1.Clinical observation on 30 cases of advanced stage non-small-cell lung cancer treated by integrated Xinjia Shashen Maidong Decoction and radiotherapy

Objective:To estimate the clinical effect of integrated Xinjia Shashen Maidong Decoction and radiotherapy on advanced non-small-cell lung cancer (NSCLC) patients.Methods:30 NSCLC patients were treated with Xinjia Shashen Maidong Decoction after radiotherapy for more than two months,then the clinical effect were evaluated by tumor-mass stable rate,symptoms,Karnofsky score and survival period.Results:Response Evaluation Criteria in Solid Tumor (RECIST) evaluation results showed that the effective rate on relieving tumor mass was 53.3%,stable rate was 86.7%.The symptom of cough was much relieved,with 56.7% effective rate and 93.3% stable rate.And effect on Karnofsky score was good,that effective rate was 56.7% and stable rate was 86.7%. Fortunately,the patients had a median survival period of 11 months.Conclusion:Xinjia Shashen Maidong Decoction enhanced the efficacy of radiotherapy and reduced its side-effects in the treatment of advanced NSCLC patients.
2.Matrine Compoand Injection Interrening the Liver Fibrosis Rat Induced by Tetrachloride.

Objective To examine the change of TGF-?1 in the bloods of rats which were induced by tetrachoride during the different times and to explain the mechanism of the change.MethodsThe experimental liver fibrosis induced by tetrachloride in rats was treated by matrine compound injection.Serum TGF-?1 and pathological changes in liver tissue were observed in control group,model group and therapy group during the time of the 4th,6th and 9th week.ResultsThere was an obvious change of TGF-?1 in the blood in different time and different groups.ConclusionMatrine compound injection has an antifibrogenetic action and TGF-?1 can reflect the change of liver fibrogenesis of different time as an index.
3.Study on the relationship between changes in Ca2+ concentration and unit discharge in the hippocampal neurons of rats with benzylpenicillin-induced epilepsy

Objective To investigate the relationship between changes in Ca2+ concentration and unit discharge in the hippocampal neurons of epileptic rats.Methods Thirty-six healthy male Wistar rats were divided into three groups. Rats of the epilepsy group received an intraperitoneal injection of benzylpenicllin sodium at 6 million unit/kg; Rats of the epilepsy pretreatment group received 20 mg/kg of flunarizine at 14 and 2 hours before injection of benzylponicllin;the control group of rats received an intraperitoneal injection of normal saline.Three groups of rats were euthanized after recording of the unit discharge,and bilateral hippocampal cells were obtained to measure the cellular concentration of Ca2+ fluorescence using the new generation Ca2+ fluorescent dye Fluo-3-AM and flow cytometry.Results (1) A total of 24 units of hippocampal discharges were recorded in the control group.The average concentration Ca2+ fluorescence of the hippocampal neurons was 37.12±5.08.(2)A total of 78 units of hippocampal discharges were recorded in the epilepsy group.The average concentration of Ca2+fluorescence of the hippocampal neurons was 52.04±6.75.(3)A total of 47 units of hippocampal discharges were recorded in the epilepsy pretreatment group. The average concentration of Ca2+ fluorescence of the hippocampal neurons was 37.79±3.09.The correlation coefficient between the average concentration of Ca2+ fluorescence of the hippocampal neurons and the number of units of hippocampal discharge ( r=0.737,P<0.01).Conclusion There is a positive correlation between changes in average concentration of Ca2+ fluorescence of the hippocampal neurons and the numbers of units of hippocampal discharge.
4.The effect of different dose of butorphanol with epidural injections on neurological function for rats

