Objective To evaluate the clinical efficacy of traditional Chinese mental intervention in treating post-apoplexy depressive disorder and its effects on recovery of activities of daily living(ADL).Methods 64 cases of post-apoplexy depressive disorder patients of light or moderate degree(evaluated by HAMD scale)were randomly separated into an experiment group and a control group.On the basis of identical basic clinical treatments,the experiment group was treated with traditional Chinese mental intervention,and the results were evaluated by HAMD scale and ADL scale at the 40th,the 70th and the 100th day.Results There were no differences in marks of the two scale between the experiment group and the control group at the beginning(P＞0.05).At the 40th day,the HAMD scale mark was 13.04±4.86 in the experiment group and 17.92±5.82 in the control group,showing significant differences(P＜0.05)between the two groups.At the same time,the ADL scale mark was 61.85±11.73 in the experiment group and 53.02±10.54 in the control group,showing significant differences between the two groups too(P＜0.05).At the 70th day and 100th day,the differences became much more notable(P＜0.01).Conclusion Traditional Chinese mental intervention was effective in promoting the motor function and activities of daily living and relieving the depress disorder for post-apoplexy depressive disorder.Post-apoplexy depress disorder patients should be treated with Chinese traditional mental intervention as soon as possible.

To study the chemical constituents from the stems of Picrasma quassioides. The constituents were isolated and purified by silica gel, Sephadex LH-20, ODS column chromatographies, and preparative HPLC. Their structures were determined on the basis of their physicochemical properties and spectral data. The cytotoxic and antibacterial activities were assessed by MTT and MIC, respectively. Three β-carboline alkaloids were obtained from the 95% ethanol extract of the stems of P. quassioides and identified as picrasidine F (1), picrasidine G (2), and picrasidine S (3). Compound 1 showed selective cytotoxicity to HeLa cell, while compounds 2 and 3 showed the potent cytotoxicity against human HeLa cervical, gastric MKN-28, and mouse melanoma B-16 cancer cells. Compounds 1-3 showed the potent antibacterial activity against two strains of pathogenic bacteria methicillin-resistant Staphylococcus aureus (MRSA) and two strains of pathogenic bacteria methicillin-sensitive Staphylococcus aureus (MSSA). The assignments of NMR data of compound 1 are reported in this paper for the first time. Compounds 1-3 show the potent cytotoxic and antibacterial activities.

BACKGROUND:Both of acupuncture and mesenchymal stem cel transplantation have therapeutic effects on cerebral palsy. OBJECTIVE:To explore the therapentic effect of acupuncture combined with placenta derived mesenchymal stem cel s on rat cerebral palsy. METHODS:Thirty-five Wistar rats were equivalently randomized into five groups, including control group, model group, acupuncture group, transplantation group and combination group. Rat models of cerebral palsy were prepared in al groups except the control group. At 7 days after modeling, in the transplantation group, 10μL placenta derived mesenchymal stem cel s were transplanted into the right cerebral cortex and striatum of rats;rats in theacupuncture group were given acupuncture at Shuigou (DU26), Dazhui (DU14), Baihui (DU20), Guanyuan (RN4) and Qihai (RN6) points;rats in the combination group given acupuncture and transplantation treatment as described above;the model and control groups received no treatment. Then, after 7-day intervention, the body mass, neurological function and pathological changes of rat brain tissues were measured and observed, respectively. . RESULTS AND CONCLUSION:The order of the growth rate of body mass was as fol ows:control group>combination group>acupuncture and transplantation groups>model group, and al groups had significant differences except the two groups of acupuncture and transplantation (P<0.05);findings from holding tract, foot error and water finding in maze tests showed the neurological function was best in the control group, fol owed by the combination group, as wel as acupuncture and transplantation groups, and the worst in the model group. Al groups had significant differences except acupuncture and transplantation groups (P<0.05). Besides, in the model group, cel s in the brain and hippocampus decreased, arranging disorderly with fuzzy structure, and inflammatory cel s appeared;pathological changes in the other groups were in between control group and model group, Additional y, brain tissues in the combination group exlibited better pathological changes than those in the acupuncture and transplantation groups, which were similar with the normal brain tissues. To conclude, acupuncture, placenta derived mesenchymal stem cel s transplantation, or their combination can improve neurological function in rats with cerebral palsy, especial y the combination treatment.

