1.Survey in related factors of nurse' compassion fatigue and compassion satisfaction in a certain tertiary hospital

Objective To describe the status and analyze the influencing factors of nurses' compassion fatigue and compassion satisfaction in a certain tertiary hospital,which may provide evidences for the emotional management of nurses by nursing managers.Methods A cross-sectional survey was used in this study and 246 nurses were recruited totally.They investigated about compassion fatigue and compassion satisfaction by questionnaires.Results The average score of nurses' compassion fatigue was (30.76± 5.61)points.The average score of nurses' compassion satisfaction was (25.68±4.12)points.There was a significant difference for nurses' compassion fatigue in various wards,and ICU nurses had the top score.There were no significant differences for nurses' compassion satisfaction.Conclusions The nurses' compassion fatigue and compassion satisfaction in this hospital was relatively moderate.ICU nurses' compassion fatigue score was higher than other wards.Education background,professional title,relationship with colleagues,extra work,support from leaders and family members,wards,education background and relationship with colleagues were the influencing factors of nurses' compassion satisfaction score.
2.Analysis of the usage of antibacterial drugs in 75 hernior-rhaphy inpatients during perioperative period

Objective: To evaluate the rationality of the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Methods: Seventy-five cases of herniorrhaphy discharged in March, June and September of 2009 were collected. Parameters including indications, the types of antibacterial drugs, time of administration, solvent choice, usage and dosage, combination, etc, were assessed for the rationality according to the "the guiding principles of clinical use of antibacterial drugs"and other relevant documents. Results: The antibacterial drugs application rate was 100%, while the rationali-ty of the drug selection, the drug administration route, the single dose and the number of dosing, was 70.7%, 100% , 100% and 93.3% respectively. Sixty-two patients(82.7%)were treated with antibacterial drugs at 0.5~2 hours before surgery, and the average time course of the prophylactic use of antibiotics after surgery was 2.3 days. Conclusions: Some problems existed for the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Surgeon should ensure the correct use of the antibacterial drugs to make the safety, efficiency, rationality, and economy for the patients.
3.Research status on regulation of Chinese herbal compound on intestinal microecology.

The ralationship between traditional Chinese medicine (TCM) and intestinal microecology is increasingly being given more and more attention. Combined with the devolopment of intestinal microecology disciplines, effects of TCM on regulation of intestinal microecology have been gradually explained. Both clinical studies and animal experiments have confirmed that TCM can maintain the balance of intestinal microecology and regulate the intestinal flora. The author arrangemented the documents related to Chinese herbal compound adjusting intestinal flora in the recent ten years, summarized that the Chinese herbal compound which can strength spleen and replenish Qi, relax bowels and regulate Qi, dissipate dampness and check diarrhea, clear away heat and toxic materials, promote digestion and relieve stasis had certain regulation effects on intestinal microecology, providing basis for revealing the TCM essence of intestinal microecology.

4.The status and related factors of daily activities in patients with chronic obstructive pulmonary disease

Objective To determine relationships between daily activities measured as modified version of Pulmonary Functional status and Dyspnea Questionaire (PFSDQ-M) and functional capacity and symptoms experienced in patients with chronic obstructive pulmonary disease (COPD). Methods Convenience sample of 94 COPD patients with stable condition were assessed by interview with PFSDQ-M Chinese version and modified Medical Research council dyspnea scale (MMRC) respectively. Pulmonary function test (PFT) and 6-minute walking test (6MWT) were undergone on the same day or within one week as interview. PFSDQ-M has three subscales,i.e.,change experienced by patient with activities (CA),dyspnea with activities (DA) and fatigue with activities (FA). Results Scores of CA,DA and FA correlated to 6-minute walking distancer= (-0.37)- (-0.42),FEV_1 r=(-0.27)-(-0.32),FEV_1/FVC r= (-0.27)- (-0.32),dyspnea rated by MMRC (r=0.55-0.60) and BODE index (r=0.35-0.40),respectively (all P<0.01). dyspnea explained 26% of the variance in changes of activities. Conclusions Physical activities moderately changed in patients with stable COPD;Dyspnea is the best predictor of limitation of daily activities.
5.Relationship between pancreatic beta cell function and liver function in hepatitis B cirrhosis

