1.Effect of lumbar dynamic fixation on intervertebral disc stress of adjacent segment

[Objective]To analyze the change of intervertebral disc Von Mises stress on the adjacent segment after "U" shape dynamic fixation for lumbar with finite element method. [Method]The three-dimensional finite element models of the lumbar dynamic fixation and rigid fixation were established by using Mimics11.11 and Abaqus6.51 softwares.Loads used in this study were axial compressive,flexion,extension,lateral bending,and rotation forces.The intervertebral disc Von Mises stress of the adjacent segment was analyzed and compared.[Result]In the same shearing load of 500 N,the intervertebral disc Von Mises stress of the adjacent segment in the dynamic fixation model was lower than rigid fixation model under axial compressive,flexion,extension,lateral bending,and rotation forces,especially at lateral of the intervertebral disc,and there were significant difference between dynamic fixation group and rigid fixation group(P
2.Drugs for prevention and treatment of immunological rejection following corneal transplantation

Corneal transplantation has been considered a primary method for treatment of keratopathy. Corneal transptantation would bring immunological rejection although cornea belongs to an immune-privileged tissue owing to no vessels and lymphatic vessels. Immunological rejection is highly likely to occur in keratopathy patients who undergo large sizes of corneal transplantation or present with newly formed vessels in the center of implant. Many patients have to replace other implants because of immunological rejection. But severer immunological rejection caused by several corneal transplantations as well as rare donor source can not satisfy patients' requirement. Therefore, much attention has been paid to studies regarding drugs for immunologic protection following surgery. There have been many clinical studies confirming that the principle drugs against immunologic rejection, such as glucocorticoid and ciclosporin A, can not acquire satisfactory curative effects and concomitantly bring many adverse reactions. For this reason, many new immunosuppressive agents should be investigated. The method of drug combination has become an important research project for keratopathy physicians.
3.Immunogenicity of osteoblasts prior to and after liquid nitrogen cryopreservation

BACKGROUND: Cryopreservation can decrease tissue and organ immunogenicity. The effects of cryopreservation on cell immunogenicity are disputed.OBJECTIVE: To investigate the effects of liquid nitrogen cryopreservation on osteoblast immunogenicity. DESIGN: Randomized,controlled ,paired-sample experiment. SETTING: This study was performed in the Laboratory Center, Qilu Hospital Affiliated to Shandong University between July 2003 and March 2004. MATERIALS: Four New Zealand rabbits of either gender were included for this study. 3H-TdR was provided by Nuclear Medicine Institute of Shandong University. METHODS: Osteoblasts were cultured from the tibial periosteum of New Zealand rabbits and then cryopreservated in the liquid nitrogen for 3 months and defrosted. Cryopreservated and thawn osteoblasts were set as cryopreserved group and freshly cultured osteoblasts were set as non-cyropreserved group. Major histocompatibility complex (MHC)-I positive rate was examined by flow cytometry assay prior to and after cryopreservation. Simultaneously, mixed lymphocyte-osteoblast cultures were established. Lymphocyte stimulation index was calculated after counting the flares using β liquid scintilloscope. MAIN OUTCOME MEASURES: MHI-I antigen positive rate and lymphocyte stimulation index prior to and after cryopreservation of osteoblasts. RESULTS: MHI-I antigen positive rate and lymphocyte stimulation index of osteoblasts was significantly higher in the non-cryopreserved group than in the cryopreserved group (P<0.01). CONCLUSION: The immunogenicity of cryopreserved osteoblasts was significantly decreased. Liquid nitrogen cryopreservation is an ideal method to decrease the immunogenicity of osteoblasts.
4.An assumption of community type 2 diabetes health intervention based on the pattern of PRECEDE-PROCEED

The assurance and continuous improvement of the effect of health intervention on type 2 diabetes mellitus depends on the scientific guidance. The health intervention model of PRECEDE-PROCEED provides a new perspective for effective intervention of diabetes mellitus because of its comprehensive and systematic, dynamic, scientific and effective characteristics. This article aimed to imagine the model of community type 2 diabetes health intervention so as to provide references for the further research of diabetes community health interventions in community by combining with the situation and problems of present interventions and on the basis of systematic analysis of PRECEDE-PROCEED model.
5.Effects of nursing based on PRECEDE-PROCEED in community patients with type 2 diabetes mellitus

