1.Single iliac screw and dual iliac screws and titanium mesh cage fixation in the reconstruction of lumbosacral defects with finite element analysis

BACKGROUND:The surgical management to reconstruct the stability in lumbosacral region is very chal enging. There are many techniques to reconstruct the stability of the lumbosacral region, but the internal fixation loosening, fracture and pseudoarthosis are not uncommon. OBJECTIVE:Finite element analysis was used to analyze the stability of two types of reconstruction procedures and the stress of the internal fixation system. METHODS:A 64-slice spiral CT scan was used in a subject from L1 to pelvis. The scan data were imported into the Mimics 15.0 software to generate a three-dimensional surface model. The three-dimensional solid model was established using in the SolidWorks software. The solid geometry model of the L3, 4 pedicle screws combined with single iliac screw fixation and L3,4 pedicle screws combined with dual iliac screws and anterior titanium mesh cage support fixation were constructed by the Solidworks software. Two kinds of reconstruction procedures were simulated. The models were given material properties and analyzed by using the ABAQUS software. RESULTS AND CONCLUSION:(1) The maximum stress was 195.3 MPa in the complete model, 189.5 MPa in single iliac screw model, and 149.2 MPa in dual iliac screw+titanium cage model when constrained the roof of double acetabulum and applied a vertical load of 1 000 N. (2) The axial compression rigidity was 551.572 N/mm in the complete model, 613.87 N/mm in the single iliac screw model and 1 683.50 N/mm in the dual iliac screw+titanium cage model. (3) The bending rigidity of the dual iliac screw+titanium cage model was bigger than other models at 6 directions when applied 7 N?m bending loads. The maximum stress of single iliac screw model was bigger than other models. (4) The results suggest that the dual iliac screw+titanium mesh cage reconstruction can effectively restore the stability of the lumbosacral area. The stress of internal fixation system is smal er and more dispersed. There is obvious stress concentration in the connecting rod and the tail of the single iliac screw. The single iliac screw internal fixation system is easy to fatigue fracture and loosening.
2.Expression of the estrogen receptor ? and ? in mouse thymus

Objective:To detect estrogen receptor ? and ?(ER-?,ER-?) protein expression in different age of mouse thymus.Methods:Protein expression of ER-? and ER-? in thymus was analyzed via immunohistochemistry.Moreover,the relationship between ER-? and cytokeratin 18(epithelial cell marker) was further tested through fluorescence double-staining.Results:Immunohistochemical analysis revealed that both ER-? and ? protein was found in nuclei of some thymocytes of 3 month mice.However,expression of ER-? was absence while ER-? was still positive in aged mice,such as 12 months and 16 months old.Double staining further confirmed that lots of ER-?/? positive cells were Foxp3 negative cells.Conclusion:Expression of ER-? is absent while ER-? is still positive in thymus of aged mice,which indicates ER-? is the critical estrogen receptor that involves in thymic involution.Moreover,ER-?/? do not participate in Treg development within thymus.
3.Obesity,Hypertension and Aging are Three Most Important Components of Metabolic Syndrome

Objective To explore the most important risk factors in metabolic syndrome(MS) components.Methods Ninety-four individuals were classified into MS and non-MS group according to the diagnostic criteria for MS proposed by Chinese Diabetes Society(CDS) revised in 2006 or International Diabetes Federal(IDF) in 2005.Age,waist circumference(WC),body mass index(BMI),fasting plasma glucose,lipid profile,blood pressure and blood cell counts in two groups were compared.Partial least squares discriminant analysis(PLSDA) was carried out to determine the most important components of MS.Results Patients with MS diagnosed by CDS or IDF criteria have significantly older age,higher BMI,WC,blood pressure,fasting plasma glucose,triglycerides,insulin levels,insulin resistance index,high sensitivity CRP and fibrinogen levels compared with non-MS group.PLSDA analysis shows WC,BMI,blood pressure and aging are most important components of MS.Conclusion Obesity,hypertension and aging are three most important components of MS with obesity is the utmost among them.
4.Optimizing the teaching of human anatomy by applying non-verbal communicative approaches

6.Advances in the Isolation Methods of Fungal Polyketide Synthase Genes

Fungal polyketide synthases are responsible for the biosynthesis of secondary metabolites such as pigments, mycotoxins, and they are very important in pharmacology , food science and agriculture. The recent advances in the methods for the isolation and manipulation of multiple classes of polyketide synthase genes from fungi were introduced. It is useful for discovery of novel fungal polyketide synthase gene clusters. These methods can also be useful for revealing the genetic potential of fungi to produce multiple types of bioactive polyketide.
7.Effects of YinlingⅠon Cell Viability and Oxidative Injury of Lead-Exposed Vero Cell

