1.The Analysis of Complications in Esophageal Stenting

Objective To study the incidence and treatment measures of complications in esophageal stenting.Methods The processes of implantation of esophageal stents in 60 cases were done,of them,59 cases were malignant disease and 1 case was benign stricture.All cases were followed-up for 3~24 months.The types of stents to be used including domestic metal-coated stainless steel Z-stent in 12 cases,domestic gridding-shape nickel-titanium memory alloy coated stent in 47 cases and imports stent in one.Results The processes were successful in all cases,the complications such as arrhythmia in 2 cases,suffocation in one case during operation,and chest discomfort and feeling of foreign body in 49 cases,nausea and vomiting in 15 cases,chronic infection of esophageal stent upper port in 2 cases,stent displacement in 6 cases,stent obstruction in 16 cases,massive hemorrhage,dyspnea and esophago-thoracic fistula in 1 case respectively for 1~36 weeks after operation.After clinical treatment,except 1 patient died due to bleeding shock,all complications were controlled or improved.Conclusion The majority of complications during or after treatment of esophageal stricture with stent implantation are slight,but the indications of operation should be strictly controlled.
2.Preparation and Content Determination of Cholecalciferol Microcapsules

OBJECTIVE:To prepare cholecalciferol(VitD3)microcapsules and establish a method for their quantitative determination.METHODS:Microcapsules were prepared by complex coacervation method and VitD3 content was determined by HPLC.RESULTS:The prepared microcapsules had well-distributed particle size with average particle size of(103.9?26.2)?m and encapsulation efficiency of(90.8?2.68)%.The linear range of VitD3 was 0.1~1.0 ?g?mL-1(r=0.999 3)with mean recovery rate of 98.65% and a dissolution rate of above 75% at 45 min.CONCLUSION:VitD3 microcapsules was successfully prepared by the established method,and the established content determination method was proved to be simple and feasible.
3.Relationship between BRCA1 protein expression in lung cancer tissues and the efficacy of chemotherapy

Objective To investigate the relationship between BRCA1 protein expression in lung cancer tissues and the effect of gemcitabine and Cisplatin based chemotherapy.Methods 80 cases of squamous cell carcinoma confirmed by pathology from May 2013 to May 2015 were selected, and treated with gemcitabine plus cisplatin chemotherapy and Ginseng and Astragalus assisted theropy.BRCA1 protein expression of all patients were detected, and relationship between the effect of chemotherapy and prognosis of patients with BRCA1 protein expression in lung cancer tissues were studied.Results In 80 cases, BRCA1 protein was positive in 40 cases, 40 cases were negative, CR 5 patients, PR 6 patients with BRCA1 positive expression, the remission rate was lower than the BRCA1 negative patients ( P <0.05 ) .80 cases of adverse reactions were seen as leukopenia, gastrointestinal symptoms (nausea and vomiting), abnormal liver function, BRCA1 expression of peripheral neurotoxicity, the incidence of complications in patients with positive expression were significantly higher than BRCA1 negative patients(P<0.05).N stage (P=0.03), BRCA1 gene (P=0.02) were independent risk factors of the prognosis of patients.Conclusion BRCA1 protein positive patients in lung cancer tissue had poor chemotherapy effect, more adverse reactions, and poor prognosis.
4.Relation Between the Situation of Use of Herbal Decoctions and Tumour Therapy in Our Hospital

OBJECTIVE:To analyse the situation and characteristics of use of herbal decoctions,so as to find out the rules of drug-use in treatment of tumour METHODS:By means of the software named "Management System for Chinese Medicinal Herb Pharmacy in Tumour Hospital",usage of the herbal decoctions in treating tumours in this hospital from March 1997 to February 2000 was comprehensively analyzed RESULTS:There were six main categories,271 kinds of herbs used in this hospital which accounted for 58 15% of the total categories The amount of consumption reached 188 135kg which amounted to 83 03% of total consumption Among the top 20 kinds of herb in consumption,the tonics accounted for about 1/3,of which 61 45% for invigorating qi and 29 95% for nourishing yin and the herbs for clearing away heat and toxic material amounted to 65 30% of the total consumption of those for clearing away heat CONCLUSION:The Chinese traditional medicine is one of the most effective methods in preventing and treating tumours The average dosage of the herbs mainly used in treating tumour was about 30g,which is much larger than the common dosage
5.Cardiac involvements in adult polymyositis or dermatomyositis: a qualitative systematic review

ObjectiveTo investigate the clinical features of cardiac involvement in polymyositis or dermatomyositis(PM/DM).MethodsAll articles published in Chinese and English were retrieved by searching the electronic resource databases.Theeligible articles that metthe predefined inclusion and exclusion criteria were included and then a systemic review was performed.ResultsTotally 23 articles were enrolled in this study,including 1452 patients.The incidence of cardiac involvement was 27.1%-85%.Heart failure was the most frequent (43.8%-76.9%) symptom.Among the abnormal ECG and UCG,the incidence of conduction abnormalities,left ventricular diastolic dysfunction andhyperkinetic left ventricular contraction 25%-38.5%,27.3%-51.5% and 11.5%-42.3% respectively.The pathological findings revealed myocardial inflamma-tion,degenerative changes and necrosis similar to those findings in skeletal muscles.Some patients'condition were improved aftergluco-corticoid stero and immunosuppressant treatment.Thirtypatients (30.9%) died as a direct result of heart disease.ConclusionHeart abnormalities are common in patients with PM/DM,most of them are subclinical,however.The efficacy of gluco corticoid steroid and immunosup-pressant treatment are uncertain.Cardiac involvement is a common cause of death.
6.The clinical significance of myositis specific autoantibodies in polymyositis and dermatomyositis

