OBJECTIVETo compare clinical effects of spinal leveraging manipulation and medicine for the treatment of degenerative scoliosis in pain and function.
METHODSFrom July 2010 to June 2013, 38 patients with degenerative scoliosis were randomly divided into spinal leveraging manipulation group and medicine group by coin tossing. In manipulation group, there were 9 males and 11 females aged from 58 to 74 years old with an average of (66.63±7.73), the courses of diseases ranged from 3 to 8 months with an average of (5.65±2.58), spinal leveraging manipulation(following meridian to straighten tendon, relieving spasm, osteopathy and massage, clearing and activating the channels and collaterals) were performed for 30 min, once a day, 4 days for a period treatment, totally 9 courses. In medicine group, there were 8 males and 10 females aged from 57 to 70 years old with an average of (63.51±6.61) the courses of diseases ranged from 3 to 5 months with an average of (4.82±1.43), celecoxib with eperisone hydrochloride were orally taken, 4 days for a period treatment, totally 9 courses. VAS score, Cobb angle and ODI score were measured.
RESULTSAfter treatment, VAS score in manipulation group was (5.38±0.99), (6.36±1.31) in medicine group, and had significant meaning (t=2.618, P<0.05); there was significant differences in Cobb angle between manipulation group (16.51±4.89)° and medicine group (19.85±5.03) °(t=2.074,P<0.05); and had obviously meaning in ODI score between manipulation group (20.20±2.93) and medicine group (26.01±3.11) (t=5.592, P<0.05).
CONCLUSIONSpinal leveraging manipulation for degenerative scoliosis could regulate muscle balance on both side of spine, correct coronal imbalances in spine, recover normal sequence of spine, reduce and remove opperssion and stimulation of nerve root, relieve pain in leg and waist and further improve quality of life.

Chinese Journal of Digestive Surgery 2009;8(4):290-293
Objective To analyze the clinicopathological characteristics, immunophenotypes and immu-noglobulin heavy chain gene rearrangement of gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Methods The clinical data of 35 patients with gastric MALT lymphoma who had been admitted to Southwest Hospital from January 1984 to June 2003 were retrospectively analyzed. The specimens of the gastric MALT lymphoma were obtained and their pathomorphological classifications were studied. The immunophenotypes and immunoglobulin heavy chain gene rearrangement were detected by immunohistochemistry and polymerase chain reaction, respectively. All the data were analyzed by chi-square test and Mann-Whitney U test. Results The main manifestations of patients with gastric MALT lymphoma were epigastric discomfort and abdominalgia. The percentage of patients with age≥40 accounted for 74% (26/35). The gastric MALT lyrnphoma has low differentia-tion in 32 patients, and high differentiation in 3 patients. There were 31 patients in Ⅰ E stage, 2 in Ⅱ E1 stage and 2 in Ⅳ stage. Pathological examination showed that gastric MALT lymphoma was composed of centrocyte-like cells and mononuclear-like B cells or lymphoplasma cells. Twenty-nine specimens were with lymphoepithelial lesions (83%), 2 with follicular colonization (6%), and 11 with plasma cells segregated beneath the surface epithelium (33%). All the 35 specimens were B cell immunotype. Restrictive expression of κ and λ light chains of the immunoglobulin was found in 19 cases, and Bcl-2 positive expression was found in 16 cases. High expres-sion of proliferating cell nuclear antigen was found in gastric MALT lymphoma with transformed blasts, which indicated that gastric MALT lymphoma with transformed blasts was more active in proliferation than those of low grade. A monoclonal pattern of immunoglobulin heavy chain gene rearrangement was detected in 33 patients (94%). Conclusions Digestive symptoms are the main manifestations of patients with gastric MALT lymphoma. The incidence of gastric MALT lymphoma increases with age. The final diagnosis of gastric MALT lymphoma depends on the pathological, immunohistochemical examination and laboratory test.