Objective To investigate the pregnancy opportunity and outcome of patients with sysmetic lupus erythematosus (SLE).Methods We carried out a retrospective analysis of the clinical information of patients who suffered from SLE complicated with pregnancy and were hospitalized to People's Hospital of Peking University from December of 1992 to February of 2012.Chi-square test were used for statistical analysis.Results Forty-nine cases of patients with SLE complicated with pregnancy had 52 pregnancies in total.In 27 cases of planned pregnancies,24 cases(89％ )resulted in live births and 5 cases( 18％ ) had flares during pregnancy.Of the 24 live births,3 cases( 12％)were premature deliveries and 4 cases(17％) had low birth weight infants.In 25 cases of unplanned pregnancies,12 cases (48％) resulted in live births and 20 cases (80％) had flares during pregnancy.Of the 12 live births,6 cases(50％ ) were premature deliveries and 6 cases (50％) were low birth weight infants.The patients in the planned pregnancy group tended to have flares during pregnancy more frequently than those in the unplanned group (P＜0.01),the former were more likely to have live births than the latter (P＜0.01) and were less likely to have premature delivery (P＜0.05).Conclusion Patients with SLE should have planned pregnancy and need collaborative supervision of both rheumatology and immunology department and obstetric department to improve live birth rate,decrease premature delivery and the activity of SLE during pregnancy.

Objective To investigate the effect of ischemia and ischemia/reperfusion(I/R)in rat heart on matrix metalloproteinase-1(MMP-1).Methods The I/R animal models were established by shutting down and reopening the anterior interventricular branch with a silver clamp,then the distribution and amount of MMP-1 of the normal and I/R rat hearts were observed by immunohistochemical staining and Western blotting and analyzed by computer image analysis.Results 1.Immunohistochemical staining showed MMP-1 existed mainly in the cardiac matrix.There were strong positive reactions in fibrocytes,smooth muscle cells of the blood vessel and endotheliaI cells of capillaries.MMP-1 didn't show distinct changes 30 minutes after ischemia,while its concentration increased dramatically 60 minutes after ischemia.The positive reaction of MMP-1 increased 30 minutes after I/R,and 60 minutes after I/R there was large fusion areas in MMP-1 existing reglons.2.Quantitative analysis showed no dramatic changes of MMP-1 after ischemia for 30 minutes(P＞0.05),while dramatic changes were seen 60 minutes after ischemia(P＜0.05).MMP-1 changed dramatically 30 minutes and 60 minutes after I/R.3.Western blotting showed that there were no distinct naked-eye-observable changes.The bands of MMP-1 became widened 30 minutes after I/R,and became obviously widened 60 minutes after I/R.Conclusion 1.MMP-1 is secreted by fibrocytes,smooth muscle cells and endothelial cells of cardiac tissue under physiological conditions,and cardiomyocytes has the potential to secrete MMP-1 under ischemia or I/R.2.The longer time the heart ischemia lasts,the greater MMP-1 concentration will increase.Reperfusion can increase MMP-1 concentration to an even higher level,which may be the main cause of the collagen destruction after heart I/R.

ObjectiveTo evaluate whether endometrial blood flow measured by three-dimensional power Doppler ultrasound can predict pregnacy after the treatment of in vitro fertilization and embryo transfer (IVF-ET).MethodsNinty women who received the treatment of IVF-ET examined by threedimensional power Doppler imaging at the day of inject human chorionic gonadotropin(HCG) were enrolled.All patients were divided into pregnancy group (43 cases,include biochemical pregnancy and clinical pregnancy ) and non-pregnancy group(47 cases).The diagnostic value of parameters derived from threedimensional power Doppler imaging was evaluated by using the receiver operating characteristic (ROC)curve.ResultsDifferences of these parameters,including vascularization index (VI),flow index(Fl) and vascularization flow index (VFI)were considered statistically significant between two groups,the parameters of uterine artery and endometrial volume had no significant differences between two groups.The area under ROC curve of VI,FI and VFI were 0.880,0.713 and 0.646,respectively.Among these parameters,VI had the highest diagnostic value for discriminating between benign and malignant endometrium,the cut-off value of which was 0.733 with a sensitivity of 84.2％.Conclusions Threedimensional power Doppler ultrasound was useful for evaluating endometrial blood flow in IVF-ET cycles.Endometrial blood flow parameters may be useful predictors for pregnancy.

OBJECTIVE To investigate the status of hospital infection of elderly in internal medicine,in order to prevent and control hospital infection of them.METHODS A restrospective survey on 157 cases of hospital(infection) of elderly from Aug 2003 to Apr 2005 was conducted in internal medicine.(RESULTS)The results showed that the high risk wards were in geriatrics,neurology,endocrinology and(cardiovascular) departments.The high risk season was in winter.Respiratory tract was the most common(infective) site.Fungi were the main pathogens.CONCLUSIONS The hospital infection of elderly should be(controlled) better,geriatrics department is the key(control) unit.Winter is the key control season.The diagnostic level of clinician about the hospital(infection) of(edlerly) should be enhanced and the antibiotic must be used reasonably.