Objective To study effects of yinlingⅠon cell viability and oxidative injury of lead-exposed cell.Methods After lead exposure Vero cell was treated with yinlingⅠof different concentrations.The cell viablitity was measured by methyl thiaxiolyl tetrazolium(MTT) method and the superoxide dismutase(SOD)and malondialdehyde(MDA) activity in cell suspensions were measured.Results When Vero cell lived in 125 ?mol/L lead acetate surrounding,the MDA concentration increased,but the viability of cell and the SOD content in the Vero cell suspension decreased.YinlingⅠcould increase the viability of lead-exposed cell during a certain extent;in the most non-toxicity concentration yinlingⅠcould elevate the SOD content in the Vero cell suspension,reduced the MDA concentration and resisting lead toxication in vitro.Conclusion YinlingⅠhas the protective effects on the cell viability and oxidative injury of lead-exposed cell.
9.MR imaging during arterial portography: an experimental study

Objective To evaluate the optimization of perfusing artery and contrast agent dose, and to determine the effect of precontrast injection of papaverine into the perfusing artery or not on the degree of contrast enhancement of the hepatic parenchyma during MRAP in dog models. Methods Sixteen dogs were included in this study and distributed to three study groups (perfusing artery, dose of Gd-DTPA and use of papaverine or not). Animals were anesthetized with Thiopental Sodium. After placement of a 5F end hole angiography catheter with no metal parts into the perfusing artery, the dogs were transferred immediately to the MR imaging unit. Unenhanced GRE MR and MRAP images were performed with a 2.0 T superconducting magnet. Signal intensity of liver parenchyma was measured with 6 different regions of interest. Relative value (%) of liver parenchymal enhancement was measured and compared. Time-enhancement curves were created and compared. Results MRAP images with good quality were obtained in all 16 dogs. The peak relative value of liver parenchymal enhancement and the time to the peak enhancement was similar in the superior mesenteric artery (SMA) group and in the splenic artery (SA) group. The peak relative value of liver parenchymal enhancement was the lowest in the 0.025 mmol/kg group among all groups. There was no statistical difference in the time to relative peak liver enhancement across the four groups. The time to relative peak liver enhancement in the group with papaverine injection was statistically earlier than that in the group without papaverine injection. Conclusion MRAP is a new and feasible imaging technique to detect hepatic masses before operation. When a 20 ml mixed solution of Gd-DTPA(0.050 -0.100 mmol/kg)was injected into perfusing artery at the rate of 1 ml/s, MRAP image of best liver parenchymal enhancement in dogs could be obtained at 21-28 s after the start of injection of Gd-DTPA. We can place catheters in SMA or SA and there was no statistical difference on MRAP images and time-enhancement curves. The dose of 0.050-0.100 mmol/kg Gd-DTPA was sufficient for MRAP. Application of papaverine precontrast advanced the time to peak liver enhancement, but the peak value of liver enhancement was not affected.
10.Treatment of acute obstructive cased by colorectal cancer

Objective To investigate the treatment of acute obstructive colorectal cancer.Methods The clinical data of 26 cases with acute obstructive colorectal cancer were analysed retrospectively and relevant literatures were reviewed.Results Radical right hemicolectomy was performed in 9 patients with obstructive right colonic cancer.Among 17 patients with obstructive left colorectal cancer,one-stage radical resection was perfomed in 12 cases,including one-stage anastomosis in 4 cases and Hartmann's operation in 8 cases.Palliative colostomy was performed in 3 cases.Right hemicolectomy and sigmoid colon loop colostomy was performed in 1 case of rectum cancer with ascending colon strangulation.One case refused surgery.Two cases died of MODS and 1 case with inflammatory ileus recovering from conservative treatment after operation.In the course of disease,septic shock and MODS happened in 3 cases,lung infection in 5 cases,heart disease in 2 cases and Hypoalbuminemia in 16 cases.Conclusions One-stage radical colectomy and anastomosis should be performed in patients with obstructive right colonic cancer.In order to reduce toxin absorption and prevent the deterioration of disease,Bowel decompression and removal of inflammatory exudate should be performed in patients with obstructive left colorectal cancer according to the damage control theory.Then effective and safe operation should be chosen in accordance with patients' status.Anti-infection and nutrition support treatment must be strengthened after operation.
