1.Observation of the effect of hyperthermia combined with chemotherapy in patients with advanced gastric carcinoma

Objective To observe the therapeutic efficacy and security of hyperthermia combined with chem-otherapy for advanced gastric carcinoma.Methods The clinical data of 126 patients with advanced gastric cancer were retrospectively analyzed.According to the different treatment methods,they were divided into simple chemotherapy group(A group,63 cases)and chemotherapy combined with hyperthermia group (B group,63 cases).A group received chemotherapy through vein,the patients received docetaxel 75mg/m2 on day 1,tegafur 80mg/m2 on day 1 -14 and cisplatin 75mg/m2 on day 2 -4 of 21 -day courses.B group received hyperthermia together with chemothera-py.Hyperthermia was maintained at 40 -42℃ for 90min.The efficacy and toxicity -side -effect after the patients received two courses were evaluated.Results In A group,the rate of complete remission(CR)was 0 case,partial remission(PR)in 28 cases,stable disease(SD)in 27 cases,progressive disease(PD)in 8 cases,and the overall response rate(CR +PR)was 44.4%,while those in B group were 3 cases,36 cases,21 cases,3 cases and 61.9%, respectively.There was significant difference between the two groups(χ2 =4.589,P =0.032).The improvement rate of life quality of B group(58.7%)was better than A group(33.3%),there was statistically significant difference between the two groups(χ2 =9.232,P =0.002).The rates of Ⅲ°+Ⅳ°abnormal liver and kidney function,myelo-suppression and gastrointestinal tract in A group were 19.0%,23.8% and 25.4%,those in B group were 15.9%, 20.6% and 30.2%,there were no statistically significant differences between the two groups (P =0.815,P =0.830, P =0.426).Conclusion Hyperthermia combined with chemotherapy is efficient and safe for advanced gastric carci-noma,and it is worthy of furthermore study and observation in clinic.
2.Electrocatalytic Properties of Dopamine and Uric Acid at Vitamin B1 Self-assembled Monolayer Gold Electrode

Vitamin B1 self-assembled monolayer modified electrode (VB1-Au/SAMs/CME) was prepared and its electrochemical behavior was studied by cyclic voltammetry. The results showed that a self-assembled monolayer (SAM) was formed on the surface of gold electrode. \3-/4-redox couple was used as the electrochemical probe to study the electrochemical characteristics of SAM modified gold electrode and the results indicated that the existence of Vitamin B1 membrane on gold surface hindered the electron transfer. The electrochemical behaviors of dopamine(DA) and uric acid (UA) were investigated at VB1-Au/SAMs/CME. The results showed that the electrochemical oxidation of DA and UA could be electrocatalyzed. There were linear relationships between the anodic peak current and the concentration of DA(8.0×10-6-4.0×10-4 mol/L) and UA (6.0×10-5-2.2×10-4mol/L) respectively in differential pulse voltammograms. This modified electrode was effective to detect DA and UA in simultaneous determination of these species in a mixture.
3.Effectiveness of community-hospital-based integrated management of diabetes patients in Xiamen, Fujian province

During 2004 to 2009, 701 patients with diabetes were under community-hospital-based integrated management at Zhonghua and Xiagang community health-care service centers, Xiamen, Fujian.Changes of clinical indicators, life-style and frequency of checks for its complications in them were compared before and after the management. All clinical indicators decreased with length of management, with fasting blood glucose from ( 7. 6 ± 2. 3 ) mmol/L before management to ( 7. 3 ± 2. 0) mmol/L half a year after management, (7. 2 ± 2. 1 ) mmol/L one year after management, (6. 9 ± 1.7 ) mmol/L two years after management. Their life-style also improved significantly and frequency of checks for complications increased with: 54. 4% for retinopathy, 52. 5% for diabetic foot, and 51.8% for neuropathy after management (P <0. 05).
4.Significance of percutaneous renal biopsy analysis for patients with acute renal failure

