Objective:To investigate the effect and safety of sertraline tablets with oral administration in the diabetes patients com-plicated with depression. Methods:Totally 74 diabetes patients complicated with depression were randomly divided into 2 groups with 37 ones in each. All the patients were orally given dimethyldiguanide 500 mg,tid, and the observation group was given sertraline tablets additionally,50-100 mg,qd. The treatment course was 8 weeks. The changes of sugar metabolism and the scores of self-rating depres-sion scale( SDS)and quality of life scale( DMQLS)in the two groups were compared. Results:After the 8-week treatment, the blood sugar,SDS scores and DMQLS scores in the two groups were significantly improved when compared with those before the treatment( P<0. 05). The levels of FBG,2hPG and HbAlc and SDS scores in the observation group were significantly lower than those in the con-trol group(P<0. 05),while the scores of DMQLS disease, psychology, society and satisfaction dimension were significantly higher in the observation group(P<0. 05). The incidence of adverse reactions showed no statistically significant difference between the two groups(P>0. 05). Conclusion:Sertraline tablets can improve the depression of diabetes patients and enhance the blood sugar control and quality of life.

Objective:To summarize the drug interactions of haloperidol used in combination with the other drugs to provide refer-ence for safe, reasonable and effective use of haloperidol in clinics. Methods:By retrieving Micromedex? , Pubmed, CNKI and so on, the interactions between haloperidol and the other drugs were summarized and analyzed. Results:The effect of haloperidol was on do-pamine receptors, and haloperidol was mainly metabolized by hepatic cytochrome P450 ( CYP) enzymes. When haloperidol was com-bined with the other drugs, significant interactions of pharmacokinetics and pharmacodynamics were induced by affecting CYP enzymes or dopamine receptor. Conclusion:In clinical practice, the other drugs combined with haloperidol should be reasonable and careful to ensure safe, effective and rational drug use.

Objective To analyze the effect of diabetes mellitus (DM) on the prognosis of patients with unresectable hepatocellular carcinoma (HCC) after receiving transcatheter arterial chemoembolization (TACE).Methods A total of 858 HCC patients, who were admitted to the Affiliated Xinhua Hospital,School of Medicine,Shanghai Jiaotong University,China, during the period from January 2010 to June 2015 to receive TACE,were selected.Among the 858 HCC patients,175 patients suffered from coexisting DM (DM group) and 683 patients had no DM (non-DM group). The differences in general clinical data and overall survival between the two groups were analyzed.Kaplan-Meier method was used to calculate the survival rate, the difference between the two groups was evaluated by log-rank method, and Cox proportional hazards regression was adopted to analyze the prognostic factors of HCC patients after receiving TACE.Results There were no statistically significant differences in sex, age, serum albumin, total bilirubin, prothrombin time, alpha fetoprotein (AFP),Child-Pugh grade, tumor size and number, cirrhosis, number of TACE treatment times and BCLC staging between the two groups (P>0.05);the fasting blood glucose (FPG)level in DM group was higher than that in non-DM group (P<0.05).The 1-,3-and5-year survival rates of the DM group were 60.9%,27.5% and 10.7% respectively, which were 70.9%,36.0% and 17.6% respectively in the non-DM group;the Log-rank test showed that the difference between the two groups was statistically significant (P=0.008).Multivariate Cox proportional hazard regression analysis revealed that the largest tumor diameter >5 cm,multiple tumor lesions,AFP>20 ng/ml,BCLC stage and coexisting DM were independent risk factors that affected the prognosis of HCC patients after receiving TACE.Conclusion The coexisting DM is an independent risk factor that may affect the prognosis of patients with inoperable HCC after receiving TACE.

Centralized drug procurement in large quantities is a major step to deepen the medical and healthcare system reform, to improve the mechanism of drug price formation, and to give full play to the role of medical insurance in guiding drug prices in China. Combined with the practice of centralized drug purchasing in public hospitals, the authors sorted out the practical problems and causes from four aspects which affected the implementation of centralized drug purchasing policy in public hospitals: selection on centrally purchased drugs, procurement and supply, clinical use, and hospital financial operation. On this basis, suggestions were put forward to provide reference for the normalization of centralized drug procurement work, such as coordinating national and regional drug collection policies, reasonably setting distribution costs, extending the agreed procurement period, and carrying out special evaluation for drug collection.

ObjectiveTo explore the characteristics of eye collaterals of patients with hepatic Wilson's disease (WD) and intervention effect of Gandouling (GDL) based on digital diagnosis information on eyes, in order to provide references for the clinical efficacy of GDL in the treatment of patients with hepatic WD. MethodEighty-two patients with hepatic WD (WD group) and 68 healthy volunteers (normal control group, NC) were enrolled. Eye images of all subjects were collected by the ophthalmoscope, followed by digital extraction of eye collateral characteristics. Differences in main eye collateral characteristics between the WD group and the NC group were analyzed. Pearson correlation test was used to analyze the eye collateral characteristics, serological indexes of liver damage [alanine aminotransferase (ALT),aspartate transaminase (AST),and albumin (ALB)], and Child-Pugh score in patients with hepatic WD. The 82 patients with hepatic WD were divided into a treatment group (42 cases) and a control group (40 cases). All patients were treated with sodium 2,3-dimercapto-1-propane sulfonate (DMPS), while those in the treatment group received additional GDL Tablets. One course of treatment contained eight days, and the treatment lasted six courses. The effects of GDL intervention on the levels of ALT,AST, and ALB,as well as the eye collateral characteristics score and Child-Pugh score were evaluated. Result① Compared with the NC group, the WD group had higher scores of "Qiu" (P<0.05) and "Xuemai" (P<0.05),which are the characteristics of eye collaterals. ② The levels of ALT and AST, and Child-Pugh score in patients with hepatic WD were positively correlated with the scores of "Qiu" and "vessel",while the ALB level was negatively correlated with the "bulge" and "Xuemai" scores. ③ Compared with the conditions before treatment, the serum ALT and AST levels,Child-Pugh score, and the "Qiu" and " Xuemai " scores of the two groups were reduced (P<0.01),and the ALB levels were increased (P<0.01). ④ Compared with the control group after treatment, the treatment group showed improved levels of ALT and ALB and the scores of "Qiu" and "Xuemai" (P<0.05)and better Child-Pugh score (P<0.01). There was no significant difference in AST between the two groups. ConclusionCompared with healthy people, hepatic WD patients showed higher scores of "Qiu" and "Xuemai", indicating that patients with hepatic WD were mainly characterized by dampness and heat, and the scores of "Qiu" and "Xuemai" were positively correlated with the severity of liver damage. Meanwhile,copper-expelling therapy combined with GDL can significantly improve the liver function of WD patients and improve the clinical efficacy.