Objective To observe the effect of epidural injection with different dose of butorphanol on the rats' neurological function.Methods A PE-530 catheter was inserted into the epidural space of all the SpragueDawley rats (male, weighting 180 ~210 g) at L1-2 level.After three days, a total of 32 rats without any motor dysfunction were randomly divided into 4 groups as follows saline(NS) group (group C, n= 8 )and butorphanol injection (B) group( B1∶ n=8;B2∶ n=8;B3∶ n=8).Rats in group C were epidurally injected NS 30 μl each ,and rats in group B1, B2 and B3 were respectively epidurally injected Butorphanol 60 μg/30μl, 120 μg/30 μl,240 μg/30 μl (all diluted with NS) ,and 1 time per day for5 days.The neurological function of rats was recorded before injection (T0) and 6h after injection on day 1 ~4(T1 ~T 4) and 6h,24h and 72h after injection on day 5 (T5 ~T7) by BBB (BASSO,BEATTIE and BRESNAHAN ) Score and the inclined plane test .Results Compared with group C ,the BBB score and the inclined plane test of group B1 showed no significant difference throughout the experimental period(P> 0.05 ).There was also no significant difference at T0 ~ T3 of group B2 and group B3 compared with group C (P > 0.05 ), while at T4, the BBB score ( ( 18.50 ± 2.00 ) points, ( 16.38 ± 2.33 ) points) and the inclined plane test( (58.75 ± 5.17 )°, (59.38 ± 3.20) ° ) of the two groups were both obviously decreased when compared with group C( (21.00 ±0.00) points, (65.00 ±3.78)°, P<0.05) ,and the same significant differences appeared at T5,T6 and T7 (P < 0.05 ).Conclusion Repeated epidural injection of butorphanol 60 μg have no effect on neurological function of rats,while repeated epidural injection of butorphanol 120 μg and 240 μg could impaire the neurological function.
5.Characteristics of neutrophils infiltration in ventilation-induced lung injury.

Neutrophils play a critical role in ventilation-induced lung injury. This study was aimed to investigate the characteristics of neutrophils influx in lungs induced by high tidal volume ventilation. Anaesthetized rats were randomly divided into low tidal volume ventilation group (Vt: 7 mL/kg, LV group) or high tidal volume ventilation group (Vt: 42 mL/kg, HV group) (n=40 in each). Rats in each group were ventilated for 0, 60, 90, 120 and 240 min. The wet/dry lung weight ratio (W/D) was measured. The levels of macrophage inflammatory protein-2 (MIP-2) and tumor necrosis factor-α (TNF-α), and the activity of myeloperoxidase (MPO) were detected by enzyme-linked immunosorbent assay (ELISA). The number of neutrophils in bronchoalveolar lavage fluid (BALF) was counted after Wright's staining, and the percentage of netrophils in lung tissues calculated. Histopatholgical examination was used to observe the changes of lung tissues after different ventilations. The results showed that the W/D weight ratio was increased, and the levels of MIP-2 and TNF-α significantly enhanced in HV group at 90, 120 and 240 min. Neutrophils in BALF and the neutrophil percentage in lung tissues were also elevated at 120 and 240 min, which coincided with the enhanced activity of MPO in HV group. The lung injury was significantly related with the ventilation time and the infiltration of neutrophils in lungs in HV group. In conclusion, in ventilation-induced lung injury, neutrophil infiltration is present in a time-dependent manner and associated with the aggravated lung injury. Pulmonary structural damage may be the main reason for ventilation-induced lung injury.
6.Effect of patient-controlled epidural analgesia with opioids on serous myelin basic protein and somatosensory evoked potential of lower limbs in puerperants

ObjectiveTo investigate the effect of patient-controlled epidural analgesia(PCEA) with opioids on serous myelin basic protein(MBP) and somatosensory evoked potential(SEP) of lower limbs in puerperants.MethodsA total of 120 puerperants,after receiving cesarean section,were divided into four groups by random number table method as group B,BR,MR and R randomly,and each group included 30 cases.After surgery,each case received PCEA:group B received 0.008% butrophanol;group BR received 0.008% butrophanol + 0.2% ropivacaine;group MR received 0.004% morphine +0.2% ropivacaine and group R received 0.2% ropivacaine only.VAS score,OAA/S score,adverse effect occurrence,concentration changes of serous MBP,SEP of both lower limbs and neurological function were observed at 2h(T1 ),4h(T2),8 h(T3),12h(T4),24 h(T5) and 48h (T6) after surgery.ResultsVASscoresofgroupBR(1.64±0.38,1.86±0.62,1.93±0.67) and MR( 1.74 ±0.39,1.91±0.58,1.98 ±0.63) at T3,T4,T5 were lower than those of group B(4.6 ±0.5,4.6 ±0.3,4.7 ±0.3)and R(2.64 ±0.41,2.83 ±0.91,3.37 ±0.87) (P<0.05).There was no significance in four groups in OAA/S score at each point (P > 0.05 ).Incidence of nausea ( 6 cases),vomiting ( 2 cases) and abdominal distention ( 5cases) of group M was higher than that of other three groups(P<0.05).There was no significant difference in concentrations of serous MBP,SEP and neurological function in all four groups between preoperative time and 48h after operation(P>0.05).ConclusionLower-dose and lower- concentration opioids used for PCEA have no influence on serum MBP and SEP.
7.Improvement of instrument for preparation of medical patch material

