Chinese Journal of General Surgery 2016;31(4):277-280
Objective To evaluate combination of traditional surgery and sclerotherapy in treating varicose veins of the lower extremity.Method This protocol includes high ligation of the great saphenous vein (GSV),stripping or laser closing of GSV above knee, injection of sclerosing agent into GSV below knee;ligating the perforator veins;sclerosing agent injection for varicose vein in 31 cases of varicose vein of the lower extremitis, among them seven were recurrent cases from April 2015 to June 2015.The clinical classification was as follows:nine cases of C2,four cases of C3,seventeen cases of C4,and one case of C6.All the perforator veins were located before the operation by color ultrasound. Results The number of perforator veins varied significantly (P <0.01) among the inner and back side of calfs (4.16 ± 1.86),the lateral calfs (1.29 ± 1.10),and thighs (0.16 ± 0.45).Twenty-four patients received high ligation of GSV combined with stripping of above-knees.Two patients received high ligation of GSV combined with the laser closing of above-knees.Five cases were treated conservatively (in which four have stripped or closed and one was on normal condition).Below-knee GSV were treated by sclerosing foam in 27 cases, and four cases were untreated (in which three have closed and one was normal). All the thirty-one cases underwent mini-incision for ligating the perforators and having the varicose veins closed using the sclerosing foam. All patients' GSV with sclerotherapy have got sclerosed. There were no complications of deep vein thrombosis, pulmonary embolism,or visual disturbance. Conclusion This operation design ensures the effect and prevents the recurrence of varicose vein,as well as reduces surgical trauma.

Objective To study the clinical value of vascular ultrasound examination in detection of carotid artery disease. Methods 53 cases of ischemic cerebrovascular disease who served as experimental group, tested 387 vessels; and used the same period 55 cases of healthy persons as control group, tested 394 blood vessels. Comparison of two blood flow parameters between groups with differences in plaque detection. Results The results of vascular ultrasound examination could detect the visual display and quantitative carotid artery lesions in the experimental group a significant thickening of carotid artery IMT,carotid artery plaque prevalence was significantly higher than control group (P <0.01). Conclusion Vascular ultrasound examination was an effective screening method to detect ischemic cerebrovascular disease.

Objective To investigate vagal effects on atrial fibrillation(AF)thresholds at different sites of pulmonary veins after autonomic denervation. Methods In 10 mongrel dogs, the cervical vagal trunks were isolated bilaterally and decentralized from Oct 2004 to May 2005.The ansae subclaviae were exposed bilaterally as they exited from the stellate ganglia, doubly ligated, and cut.Local rapid electrical stimulation(S1S1=80 ms,impulse duration 0.5 ms)was performed to right appendage(RAA)、left appendage(LAA)、left atrium(LA)and different sites of 4 pulmonary veins.AF threshold was determined at each site in the baseline state, during bilateral cervical vagal stimulation(VNS,12.5 Hz,impulse duration 0.5 ms,stimulation voltage 5~8 V)and during infusion of atropine. Results During VNS,a lower AF threshold was observed at all sites, especially at distal of 4 pulmonary veins(P0.05).Conclusion Vagal nerve not only contributes to the maintenance of AF originating from pulmonary veins, but also greatly promotes the initiation of AF.

Objective To examine the distribution and antibiotic resistance proifle of the bacterial isolates from lower respiratory tract of hospitalized patients for better empirical therapy. Methods A total of 3 155 lower respiratory tract specimens were collected from Respiratory Department of the First Afifliated Hospital of Chongqing Medical University in 2013 to identify bacterial pathogens for susceptibility testing. The results were compared with the data of 2011 and 2012.Results The top three bacterial species from lower respiratory tract in the period from 2011 to 2013 wereA. baumannii,P. aeruginosa, andK. pneumoniae. The prevalence ofS. aureusincreased, while the prevalence ofS. maltophilia andS. pneumoniae was still low.A. baumannii isolates were highly resistant to antibiotics. Not more than 40% of these strains were susceptible to the antimicrobial agents except minocycline, which was active against more than 60% of the ICU and non-ICU strains. More than 70% of theP. aeruginosa strains from non-ICU were susceptible to cefepime, meropenem, imipenem, levofloxacin, and ciprofloxacin in the three years, 50%-70% of the strains from ICU were susceptible to the antimicrobial agents tested. Carbapenem-resistantA. baumannii andP. aeruginosa strains were identiifed.S. maltophilia strains were relatively susceptible to minocycline and levolfoxacin. All the S. aureus isolates were susceptible to vancomycin, linezolid, and tigecycline. The prevalence of MRSA increased year by
year. More than 80% of theK. pneumoniae andE. coli strains were susceptible to imipenem, piperacillin-tazobactam, and cefepime. ESBLs-producingK. pneumoniae was identified. More than 85% of theH. influenzae strains were susceptible to meropenem, ceftazidime, cefuroxime, ciprolfoxacin, levolfoxacin, and amoxicillin-clavulanate potassium. All theS. pneumoniae isolates were susceptible to vancomycin, and 80%-96% of these strains were susceptible to levofloxacin and penicillin.ConclusionsA. baumannii,P. aeruginosa, andK. pneumoniae are the top three bacterial species isolated from lower respiratory tract. Carbapenem-resistantA. baumannii,P. aeruginosa and ESBLs-producingK. pneumoniaeare emerging. The prevalence of MRSA is increasing year by year, which poses a challenge to antimicrobial therapy. It is urgent to contain and control the growing antibiotic resistance.

Objective To investigate the effect and molecular mechanism of cyclooxygenase-2 (COX-2) on the benign prostatic hyperplasia (BPH) in rat model. Methods Thirty-six SD rats were randomly divided into 3 groups:normal control (group A,n=12),BPH model (group B,n=12) and BPH+selective COX-2 inhibitor celecoxib (group C,n=12). At the 5th week after treatment, the weight of the prostates was measured, and the morphological changes were examined under light microscope.Detection of ki-67 and TUNEL in prostatic epithelial and stromal cells was undertaken to assess the proliferation and apoptosis status. The protein and mRNA expression of COX-2,epidermal growth factor (EGF),basic fibroblast growth factor (bFGF) and transforming growth factor-?1 (TGF-?1) were analyzed by means of immunohistochemisty and RT-PCR. Results The prostate index [prostate wet weight (mg)/rat body weight (g)] of group B was significantly higher compared with those in groups A and C (1.88?0.17 vs 1.70?0.09 and 1.74?0.16,P0.05). Conclusions The increased expression of COX-2 may play an important role in the pathogenesis of BPH by modulating the expression of growth factors and affecting the proliferation and apoptosis of prostate cells.
