1.The clinical study on “Qing Dynasty Palace manipulation” in combination with plaster immobilization and foot bath with traditional Chinese medicine for acute soft tissue sprain of the ankle

Objective To investigate the clinical curative effect of“Qing Dynasty Palace manipulation” in combination with plaster immobilization and foot bath with traditional Chinese medicine for acute soft tissue sprain of the ankle.Method A total of 90 patients with acute soft tissue sprain of the ankle from emergency department of WangJing Hospital, CACMS were recruited from July 2013 to April 2014 and randomly divided into two groups with 45 patients in each. The patients in the treatment group were treated with the court manipulation, plaster and Chinese herbs washing, and those in the control group were only treated with cast immobilization. The clinical curative effect was evaluated using Baird-Jackson score system(excellent: 96-100; good: 91-95; fair: 81-90; poor: 0-80).Result At the second, third week after treatment, the excellent/good rates in the treatment group were significantly higher than those in the control group(at week 2: 73.3%vs. 44.4%;χ2=11.468,P<0.01; at week 3: 86.7%vs.60.0%;χ2=6.875,P<0.05). At week 2 and 3, Baird-Jackson scores in the treatment group(87.41±14.12, 93.16±9.51)and in the control group(76.32±11.41, 82.54±9.22)were significantly higher than those before treatment(treatment group: 72.11±12.61; control group: 71.67±13.12, allP<0.05); at week 3, Baird-Jackson score in the treatment group were significantly higher than that in the control group(P<0.05).Conclusion“Qing Dynasty Palace manipulation” in combination with plaster immobilization and foot bath with traditional Chinese medicine can increase Baird-Jackson score and aid recovery of the ankle function in patients with acute soft tissue sprain of the ankle.

Host genes involved in lipid metabolism are differentially affected during the early stages of hepatitis C virus (HCV) infection.Here we demonstrate that artificial up-regulation of fatty acid biosynthesis has a positive effect on the replication of the HCV full-length replicon when cells were treated with nystatin.Conversely,the HCV RNA replication was decreased when fatty acid biosynthesis was inhibited with 25-hydroxycholesterol and PDMP(D-threo-1-phenyl-2-decanoylamino-3- morpholino-1-propanol).In agreement with these results,the expression level of GlcT-1(ceramide glucosyltransferase),a host glucosyltransferase in the first step of GSL (glycosphingolipid) biosynthesis,was found to be closely associated with the expression and replication of HCV RNA.On the other hand,the viral RNA can also activate GlcT-1 in the early stage of viral RNA transfection in vitro.To identify viral factors that are responsible for GlcT-1 activation,we constructed ten stable Vero cell lines that express individual HCV proteins.Based on the analyses of these cell lines and transient transfection assay of the GlcT-1 promoter regions,we conclude that HCV proteins,especially NS5A and NS5B,have positive effects on the expression of GlcT-1.It is possible that NS5A and NS5B stimulate transcription factor(s) to activate the expression of GlcT-1 by increasing its transcription level.
4.Vessel analysis of diabetic retinopathy based on image segmentation

Objective To enhance the clinical diagnosis efficiency and accuracy of diabetic retinopathy (DR) to facilitate its early finding and diagnosis.Methods Image segmentation method was used to process and analyze DR vessel.Clinical images by fundus fluorescein angiography (FFA) were used as the objects for the research on DR vessel segmentation and extraction.Results Image processing and analysis gained higher definition and accuracy in segmenting the normal retinal image and DR vessel when they were compared with the fundus image and FFA image.Conclusion The method proposed enhances the early diagnosis of DR by processing and analyzing the retinal vessel,and thus contributes to the increased living quality of the patient.
5.Autogenous iliac bone implantation after core decompression for the treatment of ischemic necrosis of the femoral head in dogs