Objective To modify the existing preparation instrument for medical patch material to realize its scale and automatic production.Methods The instrument had its shaking table improved and a cylindrical processing system added with comprehensive analysis on raw materials and kinds of process flows.A spreading and fixation mechanism for animal materials was placed in the cylindrical processing system,which had the holes for liquid inlet and outlet respectively on its top and bottom.The cylindrical processing system was fixed to the base of the shaking table.Results The instrument enhanced preparation efficiency significantly,and had the raw material utilization rate increased by 20%,product qualification rate raised by 35%,preparation cycle reduced by 33% and total cost saved by 40%.Conclusion The instrument behaves well in adaptability to multi animals,preparation process,inter-assay difference,raw material utilization rate,product qualification rate and cost reduction,which is of great significance to promote the clinical application of medical biological patch.
8.Application of Glissonean pedicle transection method for hepatic resection in hepatocellular carcinoma

Objective To investigate the clinical application of Glissonean pedicle transection method in hepatocellular carcinoma. Methods We analyzed the clinical data of 232 cases of hepatocellular carcinoma underwent hepatic resection in our department from April 2009 to April 2013. Eighty-nine cases underwent hepatic resection by Glissonean pedicle transection method, and 143 cases underwent Pringle maneuver. Results There was no significant differences in the operation time and blood loss between the two groups (P > 0.05). However, the postoperative hospital stay time and the postoperative complications in group A were better than those in group B(P<0.05). Levels of ALT and AST in group A increased significantly slowly than those in group B. The 1, 3-year disease-free survival rates in the two groups were 87.4%, 71.1% and 85.1%, 60.0%, respectively. The 1, 3-year survival rates were 93.3%, 73.5% and 93.4%, 65.0%, respectively, with no significant differences (P>0.05). Conclusion Glissonean pedicle transection method has many advantages including effectively preventing the intraoperative bleeding, maximally reserving the remaining of liver function, decreasing the rate of tumor spread and metastasis. Meanwhile, it is a safe and effective technique for hepatectomy without dissection of the hepatic artery, portal vein and bile duct in the hepatoduodenal ligament.
9.Effects of duration of surgery flash-induced visual evoked potentials in patients undergoing spinal surgery in prone position

Objective To investigate the effects of duration of surgery on flash-induced visual evoked potentials (VEP) in patients undergoing spinal surgery in prone position.Methods Eighty-two ASA Ⅰ or Ⅱ patients of both sexes aged 20-76 yr weighing 43-96 kg undergoing spinal surgery in prone position were divided into 3 groups according to the duration of surgery:group S≤2 h ( n =34) ; group M 2-4 h ( n =38) and group L≥4 h ( n =10).VEP was monitored using protektor VEP monitoring device (Xltek Co.,Canada).The latency,amplitude and recovery time of wave P100 were recorded before and 10 min after induction of anesthesia and at the end of surgery.Results Compared with group S,the amplitude of wave P1000 was significantly decreased at the end of surgery in group M,the lantency of wave P100 was significantly prolonged,while the amplitude of wave P100 was decreased at the end of surgery in group L ( P < 0.05).Compared with group M,the lantency of wave P100 was significantly prolonged,while the amplitude of wave P100 was decreased at the end of surgery in group L ( P < 0.05).Compared with groups S and M,the recovery time of wave P100 was significantly prolonged in group L ( P <0.05).There was no significant difference in the recovery time of wave P100 between groups S and M ( P > 0.05).Conclusion Duration of surgery (≥4 h) can affect flash-induced VEP,the longer the duration,the stronger the effects.
