OBJECTIVE:To analyze the blood drug monitoring results of vancomycin and clinical administration behavior and then to provide a clinical reference for its rational utilization. METHODS:We retrospectively collected the blood drug monitoring results of vancomycin and drugrelated information from July 2007 to October 2008 in our hospital. These data were from the electronic clinical pharmacy workstation and initial written case history. RESULTS:Among the 76 cases,non-clinical significant results due to doctor’s irregular manipulation in collecting blood sample accounted for 32.89%. In renal failure group, the rate of peak concentration above the effective blood concentration range was 44.46% . Every patient was averagely administered 21.85 kinds of drugs mainly by intravenous injection during vancomycin application. Clinical response rate of vancomycin was 65.00%. CONCLUSION:During the blood concentration monitoring of vancomycin,we should take clinical need into consideration, collect blood samples rightly, consider the factors such as special pathophysiologic status of patients, drug combination, and adjust the dosage timely.

Objective To explore the relationship between interleukin-1?(IL-1?) and group Ⅱ metabotropic glutamate receptors(mGluR2/3). Methods The rats were randomly divided into five groups:1^cotrol;2^group Glu;3^group IL-1?+Glu;4^group IL-1?;5^group IL-1?+D-AP5+Glu.The behavior of animals was observed.Expression of mGluR2/3 mRNA was assayed with RT-PCR. Results The alteration of behavior showed that the latent period of seizure in group with IL-1?+Glu was shortened significantly and the degree of seizure was also more serious as compared with that in group Glu.Expression of mGluR2/3 mRNA showed marked enhancement in group with IL-1?+Glu and group with Glu compared with control. However, expression of mGluR2 mRNA in group IL-1?+Glu was markedly decreased as compared to that in group Glu;the level of mGluR3 mRNA showed no apparent difference between group IL-1?+Glu and group Glu.Conclusion Present study suggests that IL-1? enhances the degree of seizure induced by glutamate, the mechanism may be due to its direct or indirect effects on down-regulation of expression of mGluR2 mRNA.

Objective To explore the relationship between organizational justice and turnover intention among nurses of tertiary hospitals and to provide implications for human resource management. Methods Totally 511 nurses were recruited from four tertiary hospitals of Jinan City and investigated with Organizational Justice Questionnaire and Turnover Intention Scale. Results The average scores of turnover intention and organizational justice were (2.46±0.61) and (3.55±0.62),respectively.Distributive justice and informational justice were significant predictors of turnover intention. Conclusions The turnover intention of nurses in tertiary hospitals is high.It is beneficial for mitigating nurses' tumover intention to improve their organizational justice, apply post management and performance appraisal model.

Objective To probe the periodontal ligament regeneration following the implantation of bone marrow mesenchymal cells ( BMSCs ) sheet-collagen membrane-BMSCs sheet sandwich complex. Methods BMSCs cell sheet-collagen membrane-BMSCs sheet complexes were compounded on the root surface of teeth of Beagle dogs. All the dogs were killed on 4 and 12 weeks after implantation.Periodontal ligament regeneration was observed by radiological means, HE staining and Sirus-red staining. Results Compared with collagen membrane group and blank control group, there was a clearly periodontal ligament like tissue and Sharpey′s like fibres formation in test group only.Conclustion Cell sheet-collagen membrane-cell sheet sandwich complex can effectively improve the periodontal ligament regeneration.

Chinese Journal of Practical Nursing 2016;32(10):796-800
Objective To systematically evaluate the effectiveness of diabetes self-management education through telemedicine on glycosylated hemoglobin (HbA1c). Methods Randomized clinical trials (RCTs) that assessed the effectiveness of remote diabetes self-management education in patients with diabetes were systematically reviewed using multiple electronic databases. The searching time was from database establishment to May 2015. These articles were analyzed after evaluation of their quality. Results Eight RCTs were included involving 2 047 patients. The results of Meta-analysis showed the effectiveness of remote DMSE on reduction of HbA1c of patients with type 2 diabetes was more significant than that of the control group [WMD=-0.50,95%CI (-0.88,-0.12),P < 0.05],while failing to support the effectiveness of remote DMSE on patients with type 1 diabetes [WMD=0.62, 95%CI(-0.19,1.43), P>0.05]. Conclusions Remote DMSE is effective to improve the glycemic control in patients with type 2 diabetes, while the effect on patients with type 1 diabetes is still not certain. Large-scale and multi-centered RCTs are needed to confirm its effectiveness.

Objective To analyze the surgical treatment of hemangioma of limbs complicated by Kasabach-Mcrritt phenomenon in children. Methods A retrospective study was carried out on 31 children with hemangioma of limbs complicated by Kasabach-Merritt phenomenon.Lower limbs were affected in 19 patients, and upper limbs in 12 patients.Twenty-six patients showed decreased platelet count ((2.6-60) × 109/L) and fibrinogen level (< 1 g/L),and 21 prolonged partial prothrombin time (> 50 seconds). All the patients who had responded poorly to medications including glucocorticoids received surgical treatment. Results Of these patients,30 were cured, and one died of massive blood loss. After surgical treatment, the platelet count returned to normal in 1-3 days in 25 patients,3-7 days in 4 patients, and more than 10 days in one patient. Conclusions For pediatric hemangiomas of limbs complicated by Kasabach-Merritt phenomenon, surgical treatment should be given as early as possible so as to reduce mortality in patients.

Objective To systematically evaluate the risk factors for upper extremity lymphedema after breast cancer treatment and the strength of their associations. Methods PubMed,Ovid,EMbase, and the Cochrane Library were searched to identify clinical trials published up to December 2012.The quality of included studies was assessed by the Newcastle-Ottawa Scale;data analysis was performed by Stata 10.0 and RevMan 5.2;the strength of associations between risk factors and breast cancer-related upper extremity lymphedema was described as odds ratio (OR) and 95% confidence intervals (CI).Results Twenty-two studies involving 10106 patients were included in the meta-analysis.The risk factors for upper extremity lymphedema after breast cancer treatment mainly included axillary lymph node dissection (OR =2.72,95% CI=1.06-6.99,P=0.038),hypertension (OR=1.84,95% CI=1.38-2.44,P=0.000),body mass index (OR =1.68,95% CI=1.22-2.32,P =0.001),and radiotherapy (OR =1.65,95% CI =1.20-2.25,P =0.002),while no significant associations were found for such factors as chemotherapy, age, number of positive lymph nodes, and number of dissected lymph nodes. Conclusions The incidence of upper extremity lymphedema is high among patients with breast cancer after treatment, and axillary lymph node dissection, hypertension, body mass index, and radiotherapy are the main risk factors for lymphedema after breast cancer treatment.