BACKGROUND: The pathological change of femoral head was observed after treating necrosis with core decompression and autogenous iliac bone implantation, expected to obtain a better understanding of the prognosis of autogenous iliac bone implantation method.OBJECTIVE: To explore the curative effect of the ischemic necrosis of femoral head filling the hole of the core decompression with autogenous iliac bone implantation in animal models. DESIGN, TIME AND SETTING: Animal trial observation. The experiment was performed at the Central Laboratory of the Guilin Medical University from April 2006 to September 2007.MATERIALS: Nine hybrid adult dogs were prepared for necrosis of femoral head models.METHODS: The ischemic necrosis of femur head was made with the liquid nitrogen in dogs. The rod-shape autogenous cancellous bone that comes from the posterior superior iliac spine was driven into the full depth of the core r decompression tunnel to the subchondral bone. The dogs were divided into 3 groups, 3 dogs in each groups were sacrificed respectively at 3, 6, 12 weeks after operation. Radiograph and histological examination were used to evaluate the results.MAIN OUTCOME MEASURES: Histological changes of the femoral head necrosis and bone graft at the weeks 3, 6 and 12.RESULTS: No collapse of femoral head was observed at the 3 weeks after operation. The graft as well as perivascular tissues showed necrosis with empty lacune. Fiber granulation tissues could be seen ingrowth of capillaries and perivascular tissues into the bone graft. At 6 weeks after operation, basophilia blue area and new born osteoblast, cartilage, as well as chondroblasts zone could be found at the surface of bone trabecula. After 12 weeks of operation, new bone formation, representing typical process of "creeping substitution". And the cancellous bone of necrotic femoral head was rebuilt gradually. CONCLUSION: Core decompression combined with implantation of autogenous iliac bone implantation can accelerate femoral head repairing, delay or prevent femoral head from necrosis.
6.Clinical characteristics and genetic survey on four families of keloid

Objective To study the clinical and genetic characteristics of keloid through investigating on four Han Chinese pedigrees.Methods The pedigree information and clinical data from Han Chinese keloid pedigrees were collected,which consisted of 22 patients in 127 family members,and then the charts of these pedigrees were constructed according to the data.Using the genetic model and pedigree analyses we summarized the clinical features of the disease in the families.Results Four Han Chinese keloid pedigrees were discovered.The three pedigree spans included 3 generations and one was 4 generations.Incidence of KD in the consanguinity family member was 23.7% (23/93),and 20.8% (11/53) in male KD,and 27.5% (11/40) in female.Incidence of anterior chest KD was 40.9 %.The inheritance pattern observed in these pedigrees was consistent with an autosomal dominant inheritance multi-gene hereditary disease with incomplete penetrance,and its nonpenetrance of KD gene carriers was 12% (3/25).Conclusions The pattern of inheritance observed in these four Han Chinese keloid pedigrees is similar to previous reports and no gender differences are found in the incidence of disease,but differences in pathogenic site.Pedigree investigation helps to reveal the genetic characteristics of keloid.
7.The clinical effect of levofloxacin used on patients with hospital acquired pneumonia (HAP)

ObjectiveTo explore the clinical effect of levofloxacin used on patients with HAP. Methods134 cases of patients with HAP as studied object were randomly divided into observation group and control group,observation group were given therapy of levofloxacin,and control group were given therapy of azithromycin. ResultsThe efficiency cases,effective cases,vain cases in observation group were 51,13 and 3,the efficiency rate and total effective rate were 75.0% and 93.3%, the data in control group were 38,17,12,51.7 % and 81.7 %. The efficiency rate ( x2 =4. 818, P < 0.05 )and total effective rate( x2 =4. 804 ,P < 0.05 ) of observation group was significantly better than that of control group. ConclusionLevofloxacin used on patients with HAP, could acquire good clinical effect,and the side-reactions were light.
8.The preliminary study of structure variation related to keloid based on the whole-gene resequencing technique.

