremental protocol. W%, VO₂%P, VO₂/kg, AT, MET, O₂ pulse, VTe, BF and VE were measured in the end of load exercise.
RESULTS(1) In patients after pneumonectomy, the values of the above indexes of CPET in the respiratory failure group were significantly lower than those in the non-respiratory failure group ( P < 0.05 or P < 0.01) except VTe. In patients after lobectomy, the values of 9 indexes of CPET in the respiratory failure group were similar to those in the non-respiratory failure group ( P > 0.05). However, when the patients after lobectomy were further divided into groups of upper and lower lobectomy, W% in the respiratory failure group was remarkably lower than that in the non-respiratory failure group after lower lobectomy ( P < 0.05). (2) Chi-Square test showed that abnormality of CPET indexes in different degrees was related to the morbidity of respiratory failure after pneumonectomy. Logistic regression showed that O₂ pulse < 80% and BF < 30/min correlated with the morbidity of post-operative respiratory failure. (3) For predicting post-operative respiratory failure, the sensitivity and specificity of VO₂%P < 60%, BF < 30/min, VE < 35 L/min were all more than 60% and their negative predictive values were all more than 90%.
CONCLUSIONSCPET is suitable to predict post-pneumonectomy respiratory failure. As a comprehensive index indicating cardiopulmonary function during exercise, VO₂%P < 60% should be selected to predict respiratory failure and evaluate indication of lung resection for patients with lung cancer.
8.Expression of SIRT1 in human lung adenocarcinoma cells and its relation to the susceptibility of NDP

Objective To investigate the expression of Situin 1 ( SIRT1) in 5 strains of human lung adenocarcinoma cell lines, inclu-ding HCC827, H1650, H1975, A549 and H1299, and its relation to the susceptibility of nedaplatin ( NDP ) . Methods The SIRT1 mRNA and protein levels in 5 strains of human lung adenocarcinoma cells were detected by real-time quantitative PCR and Western blot, respectively. The viability of cells treated with NDP was detected by the CCK-8 method and the half growth inhibition concentra-tion ( IC50 ) was calculated. After the expressions of SIRT1 in A549, H1299, H1650 and H1975 cells were down-regulated by the siR-NA interference, the effects of NDP on the viability and apoptosis of these cells were determined by the CCK-8 method and flow cytom-etry, respectively.Results The expression levels of SIRT1 mRNA (4.53 ± 0.74, 3.11 ± 0.64, 15.76 ± 2.28 and 18.09 ± 1.17) and protein (0.23 ± 0.03, 0.21 ± 0.02, 0.52 ± 0.11 and 0.56 ± 0.08) in H1650, H1975, A549 and H1299 cells were significantly higher than that in HCC827 cells (1.00 for SIRT1 mRNA and 0.11 ± 0.02 for SIRT1 protein, F=122.10 and 26.50, respectively, P<0.01). The susceptibility of A549 and H1299 cells to NDP [IC50=(7.38 ± 1.59) and (8.14 ± 1.43) μmol/L, respectively] was significantly higher than that of HCC827, H1650 and H1975 cells [IC50=(26.16±4.35),(22.29±3.26) and (24.41 ± 2.58), respectively, F=30.86, P<0.01].The survivals of A549 and H1299 cells transfected by siSIRT1 and treated with NDP were significantly higher than that in the NC group ( F=235.10 and 39.20, respectively,P<0.01) , and the apoptotic rates were the reverse ( t=7.29 and 6.68, re-spectively, P<0.05) . However, the survivals of H1650 and H1975 cells transfected by siSIRT1 and treated with NDP were significantly lower than that in the NC group ( F=185.40 and 60.09, respectively,P<0.01) , and the apoptotic rates were the reverse ( t=6.15 and 31.36, respectively,P<0.01).Conclusion The expression of SIRT1 in A549 and H1299 cells with high expression of SIRT1 increases their susceptibility to NDP , while that in H1650 and H1975 cells with moderate expression of SIRT1 decreases their susceptibility to NDP, indicating that SIRT1 may play dual roles in the resistance of human lung adenocarcinoma cells to platinum.
9.Diagnostic strategy of primary aldosteronism based on CT scan and serum potassium level

One hundred and forty-five patients with primary aldosteronism (PA) admitted from 2006 to 2013 were enrolled in the study. The diagnosis of PA was confirmed by upright furosemide test and all patients met the following criteria: ① round-or oval-shaped lesion of low density with diameter>1 cm in one adrenal gland shown in contrast CT scan; ② no lesion or abnormality in contralateral adrenal gland; ③serum potassium level<3.5 mmol/L. Of 145 patients, 106 underwent total adrenalectomy, 36 partial adrenalectomy and 3 tumor enucleation. Serum potassium was (2.75±0.55) mmol/L before and (4.03±0.46) after surgery. Potassium was normalized after treatment in 141 cases (97.2%) with correction or improvement in hypertension; 4 patients (2.8%) remained hypokalemic and received spironolactone. Patients with normalized potassium were followed up for a medium period of 74 months (22—103 months), of whom 32 (22.7%) dropped off; the remaining 109 (77.3%) patients did not have hypokalemia. Multivariate linear correlation analysis showed that serum potassium level was negatively correlated with tumor diameter (r=?0.273,95% CI:?0.086—?0.564, P=0.026) and basal serum aldosterone level (r=?0.261,95% CI:?0.047— ?0.514, P=0.036). In PA patients with unilateral adrenal macroadenoma and hypokalemia, satisfactory surgical resolution can be achieved without adrenal venous sampling in majority of patients.
10.Efficacy and safety of CalliSpheres(R) drug-eluting beads transarterial chemoembolization in treatment of liver cancer patients

Objective To evaluate the efficacy and safety of drug-eluting beads transarterial chemoembolization (DEB-TACE) in treatment of patients with liver cancer.Methods Totally 20 patients with liver cancer undergoing DEB-TACE treatment were enrolled.Clinical efficacy included complete response (CR),partial response (PR),stable disease (SD) and progressive disease (PD).The overall response rate (ORR) was defined as proportion of patients achieved CR and PR.Results After treatment of DEB-TACE,the ORR,CR,PR,SD,and PD of patients were 80.00% (16/20),40.00% (8/20),40.00% (8/20),15.00% (3/20),and 5.00% (1/20),respectively;while for single tumor nodule,the ORR,CR,PR,SD,and PD were 81.58% (31/32),50.00 (19/38)%,31.58% (12/38),15.79% (6/38),and 2.63% (1/38),respectively.After the treatment of DEB-TACE,the relapse free survival rate in half year was 85.00%,and the overall survival rate was 95.00%.The adverse events of patients were mild pain,fever,nausea and vomiting.Conclusion CalliSpheres(R) DEB-TACE is an effective and well tolerated treatment in liver cancer patients.
