1.Functional hemodynamic monitoring should be emphasized in intensive care for burn and trauma patients.
Chinese Journal of Burns 2014;30(4):291-294
Hemodynamic monitoring is a very important measure for critically ill patients with burn and trauma, and it should be carried out throughout the course of treatment. Functional hemodynamic monitoring consists of the assessment of the dynamic interactions of hemodynamic variables in response to a defined volume change. Accordingly, response of fluid volume can be assessed in a quantitative fashion by measuring variation of both arterial pulse pressure and left ventricular stroke volunime during positive pressure breathing, or the change in cardiac output response to passive leg raising maneuver. Functional hemodynamic monitoring, contrary to that of static condition in order to realize dynamic and individual monitoring, is related to response to treatment, and it is a useful complement to static (routine) hemodynamic monitoring. At present, in the care of sepsis, shock, and mechanical ventilation, etc. related to burn injury and trauma, functional hemodynamic monitoring is more and more accepted and applied by medical personnel of ICU in burn and trauma departments. Therefore, further study on functional hemodynamic monitoring should be emphasized and practised.

2.Effect of Myocardial Ischemic Preconditioning on Ischemic/Reperfusion Injury in High Blood Fat Rat
Journal of Shanghai Jiaotong University(Medical Science) 2006;0(09):-
Objective To study the effect of myocardial ischemic preconditioning on activity of ATPase and creatine kinase(CK) in high blood fat rat. Methods High blood fat rat mode was established from SD rats. The rats were randomly divided into three groups: ischemic preconditioning(IPC), ischemic/reperfusion(I/R) and control group. The activity of CK in coronary outflow, the activity of malonyldialdehyde(MDA),superoxide dismutase(SOD),glutathione perodxidase(GSH-Px) and ATPase in myocardium were dectected. Results CK and MDA were significantly less in IPC group than those in I/R group. In IPC group, the activity of SOD,GSH-Px,Na~(+)-K~(+)-ATPase,Ca~(2+)-ATPase and Ca~(2+)-Mg~(2+)-ATPase were much higher than those in I/R group. Conclusion Myocardial ischemic preconditioning can protect high blood fat rat from ischemic/reperfusion injury.
3.Effect of folic acid on plasma homocysteine in patients with sudden sensorineural hearing loss
Journal of Shanghai Jiaotong University(Medical Science) 2006;0(02):-
Objective To study the effect of folic acid on decreasing level of plasma total homocysteine(tHcy)in patients with sudden sensorineural hearing loss(SSHL) and the optimal dosage of folic acid. Methods Ten randomized controlled trials involving treatment data on 210 patients with SSHL were retrospectively studied. They were divided into seven groups according to the daily dosage of folic acid: group A to group G,0.2 mg,0.4 mg,0.8 mg,2.0 mg,5.0 mg,10.0 mg and 15.0 mg,respectively.Besides oral administration of folic acid,Vitamine B6 and B12were supplemented, and other routine treatment were performed.Fluorescence polarization immunoassay was employed to detect the plasma tHcy before and 3 months after the treatment. And the data of plasma tHcy of 210 patients without SSHL were collected and served as controls. The levels of plasma tHcy were statistically analysed between the SSHL group and control group and among group A to group G. Results The level of plasma tHcy in the SSHL group was significantly higher than that in the control group,(18.07?1.58)?mol/L vs(13.63?1.33) ?mol/L(P0.05). Conclusion The levels of plasma tHcy are significantly increased in SSHL.Folic acid may play an important role in decreasing the levels of tHcy in patients with SSHL,and a dosage of 10 mg/d for oral adminstration is well suggested.