Objective To explore the effects of percutaneous renal biopsy(PRB) on the etiological diagnosis and therapeutic regimen of patients with acute renal failure(ARF),so as to further improve the diagnostic and therapeutic levels of ARF.Methods From Nov.1992 to Dec.2007,176 patients were admitted in the Renal Division of Nanfang Hospital.All the patients matched the diagnostic criteria and were clinically diagnosed as ARF:within 48h the serum creatinine(SCr) ascended(≥26.5?mol/L) and increased by more than 50%,and the urine volume of less than 0.5ml/kg?h persisted in 6 hours.All the patients were undergone PRB and the clinical data were analyzed retrospectively.Results The final etiological diagnosis rate elevated from 64.2%(113/176) before PRB to 96.6%(170/176) after PRB.The coincidence of etiological diagnosis before and after PRB was 95.6%(108/113).Of 176 cases,170 were finally diagnosed as ARF,and the 6 remainders who were clinically misdiagnosed as ARF were finally diagnosed as chronic renal insufficiency(CRI).The therapeutic regimen for 83 patients was supplemented and for another 10 patients was modified after PRB,the total adjusted rate was up to 52.8%(93/176).After PRB,8 patients were finally diagnosed as IgA nephropathy,of them one case was specifically diagnosed as IgA protractedly leading to CRI,6 cases were ARF complicated with IgA,and one case was crescent formation induced by IgA nephropathy that leading to ARF.Conclusion PRB is a very useful technique for the etiological diagnosis of ARF on determining the therapeutic regimen and defining the prognosis.
5.Effects of intelligent power-assisted functional electrical stimulation therapy on ankle joint function in post-stroke subjects

Objective To observe the effect of power-assisted functional electrical stimulation (PAFES)therapy on ankle joint function recovery in stroke patients.Methods Ninety hemiplegic stroke patients were randomly and evenly divided into a control group,a PAFES group,and a neuromuscular electrical stimulation (NMES)group.All groups received conventional rehabilitation training.PAFES group adopted PAFES treatment on affected lower extremities and NMES group was given the NMES therapy on the tibialis anterior of the affected lower limbs,in addition to conventional rehabilitation training.The active range of motion (AROM) of ankle dorsiflexion,FuglMeyer motor assessment (FMA),Barthel index (BI) and Ankle flexion and extension movement (AFEM) in 10 seconds were evaluated before the trial and after 4 weeks treatment.Results After treatment,there were significant differences in the AROM of ankle dorsiflexion,FMA,BI and AFEM (P < 0.05) compared with before treatment within each group.The improvement of AROM of ankle dorsiflexion in PAFES group (8.19 ± 3.39) ° and the values in NMES group (8.96 ± 3.68) ° were to a significantly greater extend than control group (3.88 ± 4.10) ° (P <0.05) ; the improvement of FMA and BI in PAFES group was also superior to those in NMES group and control group (P < 0.05).Conclusion The intelligent PAFES therapy could help improve AROM of ankle dorsiflexion,the motor function of the affected lower extremity and the ability of the activities of daily living in stroke patients.
6.The regulation and the mechanism of melatonin in calcium overload of pancreatic acinar cell

Objective To investigate the regulation and the mechanism of Melatonin in calcium overload of pancreatic acinar cell in acute necrotizing pancreatitis (ANP). Methods Fifty-four Sprague-Dawley (SD) rats were equally divided into three groups:sham-operation group (SO group),ANP group (created with retrograde cholangiopancreatography injection of sodium taurocholate) and MT group (ANP model made after intra-peritoneal injection MT for 30 mins).Rats were sacrificed at 1,4 and 8hours after operation and pancreas tissues were underwent pathological examination.The free calcium concentration of pancreas tissues was determined by fluorescence minitoring method; and the expression of CaMK Ⅱ in pancreas tissues at mRNA and protein level was tested by real-time PCR and Western Blot.Results Pancreatic pathological injury in ANP groups was progressively severe as time extended,which was obviously ameliorated in MT group compared with ANP group (the t value of compared pathological score at 1,4 and 8 hour was:-7.95,-9.72 and -7.69,all P=0.00).Compared at same time point,the free Ca2+ concentration of pancreas tissues in ANP group was significantly higher than that of SO group (the t value of 1,4 and 8 hour was 13.09,18.58 and 16.56,all P=0.00).It was a little bit higher in MT group compared with that of SO group,however was significantly lower than that of ANP group (the t value of 1,4 and 8 hour was -10.03,-11.75 and -11.02,all P =0.00).Compared with SO group,the expression of CaMK Ⅱ at mRNA and protein level significantly increased in ANP group; MT significantly inhibited its expression.Conclusions The expression of CaMK Ⅱ may be inhibited by MT interfere,and then lower the calcium overload in pancreatic acinar cell,which play a role in pancreas tissue protection.
7.Features of computed tomography examination of esophageal submucosal hematoma

