1.A study on the correlation between serum procalcitonin and cardiac troponin I levels in patients with sepsis

Objective To investigate the relationship between the levels of serum procalcitonin (PCT) and cardiac troponin I (cTnI) in patients with sepsis.Methods According to the severity of the disease, 126 patients with sepsis in the Department of Critical Care Medicine of Hubei Provincial Hospital of Integrated Chinese and Western Medicine from June 2013 to February 2015 were divided into three groups: sepsis, severe sepsis and septic shock groups, 42 cases in each group. The changes of lactate (Lac) and central venous oxygen saturation (ScvO2) were monitored in the three groups within 72 hours after admission, the mortality and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score within 24 hours after admission, the levels of serum PCT and cTnI were compared among the three groups, and the correlations between serum PCT, cTnI levels and APACHE Ⅱ score were analyzed.Results Along with the increase of patients' severity in the three groups, the levels of Lac (mmol/L) were gradually increased (prior treatment: 5.82±2.42, 7.97±3.76, 10.30±2.82; 6 hours after treatment: 3.63±1.54, 5.08±1.98, 7.50±1.35; 12 hours after treatment: 2.70±1.55, 4.13±1.96, 6.23±1.16; 24 hours after treatment: 2.58±1.35, 3.95±2.44, 5.25±1.13; 48 hours after treatment: 2.15±1.82, 3.86±2.36, 4.12±1.72; 72 hours after treatment: 1.83±1.04, 3.78±1.84, 3.74±0.87), while the levels of ScvO2 were gradually reduced (prior treatment: 0.556±0.102, 0.502±0.095, 0.402±0.092; 6 hours after treatment: 0.627±0.062, 0.557±0.083, 0.504±0.075; 12 hours after treatment: 0.658±0.076, 0.601±0.083, 0.595±0.072; 24 hours after treatment: 0.676±0.059, 0.625±0.084, 0.603±0.050; 48 hours after treatment: 0.704±0.049, 0.656±0.066, 0.615±0.035; 72 hours after treatment: 0.755±0.053, 0.707±0.066, 0.629±0.048). The higher the Lac, the lower the ScvO2, and the differences among the three groups were of statistical significance (allP < 0.01). The mortalities of patients in the septic, severe septic and septic shock groups were as follows: 11.9% (5/42), 21.4% (9/42), 38.1% (16/42); serum PCT (μg/L): prior treatment: 5.21±2.92, 17.20±4.81, 40.71±5.22; 4 days after treatment: 2.51±1.52, 10.72±3.83, 46.45±4.25; the levels of cTnI (μg/L): prior treatment: 5.31±0.82, 9.50±0.31, 15.12±3.15; 4 days after treatment: 1.16±0.62, 5.35±0.53, 9.24±1.25; APACHE Ⅱ score: prior treatment: 10.41±3.72, 20.15±5.14, 35.17±4.58; 4 days after treatment: 7.25±2.22, 14.15±4.16, 28.12±3.13. Their levels were also increased along with the increase of the septic severity. The correlation analyses showed that the serum levels of PCT was significantly positively correlated with cTnI and APACHE Ⅱ score (r value was respectively 0.922, 0.921, bothP < 0.01).Conclusion Myocardial damage often easily occurs in patients with sepsis, and the more serious the illness, the more prominent the injury.
2.A clinical study on diagnosis and treatment of renal tuberculosis:a report of 82 cases

Objective To investigate the clinical characteristics on diagnosis and treatment of renal tubercu- losis.Methods Clinical data of 82 patients with renal tuberculosis were retrospectively reviewed.Results All of the 18 cases who received medication recovered completely.64 cases undergoing surgery were pathologically diagnosed to have renal tuberculosis,of which 2 cases developed ureteral stump syndrome.Conclusion Urine PCR-TB-DNA re- mained the primary diagnostic method before operation.Computed tomography(CT)and magnetic resonance urogra- phy(MRU)could be used in diagnosis of renal tuberculosis.When the non-functioning kidney was resected,per- inephrit fat and the involved ureter should be concomitantly resected as much as possible.
3. Purification of total flavonoids from diaphragma of Juglans regia by macroporous resin

Objective: To investigate the purification technology of total flavonoids from the diaphragma of Juglans regia by macroporous resin. Methods: Twelve macroporous resins were chosn with static and dynamic adsorption and desorption experiments to optimize the purification parameters. Results: AB-8 macroporous resin was found to have good adsorption and desorption effects. The optimal purification conditions were pH value of 4.70, sample mass concentration of 0.713 5 mg/mL, the loaded amount of 4 mL/g, and loading flow rate of 1.5 BV/h. The sample was eluted by water with 2 BV and 50% ethanol of 2.5 BV, respectively. The purity of total flavonoids increased to 72.25% after the purification, and the yield was 93.94%. Conclusion: AB-8 is an ideal resin with the best enrichment for separating and purifying the total flavonoids from diaphragma of J. regia.
4.Influences of repeated propofol anesthesia on hippocampal apoptosis and long-term learning and memory abilities of neonatal rats