To evaluate the application of intracavitary three-dimensional ultrasonography in the diagnosis of polycystic ovary syndrome ( PCOS). The three-dimensional volumes of ovary were measured by intracavitary ultrasonography in 40 PCOS patients and 40 normal controls. The diagnostic value of parameters derived from three-dimensional ultrasonography was evaluated by using the receiver operating characteristic (ROC) curve. The ovarian volume, stromal volume, and the stromal/total volume ratio in PCOS group were significantly higher than those in control group (both P < 0. 05 ) ; the area under ROC curve ( AUC ) of them were 0. 876, 0. 926, 0. 886 respectively. Among these parameters, stromal volume had the highest diagnostic value for PCOS, the cut-off value of which was 7.52 cm3 with a sensitivity of 88% and a specificity of 90%. Volume parameters of ovary measured by intracavitary three-dimensional ultrasonography can offer quantitative information for the diagnosis of PCOS.

Objective To investigate the effect of ischemia and ischemia/reperfusion(I/R)in rat heart on matrix metalloproteinase-1(MMP-1).Methods The I/R animal models were established by shutting down and reopening the anterior interventricular branch with a silver clamp, then the distribution and amount of MMP-1 of the normal and I/R rat hearts were observed by immunohistochemical staining and Western blotting and analyzed by computer image analysis. Results 1.Immunohistochemical staining showed MMP-1 existed mainly in the cardiac matrix. There were strong positive reactions in fibrocytes, smooth muscle cells of the blood vessel and endotheliaI cells of capillaries.MMP-1 didn't show distinct changes 30 minutes after ischemia, while its concentration increased dramatically 60 minutes after ischemia. The positive reaction of MMP-1 increased 30 minutes after I/R,and 60 minutes after I/R there was large fusion areas in MMP-1 existing reglons.2.Quantitative analysis showed no dramatic changes of MMP-1 after ischemia for 30 minutes(P>0.05),while dramatic changes were seen 60 minutes after ischemia(P<0.05).MMP-1 changed dramatically 30 minutes and 60 minutes after I/R.3.Western blotting showed that there were no distinct naked-eye-observable changes. The bands of MMP-1 became widened 30 minutes after I/R,and became obviously widened 60 minutes after I/R.Conclusion 1.MMP-1 is secreted by fibrocytes, smooth muscle cells and endothelial cells of cardiac tissue under physiological conditions, and cardiomyocytes has the potential to secrete MMP-1 under ischemia or I/R.2.The longer time the heart ischemia lasts, the greater MMP-1 concentration will increase.Reperfusion can increase MMP-1 concentration to an even higher level, which may be the main cause of the collagen destruction after heart I/R.

Abstract: Objective To investigate the clinical manifestations, diagnosis, treatment and prognosis of COVID-19-associated multisystem inflammatory syndrome in children (MIS-C) with multisystem organ failure, and to analyze the differences between the syndrome resembling Kawasaki disease and Kawasaki disease. Methods We report a case of child diagnosed with SARS-CoV-2 infection who developed MIS-C with multi-organ failure. Literature was reviewed to further discuss clinical symptoms, diagnosis, treatment, and prognosis of MIS-C. Results The patient 's condition progressed rapidly and was severe, with acute kidney injury at admission, presenting with urine output of 0 mL in 8 hours, serum creatinine of 675.1 μmol/L, urea nitrogen of 38.2 mmol/L, creatine kinase 4 932 U/L, along with continuously elevated serum amylase, peaking at 5 809.7 U/L, aspartate aminotransferase of 995 U/L, alanine aminotransferase of 240 U/L, and frequent vomiting. The patient was categorized as critically ill. The confirmed patient with MIS-C was managed in pediatric intensive care units (PICU), received early continuous blood purification, jejunal tube feeding, and combined treatment with intravenous methylprednisolone pulse and human immunoglobulin for intravenous injection (IVIG), resulting in complete recovery and discharge. Through literature review, it can be analyzed that MIS-C is systemic inflammatory response characterized by multi-organ dysfunction from a cytokine storm, which differentiates it from Kawasaki disease characterized by acute systemic vasculitis triggered by infectious factors, ultimately leading to cardiovascular accidents as the main cause of death. Conclusions When fever (≥38 ℃) persists for at least 24 hours, accompanied by high inflammatory symptoms and dysfunction in two or more systems, requiring intensive care treatment, and there is evidence of SARS-CoV-2 infection, MIS-C should be diagnosed early, and intervention and treatment should be initiated promptly to achieve favorable outcomes.
