1.Study on the Blood Concentration of Fluorouracil Administered into Guinea Pigs by Arterial&Intravenous Infusion and Celiac Infusion

OBJECTIVE:To compare and analyze blood concentration changes of fluorouracil administered to guinea pigs by different paths.METHODS:The peripheral vein and portal vein blood samples of guinea pigs were taken at different time after administered with fluorouracil in3different ways:group A(by arterial infusion),group B(by intravenous infusion)and group C(by celiac infusion).The blood concentration changes were determined and analyzed by HPLC.RESULTS:The portal vein and peripheral vein blood concentrations in group A and B reached peak value5min after administration with flu?orouracil while group C reached peak at10min;the portal vein blood concentrations in group A reached the highest in the first20min while group C showed the highest value in the last30min.CONCLUSION:Arterial infusion combining celiac infusion path was recommended for local chemotherapy;arterial infusion combining intravenous infusion or intravenous infusion com?bining celiac infusion was recommended for the local therapy while giving consideration to systemic chemotherapy.
2.Study on Preparation and Quality Standard of Flu Mixture

OBJECTIVE: To prepare Flu mixture and establish its quality standard. METHODS: The methods of decoction- boiling and distillation were adopted to prepare the flu mixture; TLC was used to identify Radix Et Rhizoma Rhei and Rhizoma Curcumae Longae, and HPLC was used to determine the content of Artemisinin. RESULTS: The spots characteristic of Radix Et Rhizoma Rhei and Rhizoma Curcumae Longae. were clearly identified with TCL. A good linearity was seen of Artemisinin in the range of 0. 42~ 2. 10? g( r=0. 999 3) . The recovery rate was 99. 84% ( RSD=2. 15% ) . CONCLUSIONS: The preparation is simple in preparation technique and good in stability. The TLC method is highly exclusive. The HPLC method is simple, accurate, and can be used for the quality control of flu mixture.
3.STUDIES ON THE PHOSPHOLIPIDS AND FATTY ACIDS IN TRADITIONAL CHINESE MEDICINAL MATERIALS HIPPOCAMPUS AND SYNGNATHUS

This paper deals with the analysis of the phospholipids and fatty acids in Hippocomnpus kelloggi Jordan et Synder, H. Histrix Kaup, II. trimaculalus Leach, II. kuda Bleeder, II. japonicus Kaup, Solenognathus hardwickii(Gray), Syngnathoides biacufcatus(Bloch) and Tracliyrliamphus serralus (Temminck &. Schlegel).The total phospholipids were extracted with Folch solvent and determined by molybdate blue spec-trophotometric method. The result showed that the contents of total phospholipids in these spccimins were 2. 56-7. 82mg/g.The nine kinds of phospholipid standards of absorbance proportional coefficient were detected in this paper. The phospholipid components were separated and determined by using thin - layer chro-matographic method and Vaskovsky solvent as a specific spray reagent. The quantitative analysis of the distribution of various phospholipids components in these specimen ts were carried out, by using TLC scanning and the corrective method of absorbance proportional coefficient. The resut showed that the Hippocampus contains about five phospholipids: lysophosphatidylcholine, sphingornyelin, phophatidylcholine, phosphatidylinositol, and phosphatidylethanolamine. The phosphatidylserine , phosphatidylglycerol, diphosphatidylglycerol and phosphatidic acid were more in Syugnathus than in Hippocampus, But in all these specimens, the main phospholipid components were lysophos- phatidylcholine, sphingornyelin and phosphatidylcholine.Fourteen fatty acid compositions in these specimens were analysed by GC -MS -DS. The result showed that the main fatty acid were hexadecanoic acid, 9 - octadecenoic acid, 8,11- octadeca-dienoic acid and 1,7,10,13,16,19 - docosahexaenoic acid.

5.Analysis on the pathogenic bacteria distribution and drug resistance of the patients in PICU with lower respiratory tract infection

Objective To explore the significance of sample cultivation and drug resistance of the patients in PICU with severe lower respiratory tract specimens.Methods 1 107 pathogenic bacteria of lower respiratory tract samples collected were analyzed and the drug resistance was kept watch on.Results A total of 1107 samples were cultured into 948 strains of pathogens,with the positive rate of 85.64%,and 616 strains of Gram negative bacteria accounting for 64.98%,of which 145 strains of Pseudomonas aeruginosa(15.30%),Klebsiella pneumoniae 135(14.24%),Acinetobacter spp 131(13.82%),large intestine Egypt Greek bacteria 89(9.4%);Gram positive cocci 129(13.61%),the main pathogen was staphylococcus aureus and Fungi 203(21.41%),the main pathogen was candida albicans.Carbapenems was the most active antibiotics for gram negative bacill,and vancomycin for gram positive cocci.Conclusion Infectious bacteria have high selectivity to the sensitive antibiotics,and doctors should carefully use antibiotic according to the susceptibility test results in order to reduce hospital infection and the emergence and spread of resistant strains.
6.Effect of glucocorticoid by oral medication or intrathoracic injection on the expression of IL-6,IL-8 and sIL-2R in serum and pleural fluid in patients of tuberculous pleurisy