Objective:To investigate the influences of repeated propofol anesthesia on the hippocampal apoptosis and the long-term learning and memory abilities of neonatal rats.Methods: In the study,45 male Sprague-Dawley rats,aged 7 days,were randomly divided into 3 group (n=15 each): control group (C group) that received intralipid 7.5 mL/kg intraperitioneal (IP) once a day×7 days;group propofol 1 (P1) that received propofol 75 mg/kg IP once a day×7 days;group propofol 2 (P2) that received intralipid 7.5 mL/kg IP once a day×6 days+propofol 75 mg/kg IP on the 7th day.In each group,5 animals were chosen and arterial blood samples were obtained immediately after the animals were fully awake for blood gas analysis.Learning and memory abilities were assessed using Morris water maze when the other rats were 4 weeks old.The animals were decapitated after the tests.The hippocampi were isolated for detection of neuron-specific nucleoprotein (NeuN) expression by immunohistochemistry method and the expression of caspase-3 using the Western blot.Results: There was no significant difference in the indexes of blood gas analysis among the 3 groups.Morris water maze test: compared with group C,the escape latency and the length of searching on the 5th day were significantly prolonged,and the searching time in target quadrant and platform crossing on the 6th day were significantly decreased in group P1 (P<0.05) but not in group P2.Compared with group C,NeuN-positive neurons were decreased,and the expression of caspase-3 was increased in the rats of group P1 (P<0.05) but not in group P2.Conclusion: Repeated propofol anesthesia may destroy long-term learning and memory abilities by inducing apoptosis of hippocampal neurons in neonatal rats,while single dose of propofol has no obvious effect on the hippocampal apoptosis and long-term learning and memory of neonatal rats.
5.Influence of 3' untranslated region of vascular endothelial growth factor-C gene on the expression of luciferase

Objective To construct a luciferase gene expression vector containing full-length 3' untranslated region(3'UTR)of mouse vascular endothelial growth factor-C(VEGF-C)gene,and to observe the effects of VEGF-C 3'UTR on luciferase gene expression by a double-fluorescence report system.Methods Polymerase chain reaction(PCR)was used to amplify VEGF-C 3'UTR and a 312bp VEGF-C coding region(CR)fragment from full-length VEGF-C cDNA in mouse Lewis lung cancer cells.The luciferase expression vectors containing VEGF-C 3'UTR or VEGF-C CR were constructed by subcloning the PCR products to luciferase reporter vector pGL3-Promoter using gene engineering technology,and then they were transfected to mouse Lewis lung carcinoma cells by LipofectamineTM 2000.The activities and mRNA expression of luciferase were detected by Dual-Luciferase Reporter System and quantitative RT-PCR,respectively.Results Mouse VEGF-C 3'UTR(429bp)and VEGF-C CR(312bp)were successfully amplified by PCR.The VEGF-C 3'UTR and VEGF-C CR fragments were successfully inserted into pTA2 vector,and then subcloned to pGL3-Promoter vector at Xba Ⅰ site by using restriction endonucleases analysis.The DNA sequences and insertion orientation of PCR products were all correct by sequencing analysis.The resulted luciferase expression plasmids were named pGL3-VEGF-C 3'UTR and pGL3-VEGF-C CR,respectively.Dual-Luciferase Reporter System detection and quantitative RT-PCR showed that in Lewis lung carcinoma cells,the activities of luciferase and expression of luciferase mRNA in the pGL3-Promoter group were higher than that in the pGL3-VEGF-C 3'UTR group,and there was no significant difference between pGL3-VEGF-C CR group and pGL3-Promoter group.Conclusion VEGF-C 3'UTR can inhibit luciferase gene expression.

8.Clinical research on the effects of propranolol on infantile hemangiomas

Objective To study the efficacy and adverse reactions of the treatment on different types of infantile hemangiomas (IH) with different doses of propranolol .Methods One hundred and fifty cases of IH without contraindications were randomly treated with different doses of propranolol (low dose group:1 mg?kg -1 ?d-1 ;high dose group:2 mg?kg -1 ?d-1 ) under ECG monitoring inferior half an hour to breastfeeding taking ,and stay in hospital for observation for 6 to 24 hours after the treatment . The patients returned to the hospital for review every month(ECG and blood RT ;lung function in necessary) .Results The curative effect is better on strawberry IH than that on flat IH ,also better on deeply IH than that on superficial IH .The adverse reactions oc‐curred relatively slight in the low dose group and there was no significantly statistical differences on the treatment effects between the both groups(P>0 .05) .Conclusion It is safe and effective to treat IH with propranolol .

[Summary] Postoperative cognitive dysfunction ( POCD) is a common central nervous system complication after anesthesia and surgery.The mechanisms of POCD are not clear yet.The article reviewed several potential mechanisms that have been reported. Further researches are needed to identify the pathophysiologic progress of POCD, which have great clinical and economic values.
10.Early effect of extracorporeal shockwave treatment on distraction osteogenesis of long bone in rabbits

Objective To investigate the early effect of extracorporeal shockwave(ESW) treatment on distraction osteogenesis of rabbit tibias and its mechanism. Methods 30 adult New Zealand white rabbits were used. After osteotomy of left tibias, improved Orthofix M- 100 bone fixator was applied. After 5 days, bone lengthening of 1 cm was obtained over 10 days. Randomly the animals were divided into ESW and control groups. No ESW treatment was given to the control group. The shock wave was set at 0.54 mJ/mm2 and 1000 shots each time were applied to the central areas in the ESW group. Then the animals were divided into 3 sub- groups, and were killed respectively at 45, 60 and 75 days after osteotomy. During the process, X- ray, bone mineral density (BMD) and histological examinations were conducted for every animal.