Objective To investigate the relationship between pancreatic beta cell function and liver function in hepatitis B cirrhosis with different glucose metabolism status.Methods A total of 247 patients with hepatitis B cirrhosis were included and divided into 3 groups according to measurement of fasting blood glucose (FBG),and 2h blood glucose in 75g oral glucose tolerance test(2hPG),normal glucose metabolic status group(group A,n =47),glucose tolerance impairment group(group B,n =103) and diabetes mellitus group(group C,n =97).Data of fasting and 2h postprandial blood glucose,C-peptide,insulin and glycosylated hemoglobin (HbA1c),pancreatic beta cell function index(HBCI),insulin sensitivity index (ISI),hepatitis B virus load were collected and analyzed.Results Abnormal glucose metabolism was observed in 81% patients with hepatitis B cirrhosis,while hepatogenic diabetes accounted for 39.3%.2 hPG[(6.29 ± 3.78) mmol/L,(10.56 ± 4.26) mmol/L,(17.34 ± 5.9) mmol/L],FBG [(4.72 ±2.15)mmot/L,(5.68 ±2.81) mmol/L,(9.82 ±5.1) mmol/L],HbA1c [(4.5 ± 1.2)% (10.56 ±4.26) % (9.5 ± 3.0) %],HBV-DNA [(3.78 ± 0.52),(4.82 ± 0.61),(6.02 ± 0.63)] were compared in group A,group B and C.2hPG,FBG,HbA1c and HBV viral loads in group A were significantly lower than group B and group C (F =93.23,41.35,84.93,237.2,P < 0.05).Fasting insulin [(15.65 ± 4.17) mU/L,(26.53 ± 7.22) mU/L,(30.18 ± 3.23) mU/L],postprandial insulin [(45.28 ± 10.22) mU/L,(106.8 ± 20.74) mU/L,(141.68 ±20.25) mU/L],postprandial C peptide [(5.96 ± 4.82) mU/L,(9.86 ± 5.46) mU/L,(9.54 ± 6.42) mU/L] and ISI [(-5.96 ± 0.61),(-4.92 ± 0.42),(-5.03 ± 0.51)] were compared in group C,group B and A,those values in group C were lower than group A and B,the defferences were stastistically significant (P < 0.05).HBCI in three groups were (5.66 ± 0.64),(5.32 ± 1.01),(4.30 ± 1.53),respectively,the defferences were stastistically significant(F =27.55,P <0.05).Patients in group C with Child-Pugh C score was much more than group A and B,the defference was stastistically significant (x2 =48.6,P < 0.01).Conclusion Hyperinsulinemia,increased insulin resistance and decreased insulin secretion exist in hepatitis B cirrhosis patients,and they are closely related to liver function.
6.Effects ofLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets on complications of type 2 diabetes mellitus

Objective To investigate the effects ofLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets on diabetic complications in patients with type 2 diabetes mellitus (T2DM). Methods Seventy-eight patients with T2DM were randomized into a TCM treatment group and a conventional treatment group by random number table method, 39 in each group. The patients in the conventional treatment group were regularly using antihyperglycemic drugs or insulin. On the basis of conventional treatment group, the patients in the TCM treatment group were additionally treated withLiuwei-Dihuang Pills and Ginkgo Biloba Leaves Extract Tablets. Intima-media thickness (IMT) of internal carotid artery, vibration perception threshold (VPT) and diabetic retinopathy in both groups were detected before the treatment, 12 and 36 months after the treatment.Results The VPT in the left foot 12 months after the treatment in the TCM treatment group were significantly lower than that in the conventional treatment group (13.98 ±4.38 Vvs. 18.70 ±2.43 V;t=2.764, P=0.008). IMT of internal carotid artery (left: 0.81 ± 0.16 mmvs.0.70 ± 0.10 mm,t=3.120,P=0.003; right: 0.81 ± 0.17 mmvs. 0.73 ± 0.12 mm,t=2.286,P=0.026), VPT (left foot: 14.82 ± 6.45 Vvs. 20.63 ± 9.75 V,t=2.714, P=0.009; right foot: 16.73 ± 7.10 Vvs. 20.73 ± 10.35 V,t=2.001,P=0.048) and incidence of diabetic retinopathy (35.9%vs. 53.8%;χ2=5.804,P=0.016) 36 months after the treatment in the TCM treatment group were significantly lower than those in the conventional treatment group.ConclusionsLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets can effectively reduce the progression of diabetic complications in patients with T2DM.
7.Detection of expressions of Ck2α,β-catenin and survivin in ovarian cancer tissue and its clinical application value