Objective:To implement PRECEDE-PROCEED based intervention in community patients with type 2 diabetes mellitus (T2DM) and explore its effects on patients' diabetes knowledge and self-efficacy.Methods:Totally 86 T2DM patients treated in a community health service center in Hangzhou between May and October 2014 were selected using convenient sampling and divided into intervention group ( n=43) and control group ( n=43) . Patients in the intervention group received 6-month intervention based on PRECEDE-PROCEED, while patients in the control group received a regular community health education course once during the same period. The diabetes knowledge and self-efficacy scores were evaluated between the two groups before and 3 and 6 months after intervention. At last, a total of 40 patients in the intervention group and 42 patients in the control group completed the study. Results:Repeated measures analysis of variance showed that there were time-dependent, between-group and combined effects in diabetes knowledge and self-efficacy scores in the two groups ( P<0.05) . Pairwise comparison between the two groups revealed that there was no statistically significant difference in diabetes knowledge and self-efficacy scores between the two groups before intervention ( P>0.05) , but there were statistically significant differences in diabetes knowledge and self-efficacy scores between the two groups 3 and 6 months after intervention ( P<0.05) . Conclusions:The PRECEDE-PROCEED based intervention can effectively improve the diabetes knowledge and self-efficacy of T2DM patients, which is worth promoting in clinical practice.

Objective To understand the present situation and existing problems of the pharmaceutical care of Nutrition Support Pharmacists (NSP) in China and provide reference for pharmaceutical care of NSP. Method Literatures about pharmaceutical care from NSP were retrieved in domestic medical journals from January 2011 to January 2017, and totally 24 articles were analyzed and discussed. Result There were still many problems in NSP, such as the use of Nutritional Risk Screening, and the ratio of glucose/lipid ratio, ratio of non-protein energy to ni-trogen ratio, the lack or excess of vitamin and trace elements and the use of insulin and propylene ammonia acyl glu-tamine in total parenteral nutrition prescription, TPN osmotic pressure and drip time, and the application of glucose and fat milk energy density. Conclusion The pharmaceutical care of NSP starts relatively late, there are still many problems, and the capacity and theoretical level of NSP need to be improved.
8.Tympanoplasty with soft-wall reconstruction of ear canal.

OBJECTIVE:
To observe the therapeutic effect of tympanoplasty with soft-wall reconstruction of ear canal for chronic otitis media with cholesteatoma.
METHOD:
Seventy-three patients (76 ears) suffering from chronic otitis media with cholesteatoma were treated with canal wall down mastoidectomy with tympanoplasty. Postauricular myo-periosteal flap was used to the soft-wall reconstruction of ear canal, and the cavityplasty of auricular concha was not performed. The auricular bone prosthesis was made of the autogeneic mastoid cortical bone or residual incus. The postoperative modality and the function of external auditory canal and the postoperative hearing and the postoperative complications were observed.
RESULT:
The mean dry ear time was (21.1 +/- 3. 1) days after surgery in this study. The postoperative modality of external auditory canal was normal on the whole. The patients were followed up between 6 months and 24 months after surgery. The postoperative average air conduction hearing was improved by (14.5 +/- 6.1) dB HL.
CONCLUSION
Tympanoplasty with soft-wall reconstruction of ear canal using the postauricular myo-periosteal flap can recover the modality and function of external auditory canal on the whole, and the cavityplasty of auricular concha is not needed. The postoperative hearing can be improved by this technique satisfactorily.

9.Anti-tumor effects on human laryngeal carcinoma Hep-2 of recombinant fowlpox virus expressing chicken anemia virus Apoptin gene.

OBJECTIVE:
To investigate the anti-tumor effects and the mechanism of the recombinant fowlpox virus expressing Apoptin gene on human laryngeal carcinoma Hep-2.
METHOD:
Hep-2 cells cultured in vitro were infected with vFVApoptin. The anti-tumor effects on Hep-2 cells were measured through MTT staining and, the mitochondrial trans-membrane potential (delta psi m) and reactive oxygen species (ROS) were analyzed by flow cytometry. Western blot was used to detect the release of cytochrome c (Cyto c). Caspase-3/9 activities were measured by colorimetric assay.
RESULT:
vFVApoptin could restrain Hep-2 cells significantly and, had the function of down-regulating delta psi m, up-regulating ROS, promoting Cyto c release and activating Caspase-3/9.
CONCLUSION
Cyto c were released from mitochondria by the function of up-regulating ROS of vFVApoptin. Cyto c triggered Caspase-9 and, after the activation of Caspase-9, downstream apoptotic factors, such as caspase-3, were activated. Eventually, Hep-2 cells were suppressed by mitochondrial pathway apoptosis induced by vFVApoptin.

10.Problems and countermeasures on the centralized drug procurement in large quantities in public hospitals

Centralized drug procurement in large quantities is a major step to deepen the medical and healthcare system reform, to improve the mechanism of drug price formation, and to give full play to the role of medical insurance in guiding drug prices in China. Combined with the practice of centralized drug purchasing in public hospitals, the authors sorted out the practical problems and causes from four aspects which affected the implementation of centralized drug purchasing policy in public hospitals: selection on centrally purchased drugs, procurement and supply, clinical use, and hospital financial operation. On this basis, suggestions were put forward to provide reference for the normalization of centralized drug procurement work, such as coordinating national and regional drug collection policies, reasonably setting distribution costs, extending the agreed procurement period, and carrying out special evaluation for drug collection.
