Objective To study the chemical constituents of the South China Sea black coral Amtipathes dichotoma. Methods Compounds were isolated and purified by repeated column chromatography on silica gel and their structures were identified by spectral analyses and compared with literatures. Results Ten compounds were obtained from A. dichotoma and characterized as stigma-7,22-dien-3?,5?,6?-triol (Ⅰ),5,8-epidioxycampesta-6,22-dien-3-ol (Ⅱ),cholesterol (Ⅲ),(2S,3R,4E,8E)-2-N-(2'-hydroxyhexadecanoyl) hexadecasphinga-4,8-diene (Ⅳ),(2S,3R,4E)-2-N-(2'-hydroxytetracosanoyl) octadecasphinga-4-ene (Ⅴ),batyl alcohol (Ⅵ),thymine (Ⅶ),thymidine (Ⅷ),uracil (Ⅸ),and guanine (Ⅹ). Conclusion All compounds are obtained from Amtipathes dichotoma. for the first time.

Objective To observe the clinical efficacy of Lao Shi Jiu (moxibustion at a group of ten acupoints) plus Wu Zhu Xiang Sha Liu Jun decoction in treating chronic gastritis due to deficient cold in spleen-stomach. Method Ninety patients with chronic gastritis due to deficient cold in spleen-stomach were randomized into a moxibustion-medication group and a Western medication group by using the random number table, 45 cases each. The moxibustion-medication group was intervened by Lao Shi Jiu plus Wu Zhu Xiang Sha Liu Jun decoction, while the Western medication group was prescribed with conventional medications. The clinical efficacy and changes in symptoms were observed after the intervention. Result The total effective rates were respectively 88.9% and 75.6% in the moxibustion-medication group and Western medication group, and the rate in the moxibustion-medication group was significantly higher than that in the Western medication group (P<0.05); the gastroscopy-based total effective rates were respectively 91.1% and 77.8% in the moxibustion-medication group and Western medication group, indicating that the moxibustion-medication group was superior to the Western medication group (P<0.05); the improvement of stomachache in the moxibustion-medication group was more significant than that in the Western medication group (P<0.05). Conclusion The moxibustion-medication group is superior to the Western medication group in comparing the clinical efficacy and improvement of symptoms in treating chronic gastritis due to deficient cold in spleen-stomach.

Objective To compare the serum anti-survivin antibody levels between benign and malignant lung tumor,thus to provide evidence for using anti-survivin antibody as an indicator in non-small cell lung cancer.Methods ELISA was used to measure the level of anti-survivin antibody in healthy population(control group,n=60),benign lung tumor patients(benign lung tumor group,n=60) and non-small cell lung cancer patients(non-small cell lung cancer group,n=60). Results The anti-surviving antibody did not express 11.7%(7/60) in the control group and almost no expression 20.0%(12/60) in the benign lung tumor group,with no significant difference between the two groups(P＞0.05).In the non-small cell lung cancer group,the anti-survivin antibody expressed in 41 patients,which was significantly higher than those in the benign lung tumor group(x2=38.352,P＜0.01).Conclusion Anti-survivin antibody does not express in the healthy population and the benign lung tumor patients,whereas shows high expression in non-small cell lung cancer.This finding indicates that anti-survivin antibody can provide important evidence for non-small cell lung cancer diagnosis,and can be used as an indicator for non-small lung cancer screening.

[A BSTR ACT] OB J ECTI V E To i mprove the recognization of clinical, imaging and pathological characteristics of angioleiomyoma (ALM) in the head and neck region. METHODS We retrospectively reviewed the data of 20 patients with ALM in the head and neck region between 2000 and 2012. RESULTS Seven male and 13 female patients were included in this study. The average age was 52.5 (from 28 to 74 years). The symptom in most cases (n=14) was the painless mass, 4 tumors originating in the nasal cavity presented with nasal obstruction or (and) epistaxis, and the other 3 cases were accidentally found by physical examination. The results of B-ultrasonography in 10 ALM cases of subcutaneous or deep space were homogeneously hypoechoic echo texture, straight and linear vessels in the tumor with convergence to one point with a circumscribed margin. MRI in 5 cases demonstrated typically a well-defined mass, which showed hypointensity or isointensity to muscle on T1WI, and heterogeneous hyperintensity on T2WI. All lesions showed obviously delayed enhancement on contrast MRI. HE stain showed that the tumors were formed by bundles of spindle-shape smooth muscle cells circumscribing numerous slit-like blood vessels in most cases. Immunoperoxidase staining revealed that the tumor cells were strongly positive for calponin, desmin and smooth muscle actin (SMA) in the cytoplasm of the smooth muscle cells. The positive expression of progestogen receptor and estrogen receptor was seen in 7 cases and 4 cases respectively among 10 cases. All patients underwent surgery, and recovered well postoperatively without recurrence or malignancy. CONCLUSION The clinical manifestations of ALM are nonspecific. ALM has distinctive imaging features in B-ultrasonic and MRI examination. Histological examination and immunoperoxidase staining can make a definite diagnosis of the disease. Progestogen receptor and estrogen receptor can be expressed in ALM. The postoperative prognosis is good.

A series of valproic acid salicylanilide esters were designed and synthesized based on the principle of prodrug. The structures of the target compounds were confirmed by MS, 1H NMR and 13C NMR. Anti-tumor activities of these compounds against K562, A549, A431 cells in vitro were investigated by MTT assay and SRB assay. The results indicated that the compounds 6h-6j were found to have stronger cell growth inhibitory action than gefitinib, and comparable to niclosamide, which are worth to be intensively studied further.

