Objective To characterize the growth curve of Brucella in Bact/Alert blood culture system which may be helpful to predict the growth of Brucella strain for earlier clinical diagnosis of brucellosis. Methods The epidemiological, clinical and labo-ratory data including growth curve of Brucella were reviewed and analyzed retrospectively for 15 cases of brucellosis as con-firmed by positive serological test and brucellosis agglutination test. Results All the 15 patients had irregular fever and history of animal exposure, even though their clinical feature and signs varied greatly. The growth curve of Brucella in blood culture showed the same chracteristics in the 15 patients, including:the time to positive alarm was about 72 hours,a longer lag phase, shorter logarithmic phase,a shorter vertical axis corresponding to the logarithmic phase, and a flat stable phase. Conclusions The clinical manifestation of brucellosis is variable and non-specific.Lack of awareness of this disease makes the clinicians mis-diagnose it easily. Therefore, blood culture is critical for clarifying the etiology in febrile patients. The growth curve of bacteria during blood culture is useful for early diagnosis of brucellosis and prevention of laboratory infections.

Chinese Journal of Stomatology 2007;42(5):288-291
OBJECTIVETo explore the feasibility of transplantation of frozen autogenous mandible with delayed implantation.
METHODSOperations were performed to create two defects in the bilateral mandible of 16 dogs. The left defect was grafted by composite transplantation of frozen autogenous mandible (immersed in -196 degrees C liquid nitrogen) with fresh cancellous ilium (composite transplantation group, CTG). The right defect was grafted by fresh ilium (iliac transplantation group, ITG). Three months after transplantation one IMZ TPS dental implant was placed into the graft of each side. At 3, 6, 9, 12 weeks postoperatively, 4 animals were sacrificed respectively and the grafts with dental implant were harvested for gross observation, X-Ray examination and histological evaluation to compare peri-implant bone healing between composite transplantation group and iliac transplantation group.
RESULTSThere was no absorbing bone density reducing image of peri-implant at each stage. The quantified X-Ray gray extent displayed obvious variation of interfacial bone density between two kinds of grafts at 3 weeks, 6 weeks and 9 weeks after implantation. The composite transplantation group obviously surpassed the iliac transplantation group. At 12 weeks after the implantation, there was no significant difference between the peri-implant bones of both sides. There was satisfactory osseointegration between the implants and the two kinds of grafts. The healing style of peri-implant bone was similar.
CONCLUSIONSGood osseointegration was performed between the implant and the composite transplantation of frozen autogenous mandible following delayed implantation.

Objective To investigate the clinical and pathological features of small cell carcinoma of the urinary bladder. Methods The pathological and clinical data of 9 cases of small cell carcinoma were analyzed retrospectively. There were 6 males and 3 females, ages 45 to 79 years (mean age, 62 years). Clinical manifestations of 7 cases included gross hematuria and dysuria, the other 2 cases experienced lower abdominal pain. The mean tumor size was 2.0 cm (ranged, 0.5 to 7.0 cm). Two cases had multiple tumors and 5 cases had single tumors. The growth pattern in 2 cases was diffuse growth in the whole bladder. In 4 cases tumor cells were found in urine cytology. All 9 patients underwent surgical treatment, including TURBt. Four patients were diagnosed as superficial tumors before operation. All the patients underwent regular theprubicine irrigation in the bladder. One case underwent additional intravenous chemotherapy for 3 cycles. Partial cystectomy was performed in 2 cases, with regular theprubicine irrigation in bladder and 1 case underwent intravenous chemotherapy for 2 cycles. Radical cystectomy was performed in 3 cases, with 2 cases undergoing intravenous chemotherapy after operation. Results Pathological findings showed that tumor cells were small and round in shape. These hyperchromatic nuclei showed limited cytoplasm with lack of nesting characters. CgA and NSE were positive in immunohistochemistry. The final diagnosis was small cell carcinoma, with 1 case accompanied with transitional cell carcinoma and 1 case accompanied with prostate cancer. One case showed high preoperative serum calcium (3.15 mmol/L) and low serum phosphate (0.61 mmol/L), which returned to normal 1 month after operation. Four cases who′s bladder was preserved were followed up, 3 cases were alive for 4, 9 and 25 months after operation. The 1 case who underwent intravenous chemotherapy was followed up for 24 months and there was no sign of relapse or metastasis. In all the 3 cases with radical cystectomy, 2 cases died 2 and 28 months postoperativly. Another case with adjuvant chemotherapy was followed up for 24 months without recurrence or metastasis. Conclusions Small cell carcinoma of the urinary bladder is highly malignant with poor prognosis. Radical cystectomy in combination with systemic chemotherapy has better efficacy. Retained bladder surgery with systemic chemotherapy is an alternative choice. The most important factors which influence the prognosis of the tumor are clinical stage and therapeutic methods.

