1.Computer-assisted external frame for correction of tibial and fibular deformity

Objective To introduce a new computer-assisted external frame for osteotomy to correct complex tibial and fibular deformity.Methods From January 2012 to December 2013,15 patients with complex tibial and fibular deformity were treated with a computer-assisted external frame and related computer software for surgical correction.All were unilateral limb deformity,involving the left lower limb in 8 cases and the right lower limb in 7 cases.They were 8 males and 7 females,15 to 43 years of age (average,26.8 years).The deformity was caused by old osteomyelitis in 9 cases and trauma sequela in 6.The related angles,distances and rotations were measured on the postoperative radiographs of the lower limbs.The data were input into the software to calculate the daily adjustment lengths of the frame rods.Daily frame adjustments were conducted according to the data calculated.The frame was removed when expected results were achieved.The patients lived independently during the adjustment.Results The 15 patients were effectively followed up for 10 to 22 months (average,15 months).Frame fixation time averaged 8.4 months (from 5.5 to 13.1 months).The reunion time averaged 8.0 months (from 5.4 to 12.1 months).The hospitalization time averaged 2.4 months (from 2.0 to 2.8 months).In 10 cases of limb shortening,the shortening lengths averaged 0.5 cm (from 0 to 0.9 cm) after adjustment.In 12 cases of angulation deformity,the angulation averaged 3.3° (from 0° to 5.4°) after adjustment.In 5 cases of rotation deformity,the maximum internal rotation was 5° and the maximum external rotation 10° after correction,deviating from the normal values by 3.5° on average.In 6 cases of horizontal displacement,the displacements averaged 0.5 crn after correction.All patients were satisfactory with corrected lower limb alignment,improved symptoms and appearance,giving a subjective satisfaction rate of 100%.Conclusion Use of computer-assisted external frame in surgical correction of tibial and fibular deformity can achieve satisfactory outcomes in simple,accurate,standardized,minimally invasive manners.
3.Nursing care of patients with cosmetic dermatitis receiving patch test

Objective To summarize key points of nursing patients with cosmetic dermatitis during patch test.Method One hundred and forty one patients with contact dermatitis induced by cosmetics underwent patch test and the nursing care was performed. Results The top five allergens based on the positive rate ranked in descending order were potassium dichromate(43?97%),nickel sulfate (43?26%),cobalt chloride(36?17%),mixture of hydroxy benzoic acid(23?40%)and fragrance mix(20?57%).The positive suspicious cosmetics ranked in ascending order included body spray(80?00%),freckle cream(56?00%),liquid products(38?89%),cleaning supplies (31?58%)and skin care cream(25?62%).Conclusion During the cosmetic patch test for the patients with contact dermatitis,the nurses should conduct mental care,carefully observe the patients and make instructions to them on cosmetic health knowledge.
4.Significance of Changes of Blood Lipids,Endothelin and Carbohydrate Tolerance in Simple Obesity Children

Objective To investigate the changes of blood lipid,endothelin(ET)and carbohydrate tolerance in simple obesity children.Methods Serum total cholesterol(TC),triglyeride(TG),low density lipoprotein(LDL),endothelin(ET) and oral glucose tolerance test(OGTT) of 436 cases(light obesity group 242,moderate obesity group 138,heavy obesity group 56) were detected respectively.Results The levels of TC,TG,LDL,ET and OGTT in 3 simple obesity groups were all higher than those in control group obviously and there were significant differences.The significant positive correlation between the obesity children serum lipids,ET and obese degrees were observed.Four insulin resistant children were found in heavy obesity group and none in other groups.Conclusions The levels of serum lipids and ET increases in obesity children and the OGTT decreases in some heavy obesity children.For preventing the obesity related health problems,the simple obesity children should be paid more attention to earlier.
5.Application of integrated collaborative service mode in obstetrics and gynecology clinical service

Objective To explore and assess the reasonable application of integrated collaborative service mode in obstetrics and gynecology clinical service.Methods 152 pregnant women admitted in obstetric outpatient from March 2016 to May 2016 were randomly assigned into observation group and control group,76 cases in each group.The control group received obstetric traditional outpatient treatment and hospitalization mode,while the observation group implemented the integrated collaborative service mode.It was a integrated mode based on the traditional mode and multidisciplinary professionals integrated into groups for synergistic services,from the beginning of pregnancy to discharge.Results The number of cases who were knowledged maternal diseases,intraoperative coordination,reasonable diet and breastfeeding of the observation group (69,70,69,66) were superior to those of the control group (51,40,54,50) (x2=17.56,52.58,12.44,11.11,all P<0.001).The incidence rate of maternal and neonatal complications of the observation group were significantly lower than those of the control group(t=12.04,6.49,2.87,9.26,-7.14,all P<0.005).And patients satisfaction survey was improved,the number of satisfaction and complaint in the observation group was 70 persons,1 person,which of the control group was 70 persons,6 persons (x2=7.65,P<0.01).Conclusion Integrated collaborative service mode can improve the quality of medical service and patients' satisfaction,while reduce cesarean section rate,postpartum hemorrhage and maternal complications.Integrated collaborative service mode is an effective method to promote the development of perinatal medicine.
6.Rituximab intrathecal injection combined with intravenous chemotherapy in the successful treatment of splenic marginal zone lymphoma with the central nervous system relapse: a case report and literatures review

