1.Changes in serum C-reactive protein level and erythrocyte sedimentation rate in patients after use of orthopedic metal implants

BACKGROUND:Most orthopedic surgeries use metal implants. Although this approach wil benefit for patients, it simultaneously makes patients infected by some bacteria. Serum C-reactive protein level and erythrocyte sedimentation rate are two indices for clinical y determining the inflammation. OBJECTIVE:To investigate the changes in serum C-reactive protein level and erythrocyte sedimentation rate in patients after use of orthopedic metal implants. METHODS:The clinical data of 100 patients who used metal implants between October 2012 and October 2014 at the Department of Orthopedics, Linqu People’s Hospital of Shandong Province of China were retrospectively analyzed. Among these patients, 36 patients received implantation of intramedul ary nails, 34 patients artificial joint, and 30 patients steel plates. At 1, 3, 7, 14, and 21 days after implantation, infection was determined according to serum C-reactive protein level and erythrocyte sedimentation rate, as wel as other laboratory indices, wound healing, and body temperature. According to these indices, the patients were assigned to two groups:no infection (n=52) and infection (n=48). Disease progression was analyzed for providing reliable evidence for rational use of antibiotics. RESULTS AND CONCLUSION:There were significant differences in serum C-reactive protein level and erythrocyte sedimentation rate in patients between infection and non-infection groups at 1 (t=3.43, P<0.05;t=3.78, P<0.05), 3 (t=3.35, P<0.05;t=3.68, P<0.05), 7 (t=3.28, P<0.05;t=3.54, P<0.05), and 14 days after use of orthopedic metal implants (t=2.56, P<0.05;t=3.02, P<0.05). No significant differences in these two indices were observed between infection and non-infection groups at 21 days after use of orthopedic metal implants (t=1.76, P>0.05;t=1.98, P>0.05). These findings suggest that after use of orthopedic metal implants, serum C-reactive protein level and erythrocyte sedimentation rate in patients should be closely monitored and simultaneously other indices like wound healing and body temperature should be determined to determine whether infection occurs after use of orthopedic metal implants so as to develop effective repair strategies. This wil provide reliable clinical evidence for rational treatment and prevention of orthopedics-related diseases.
2.Apoptosis Changes in Rat Retina after Ischemia-reperfusion Injury

Objective To establish the rat ischemia-reperfusion injury model and learn the rat apoptosis changes after ischemia-reperfusion injury.Methods Ligate rat left common carotid artery 1h,reperfuse and exam retinal apoptosis in 1,6,12,24,48 and 72h groups.Result Apoptosis mainly occurs in retina ganglion cell layer and inner nuclear layer,occasionally in outer nuclear layer.Apoptosis occurs in 6h after reperfusion.In 24h,apoptosis achieves the peak,then decrease gradually.However,in 72h after reperfusion,there is apoptosis in rat retina.Conclusion rat retina ischemia-reperfusion injury can be observed after ligating common carotid artery.Apoptosis may play an important role in rat retina ischemia-reperfusion injury.In 72h after the operation,the injured retina didn't recover completely.
3.Effect of dexmedetomidine combined with propofol for gastroscopy in elderly patients

Objective To evaluate the efficacy and safety of dexmedetomidine combined with propofol for gastroscopy in elderly patients.Methods Ninety ASA physical status Ⅰ-Ⅲ patients,aged 65-80 yr,weighing 45-80 kg,scheduled for elective gastroscopy,were randomized to receive either dexmedetomidine combined with propofol (group DP,n =45) or propofol (group P,n =45).In group DP,dexmedetomidine 0.4 μg/kg was infused over 5 min and propofol 1.0 mg/kg was injected intravenously 25 min later.Propofol 2.0 mg/kg was injected intravenously in group P.When body movement occurred during operation,additional propofol 0.2 mg/kg was given intermittently.Mean arterial pressure (MAP),heart rate (HR),pulse oxygen saturation (SpO2) and Narcotrend index were monitored during the whole procedures.The consumption of propofol and vasoactive agents,adverse cardiovascular events,and respiratory depression were recorded.Results Compared with group P,the consumption of propofol,incidences of respiratory depression and tachycardia,and degree of body movement were significantly decreased (P < 0.05),Narcotrend index was increased during operation (P < 0.01),and no significant change in the incidence of circulatory depression was found in group DP (P > 0.05).Conclusion Small dose of dexmedetomidine 0.4 μg/kg combined with small dose of propofol 1.0 mg/kg provides good efficacy and safety for gastroscopy in elderly patients.

