1.Tissue-engineered neural complex seeded in modified collagen scaffold for treatment of acute spinal cord injury in adult rats

Objective To repair the injured spinal cord in adult rats using tissue-engineered neural complex constructed in vitro by tissue engineering techniques and discuss the treatment effect.Methods Neural stem cells (NSCs) from rats were cultured separately and modified collagen scaffold was also prepared to construct complex of NSCs /modified collagen scaffold in vitro.Embryo spine cord extract was administered to induce differentiation of NSCs,the structure of which was revealed by histochemical technique and scanning electron microscope.The rat models of hemisected spinal cord injury were built and grafted with tissue-engineered neural complex.Functional recovery of the models was evaluated every two weeks postoperatively.Effect in repair of spinal cord injury was assessed by histological method at postoperative three and eight weeks.Results After tissue-engineered neural complex planted in the modified collagen scaffold was grafted into the injured spinal cord in adult rats,the seed cells survived and further differentiated into functionally active neurons.The differentiated glia cells presented quite regular reticular structure.Motor function below the level of the injured spinal cord in adult rats obtained obvious recovery.Conclusion Tissue-engineered neural complex seeded in the modified collagen scaffold takes effect in structural reconstruction and functional recovery of the injured spinal cord in adult rats.
2.Clinical effect of lumbar catheter drainage combined with intrathecal injection on patients with cerebrospinal fluid leakage and intracranial infection

Objective To investigate the clinical effect of lumbar catheter drainage combined with intrathecal injection on patients with cerebrospinal fluid leakage and intracranial infection.Methods A retrospective study was conducted.One hundred and fifty-two cases with cerebrospinal fluid leakage and intracranial infection were selected as our subjects who were hospitalized in the First Hospital of Yuncheng from 2006 to 2014.The patients were divided into lumbar puncture + intrathecal group (A),lumbar (group B) and lumbar intrathecal large pool + group (group C) based on post-processing methods.A experimental data were recorded and compared in terms of the total efficiency of treatment,the therapeutically effective time,bacterial clearance and security differences.Results After treatment,the levels of white blood cells,protein,glucose and intracranial pressure were changed compared with that of before treatment in three group(P < 0.01),but there was no significant difference among the three groups(P > 0.05).The therapy periods in group A,group B and group C were (12.80 ± 2.25) d,(12.64 ± 2.00) d and (9.44 ± 1.50) d respectively and the difference was significant(F =25.94,P < 0.05).Compared with Group C,the therapy periods in group A and B were significant different(t =2.769,2.854;P < 0.05),but there was no significant difference between group A and B (t =0.119,P =0.908).The cases with success.effect was 45 (89.1%) in group A,53 (94.6%) in group B,46 (95.8%) in group C,and there was no significant difference among three groups (P > 0.05).In terms of bacterial clearance rate,33 cases(68.75%) was in group A,35 cases(72.91%) in group C and 23 cases (41.07%) in group B and the effective rate in group A or C were higher than that in group B (x2 =9.478,10.63 ; P < 0.05).Conclusion The methods of lumbar catheter drainage combinedwith intrathecal injection is proved with a high clinical value of therapy,effective treatment can effectively shorten the time and improve the overall treatment effect.

