1.Appealing for Freer Academic Atmosphere in Bioethics

Bioethics, one of the focus subjects in 21 century,attracts the attention in the worldwide.Originated from western developed countries,it has been branded with distinct western civilization,which is disadvantageous to the development of the subject.Some suggestions are put forward as following: 1.Communication between the East and the West should be promoted in order to form a solid multi-cultural base for bioethics. For instance, in America ethics bears the imprint of religious conventionality,while in China politics is infiltrated into traditional ethical ideas. Both of them are not perfect. So they cannot form the base of bioethics. 2.Research on the "base line" of the bioethics should be emphasized in order to provide freer academic atmosphere. Transplanting the western bioethical ideas to the developing countries will never work. As for the bioethical"base line",different countries should establish feasible ethical codes according to their own actual situations. 3.Scientists should be encouraged to adopt various technical ways to explore the problems in the field of bioethics.Such methods as tissue-engineering,antologous stem-cell replanting and cell diffrenttation inducing may bring about earlier breakthrough than fetus stem-cell.Equal attention,encouragement and support should be given to bioethics.
2.Low testosterone levels are inversely correlated with carotid artery plaque formation in elderly women.

OBJECTIVETo study the relationship between serum testosterone levels and the plaque formation of the carotid artery in a population-based cohort of independently living healthy women above 60 years of age.
METHODSAnalysis of the healthy elders from a population-based cohort study in 9 communities of Beijing. Carotid intima-media thickness and atherosclerotic plaques were determined ultrasonographically. Serum testosterone levels were measured by immunoassay. The data were analyzed with ANOVA and logistic regression analysis.
RESULTSThere was an inverse correlation between testosterone and plaque formation in old females (P < 0.01), while no association was found in males. Female with testosterone levels in the lowest quartile (< 0.49 nmol/L) had more risk of plaque formation (OR = 3.805, P < 0.01) after adjusted with age and other traditional factors of atherosclerosis.
CONCLUSIONTestosterone concentrations are negatively associated with carotid artery atherosclerosis in old women in Beijing, experimental and prospective studies are needed to determine the possible therapeutic role of testosterone in atherosclerosis.

4.MRI Diagnosis of Discoid Meniscus

Objective To investigate the MRI signs of discoid meniscus and to establish the criterion of MRI diagnosis.Methods 40 cases of discoid meniscus were analysed retrospectively. The methods of measurement and the classification of discoid meniscus were studied. Besides, the injurious criterion of diagnosis were discussed.Results All of 40 cases were lateral discoid meniscus. 24 cases were classified as slab type and 16 cases wedge type. 38 cases developed in different degree of meniscal injury. The rate of injury was 95%. Conclusion MRI examination is principal in diagnosis of discoid meniscus. Proportion of transverse diameter of the midbody of meniscus to the transverse width of the tibia exceeded 40%, which is necessary to diagnose discoid meniscus.
5.Analysis of Technique in 64-slice Spiral CT Angiography

Objective To explore scanning technique in 64-slice spiral CT angiography, and to evaluate the clinical value of various of post-processing methods.Methods 45 patients were examined with CT angiography, including 4 cases of thoracic aorta, 15 cases of abdominal aorta,10 cases of pulmonary artery, which delay time was determined by bolus tracking. The delay time of others ,including 4 cases of cerebrovascular, 6 cases of cervicum blood vessel and 6 cases of lower limb artery were determined by artifical. The initial images were processed with MPR, CPR, MIP ,SSD and VRT.Results According to standard valuation of imaging quality ,the good images were 37 cases, the better were 8 cases and none was bad.Conclusion As far as the diagnosis of great vessels is concerned, 64-slice spiral CTA can instead of DSA. MIP and VRT are important methods in post-processing of image.
6.Effect of standardized treatment process on early enteral nutrition combined with gastric retention after operation for patients with esophageal cancer

