1.Endoscopic Thyroidectomy via Breast Areola Approach:Report of 18 Cases

Objective To investigate the feasibility and cosmetic results of endoscopic thyroidectomy via breast areola approach.Methods From January 2004 to January 2006,endoscopic thyroidectomy was performed through the breast areola approach in 18 patients with benign thyroid mass.Results The surgery was accomplished successfully in all the 18 cases.The operation time ranged 45-120 min(mean,62 min) for unilateral resection and 90-180 min(mean,98 min) for bilateral resection.The intraoperative blood loss ranged 6-20 ml(mean,11 ml) for unilateral resection,and 15-70 ml(mean,29.5 ml) for bilateral resection.Postoperative hospital stay was 3-5 days.No complication occurred after the operation.The patients were followed up for 6-12 months(mean,9 months).The wound healed well in all the patients,inconspicuous scars or no recurrence was observed during the follow-up.Conclusions Endoscopic thyroidectomy via breast areola approach is a safe and feasible procedure with satisfying neck cosmetic outcome.

Objective To observe the protective effect of Ulinastatin in rat islet isolation and advance an improved method in rat pancreas islet isolation.Methods Rat pancreas was perfused with Hanks solution containing collagenase. The rats were divided into 3 group according the Ulinastatin concentrations in collagense: low-concentration Ulinastatin ( 2500 U/ml) group, high-concentration Ulinastatin ( 5000 U/ml) and control group (no Ulinastatin). Objective To explore the effect of hypoxia-inducible factor 1α(HIF-1α)in pathological scar and its specific mechanism,and the therapy target of pathological scar.Methods Real time PCR and Western blot was used to test the expression of HIF-1αin normal tissue and pathological scar,meanwhile to detect the effect of hypoxic environment on the expression of HIF-1α.Fibroblasts activity in pathological scar and normal tissue under different oxygen concentration(20%,10%,5%,2% and 1%)was determined by MTT method.Determine the different expression of HIF-1αmRNA and protein in normal environment(20% oxygen) and hypoxic environment(5%oxygen).The changes of the number of fibroblasts after the silence of HIF-1αby HIF-1αshRNA virus using flow cytometer.Results The Real time PCR and Western blot results showed that the expressions of HIF-1αmRNA and protein in fibroblasts of pathological scar was significantly higher than that in normal tissue(P<0.05),and the activity of the keloid fibroblasts in hypoxia environment(5%,2% and 1%)was also higher than that in normal tissue(P<0.05).The expressions of HIF-1αmRNA and protein in keloid fiber cells were higher in hypoxia environment(5% oxygen)than those in normal environment(20% oxygen),but when silence the expression of HIF-1αin keloid fibroblasts,the apoptosis of fibroblast in hypoxia environment(5%oxygen)were significantly higher than control group[(0.021 ±0.001)%vs.(3.739 ± 0.039)%,P<0.05].Conclusion HIF-1αincreases significantly in the pathological keloid fibroblasts and could promote the proliferation of fibroblasts and its vitality,which could accelerate the formation of pathological scar.
4.Clinical effect of Kang Shuling gel on the inflammatory factors in Ⅱ degree burns

Objective To investigate the effect of Kang Shueling gel on the degree of inflammation in the patients with Ⅱ degree burns. Methods 86 patients with Ⅱ degree burns from July 2014 to July 2016 in our hospital were divided into the control group and the experimental group by lottery method with 43 cases in each group. The control group was treated with silver sulfadiazine, the experimental group was treated with Kang Shuling gel, the two groups of clinical curative efficacy, inflammatory factors, visual analogue scale (VAS), wound healing rate and wound healing time, wound infection rate and adverse drug reactions were compared. Results After treatment, the total effective rate of the experimental group was 95.34% higher than that of the control group 72.09%, the difference was statistically significant (P<0.05). The tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) levels of the experimental group was lower than the control group, the difference was statistically significant (P<0.05). The VAS of the experimental group was lower than the control group, the difference was statistically significant (P<0.05).The wound healing rate of the experimental group was higher than that of the control group on the seventh day and the fourteenth day, and the wound healing time was better than that of the control group, the difference was statistically significant (P<0.05). The wound infection rate of the experimental group was lower than that of the control group (P<0.05). There was no significant difference in adverse drug reactions between the two groups. Conclusion The curative effect of Kang Shuling gel on the treatment of Ⅱ degree burns patients worthy of affirmation, could reduce serum inflammatory factors level.
5.Advances in AEG-1 mediated drug resistance