Objective To evaluate the feasibility of monitoring the gene expression of VEGF165 via the diglycylcysteine (GGC) reporter gene system by reporter probe of 99Tcm-GH. Methods DNA fragments encoding GGC binding motifs were prepared by PCR and positioned at the C end of VEGF165 gene after the linearization of pcDNA3-VEGF165 plasmid. A replication-defective adenovirus vector Ad5-VEGF165GGC motif-internal ribosomal entry site(IRES) -enhanced green fluorescent protein (EGFP) (Ad5-VIE)was constructed, with a cytomegalovirus (CMV) early promoter driving the expression of VEGF165 gene,GGC motif and EGFP, under the aid of an IRFS. A replication-defective adenovirus carrying the Ad5-EGFP was used as the control. Mensenchymal stem cells (MSC) were infected with the recombinant adenovirus at a multiplicity of infection (MOI) from 0 to 100 infectious units (0,10,25,50,100). The cellular uptake of 99Tcm-GH in infected MSC were then studied at 30, 60, 90 and 120 min. VEGF165 was detected by quantitative reverse transcriptase real-time PCR (RT-PCR), Western-blot, and immunohistochemisty. EGFP was observed by RT-PCR and fluorescence microscopy. The correlation analysis was studied between the cellular uptake of 99Tcm-GH and the expression of VEGF165. SPSS 13.0 was applied for statistical analysis. Independent samples t-test, q-test and Pearson correlation coefficient were used. Results After infected with different viral titer of Ad-VIE, the cellular uptake of 99Tcm-GH increased with the increasing virus titer(r2 =0.86, P <0.05), with the peak rate (7.94 ±0.75) % at MOI = 100. In time-dependent uptake study, the cellular uptake rates increased rapidly with the time extension, and the highest uptake occurred at 120 min with the peak uptake rate (7.72 ±0.22)%. The uptake rates of 99Tcm-GH in Ad5-VIE-infected cells were significantly higher than those of Ad5 -EGFP-transfected cells at all time points (t = 15.10- 54.92, all P <0.05). The VEGF165 and EGFP mRNA levels increased with increasing virus titer, and the VEGF165 mRNA correlated well with the EGFP mRNA(r2 = 0. 99, P < 0.05). After infected with different MOI of Ad5-VIE, good relationship was found between the cellular uptake of 99Tcm-GH and the expression of VEGF165protein in MSC(r2 =0.90, P <0.05). Inmunohistochemisty showed VEGF165 protein expressed obviously at Ad5-VIE-infected MSC, and the EGFP was observed by fluorescence microscopy. Conclusions The cellular uptake of 99Tcm-GH in Ad5-VIE-infected MSC are well correlated with the expression of VEGF165 in vitro. The expression of therapeutic gene VEGF165 can be monitored by the GGC peptide expression.

PurposeTo explore MRI manifestations and diagnostic value of progressive massive fibrosis (PMF) in patient with coal workers' pneumoconiosis. Materials and Methods Twenty-four patients with PMF coal workers' pneumoconiosis underwent conventional MRI including T1WI, T2WI and SPIR. The signal intensity and morphological changes were analyzed. Results In 24 patients, there were 16 cases with bilateral pulmonary lesions and 8 cases with unilateral lesions. PMF was located in the upper lung field in 11 cases and in the upper and middle lung fields in 13 cases. The morphology of the lesions was stripe, round or irregular in shape. PMF were mainly isointense on T1WI, similar to adjacent soft tissue, and heterogeneous iso- or hypointense on T2WI and SPIR with patchy or stripe-like hypointensity. Conclusion PMF of coal workers' pneumoconiosis appears as a mass with iso- or hypointensity on T2WI and SPIR, with characteristic patchy-or stripe-like hypointensity. MRI is helpful in diagnosing PMF.

OBJECTIVETo study clinical value of shear wave elastography (SWE) in the evaluation of neck-shoulder myofascial pain syndrome.
METHODSFrom December 2013 to July 2014,30 patients diagnosed as neck-shoulder myofascial pain syndrome were in the treatment group, including 17 males and 13 females, with an average age of (44 ± 3) years old. Thirty healthy people were in the control group, including 22 males and 8 females, with a mean age of (37 ± 5) years old. The patients in the treatment group were treated with manipulation, once every other day, total 7 times. The SWE was used to detect tension part of trapezius muscle of patients in the treatment group before and after treatment, as well as to detect muscle belly at the descending part of trapezius muscle in the control group. The tissue elasticity and Yang's modulus value were recorded and compared.
RESULTSThe tissue elasticity chart of patients in the treatment group before treatment was mainly greenish blue with the score of 3.70 ± 1.53, and the Yang's modulus was (43.4 ± 15.6) kPa. The tissue elasticity figure after treatment was mainly blue with the score of 2.40 ± 0.87, and the Yang's modulus was (29.0 ± 5.9) kPa. Whereas in the control group, the tissue elasticity figure was mainly blue with the score of 1.60 ± 0.72, and the Yang's modulus was (24.0 ± 7.6) kPa. These were statistical differences between the two groups (P = 0.000).
CONCLUSIONSWE can be used as an evaluation method of manipulation treatment for neck-shoulder myofascial pain syndrome, which is an objective and sensitive detection method.