4.Treatment of 225 patiens with chronic infective wounds

Objective To explore the treatment of chronic infective wounds .Methods A retrospective analysis of clinical data in 225 patients were admitted from 2000 to 2010 .Results (1) They were mainly traumatic ulcers ,pressure ulcers ,postoperative ul-cers ,diabetes ulcers ,vascular ulcers in the group ,accounted for 80 .4% (181/225) .(2) Bacterial culture positive rate was 87 .1%(196/225) ,a total of 46 kinds with 342 pathogens were cultured ,gram-positive bacteria 40 .6% (139/342);gram-negative bacteria 57 .6% (197/342);Fungi 1 .8% (6/342) .The main pathogens were S .aureus(52) ,E .coli(43) ,P .aeruginosa(44) ,Klebsiella .SPP (27) ,which were highly resistant to penicillin ,erythromycin ,ampicillin ,gentamicin ,cotrimoxazole and the multidrug resistance rate was 37 .1% (127/342) .Chronic wounds and multidrug resistant bacteria showed rapidly increasing trend from 2007 .(3) 201 pa-tients with topical antibiotic treatment ,208 patients(49 patiens underwent re-operation)underwent operations to close wounds ;213 patiens were recovery ,12 patiens had to leave hospital because economic burden .Conclusion Chronic infective wounds were affect-ed by many factors .emphasizing on debriding ,reasonablechoice ,circulative ,alternate use of antibiotics and wound bed preparation , appling surgery to close wounds in early stage could effectively control wound infection and promote wound healing .
5.Ultraviolet Spectroscopy Combined with Chemometrics for Simultaneous Quantitative Determination of 2,4,6-Trinitrotoluene andIts Degraded Products in Environmental Water Sample

2,4,6-Trinitrotoluene (TNT) and its by-products dinitrotoluene (DNT) pose a significant threat to human health and other living organisms.However, the conventional analytical methods involved in bulky and expensive instruments are complicated and time-consuming, impeding quick and on-line determination.In this work, a facile yet effective strategy of utilizing UV-vis spectroscopy coupled with partial least squares (PLS) was proposed, through which TNT and two isomers of DNT (2,4-DNT and 2,6-DNT) in nature water could be rapidly and simultaneously determined without any pre-separation.Variable combination population analysis (VCPA) was utilized to select important feather variables and significantly improved the predictive performance of the PLS model.The calibration set contained 25 samples constructed by orthogonal array design (OAD).The predictive ability of the models was validated by an independent prediction set including 15 samples, achieving up to 0.99 of the determination coefficients (R2) for each of the analytes.The optimized models were successfully applied to determine the 3 ingredients in 8 environmental samples involving in tap, lake and two kinds of river water with the recovery values of great than 97%.Finally, the proposed method was further validated by high performance liquid chromatography method.UV-vis spectroscopy coupled with chemometrics may be used as simple and effective strategy with high potential in environmental monitoring.
6.Evaluation of Effectiveness of Continous Passive Motion after Total Knee Arthroplasty: A Meta-analysis of Randomized Controlled Trails

ObjectiveTo evaluate the effectivenes of continous passive motion(CPM) on limb function in patients after total knee arthroplasty.MethodsThe Cochrane Database of Systematic Reviews, MEDLINE, EMBASE, PEDro, CNKI and VIP were retrieved from the date of establishment of the databases to July, 2010 to collect randomized controlled trials(RCTs). The quality of RCTs was critically appraised and data were extracted by 4 reviewers independently. Meta-analysis was conducted for the eligible RCTs.Results26 RCTs were included. The result of Meta-analysis showed that CPM improved the range of motion (ROM) in active knee flexion and passive knee flexion, eliminate swelling in short term(<6 weeks) and alleviate pain in short and medium term (less than 6 months). CPM showed no significant difference in improving the ROM in active and passive knee extention and quadriceps strength, and shortening hospital stay.ConclusionThe patients carried out CPM training after total knee arthroplasty can effectively improve limb flexion function, eliminate joint swelling, and relieve pain in the short term. But more high-quality and large-scale RCTs are needed.
7.Change of energy metabolism and body composition before and after chemotherapy in patients with colon cancer

Objective:To evaluate the nutritional status before and after chemotherapy in patients with colon cancer by measuring energy metabolism and body composition.Methods:Fifty-one patients with colon cancer were included and were investigated using self-control method (before and after chemotherapy).Resting energy expenditure (REE) was determined by bed metabolic instrument,and body composition was measured by body composition analyzer.Results:For all patients,the REE on the 2nd day after chemotherapy was decreased significantly compared with that before chemotherapy (P < 0.05).After chemotherapy,body fat percentage and body moisture percentage were increased significantly (P < 0.05),and lean body mass percentage was decreased significantly (P < 0.05).Conclusion:After chemotherapy,the REE and lean body mass percentage of the patients with colon cancer were decreased,and body fat percentage and body moisture percentage were increased.
8.Surgical treatment for diabetic foot ulcers