Objective To observe the effect of gheocorticoid by oral medication or intrathoracic iniection on the expression of IL-6,IL-8 and sIL-2R in gerum and pleural fluid in patients of tuberculous pleurisy.Methods Twenty tuberculous pleurisy patients were treated with ghcocortieoid by oral medication (oral group)and 20 cases by intrathoracic injection(injection group).ELISA wag employed to detect the levels of IL-6,IL-8 and sIL-2R in serum and pleural fluid before treatment and 3,6 and 9 days after treatment.Results The levels of serum IL-6 and IL-8 decreased and sIL-2R increased obviously in injection group at 3,6 and 9 days after treatment(P<0.01).Meanwhile the levels of IL-6 and IL-8 in pleural fluid decreased and sIL-2R increased obviously(P<0.01).The levels of IL-6 and IL-8 were significantly lower and the levels of aIL-2R were significantly higher in injection group than those in oral group(P<0.01).One month after treatment,the rate of complete absorpion of pleural fluid was higher in injection group(80%)than thatin oral group(45%),the rate of pachynsis pleurae was lower in injection group(10%)than that in oral group(40%),P<0.05.Conclusions Intrathoracic injection of ghcocorticoid shows stronger suppression of cellular immune function in cavum pleurae and weaker suppression of the general immune sysmm than oral medication.Intrathoracic injection of glucocorticoid can increase the therapeutic effect and decrease side effect in tuberculous pleurisy.
7.The clinical features of acute coronary syndrome combined with low T3 syndrome

Objective To investigate the clinical features of acute coronary syndrome (ACS) combined with low T3 syndrome and its prognosis.Method Eighty-seven ACS patients with low free triiodothyronine (FT3) level (low FT3 group) and additional 64 ACS patients with normal FT3 level(control group) were selected in parallel; all the patients underwent coronary angiography.The contents of FT3,N-terminal pro-brain natriuretic peptide (NT-proBNP),cardiac troponin I (cTnI) and high sensitivity-C reactive protein (hs-CRP) were recorded,and the related factors were evaluated.The number of stenotic vessels was calculated.Results The contents of NT-proBNP,cTnI and hs-CRP in low FT3 group were elevated significantly compared with those in control group [(1242.14 ± 226.32) ng/L vs.(1126.36 ±195.55) ng/L,(0.92 ± 0.23) ng/L vs.(0.84 ± 0.19) ng/L,(6.46 ± 2.25) mg/L vs.(5.71 ± 1.78) mg/L],and there were significant differences (P < 0.01 or < 0.05).The contents of FT3 were negatively correlated with NT-proBNP,cTnI and hs-CRP in two groups (P < 0.05).There was no significant difference in the number of stenotic vessels between two groups (x2 =4.512,P =0.105).Conclusion In patients with ACS,there is a negative correlation between contents of FT3 and the prognosis,but there is no correlation between contents of FT3 and the number of stenotic vessels.
8.Clinicopathological analysis of gastric mucosa-associated lymphoid tissue lymphoma