Objective To investigate thyroid hormone nuclear receptor expression in peripheral blood mononuclear cells (PBMCs) and its relation with injury severity in patients with severe multiple trauma. Methods Twenty-eight patients with severe muhiple trauma and another 26 healthy subjects as control were enrolled in the study. At 2 days after injury, the levels of free triiodothyronine (FT3),free thyroxine (FT4) and thyroid-stimulating hormone (TSH) in peripheral venous blood were determined by using immunoradiometry, and the expressions of mRNA of the thyroid nuclear receptors (TRα and TRβ) in PBMCs were measured by using real-time RT-PCR.Results FT3 concentration in peripheral venous blood was significantly decreased in patients with severe multiple trauma versus healthy subjects.Concurrently, the expressions of TR mRNA (TRα and TRβ) were significantly decreased in PBMCs of patients with severe multiple trauma compared with healthy subjects (TRα mRNA:3.86 ±0.54 vs.5.24 ± 1.17,P <0.05;TRβ mRNA:9.86±2.27 vs.13.57 ±2.45,P <0.05).Pearson correlation analysis showed that FT3 concentration and the expression of TRβ mRNA correlated negatively with injury severity score (ISS) in patients with severe multiple trauma (r=-0.445,-0.496,P=0.018,0.007).Conclusions These data provide the evidence of a lowered activity of the thyroid signaling pathway in PBMC and a significantly negative correlation between FT3 concentration and the expression of TRβ mRNA in respect of injury severity in patients with severe multiple trauma.Additional investigations are needed to further determine the roles of the thyroid signaling pathway in adverse outcomes in the wake of severe multiple trauma.

Objective To analyse a novel splice mutation in EXT1 gene of hereditary multiple osteochondroma, and study its pathogenic mechanism. Methods In April of 2013, the proband was hospitalized from the outpatient department with multiple joint deformity for more than 20 years, peripheral blood of the proband and his parents were collected and genomic DNA was extracted .Coding regions and adjacent intron sequences of EXT1/EXT2 genes in genomic DNA of the family members were amplified and sequenced.Bioinformatics was used to analyze the mutation from sequencing .cDNA from peripheral blood of the proband ,the mother and normal control was made respectively as a template for amplifying coding regions of EXT1 gene, and the product was T-A cloned and sequenced. The abnormal transcripts of each group were counted and analyzed using chi square test to study the pathogenic mechanism of the mutation .Results Sequencing results of family members revealed that there was a heterozygous deletion mutation ( c.1284 +2del) in the 5′splice site of intron 4 in EXT1 gene of the proband and his mother .Bioinformatics predicted that exon 4 of EXT1 gene was skipping or spliced aberrantly due to the mutation .T-A clone and sequencing results as well as the statistical analysis suggested that there was a significantly higher proportion of transcripts with skipping exon 4 in the proband and his mother compared with the normal control (P=0.000, P<0.01).Conclusions c.1284+2del in EXT1 gene is reported for the first time internationally , which results in a considerable number of abnormal transcripts with skipping exon 4 in EXT1 gene, thereby influences the normal transcription and translation of EXT1 gene.

Aim To investigate the method of culturing vascular smooth muscle cells (VSMC) of coronary arteries derived from experimental diabetic rats and to establish the model of VSMC for the study of chronic cardiovascular diseases of diabetes. Method After the establishment of the model of diabetic rats, the coronary arteries of rats were separated carefully to culture VSMC using the method of explanting culture. Results The primary culture of VSMC was performed successfully by the method mentioned above. The cells could be isolated by different kinds of digestive enzyme in three steps. 7 ~10 days afterward, the cells overlapped in layers and displayed the typical ‘hill and valley’ pattern and positive immunochemical staining for smooth muscle actin. Conclusion VSMC from experimental diabetes proliferates faster than normal VSMC and its cultivating condition is strictly limited. The morphology of diabetic VSMC is as same as that of normal rats.

ObjectiveTo explore the mechanism and prevention for the hypertension complicated by the retroperinoneal paraganglioma operation.MethodsBlood pressures were monitored during operations on 15 patients with retroperinoneal paraganglioma. Also, the reason and the prevention for the changed blood pressure were studied emphatically during operation.ResultsThe elevated blood pressure of 12 patients was detected during operation, and all dropped to a varied degree after operation, with 2 of them below the normal value.ConclusionThe retroperinoneal paraganglioma operation was liable to cause blood pressure elevated, which was related to the anesthesia induction and the operation stimulation of tumor. After the excision of the tumor, blood pressure of most of the patients dropped to some degree. Therefore, during the retroperinoneal paraganglioma operation, the blood pressure should be monitored closely, and the short-acting antihypertensives should be applied on time. Meantime, occurrence of hypotension was also considered after the excision of the tumor.

Class Ⅱa bacteriocins can be considered as the major subgroup of bacteriocins from lactic acid bacteria, not only because of their large number, but also because of their activities and potential applications. They have first attracted particular attention as listericidal compounds and are now believed to be the next in line if more bacteriocins are to be approved in the future. The present review attempts to provide an insight into general knowledge available for class Ⅱa bacteriocins and discuss common features and recent findings concerning these substances.