Objective:To explore the application of standardized treatment process for early enteral nutrition combined with gastric retention after operation in patients with esophageal cancer.Methods:Using convenience sampling, 84 patients with gastric retention complicated by early enteral nutrition support after esophagectomy in Department of Thoracic Surgery, Third People′s Hospital of Dalian from July 2015 to June 2020 were selected as the subjects. They were divided into control group and experimental group according to admission time with 42 cases in each group. The control group received routine nursing, while the experimental group received early enteral nutrition and gastric retention standardized treatment process nursing. The differences between the two groups in nutritional status and immune indexes before and 7 days after operation, the standard feeding rate within 7 days after operation, incidence of postoperative complications and enteral nutrition-related complications, length of hospitalization time and hospitalization expenses were compared.Results:In the control group, 2 cases were lost to follow-up. There was no statistically significant difference in preoperative nutritional status and immune indexes between the two groups ( P<0.05); 7 days after operation, the body mass index, serum total protein, prealbumin, nitrogen balance, IgM, CD3, CD4, and CD4/CD8 of the experimental group were all higher than those of the control group, and the differences between the two groups were statistically significant ( t values were 2.228-5.332, P<0.05 or 0.01).The standard feeding rate within 7 days was (68.93±8.12)% in the experimental group and (51.19±6.96)% in the control group, respectively, and the difference between the two groups was statistically significant ( t value was -4.38, P<0.01). The incidence of postoperative nausea and vomiting, Ⅱ B healing, anastomotic fistula and aspiration was 2.38% (1/42), 2.38% (1/42) and 0 (0/42), 2.38% (1/42) in the experimental group and 15.00%(6/40), 7.50% (3/40) and 5.00% (2/40) and 10.00% (4/40) in the control group, respectively, and the differences were statistically significant ( χ2 values were 5.99 - 9.88, P< 0.05 or 0.01). The length of hospitalization time in the experimental group was (10.18±1.69) d, and the hospitalization cost was (53 268.46 ±3 651.56) yuan, both lower than (13.66 ± 2.18) d and (64 972.39 ± 4 029.81) yuan in the control group, and the difference was statistically significant ( t values were 2.14, 2.89, P< 0.05). Conclusions:The standardized treatment process of enteral nutrition combined with gastric retention can improve the feeding rate, nutritional index and immune index within 7 days after operation, reduce postoperative complications and incidence of enteral nutrition related complications, reduce the hospitalization time and reduce hospitalization expenses.
7.Development of mycobacterial inducible expression system and application for immunological diagnostics on tuberculosis

Objective To develop mycobacterial inducible expression vectors which permit to overexpress Mycobacterium tuberculosis (Mtb) immunodominant antigen, and to analyze its immunogenicity after purification by affinity chromatography. Methods The regulatory region of M. smegmatis (Ms) acet-amidase(pACE) was obtained by PCR amplification, and was used as promoter to construct the mycobacteri-al inducible expression vectors, pMF series. The coding gene of Mtb chimeric antigen Ag856A2 which is a recombinant Ag85A with 2 copies of ESAT-6 inserted in its Acc Ⅰ site and showed excellent immunogenicity in the animal experiments we described previously, was cloned into the pMF vector series, and was induced to express by the addition of acetamide. The recombinant protein expressed in the Ms was purified by the Ni~(2+)-NTA affinity chromatography, the resulted homologous recombinant antigen was added into the spleen cells separated from BCG vaccinated mice, and the immunogenicity was analyzed by the IFN-γ ELISPOT as-say. Results The mycobacterial inducible expression vectors, pMF series was constructed successfully, target antigen could be. induced to express in the Ms by the addition of 0.02% acetamide, and could be puri-fied by the Ni~(2+) -NTA affinity chromatography due to the addition of 6×His tag in the vector pMF406. Fur-thermore, the mycobactefial homologous antigen could induce more IFN-γ secretion than the heterogonous one. Conclusion The mycobacterial inducible expression system based on the regulatory region of Ms acet-amidase as promoter could permit the Mtb target antigen of interest overexpression and purification, and the immunogenicity of the homologous antigen from Ms is better than that of be expressed from E. coli, which may be more potential for immunological detection of tuberculosis.

