1.The Portal Hemodynamic Influence of Patients with Portal Hypertension Treated by PSE

Objective To evaluate the changes of hypersplenism patients with post-hepatitis cirrhosis and hypertension who treated by partal splenic embolization.Methods 24 patients with hypertension and hypersplenism were treated by PSE.In the operations we measured the free portal pressure(FPP).Before and after the operation,all patients diameter and velocity of portal veins and splenic veins were measured by Doppler ultrasonography.Results Compared with the condition before operation,the FPP decreased 3.2?0.4 mmHg,the quantity of blood flow of portal veins from 1104 ml/min dropped to 793 ml/min,and that of splenic veins from 897 ml/min dropped to 355 ml/min.Conclusion PSE can improve the state of hyperdynamic circulation of portal veins system in a little time.
2.Clinical significance of N-terminal pro-brain natriuretic peptide and D-dimer in patients with chronic obstructive pulmonary disease

Objective To explore the clinical significance of serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and plasma D-dimer (D-D) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and stable chronic obstructive pulmonary disease (COPD).Methods The clinical data of 76 patients with AECOPD (AECOPD group) and 68 patients with stable COPD (stable COPD group) were analyzed retrospectively.Serum NT-proBNP,plasma D-D levels and arterial blood gas analysis were measured and compared.Results The levels of serum NT-proBNP and plasma D-D in AECOPD group were (918 ± 271) ng/L and (0.269 ±0.048) mg/L,in stable COPD group were (264 ± 32) ng/L and (0.223 ± 0.042) mg/L.The levels of serum NT-proBNP and plasma D-D in AECOPD group were significantly higher than those in stable COPD group (P < 0.01).The levels of serum NT-proBNP and plasma D-D were negatively correlated with arterial blood oxygen partial pressure (r =-0.181,P =0.030;r =-0.166,P =0.047),and were positively correlated with arterial blood carbon dioxide partial pressure (r =0.763,P < 0.01;r =0.424,P < 0.01).Conclusion Combined detection of serum NT-proBNP and plasma D-D has important clinical significance in evaluating severity of COPD,and may indicate AECOPD.
4.Development and application of a perioral force measurement system for infants with cleft lip and palate

Objective:To develop a perioral force measurement system for the infants with cleft lip and palate.Methods:The peri-oral force measurement system of infant with cleft lip and palate is composed of hardware and software.The sensor is metal cantilever. The measurement ranges are 0 -20 and 0 -1 00 g/cm2 ,and the precision is 0.1 g/cm2 .The system was used in 4 cases of infants with unilateral cleft lip and palate before and after cheiloplasty.The results were analyzed by SPSS 1 9.0 software.Results:Before cheilo-plasty the perioral force of labial frenum area was (1 .79 ±0.94)g/cm2 ,that of angulus oris area of normal side and cleft side was (5. 41 ±1 .01 )g/cm2 and (3.1 2 ±1 .55)g/cm2 (P <0.05);after cheiloplasty:the perioral force of labial frenum area was (1 2.73 ±3. 51 )g/cm2 ,that of angulus oris area of normal side and cleft side was (7.64 ±1 .64)g/cm2 and (7.27 ±1 .89)g/cm2 .Conclusion:The perioral force measurement system can be used to measure the perioral force of the infants with cleft lip and palate.
5.Clinical analysis of minimally invasive surgery and drug treatment for hypertensive cerebral hemorrhage

Objective To compare the clinical efficacy of minimally invasive removal of intracranial hematomas and medical con-servative treatment for hypertensive cerebral hemorrhage .Methods A total of 75 patients with hypertensive cerebral hemorrhage were collected and randomly divided into two groups according to the treatments :38 patients treated with conservative medicine and 37 patients treated with minimally invasive removal of intracranial hematomas based on standard drug therapy .Neurological impair-ment and recovery of self-care ability of patients in both groups before and after three weeks and six weeks of treatment were scored by National Institute of Health Stroke Scale (NIHSS) .Changes of diffusion tensor imaging before treatment and after two weeks of treatment were evaluated by fractional anisotropy value (FA) .Results There was no significant difference between neurological im-pairments of the patients in two groups before treatment .But the recovery degree of functional impairment and muscle nerve in min-imally traumatic treatment group was more significant than those in the conservative medical treatment group after treatments . There was significant difference between two groups (P< 0 .05) .Conclusion Minimally traumatic of hematomas is an effective method of treating hypertensive intracerebral hemorrhage .
6.A preliminary study on the relevance of facial morphologic angles of Xi'an youths

Objective:To explore the correlativity between facial morphologic angles of Xi'an youths.Methods:60 Xi'an residents aged 18 -40 years(30 males and 30 females)with normal facial profile,without operation and trauma experience,received 3dMD scaning.12 morphologic angles were measured.Data were statistically analysed by SPSS 19.0 software.The correlative relationship and formulas of facial angles were studied.Results:Nasofrontal angle of the males was larger than that of the females(P <0.05), the other angles showed no difference between sexes(P >0.05).Correlativity between each pair of 5 angles was concluded by a logic diagram(R≥0.7),the correlativity of 4 pairs was positive and 3 negative.Conclusion:The facial morphologic angles of Xi'an youths are correlated and coordinated.