Objective To evaluate the effect and safety of point injection and em bedding therapy on postoperative pain of hemorrhoid patients. 8.Isolation, culture and osteogenic differentiation of mesenchymal stem cells from human umbilical cord: Ultrastructure of cell membrane observed using atomic force microscope

BACKGROUND: Mesenchymal stem cells (MSCs) are commonly observed under the scanning electron microscope, transmission electron microscope and inverted microscope. However, above-mentioned observation methods have disadvantages on observing ultrastructure of cell membrane and cytoskeleton. OBJECTIVE: To establish a more effective and appropriate method to isolate, culture and identification of human umbilical cord mesenchymal stem cells (hUCMSCs) in vitro and to study the ultrastructure of osteogenic differentiation with the atomic force microscope. METHODS: The hUCMSCs were isolated from human umbilical cord by digested with collagenase. After serial subcultivation in vitro, the stem cells were passaged, and osteogenic differentiation was determined by alkaline phosphatase calcium-cobalt staining and alizarin red staining. hUCMSCs immunophenotype was measured by Flow cytometry.The membrane surface ultrastructure of osteogenic differentiation was observed by Atomic Force Microscope before and after induction. RESULTS AND CONCLUSION: The isolated hUCMSCs by digested with collagenase was efficient. After seeded 24 hours, the adherent cell showed spindle shape, polygonal shape and fibroblast-cell-like shape and the size of hUCMSCs was homogeneous. Flow cytometry analysis revealed that CD29, CD44, CD105 were highly expressed on the surface of passages 3 cells, but there was negative for CD34, CD45 and HLA-DR. These cells were high positive for alkaline phosphate staining and also showed significant calcium node using alizarin red staining after 4 weeks culture induction of osteogenic differentiation. Under atomic force microscopy, undifferentiated stem cells demonstrated insignificant microtubule protrution in a parallel distribution following analysis of cytoskeleton before and after differentiation.
9.Application of combined penile and scrotum skin flap in reoperation for failed hypospadias repair

Objective To explore the application of combined penile and scrotum skin flap in the reoperation process for operation failure cases with hypospadias.Methods A retrospective analysis was performed with the inclu-sion of 1 5 failure cases with hypospadias who received combined penile and scrotal skin flap treatment for reoperation from January 201 3 to December 201 5 in the First Affiliated Hospital of Guangxi Medical University.These cases aged from 7 to 1 5 years old,with a median age of 1 0.2 years old.Among them,a 3 rd operation was performed in 4 cases,4th operation in 5 cases,5 th operation in 4 cases,and 6 th operation in 2 cases.The procedure of Onlay island flap urethro-plasty +tubularized incised plate (Onlay +TIP)urethroplasty was performed in 4 patients,and the other 1 1 cases were treated with urethral mouth basal vascular skin tube urethroplasty (Mathieu)+TIP urethroplasty.F8 or F1 0 silicone balloon catheter were used as stent in new urethra,outer dressing after operation was replaced every 2 to 3 d,the outer dressing was demolished 6 -8 d after operation.Results All patients were followed up after surgery until March 20, 201 6,the follow -up time was 4 -26 months.In these 1 5 patients,the success rate of operation was 86.7%(1 3 /1 5 ca-ses),among them,3 cases were success and 1 case was failed in Onlay +TIP group,and 1 0 cases were success and 1 case was failed in Mathieu +TIP group.The patients with successful operation were with beautiful appearance of the pe-nis,normal position of the meatus urinarius and normal urination.No peristome of urethra,urethrovaginal fistula or ure-thral diverticulum was found after surgery.However,urethral cracking of the glans of penis appeared in 2 operation failed patients,and the orificium urethrae externum was low -set,and urethroplasty was needed 1 year postoperatively. Conclusions For some failure cases with hypospadias showing suitable anatomy conditions,Onlay +TIP or Mathieu +TIP combined with penile and scrotal flap may be a good choice for the completion of reoperation.
10.Mechanism of anticarcinogenic function of ellagic acid in gastrointestinal cancers

Recent in vitro and in vivo experiments have revealed that ellagic acid and its metabolites can inhibit the growth of digestive system malignant tumor cells by inhibiting tumor cell proliferation,inducing apoptosis,breaking DNA binding to carcinogens,blocking virus infection,disturbing inflammation,angiogenesis and drug-resistance processes required for tumor metastasis.Ellagic acid and its metabolites are potential chemoprevention and therapeutic drugs against human cancers.