Esophageal submucosal hematoma is rarely seen in clinical practice and its clinical presentations are untypical.Gastroscopy might injure the esophageal mucosa and cause bleeding.Computed tomography had the advantages of quick examination and formation of imaging,as well as non-invasiveness,which is helpful for the diagnosis and differential diagnosis of the esophageal submucosal hematoma.The clinical data of 9 patients with esophageal submucosal hematoma who were admitted to the Yueqing People's Hospital,the First Affiliated Hospital of Wenzhou Medical University,the First Affiliated Hospital of the Zhengzhou University and the Third People's Hospital of Wenzhou from July 2005 to February 2014 were retrospectively analyzed.The features of computed tomography examination of esophageal submocosal hematoma include the range of the esophageal submucosal hematoma exceeds 2 parts of the esophagus,the rip of the esophageal mucosa begins and ends at the physiological narrowing of the esophagus,thickening of the esophageal wall,stricture of the esophagus,annular thickening and eccentric thickening of the esophageal wall,crescent,spot or gas shadows of the esophageal cavity,the density of the hematoma decreases as the time passed by.These features of computed tomography are of great importance for the diagnosis of esophageal submucosal hematoma.
8.Analysis of Therapeutic Efficacy of Hyperthermia Combined with Pemetrexed and Cisplatin in the Treat-ment of Medium and Advanced Lung Adenocarcinoma

OBJECTIVE:To observe therapeutic efficacy and safety of hyperthermia combined with pemetrexed and cisplatin for medium and advanced lung adenocarcinoma. METHODS:Medical information of 120 patients with medium and advanced lung ade-nocarcinoma were analyzed retrospectively. They were divided into control group(60 cases)and observation group(60 cases)ac-cording to therapy method. Control group was given Pemetrexed disodium for injection 500 mg/m2 intravenously,d1+Cisplatin for injection 75 mg/m2 intravenously,d2-4. A treatment course lasted for 21 d,and both received 2 courses of treatment at least. Obser-vation group was additionally given hyperthermia,once a month,2-4 times in total,on the basis of control group. Clinical effica-cies,the improvement of life quality and the occurrence of toxic reaction were observed in 2 groups. RESULTS:Both groups re-ceived 2 courses of treatment at least. Total response rate of observation group(63.3%)was significantly higher than that of con-trol group(36.7%),and the improvement rate of life quality(60.0%)was significantly higher than control group(33.3%),with statistical significance (P<0.05). There was no statistical significance in Ⅲ-Ⅳ degree liver and renal function injury,myelosup-pression or the incidence of gastrointestinal tract between 2 groups (P>0.05). CONCLUSIONS: Hyperthermia combined with pemetrexed and cisplatin shows significant therapeutic efficacy for medium and advanced lung adenocarcinoma and improves surviv-al quality without increasing the occurrence of toxic reaction.
9.Effects of avastin on expression of AQP4 in Müller cells under hypoxia.