Objective To analyze the clinicopathological characteristics, immunophenotypes and immu-noglobulin heavy chain gene rearrangement of gastric mucosa-associated lymphoid tissue (MALT) lymphoma. Methods The clinical data of 35 patients with gastric MALT lymphoma who had been admitted to Southwest Hospital from January 1984 to June 2003 were retrospectively analyzed. The specimens of the gastric MALT lymphoma were obtained and their pathomorphological classifications were studied. The immunophenotypes and immunoglobulin heavy chain gene rearrangement were detected by immunohistochemistry and polymerase chain reaction, respectively. All the data were analyzed by chi-square test and Mann-Whitney U test. Results The main manifestations of patients with gastric MALT lymphoma were epigastric discomfort and abdominalgia. The percentage of patients with age≥40 accounted for 74% (26/35). The gastric MALT lyrnphoma has low differentia-tion in 32 patients, and high differentiation in 3 patients. There were 31 patients in Ⅰ E stage, 2 in Ⅱ E1 stage and 2 in Ⅳ stage. Pathological examination showed that gastric MALT lymphoma was composed of centrocyte-like cells and mononuclear-like B cells or lymphoplasma cells. Twenty-nine specimens were with lymphoepithelial lesions (83%), 2 with follicular colonization (6%), and 11 with plasma cells segregated beneath the surface epithelium (33%). All the 35 specimens were B cell immunotype. Restrictive expression of κ and λ light chains of the immunoglobulin was found in 19 cases, and Bcl-2 positive expression was found in 16 cases. High expres-sion of proliferating cell nuclear antigen was found in gastric MALT lymphoma with transformed blasts, which indicated that gastric MALT lymphoma with transformed blasts was more active in proliferation than those of low grade. A monoclonal pattern of immunoglobulin heavy chain gene rearrangement was detected in 33 patients (94%). Conclusions Digestive symptoms are the main manifestations of patients with gastric MALT lymphoma. The incidence of gastric MALT lymphoma increases with age. The final diagnosis of gastric MALT lymphoma depends on the pathological, immunohistochemical examination and laboratory test.
9.Bone ultrasound backscatter in the detection of bone nutritional status in term infants

Objective To establish a reference range of cancellous bone ultrasonic backscatter coefficient (BSC) in term neonates,and to determine the significance of BSC in bone nutritional status in term newborns and infants.Methods Six hundred and sixty-five term newborns,who were born in Shanghai First Maternity and Infant Hospital from July 2014 to August 2014,were enrolled.On the third day and the 42nd day after birth,all infants underwent ultrasonic measurement of the calcaneum bone using a broadband-non-focus ultrasonic transducer at three frequencies (2.25,3.5 and 5 MHz).MATLAB software was used to analyze the backscatter signals.Clinical data of the infants,including gestational age,birth weight and birth age,were recorded at the same time.Statistical analyses of the data included variance analysis,Tukey's test,rank sum test,Dunn test and Pearson or Spearman correlation analysis.Results (1) Of the 665 infants,335 were male.The mean gestational age at birth was (38.0± 1.9) weeks,and the mean birth weight was (3 1524±226) g.(2) On the third day,there were significant differences in BSC values acquired at 2.25 and 3.5 MHz among the groups with different gestational ages or birth weights (2.25 MHz:H=9.842 and 17.271;3.5 MHz:H=6.275 and 21.450,respectively,all P<0.05).At 3.5 and 2.25 MHz,BSC values were positively correlated with gestational age (r=0.047 and 0.035,respectively,both P<0.05) and birth weight (r=0.125 and 0.186,both P<0.05).(3) On the 42nd day,significant differences in BSC were observed at each frequency among the groups with different gestational ages or birth weights (gestational age:H=76.832,16.498 and 32.756;birth weight:H=70.014,18.095 and 34.126;all P<0.01).Gestational age and birth weight were positively correlated with BSC value acquired at each frequency (r=0.397,0.286 and 0.272 for gestational age,0.451,0.223 and 0.196 for birth weight;all P<0.01).Conclusions A preliminary BSC reference percentile in term-born infants is established.There are positive correlations between BSC value and newborn gestational age and birth weight at certain frequencies,which can reflect the status of the newborn's cancellous bone.Determination of BSC value should be performed during the follow-up period to assess the nutritional status of neonatal bone.
10.The Research and Application Effects of Systematic Anatomy Learning System Based on Android Platform

Objective To implement a systematic anatomy learning system based on android platform and evaluate the application effects of the learning system in clinical medical undergraduate students.Methods The systematic anatomy learning system was based on android development package and correlation functions were implemented using java technique.108 undergraduate students in clinical major of 2014 in Kunming Medical University were selected and randomly divided into experimental group and control group.Students of the experimental group were given the traditional teaching method and this learning system for autonomous learning,while students of the control group were given the traditional teaching method.These two teaching methods were subjectively and objectively evaluated by questionnaire survey and final examination respectively.Results The questionnaire showed that the 87.5% of students thought that it was necessary to use the learning system.The 90.74% of students in experimental group thought their learning effect were improved and the 98.15% of them thought themselves learning ability were improved.The average score of the experimental group and the control group was 83.56 ± 5.61 and 75.33 ± 4.62 respectively.The difference between the two groups was statistically significant (P<0.05).Conclusion The systematic anatomy learning system can help students to understand and grasp the knowledge points of the systematic anatomy,and it can improve the teaching effect too.It will provide research foundation for the next step of applying for patent.