BACKGROUND:Acupuncture is proved to have therapeutic effect on cerebral palsy of immature rats.
OBJECTIVE:To discuss the possible effect on cerebral amino acids and neurotransmitters in immature rats with cerebral palsy by using fire-needle.
METHODS:Cerebral palsy models were established in 45 Wistar rats aged 1 week by left common carotid artery ligation and randomly assigned into model, acupuncture and positive groups (n=15 per group). The other five rats were defined as controls, without any management. At 3 days, positive group rats were oral y administrated with brain enzyme solution, and acupuncture group given fire-needle treatment at points of Neiguan (P 6), Yongquan (K 1), Quchi (LI 11) and Baihui (Du 20) for 21 days. Hanging test, inclined plane test and Y maze test were performed 1 hour after the last treatment, and the cerebral cortex and hippocampus were extracted and homogenized after execution. Lelves of acethl choline, norepinephrine, 5-hydroxytryptamine, glutamic acid andγ-aminobutyric acid were recruited, and morphology of hippocampus was observed by pathological section.
RESULTS AND CONCLUSION:(1) Within 21-day treatment, positive and acupuncture treatment rats were more active than model group. Compared with the control group, the body mass in the model group was significantly decreased (P<0.05);compared with the model group, the body mass in the positive and fire-needle groups were significantly increased (P<0.05), while it showed no significant difference between latter two groups. (2) The hanging time of model group rats was significantly shorter than that of control group (P<0.05), and significantly lengthened in the positive and fire-needle groups compared with the model group (P<0.05). Compared with the control rats, the error numbers of model group rats were significantly increased (P<0.05), and conversely, significantly decreased in the positive and fire-needle groups (P<0.05). (3) In the model group, the levels of norepinephrine, 5-hydroxytryptamine andγ-aminobutyric acid were significantly decreased, while glutamic acid significantly increased compared with the control group (P<0.05);compare with the model group, the positive and fire-needle group rats held significantly increased levels of norepinephrine, 5-hydroxytryptamine andγ-aminobutyric acid and decrease of glutamic acid (P<0.05). (4) Hematoxylin-eosin staining showed that in the mode group, there were abundant neurons damaged, cel volume reduced, nucleus pycnosis and unclear border with nucleus;both positive and fire-needle groups could improve neuron structure induced by cerebral ischemia. (5) In conclusion, fire-needle can treat cerebral palsy effectively.

Objective To study the relationship between ultrasonography of arteria dorsalis pedis and Hemorheology in patient with diabetic foot.Methods According to with or without frank anabrosis,51 patients were devided into two sub-groups:anabrosis group (23 patients) and no-anabrosis group (28 patients).The correlativity was analysed between the parameters of ultrasonography in arteria dorsalis pedis and hemorheological indices on patients with diabetic foot.Results When compared to patients without ulcers,vascular inner diameter (D),peak systolic velocity (PSV),resistent index (RI) and pulsatility index (PI) on patients with frank anabrosis were decreased significantly,but whole blood viscosity at high shear rate (BVH),whole blood viscosity at low shear rate (BVL),plasma viscosity (PV),fibrinogen (Fib) were increased.Multiple regression analyses indicated that the relationship between BVH,BVL and D was presence,partial regression coefficient (r) was -0.004 and -0.009 ; the relationship between PSV and BVH,PV was intimate,r was -0.957 and -25.655 ; the relationship between RI and BVH,PI and BVH,PV,Fib were relatively intimate,r was -0.009 , -0.141 , -2.666 , -0.383 ,respectively.Conclusions The changes of hemorheology can aggravate the abnormality of hemodynamics and the degree of vascular lesion in small vessels of lower limb,so it is one of influencing factor leading to lower limb arterial ischaemia and diabetic foot deterioration.

Objective To evaluate the value of three-dimensional power Doppler imaging(3D-PDI) in the discrimination between benign and malignant endometrium in women with postmenopausal bleeding. Methods Eighty-four women with post menopausal bleeding who examined by 3D-PDI were enrolled. All patients scheduled for pathological examination were divided into benign group (30 cases with endometrial polyps and 34 cases with hyperplasia) and malignant group (20 cases with endometrial carcinoma). Endometrial volume, vascularity index ( VI) , flow index ( FI) and vascularity flow index ( VFI) were measured. The diagnostic value of parameters derived from 3D-PDI was evaluated by using the receiver operating characteristic ( ROC) curve. Results Differences of these parameters (VI, FI and VFI) were considered statistically significant between two groups, the parameters of endometrial volume had no significant differences between two groups; the area under ROC curve(AUC) of the meaningful data were 0.976,0.888 and 0.894 respectively. Among these parameters, VI had the highest diagnostic value for discriminating between benign and malignant endometrium, the cut-off value of which was 2. 97 with a sensitivity of 95% and a specificity of 86%. Conclusions 3D-PDI is a good diagnostic tools in the discriminations between benign and malignant endometrium in women with postmenopausal bleeding.