Objective To detect the prevalence of myositis-specific autoantibodies (MSAs) and myositis-associated autoantibodies (MAAs) in patients with polymyositis (PM) and dermatomyositis (DM), and analyse the correlation between MSAs and the clinical features and prognosis of PM/DM. Methods Serum samples of 31 PM and DM patients were screened for MSAs (including anti Jo-1, anti Mi-2, anti PL-7, anti PL-12 antibodies) and MAAs (including anti Ku, anti PM-Scl antibodies) by immunoblotting test. Results Serum MSAs/MAAs were detected in 18 out of 31 PM/DM patients (58%). MSAs were present in 12 patients (39%). The most frequently encountered MSAs was anti-Jo-1 autoantibody (29%), followed by anti-Mi-2 (7%), anti-PL-7 (3%), and anti-PL-12 (3%). MAAs were present in 10 patients (32%), including anti-Ku-72 (16%), anti-Ku-86 (23%) and anti-PM/Scl (7%). 7.Relationship of chemosensitivity on cancer in vitro and the clinical experience

Purpose:To study the relationship of chemosen sitivity in vitro and clinical experience.Methods:To analyze the results of chemosensitivity test in v itro using the MTT assay for 210 cases of five kinds of tumor specimens(85 ova rian cancer, 30 cervical cancer, 26 gastric cancer,41 colorectal cancer and 28 breast cancer), calculate the inhibitory average of each paclitaxel drug to diff erent tumor specimens,then compare the drugs haveing higher inhibitory average w ith the drugs frequently applied clinicaly to that tumor.Results:there is obvious individual variation in in vitro c hemosensitivity test, the variation can be from 0 to more than 60%;however the d rugs having higher inhibitory average correlated to clinical experience.The main theraputic drugs used clinically and also sensitive in in vitro assay were as follows:Taxol,DDP for ovarian cancer; DDP,e-ADM,MMC for cervical cancer ; DDP,5-FU,BLM,MMC for gastric cancer; DDP,MMC,BLM,5-FU for colorectal cancer; e-ADM,ADM,NVB for breast cancer.Conclusions:the chemosensitivity test using the MTT assay may b e useful for individual chemotherapy.
8.EFFECT OF THYROID FUNCTION ON THE SECRETION OF ACTH AND CORTISOL

Patients with thyrotoxicosis had a normal circadian rhythm of ACTH and F,but the level of serum F was lower than controls due to excessive degradation.The responsiveness of ACTH to dexamet-hasone suppression or insulin hypoglycemic stimulation was normal.Whereas the circadian secretion of ACTH and F was also normal in patients with primary hypothyroidism,and the concentration of F was inreased with normal ACTH level.There were stunted responses of ACTH to suppressing and stimulating tests,which were thought to be due to the fact that the corticotropes were impaired by hypertrophy or hyperplasia of thyrotropes in primary hypothyroidism.

10.Analysis of serum soluble human leukocyte antigen-G levels in patients with polymyositis or dermatomyositis

Objective To investigate the serum levels of soluble human leukocyte antigen (sHLA)-G in patients with polymyositis (PM) or dermatomyositis (DM),and to analyze its association with clinical features and possible role in the pathogenesis of PM/DM.Methods Serum sHLA-G levels of 26 patients with PM,70 patients with DM and 35 matched healthy controls were measured by ELISA.The relationship between the sHLA-G levels and clinical features or seroimmunological data in the patients with PM/DM was analyzed.Results Serum levels of sHLA-G in PM/DM patients were significantly higher compared to healthy controls [(44±70) U/ml vs (4±5) U/ml,P<0.01].There was statistically significant difference between DM patients and PM patients [(54±81) U/ml vs (27±41) U/ml,P=0.004].The incidence of dysphagia was significantly higher in sHLA-G elevated group than those in sHLA-G normal group (P=0.001).Additionally,Spearman rank correlation analysis showed that the serum sHLA-G levels were positively correlated with serum C3 (r=0.284,P=0.021),but negatively correlated with CD3+ T cells (r=-0.233,P=0.047) and CD4+ T cells (r=-0.287,P=0.015) in the peripheral blood in patients with PM/DM.Serum levels of sHLA-G in non-treated PM/DM patients were significantly higher compared to treated patients [(77±99) U/ml vs (34±52) U/ml,P=0.021].No relationship between serum sHLA-G levels and PM/DM disease activity,or different drug therapy was found.Conclusion Serum levels of sHLA-G are increased in PM/DM patients.The increased production of sHLA-G,paralleled with higher incidence of dysphagia and lower level of CD3+ T cells and CD4+ T cells,indicates that sHLA-G may play an important role in the pathogenesis of PM/DM.