It has been demonstrated that astrocyte elevated gene 1 ( AEG-1 ) can promote tumor initia-tion and progression .Over expression of AEG -1 is correlated with tumor angiogenesis ,metastasis and chemother-apy resistance of tumor cells of different origins .The present article is a review on the mechanism of AEG -1 me-diated drug resistance .Studies have shown that AEG -1 participates in carcinogenesis through Ha -ras,myc,NF-κB and PI3K/Akt signalling pathways .AEG-1 can also promote autophagy through activating AMP Kinase . Other researchers demonstrate that AEG -1 promotes MDR1 protein translation by up-regulating MDR1 mRNA expression ,and thus increases polyribosome .It is testified that AEG-1 can influence drug susceptibility and ex-pression of MDR gene as a RNA binding protein .Multiple functions of AEG -1 in drug resistance in multiple cancers demonstrate that AEG -1 can be used as a novel target for antitumor drugs .

7.Survey and intervention on first aid skills for spinal cord injury in army members

Objective To comprehensively understand the first aid skills for spinal cord injury of army members and improve their first aid skills through interventions.Methods A total of 2 200 troops members were selected within the army (Navy,Army and Air Forces).Intervention methods included questionnaire assessment,multimedia teaching and demonstration of first aid for spinal injuries.The total intervention time was 1 year,with once every four months.Results There distributed 2 200 copies of questionnaire before intervention and received 2 118 valid copies,with the total reclaim rate of 96.27%.A total of 2 118 copies of questionnaires were distributed after intervention and received 2 074 valid copies,with the total reclaim rate of 97.92%.Theoretical examination and skill test results of the army members were significantly improved after the intervention (all P <0.01).The general individual factors showed no effect on first aid of spinal cord injury before and after intervention.Before the intervention,the navy members had higher score than the land forces members and the air force members; however,no significant difference was found on the scores of different forces after the intervention.Conclusions The first aid skills for spinal cord injury of the army members has a big gap from the actual requirements.Improvement of first aid skills for spinal cord injury of the officers and soldiers can save the lives of themselves or comrades and hence is important in minimizing the combat attrition in future potential local high-tech wars.

In this study, the rapid resolution liquid chromatography-quadrupole time-of-flight mass spectrometry (RRLC-QTOF/MS) was used to profile the metabolites of urine samples from chronic heart failure (CHF) patients and healthy controls to find the differential metabolites which could provide the scientific evidence to explain the pathogenesis of the disease and supply a better therapy plan.Urine samples from 15 CHF patients (age (62.27±3.14) years) and 15 healthy controls (age (65.41±4.63) years) were analyzed by RRLC-QTOF/MS.After processing the data, the multivariate statistical analysis (principal component analysis, PCA) was performed to find the potential biomarkers.Result showed that urine samples of CHF patients were successfully distinguished from those of healthy controls.Two significantly differentially expressed metabolites, uridine and alanyltryptophan, were found and identified as potential biomarkers.The result showed that the LC-MS based metabolomics approach had good performance to identify potential biomarkers, and the disorder of uracil metabolism and Tryptophan metabolism may play an important role in the mechanism of CHF.
10.The characteristics and risk factors for post-transplantation diabetes mellitus

Objective To investigate the characteristics of post-transplantation diabetes mellitus and analyze its risk factors. Methods Extensive survey was carried out to understand the characteristics of post-transplantation diabetes mellitus in patients who received kidney grafting from February 1984 to December 2006. Results Three hundred forty-four post-transplantatian diabetes mellitus patients from 1872 ones after kidney grafting were found from February 1984 to December 2006. The prevalence of new onset post-transplant diabetes mellitus and impaired fasting glucose in kidney allograft recipients were 18. 4% and 12. 7% respectively, being significantly higher than that in general population and other inpatients. The options of immunosuppressants were significantly associated with the prevalence of post-transplantation diabetes mellitus. By multivariate logistic regression analysis, the baseline characteristics of the post transplantation diabetes mellitus patients were significantly associated with increased age(OR: 1. 309, P = 0. 049), elevated level of the triglyceride (OR: 1. 311, P = 0. 005), treatment with taerolimus (FK506) (OR: 1. 522, P = 0. 008), and large dose of intravenous pulsed prednisolane(OR: 1. 239, P = 0. 011), as compared with patients without post-transplantation diabetes mellitus. Besides, the number of patients with at least one acute rejection episode was significantly greater in the post-transplantation diabetic patients. Mycophenolate mofetil (OR: 0. 716, P = 0. 028) and diltiazem (OR: 0. 737, P =0. 015) were associated with lower risk of post-transplantation diabetes mellitus. Conclusions High prevalence of abnormal glucose metabolism in renal allograft recipients during hospitalization was observed. Many risk factors contributed to the development of post transplantation diabetes mellitus.