Objective To get the experience of surgical treatment for diabetic foot (DF) ulcer.Methods Clinical data of 85 patiems (108 limbs in total) admitted in Department of Bum and Plastic Surgery in The First Affiliated Hospital of Chongqing Medical University from Jan 2010 to Dec 2012 were retrospectively analyzed.Results Wound culture results of the 85 patients were:bacteria in 142 limbs,and fungi in 7 limbs.The main bacteria cultured were staphylococcus aureus,escherichia coli,coagulase negative staphylococcus,enterococcus,pseudomonas aeruginosa,and acinetobacter baumannii.DF healing and Wagner classification was negatively correlated:the higher the class,the less the primary healing rate.The higher amputation rate were found in the higher class patients.The more delayed healing,and the longer the healing time and average hospital stay were also found in the higher class patients.85 patients underwent surgery:skin grafting in 45 cases (52.9%),flap in 8 cases (9.4%),skin repair + flap grafting in 7 cases,toe (limb) amputation in 28 cases,2 cases were sutured after debridement.All cases were cured after one or multiple surgeries.No recurrence was found during the follow-up of 10 days to 2 months.No delayed infection occurred to the repairments.The shape and weight bearing walking function were good.Conclusions The treatment of DF need to follow the principle of multidisciplinary cooperation,professional treatmem,systemic and local treatment.We need to pay attention to the etiology and prevention of the disease,focus on wound bed preparation,choose the best treatment.Early operation can significandy fascilitate wound healing.

BACKGROUND: The indicant of successful transplanted bone marrow stem cells is that the labeled transplanted ceils can survive in the target organs and can their biological functions. Up to date, at cell level, there are several labeled methods such as enzyme-linked, 3H-TdR, fluorescent and 5-bromodexyuridine method, etc.OBJECTIVE: To observe the biological characteristics of bone marrowderived mesenchymal stem cells labeled with 5-bromodexyuridine. DESIGN: Observations in single kind of sample SETTING: Department of Thoracic Cardiac Surgery, Affiliated Hospital of North China Coal Medical College MATERIALS: The experiment was carried out in the Experimental Center of North China Coal Medical College from July, 2003 to November,2004 on six Japanese long-eared rabbits ,with the age of 3 months , of either gender and the body mass of (3.00±0.25)kg.METHODS:①Monocytes were isolated from the bone marrow with Percoll reagent of the concentration of 1 073 g/L. The mesenchymal stem cells were cultured and proliferated with Eagle's medium, which was improved by adding 10% low sugar Dulbecco's to fetal bovine serum, so that their purity could reach about 95%. Secondly, in the experimental groups, the percentage of bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuridine was detected after 24 hours, 48 hours, 72 hours and 96hours, respectively. The negative-controlled group was composed of bone marrow-derived mesenchymal stem cells non-labeled with 5-bromodexyuridine while the blank-controlled group was made by substituting phosphate buffer or normal mice serum for primary antibody.MAIN OUTCOME MEASURES: In three different groups, 200 cells of each group were detected and observed at different time. Outcomes of the immunochemical stauning and counting of bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuridine were also determined.RESULTS: The positive bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuridine were observed in all the experimental groups. The positive materials were brown, granular and dfiffusedly scattered in the nucleus while no positive cell was observed in the controlled groups. After 24 hours, 48 hours, 72 hours and 96 hours, the numbers of bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuridine were 48±2, 100±4, 173:t:2, 178±3 respectively. The labeling percentage was raised gradually with the elongation of time. 72 hours later,the labeling percentage is above 85%. The numbers in negative-controlled and blank-controlled groups were all zero.CONCLUSION: The appropriate time of bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuriding is 72 hours. The sensibility of post-labeling detection is high due to the results being observed in the low power microscope, which makes this method suitable to quantitative analysis in massive tissue. These results show that the method of bone marrow-derived mesenchymal stem cells labeled with 5-bromodexyuridine is feasible.