OBJECTIVE:
To investigate the correlation between tongue body MRI measurements and the lingual region obstruction in patients with obstructive sleep apnea hypopnea syndrome (obstructive sleep apnea hypopnea syndrome, OSAHS), and the relationship of two diagnosis methods in positioning obstructive level, in order to improve the preoperative diagnostic accuracy of obstructive level in OSAHS patients .
METHOD:
Fifty-nine patients with OSAHS definited by the AG200 sleep apnea monitoring and obstruction plane positioning system was included. They all underwent tongue MRI scan, after three-dimensional reconstruction, the tongue body length to diameter, relative thickness, basal diameter, wide around central about wide diameter and tongue body volume were measured. The lower blocking (mainly composed of lingual region) constituent ratio P established by AG200 was correlated with tongue parameters measured by MRI. At the same time, To analysis of differences of P in patients with different degree of OSAHS.
RESULT:
Among these patients with OSAHS in different degree, the coefficient correlation between tongue parameters(tongue length, relative thickness, basal diameter, wide middle diameter, tongue body volume) and lower blocking constitute ratio P werer LP = 0.051,rHP = 0.069, r1 = 0.215, r2 = 0.147, rVP = 0.259, respectively. lower obstruction of form than the differences had statistical significance. The measured tongue parameters and value P(the lower blocking constituent ratio) were negatively correlated, which demonstrated that exclusing of the larynx and hypopharynx airway obstruction, the lower airway obstruction detected by AG200 was related with tongue parameters measured by MRI, and was strongestly related with the tongue body volume.
CONCLUSION
The volume of tongue and AG had correlation in judgement of lower blocking. In OSAHS patients the lower blocking constituent ratio varies between the defferent severity groups of the disease, the combina tion of the two method can be used to predict the obstructive plane in OSAHS patients with upper airway obstruction, and provide the basis for the individual treatment of patients with OSAHS.

9.Use of Chinese patent drugs: rationalization analysis of 1780 outpatient prescriptions in our hospital

Objective To Analyze the proprietary usage of Chinese medicine prescriptions in our hospital and to improve the level of rational drug use.Methods 1780 prescriptions were randomly collected from our hospital during January to June,2011 and analyzed.Results The rational prescriptions had a 83.01% share in the whole.while the irrational prescriptions had a 21.2% share in the whole,among which 13.37% were unqualified writing,3.45% were irrational drug use.The irrationality mainly manifested as missing items in the prescription,and improper usage and dosage.Conclusion It is suggested to strengthen the awareness of physicians on standard prescription writing,strict pharmacist review,and establish prescription quality management system to improve the quality of prescription.
10.Changes of CD4(+) CD25 (+) regulatory T cells in peripheral blood in patients with hepatocellular carcinoma before and after TACE.

This study investigated the changes of CD4(+) CD25(+) regulatory T cells (Tregs) in peripheral blood of patients with hepatocellular carcinoma before and after transcatheter arterial chemoembolization (TACE). The proportion of CD4(+) CD25(+) Tregs among CD4(+) T lymphocytes in peripheral blood of 33 patients with hepatocellular carcinoma was determined by flow cytometry before, 1 week and 1 month after TACE. And 25 healthy volunteers served as control. One month after TACE, the patients were divided into two groups: 22 in group A, who were in stable condition or getting better; and 10 in group B, who were deteriorating. One patient died and was excluded. The results showed that the percentage of CD4(+)CD25(+) Tregs among CD4(+) T lymphocytes did not significantly change in the 33 patients 1 week after TACE as compared with that before TACE, however, the difference was significant (P<0.01) between the patients with hepatocellular carcinoma and the healthy subjects. The percentage of CD4(+) CD25(+) Tregs among CD4(+) T lymphocytes in group A 1 month after TACE was decreased significantly in comparison with that before and 1 week after TACE (P<0.01), whereas, that in group B was increased significantly 1 month after TACE (P<0.01). It was concluded that patients with hepatocellular carcinoma had a higher proportion of CD4(+)CD25(+) Tregs in peripheral blood. TACE did not significantly affect the level of CD4(+) CD25(+) Tregs within short time (such as 1 week). The proportion of CD4(+)CD25(+) Tregs in peripheral blood 1 month after TACE was related to the prognosis of hepatocellular carcinoma.