Objective To study the expressions of casein kinase 2α(Ck2α),β-catenin, survivin in ovarian cancer tissue and analyze the relationships between them, and to explore the clinical application value. Methods Immunohistochemistry staining was used to detect the expressions of Ck2α,β-catenin and survivin in 8 cases of simple ovarian cysts tissue,8 cases of ovarian benign tumor tissue and 29 cases of ovarian cancer tissue;the correlations of the expressions of Ck2α,β-catenin,survivin in ovarian cancer tissue and their associations with the clinicopathologic characteristics were analyzed.Results The expression levels of Ck2α,β-catenin and survivin in ovarian cancer tissue were significantly higher than those in simple ovarian cysts tissue and ovarian benign tumor tissue(P<0.05);the Ck2αexpression level in ovarian cancer tissue was positively corelated with the expression levels ofβ-catenin and survivin(r=0.438,r=0.479,P<0.05);the expression levels of Ck2α,β-catenin and survivin in low-medium differentiation group were significantly higher than those in high differentiation group (P<0.05);the expression levels of Ck2α,β-catenin and survivin in stagesⅡandⅢ group were significantly higher than those in stage Ⅰ group(P<0.05).Conclusion The protein expression levels of Ck2α,β-catenin and survivin are increased in ovarian cancer tissue, and three are positive corelations between them;Ck2α,β-catenin and survivin may play an important role in the occurrence and development of ovarian cancer;the combined detection of them has important clinical value.
8.Suppression of chondrocyte apoptosis in the osteoarthritis by interleukin-1Ra ex vivo gene therapy

Objective To explore suppression of chondrocyte apoptosis by IL-1Ra gene transfer to the knees of experimental osteoarthritis (OA).Method ①Thirty New Zealand white rabbits were divided into 3 groups,partial synovectomy of the left knee had been done.Synovial fibroblasts (SF) were isolated and cultured.②Fibroblasts from each rabbit were transduced by using the retrovirus with either IL-1Ra gene (group 3) or marker gene to (group2) in vitro.③ OA was induced in right knee of rabbits by anterior cruciate ligament transection (ACLT).④ The autologous cells transduced with IL-1Ra gene or marker gene were injected into the right knee of group 3 or 2.⑤At the second week and at the forth week,IL-1Ra level in SF was determined by ELISA.⑥Cartilage section were stained for checking histologic changes and chondrocyte apoptosis were determined using TUNEL method.Results The IL-1Ra level in the SF of group 3 were (20.6?1.8)ng/ml and (4.82?0.52)ng/ml after 2 and 4 weeks,after gene delivered respectively,and the IL-1Ra level in the group 2 and group 1 were very lowat both 2 and 4 weeks.Approximately 40% of chondrocytes underwent apoptosis in group 2,whereas apoptosis were inhibited in group 3,only 18% of chondrocytes underwent apoptosis,and 2%~5% of chondrocytes underwent apoptosis in normal joint.Conclusion This study showes that a local increase IL-1Ra level in OA keen joints by intraartilcular injection of IL-1Ra gene transduced synovial cells can suppress articular cartilage chondrocyte apoptosis.
9.Study on mechanism of sleep disorders associated with Parkinson's disease

Sleep disorders are commonly occurred among patients with Parkinson's disease,such as difficulties in the initiation of sleep,fragmented sleep,sleep behavior disorder and excessive daytime sleepiness.The mechanism of sleep disorders associated with Parkinson's disease is not clear,which may be associated with the injury of brain stem,nuclei of median raphe,nuclei fasciculi solitarii,thalamencephalon and the changes of neurotransmitters as dopamine,hypocretin(orexin) and melatonin.This article gives an overview of the mechanism of sleep disorders associated with Parkinson's disease.
10.Effect of specific immunotherapy on GM-CSF and IL-5 in the tissues of recurrent nasal polyps.

OBJECTIVE:
To study the mechanism and clinical significance of specific immunotherapy (SIT) on the expression changes of GM-CSF and IL-5 in the tissue samples of recurrent nasal polyps.
METHOD:
Perennial allergic rhinitis patients with recurrent nasal polyps were randomly divided into 2 groups. The experimental group of 19 patients was treated by SIT and standardized treatment (glucocorticoid nasal spray) , and the control group of 17 patients was only treated by standardized treatment (glucocorticoid nasal spray). We measured the expression levels of GM-CSF and IL-5 in the tissue samples of the nasal polyps by ELISA, and compared the results obtained before treatment with expression levels detected at 6 months and 1 year after the treatment.
RESULT:
The expression of GM-CSF and IL-5 in the recurrent nasal polyps reduced significantly (P < 0.05) in both groups after 6 months and 1 year post-treatment compared with pre-treatment, and the expression of GM-CSF and IL-5 in the experimental group was much lower than that of the control group.
CONCLUSION
SIT decreases the expression of GM-CSF and IL-5 and reduces the inflammatory reaction in the tissue samples of recurrent nasal polyps.