The aim of this study was to investigate the effects of Avastin on aquaporin4 (AQP4) expression in human retinal Müller cells in vitro under hypoxia, so as to explore the mechanism of Avastin treating retinal edema. The human Müller cells were cultured using the enzymatic digestion method. Müller cells were identified under the transmission electron microscopy and by using immunofluorescence staining. By using semi-quantitative reverse transcription polymerase chain reaction (RT-PCR), the expression of AQP4 mRNA and VEGF mRNA in Müller cells cultured with 500 μmol/L CoCl(2) for 0, 3, 6, 12 and 24 h, and with 0, 100, 300, 500 and 700 μmol/L CoCl(2) for 24 h was detected. The expression of AQP4 mRNA in Müller cells cultured with 50 ng/mL exogenous vascular endothelial growth factor (VEGF) for 0, 0.5, 1, 2 and 4 h, and with 0, 25, 50 and 75 ng/mL VEGF for 24 h was detected. Amplified cDNA products of AQP4 mRNA in Müller cells cultured with 500 μmol/L CoCl(2) and 200 μg/mL Avastin for 24 h were detected. The results showed that more than 95% cells displayed positive immunofluorescence reaction. Characteristic 8-10 nm intracellular filaments could be seen in the cytoplasm under the transmission electron microscopy. In the CoCl(2) experimental groups, the expression of AQP4 mRNA and VEGF mRNA in Müller cells was increased as compared with the control group. Alteration of AQP4 mRNA and VEGF mRNA levels showed a significantly positive correlation (r (2)=0.822, P<0.05). The expression of AQP4 mRNA in Müller cells was increased by VEGF. The expression of AQP4 mRNA was significantly decreased by Avastin as compared with the control group. It is suggested that Avastin can decrease the expression of AQP4 mRNA in human Müller cells under chemical hypoxic conditions partially via VEGF path, which may be one of the mechanisms of Avastin treating retinal edema.
10.Analysis of the effect of home quarantine on blood lipid and glucose of healthy adults during the COVID-19 epidemic times based on the data of physical examination

Objective:To analyze the effect of home quarantine on blood glucose and lipids in healthy adults during the COVID-19 epidemic times.Method:From April 7, 2020 to May 1, 2020, 512 adults wereexamined in Beijing physical examination center, of which 87 adults aged olderthan 18, received community closed management or home quarantine from January 23, 2020 to March 30, 2020, and theadultshad physical examination in Beijing physical examination center in the same month of 2018 and 2019 were selected as controls. The change trend of blood glucose and blood lipid acrossthe three years was analyzed by one-way repeated measurement of variance, and the difference of blood lipid and blood glucose between two years of physical examination was further analyzed by Bonferroni method. Datawereanalyzed by gender.Result:Among the 87 subjects, 36 (41.4%) were male and 51(58.6%) were female. The average age was (46.3±13.2) years and ranged from 24 to 74. The difference of high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglyceride (TG), total cholesterol (TC) and fasting blood glucose (FBG) among 2018, 2019 and 2020 was statistically significant (all P<0.05), while differences in LDL-C, TC and FBG were statistically significant for males (all P<0.05) and HDL-C, LDL-C, TC and FBG were statistically significant for females (all P<0.05). HDL-C in 2018and 2020were significantly lower than that in 2019 [(1.27±0.29), (1.30±0.31) vs. (1.36±0.34) mmol/L], LDL-C in 2020was significantly higher than that in 2018and 2019 [(3.11±0.88) vs. (2.81±0.77), (2.84±0.71) mmol/L], TG in 2020 was significantly higher than that in 2019[(1.54±1.17) vs. (1.32±0.80) mmol/L], TC in 2019and 2020were significantly higher than that in 2018 [(4.88±0.94), (5.10±0.99) vs. (4.63±0.90) mmol/L], and the FBG in 2019and 2020were significantly lower than that in 2018 [(5.34±1.17), (5.44±1.58) vs. (5.84±1.70) mmol/L] (all P<0.05). The comparative analysis of men and women showed that the means of LDL-C, TG, TC and FBG of men were higher than those of women while the mean of HDL-C was lower than that of women in each year; compared with 2019, the increase of LDL-C and TC of men was obvious while the decrease of HDL-C of women was obvious (all P<0.05) in 2020. Conclusion:During the epidemic period of COVID-19, the home quarantine has adverse effects on blood glucose and lipid.
