Journal of Chinese Physician 2002;0(S1):-
Objective To analyze the clinical conditions and emergency treatment experiences of acute and chronic renal failure with hyperpotassemia. Methods Retrospective analyzed 542 clinical cases of supervening hyperpotassemia, which were treated by hemodialysis on 186 patients of renal failure with hyperpotassemia for 5 years. Results 540 cases clinical symptom was improved, serum potassium concentration was down to normal range and hyperpotassemia disappeared in electro cardiogram. 2 cases died after salvage. Rate of success was 99.6%. Conclusion Hemodialysis therapy is the most effective treatment to hyperpotassemia. Use bicarbonate with kalium pH indicator of 3.0~3.5mmol/L in dialysis is useful to lower hyperpotassemia, and safe also. In the early stage of hemodialysis, pure ultrafiltration dehydration treatment is not suitable. There is still danger of heartbeat stop deal to high kalium in the early stage of dialysis; emergency treatments should be standby all the time.

This paper presents a surgical optical navigation system with non-invasive, real-time, and positioning characteristics for open surgical procedure. The design was based on the principle of near-infrared fluorescence molecular imaging. The in vivo fluorescence excitation technology, multi-channel spectral camera technology and image fusion software technology were used. Visible and near-infrared light ring LED excitation source, multi-channel band pass filters, spectral camera 2 CCD optical sensor technology and computer systems were integrated, and, as a result, a new surgical optical navigation system was successfully developed. When the near-infrared fluorescence was injected, the system could display anatomical images of the tissue surface and near-infrared fluorescent functional images of surgical field simultaneously. The system can identify the lymphatic vessels, lymph node, tumor edge which doctor cannot find out with naked eye intra-operatively. Our research will guide effectively the surgeon to remove the tumor tissue to improve significantly the success rate of surgery. The technologies have obtained a national patent, with patent No. ZI. 2011 1 0292374. 1.

Chinese Journal of Orthopaedics 2010;30(4):325-329
Objective To measure the vertebral rotational centers in sagittal and transverse plane. These data may be quite valuable for the design of artificial disc. Methods Ten healthy volunteers of 40-60 years old were recruited under IRB approval and informed consent. Three-dimension lumbar spine models of L_2, L_3 and L_4 were reconstructed from MR scans. Spine motions were then reproduced using combined dual fluoroscopic imaging system (DFIS) and MR scan technique during flexion-extension and left-right twisting of the body. Based on the geometrical features of the vertebrae, ranges of motion (ROM) of 3 representative lo-cations were measured from anterior to posterior, the vertebral body center, the spinal cord canal center and the spinous process tip. Rotational centers of the vertebral segments were then located by calculating the point of zero ROM. Results Motion patterns: The ROMs of L_2 with respect to L_3 (L_(2,3)) and L_(3,4) increased pro-portionally from anterior to posterior locations. During flexion-extension motion, the vertebral body center moved within a range of 0.6 mm, while the spinous process tip moved within 7.5 mm in the sagittal plane. During left-right twisting, the vertebral body center moved within 1.0 mm, while the tip moved within 1.6 mm in the transverse plane. No statistical differences were found in the ROMs between L_(2,3) and L_(3,4). Rotational centers: the rotational centers of flexion-extension in sagittal plane for both L_(2,3) and L_(3,4) segments were located at posterior one-third of the vertebral body. The rotational centers of both L_(2,3) and L_(3,4) segments were located approximately 30 mm anterior to the front edge of the vertebral body. Conclusion The anterior portion of the vertebrae was found to have smaller ROM than the posterior portion. The vertebra rotates with the center of rotation located at approximately the posterior one-third of the vertebral body in sagittal plane. However, the vertebra rotates in transverse plane with respect to a point about 30 mm in front of the vertebra. The data indicated that the different portions of the vertebra have distinct motion characters during different motions.

Objective To evaluate the clinic efficacy of laparoscopic totally extraperitoneal technique in complex inguinal hernia repair. Methods Totally 42 patients with complex inguinal hernia were repaired by laparoscopic totally extraperitoneal technique in our hospital during July 2004 to June 2008. Among the cases,18 patients had bilateral indirect inguinal hernia,8 had unilateral indirect and direct inguinal hernia,5 suffered from indirect inguinal hernia at one side and direct inguinal hernia in the other side,4 patients showed bilateral direct inguinal,3 showed bilateral indirect inguinal hernia and unilateral direct inguinal hernia,2 had bilateral direct inguinal hernia and unilateral indirect inguinal hernia,1 patient showed indirect inguinal hernia complicated with femoral hernia at the same side, and 1 patient had indirect and direct inguinal hernia at the both sides.During the operation,a 10-mm incision was made at 15 mm below the umbilicus reaching the Hunter's line,to place the laparoscope and separate the extraperitoneal space.Afterwards, two 5-mm trocars were inserted and then extraperitoneal pneumorperitoneum was established involving the retropublic space extending up to the anterior superior spine,so that to return the complex hernias.A polypropylene patch was then used to cover the pubic foramen and Hesselbach triangle. Results TEP was successfully completed in all of the 42 patients with a mean operation time of 85-165 min (mean,107.3 min) and estimated intraoperative blood loss of 10-20 ml. Scrotum aerocele was detected in all of the cases during the operation. The patients received normal diet in 2 days postoperation. They were discharged from our hospital in 7 to 9 days (mean,7.6 days). Follow-up was achieved for 3 to 32 months (mean,15.3 months) in all of the patients, during the period,2 patients showed recurrent direct inguinal hernia, and 5 had mild paraesthesia in the lower abdominal wall. Conclusions TEP is safe and effective for complex inguinal hernia.

Objective To explore the feasibility of mastoscopic resection of benign mammary gland tumors. Methods A total of 68 cases were diagnosed as having benign mammary gland tumors(42 cases of fibroma and 26 cases of cystic adenoma) by using molybdenum target X-ray examinations. The tumor was located at lower outer quadrant in 26 cases, upper inner quadrant in 16 cases, lower inner quadrant in 14 cases, and upper outer quadrant in 12 cases, respectively. The operation was conducted via a transaxillary approach.A specially-designed flap dissector was utilized to establish a tunnel underneath the breast skin, where the air was inflated to build a surgical space. Then the tumor was resected by using an electrotome or harmonic scalpel. Frozen-section examination was routinely carried out during operation. Results The operation was successfully completed in all the 68 cases, without intraoperative complications. The operation time was 28~45 min(mean,35 min). The drainage tube was indwelled for 1~2 d.Subcutaneous effusion occurred in 1 case.Postoperative recovery was uneventful without skin necrosis or other complications. The length of postoperative hospital stay was 2~5 d(3.0?1.5 d).Follow-up checkups in the 68 cases for 3~36 months(mean,12 months) revealed good cosmetic outcomes. Conclusions Transaxillary mastoscopic resection of benign mammary gland tumors is safe and feasible, with good cosmetic results.