Objective: To explore the application value of intraoperative ultrasound (IOUS) combined with neuronavigation in the microsurgery of low-grade glioma (LGG). Methods Totally 34 patients with LGG underwent microsurgery assisted by IOUS combined with neuronavigation. The degree of tumor resection was evaluated by MRI within 72 hours after operation. Occurrence of IOUS related adverse events such as brain contusion, cerebral hemorrhage and complications such as intracranial infection and cerebral hematoma were observed. Results: The detection rate of tumors by IOUS combined with neuronavigation was 100% (34/34) and accuracy rate of localization was 100% (34/34) in the 34 LGG patients. MRI within 72 hours after operation showed total resection in 27 cases (27/34, 79.41%), subtotal resection in 3 cases (3/34, 8.82%), and partial resection in 4 cases (4/34, 11.76%). Intracranial infection occurred in 2 cases which were improved after anti-infection treatment, but no IOUS scan related adverse events occurred. Conclusion: IOUS combined with neuronavigation can accurately locate LGG and achieve maximum extent safely resection of tumors.

Objective To evaluate the clinical effect of the intertrochanteric fractures with or without lateral femoral wall fractures using proximal femoral nail antirotation (PFNA).Methods From May 2008 to June 2011,102 patients with intertrochanteric fractures were treated with PFNA.In accordance with the preoperative three dimensional CT reconstruction(3D CT) images,the group A included 41 cases with lateral femoral wall fractures,and the other 61 cases with an intact lateral wall were in group B.According to the AO/OTA classification,there were 5 cases in 31-A2,36 in 31-A3 in group A,and 61 in 31-A2 in group B.The operative time,operative blood loss,average length of stay,postoperative X-ray images,and 3D CT images were collected for each patient.Time of partial weight-bearing,full weight-bearing and fracture healing were also recorded.Clinical evaluation was made using the functional recovery scale (FRS) of hip fractures.Results The mean operation time was 56±8 min in group A vs 45±6 min in group B; the mean blood loss was 238±21 ml vs 175±11 ml; the average length of stay was 17±3 days vs 15±3 days.On the postoperative radiography,the blowout of lateral trochanteric wall only occurred in 8 (19.5％,8/41) cases in group A and 3 (4.9％,3/61) in group B.According to the postoperative 3D CT,the similar findings were seen in 36 (87.8％.36/41) cases in group A and 45 (73.8％,45/61) in group B.Eighty-two cases were followed up for 6 to 35 months (mean.19.5 months).The mean FRS score was 64.2±4.8 points in group A and 76.5±7.9 points in group B.Conclusion When treating unstable intertrochanteric fractures,iatrogenic fractures in lateral trochanteric wall could be easily caused with using PFNA.3D CT could effectively evaluate iatrogenic trauma in the intertrochanteric fractures.

Objective To investigate the effectiveness of endovascular technique in treatment of acute arterial hemorrhage of abdominal organs.Methods Totally 159 cases (145 patients) acute arterial hemorrhage of abdominal organs were enrolled.All patients underwent emergency transarterial angiography.Endovascular treatment by spring coil,gelatin sponge,or covered stent were in 40 cases.The success rate of immediate and long-term hemostasis was judged by the postoperative radiography,symptoms and the changes of vital signs and laboratory results.Results The positive signs in angiography were observed in 47 among the total 159 cases (47/159,29.56％).Seventy-seven postoperative hemorrhage cases underwent angiography and 29 cases (29/77,37.66 ％) were positive,82 non-operative hemorrhage cases underwent angiography and 18 cases (18/82,21.95％) were positive.The difference was statistical significant (P＜0.05).Endovascular treat ment were performed in 40 cases (33 by spring coils,4 by gelatin sponge and 3 by covered stent).Hemorrhages were con trolled immediately in all the 40 cases after embolization and the success rate were 100％.Conclusion Endovascular tech nique is minimal invasive,and it is an effective method for treatment of acute arterial hemorrhage in abdominal organs.

OBJECTIVE: To investigate the clinical value of sentinel lymph node (SLN) detection in laryngeal and hypopharyngeal carcinoma patients with clinically negative neck (cN0) by methylene blue method, radiolabeled tracer method and combination of these two methods. METHOD: Thirty-three patients with cN0 laryngeal carcinoma and six patients with cN0 hypopharyngeal carcinoma underwent SLN detection using both of methylene blue and radiolabeled tracer method. All these patients were accepted received the injection of radioactive isotope 99 Tc(m)-sulfur colloid (SC) and methylene blue into the carcinoma before surgery, then all these patients underwent intraopertive lymphatic mapping with a handheld gamma-detecting probe and blue-dyed SLN. After the mapping of SLN, selected neck dissections and tumor resections were peformed. The results of SLN detection by radiolabeled tracer, dye and combination of both methods were compared. RESULT: The detection rate of SLN by radiolabeled tracer, methylene blue and combined method were 89.7%, 79.5%, 92.3% respectively. The number of detected SLN was significantly different between radiolabeled tracer method and combined method, and also between methylene blue method and combined method. The detection rate of methylene blue and radiolabeled tracer method were significantly different from combined method (P < 0.05). Nine patients were found to have lymph node metastasis by final pathological examination. The accuracy and negative rate of SLN detection of the combined method were 97.2% and 11.1%. CONCLUSION The combined method using radiolabeled tracer and methylene blue can improve the detection rate and accuracy of sentinel lymph node detection. Furthermore, sentinel lymph node detection can accurately represent the cervical lymph node status in cN0 laryngeal and hypopharyngeal carcinoma.