Objective To analyse the correlation between hepatitis B virus infection in mother and immune effect of hepatitis B vaccine in infant so as to explore ways to prevent mother-to-infant transmission.Methods 8 022 aged from 7 months to 2 years old children and their mothers were selected.The children's HepB immunization were investigated.The serological investigation of mother and children were tested by the colloidal gold tripes and ELISA methods.The HBV genotype were detected among HBsAg positive mother.Results The mother's carry rate of HBsAg was 2.43% while the children's was 0.45%.The protect rate of HepB was 81.48%.127 genotype C were detected among 146 HBsAg positive mothers.There were 26 pair of mothers and their children whose's HBsAg were both positive.Nine of the mother's HBeAg and HBcAb were positive.While five of the mother's HBeAb and HBcAb were positive,and ten of the mother's HBcAb were positive.The differences of the three were statistically significant (?2=6.03,P

Objective To observe the effects of static pressure on the number of cultured retinal M?ller glial cells(RMGC)and expression of glial fibrillary acidic protein(GFAP)and heat shock protein(HSP)70 by these cells.Design Experimental study. Participants Cultured rat RMGC.Methods Rat RMGCs were cultured and identified according to previous method described by Reichenbach.These cells were treated with different static pressures and divided into 4 groups:A(1.33kPa),B(2.67kPa),C(5.33kPa)and D(10.67 kPa)while the cells without treatment was as control group(NC).The morphologies of RMGC in these groups were observed under inverted phased contrast microscope,the number of RMGC counted with conservative method and the viability were studied with trypan blue staining.The expressions of GFAP and HSP70 in RMGCs were detected with the method of western blot.Main Outcome Measures The morphologies of RMGC,cell number,cell viability.Results There were pressure-dependent changes of RMGC number. The cell number of group C and D was less than that of group NC,A and B(P<0.01).High static pressure resulted directly in the decreased ratio of unstained RMGCs(P<0.01).The ratio of unstained RMGCs in group C and D was less than that in group NC,A and B(P<0.01).Many cells in group C and D were injured and the higher the pressure elevated,the more the degree of injury became.The expressions of GFAP and HSP70 in group NC were less than other pressure treated groups and the expression of GFAP in group C and D was higher than that in group A and B.There was no obvious difference between these pressure treated groups.Conclusions High static pressure could cause the injuries of RMGCs.The increased expression of GFAP and HSP70 in RMGC might be regarded as a sign of retinal injury response to high intraocular pressure.
10.Endovascular therapy for complex subclavian artery occlusive diseases

Objective To discuss the key points of endovascular therapy for complex subclavian artery occlusive diseases. Methods During the period from January 2012 to December 2013, a total of 92 patients with complex subclavian artery occlusive disease were admitted to Xuanwu Hospital of Capital Medical University, Beijing, China. The clinical data were retrospectively analyzed. The features of the lesions, the success rate of endovascular therapy, the use of combined approaches, the relief of symptoms after treatment, etc. were evaluated. Results The complex subclavian artery occlusive diseases could be divided into three types. Type Ⅰ: long segment of the left subclavian artery was occluded; type Ⅱ: ostial stenosis or occlusion of the right subclavian artery; and type Ⅲ: subclavian artery stenosis or occlusion was associated with the ostial disorder of the vertebral artery, or the opening of vertebral artery was affected by the subclavian artery stenosis or occlusion. The technical success rate was 82.6%. Combination use of femoral artery and brachial artery approach was employed in 27.2% of patients, which had improved the technical success rate. After the treatment the symptom improvement rate was 81.6%. Conclusion Upper limb artery approach can improve the re-canalization rate of left subclavian artery with long segment occlusion, and can ensure the accurate positioning of stent at the site of right subclavian artery opening. During the procedure of endovascular intervention for subclavian artery occlusion disease, attention should be paid to the protection of the vertebral artery.