Objective To explore the clinical characteristics,pathological features,therapy and prognosis of a case of splenic marginal zone lymphoma (SMZL) with central recurrence,evaluation the safety and efficacy of rituximab intrathecal injection combined with intravenous chemotherapy.Methods Retrospectively analyze the case of SMZL with the central recurrence,and review the relevant literatures.Results According to the tests of MICM of bone marrow,spleen pathology,PET-CT and laboratory examination (LDH),patient was diagnosed as SMZL,IV group B,IPI and aaIPI was high risk group.After first-line therapy,the patient achieved complete remission.But then the central nervous system relapsed.The patients was treated with rituximab intrathecal injection combined with intravenous chemotherapy,the central focus disappeared.Conclusion SMZL belongs to low-grade lymphoma,which combined with central nervous system relapse is very rare.Rituximab intrathecal injection combined with intravenous chemotherapy in the treatment of the patients is safe to use,and have good clinical efficacy.
7.Comparison between the developments of larval Schistosoma japonicum in Oncomelania hupensis with and without pre-infection by larval Exorchis

Larvae of Schistosoma japonicum could be blocked in Oncomelania hupensis with pre-infection by larval Exorchis trematodes (Tang et al., 2008, 2009). The present manuscript reports the result of comparison between the two different developments of larval S.japonicum in O.hupensis snails with and without pre-infection by larval Exorchis trematodes. Of 300 (invader rate 26.74%) abnormal larvae of S.japonicum for 4-82 days old were found from 28 (73.7%) O.hupensis snails which were pre-infected with Exorchis eggs, their body structure were unusual and developed astray at early mother sporocyst stage. From 25 (69.4%) O.hupensis snails without pre- infection by larval Exorchis, 67 (invader rate 13.96%) normal mother sporocysts of S.japonicum for 5-61 days old in different development stages and many early or middle stage daughter sporocysts were found, in the 75th day after infection many mature daughter sporocysts and cercariae of S.japonicum were found. Different increase of lymphocyte in body tissues of snails were found from the two experimental O.hupensis snail groups.
8.Ventilation/perfusion scan in the diagnosis of acute pulmonary embolism in elderly patients

Objective To compare the diagnostic efficacy of the V/Q scan and CT pulmonary angiography (CTPA) for the detection of acute pulmonary embolism (PE) in elderly patients.Methods Fortyfour patients (age ≥60 years old) with suspected acute PE underwent V/Q scan and CTPA.The diagnosis of PE by V/Q scan was based on the criteria of prospective investigation of PE diagnosis (PIOPED) Ⅱ and the prospective investigative study of acute PE diagnosis (PISA-PED).The final diagnosis was made clinically.The sensitivities,specificities and accuracies of PIOPED Ⅱ,PISA-PED,CTPA and Wills score were calculated and compared using x2 and Fisher's exact tests.Kappa analysis was used to analyze the diagnostic accordance rate of PIOPED Ⅱ and PISA-PED.Results The sensitivities of PIOPED Ⅱ,PISA-PED and CTPA in the diagnosis of PE were 70.00% (14/20),84.62% (22/26) and 65.22% (15/23),respectively (x2 =0.069-1.545,all P>0.05).The sensitivity of Wills score was significantly lower (23.08%,3/13).The specificity of CTPA (93.75%,15/16) was significantly higher than those of PIOPED lⅡ and PISAPED (80.00%,12/15 and 61.11%,11/18,both P<0.05).The accuracies of PIOPED Ⅱ,PISA-PED and CTPA were 74.29% (26/35),75.00% (33/44) and 76.92% (30/39),respectively (x2 =0.005-0.070,all P>0.05).The accuracy of Wills score was significantly lower (52.17%,12/23).The diagnostic accordance rate of PIOPED Ⅱ and PISA-PED criteria was 77.14%(27/35),Kappa=0.547,P<0.05.Conclusion V/Q scan and CTPA have no significant difference for the diagnosis of PE in the elderly patients.

Objective To explore the effects of silybin-phosphatidylcholine compound (SPC )on lipopolysaccharide (LPS)-induced activation of nuclear factor-?B (NF-?B) and phosphorylation and degradation of inhibitors of NF-?B?(I?B?).Methods Phagocyte were collected in abdominal cavity of Kunming mousse aged 6 to 8 weeks,cultured phagocyte(2?105 mL-1) were divided into control, LPS and SPC groups randomly, phagocyte in control group were added into the same volume of 9 g/L sodium chloride.Phagocyte in LPS group were added into a single bolus of LPS(10 ?g/mL LPS) for 24 hours, phagocyte in SPC groups were preincubated with different concentration of SPC for 2 hours followed by a 24 hours incubation with 10 ?g/mL LPS.