OBJECTIVETo investigate the genome structure variation (SV) related with keloid using the whole-gene resequencing technology.
METHODSWe studied a keloid pedigree containing 4 generation of 27 people. 5 people (4 cases of keloid patients, and 1 case of normal) were selected to extract the genomic DNA. Then the whole-gene resequencing technique was used to check the variations.
RESULTSThrough database comparison and variation annotation analysis, we obtained 2 SVs associated with keloid formation. We used DAVID software to do the gene ontology and pathway analysis. We found a 168 bp inversion in gene tetraspanin 8 (TSPAN8) in all keloid patients, which contained the forth exon of TSPAN8.
CONCLUSIONSThere was no report about SVs related to keloid. In this study, we found 2 SVs associated with keloid, especially TSPAN8. The tumor cells express the TSPAN8 can up-regulate the vascular endothelial growth factor and its receptors, promote the adjacent fibroblasts secrete matrix metalloproteinases and uridylyl phosphate adenosine. So we hypothesis that the inversion of the forth exon in TSPAN8 may lead to the signal transduction disorder in the keloid patients. This study was a preliminary research. It needs a further study containing large sample to confirm.

9.Prevalence of chronic obstructive pulmonary disease withobstructive sleep apnea-hypopnea syndrome in Kunming, Yunnan province

Objective To know the prevalence of chronic obstructive pulmonary disease(COPD) in patients with obstructive sleep apnea-hypopnea syndrome(OSAHS) in Kunming Yunnan Province, and the clinical symptom of OS.Methods Retrospective study of 4 636 cases of patients with snoring, excluding COPD in addition to chronic respiratory disease and assess the patient`s condition.The test included AHI, BMI, Epworth sleepiness scale, lung function.The index of OS was confirmed by AHI>5 times/h and FEV1/FVC<70%.Results During the period of 2006 to 2012, he prevalence of COPD in patients with OSAHS was 10.1% [95% confidence interval (CI) 9.1%-11.1%] in Kunming Yunnan Province.And male`s OS prevalence rate is higher than the female(male 10.2%;female 9.7%).The OS patients` average age was 56.9±14.1, the mean AHI was (47.46±26.79) times/h, and the average FEV1/FVC was 60.09%±23.57%.Polysomnographyresults show that patients suffered obvious hypoxiaat night in both OSAHS group and OS group, but it was more significant in OS group.Pulmonary function test showed that OS patients have more serious chronic obstructive pulmonary disease.Conclusions The prevalence of COPD in OSAHS patients was high in Kunming, Yunnan Province, and the prevalence rate in old group reached more than 24%.In addition, the sympotms of patients with OS were more severe than those with only OSAHS or COPD in lung function and hyoxemia.
10.Screening for Kaposi sarcoma-associated genes by using Genechip technology

Objective To screen for Kaposi sarcoma (KS)-related genes. Methods Tissue samples were obtained from the lesion and normal skin of a patient with KS in Xinjiang Uygur Autonomous Region,and total RNA was extracted from these samples and reverse transcribed into cDNA. Real-time fluorescent quantitative reverse transcription PCR (RT-qPCR) was performed to determine the expression of K8.1, K2 and ORF50 in these samples. The cDNA was labeled with fluorescein and hybridized to a human 35K genome array containing 25 100 genes. Subsequently, the signal images were scanned by a laser scanner and acquired images were analyzed by software. Results RT-qPCR revealed the mRNA expression of K8.1, K2 and ORF50 in the KS tissues but not in the normal skin tissues, indicating that there was no crossed contamination in these specimens. Among the 25 100 genes, 1313 genes were identified to be differentially expressed between KS and normal skin tissues, including 756 up-regulated genes and 557 down-regulated genes. These differentially expressed genes, such as myeloid cell leukemia-1 gene (MCI-1), annexins (ANX) and serine proteinase inhibitor Kazal type 5 (SPINK5), were associated with apoptosis, angiogenesis, cell signaling, protein processing, cell cycle regulation, and so on. Conclusion The differentially expressed genes such as MCI-1 and SPINK5 may be associated with the development of KS.
