2.Value of ductus arteriosus in the diagnosis of fetal congenital heart disease by echocardiography

Objective To explore the value of ductus arteriosus in ultrasound diagnosis of fetal congenital heart disease by echocardiography. Methods The fetal echocardiography was performed with three segments diagnosis method. According to the fetal position, the ductus arteriosus was examined in different sections which include the V shape sign section formed by ductus arteriosus and right pulmonary artery,the V shape sign section formed by ductus arteriosus and the gorge of aortic arch,the three fingers sign section formed by ductus arteriosus,left and right pulmonary artery,the Z shape sign section formed by left pulmonary artery, ductus arteriosus and the gorge of aortic arch. The flow velocity and the inner diameter of the ductus arteriosus were measured. Results Thirty-one cases with congenital heart disease were evaluted. Thirteen cases were approved by autopsy. Among those fetus, 4 cases went with ductus arteriosus stenosis and they all accompanied with right heart obstructing disease,3 cases went with dilated ductus arteriosus and they all accompanied with left heart obstructing disease. Retrograde were explored in 2 fetus. The flow velocity of 5 fetus ductus arteriosus were higher than the normal fetus and 4 cases of them with right heart obstruction, 1 cases with left heart obstruction. Five fetal ductus arteriosus were normal. One fetal ductus arteriosus was absent. Conclusions Assessant of ductus arteriosus by echocardiography is an important content in screening fetal congenital heart disease. It can help us to diagnose quickly.
3.Ultrasound findings in urinary calculi caused by melamine

Objective To analyze the ultrasound findings of urinary calculi caused by melamine. Methods The urinary system of totally 5337 children were examined with high frequency convex ultrasound probe,the characteristics of the images were analysed. CT plain scan method was used when ultrasound result was in difficulty. Results There were 360 cases with calculus in the 5337 children. In the 360 cases, unilateral calculus was in 92 cases, bilateral calculus in 48, bladder stone in 3, urethra stone in 1, kidney calcium deposition in 2, suspicion stone in 214 cases, urinary obstruction in 28 of them. The size of the stones ranged from 3 mm×2 mm to stag-horn calculus. Conclusions The urinary calculi caused by melamine is sandy stone. Ultrasound result is equivocal in the disease, CT plain scan can be a method of choice. High frequency convex probe provides more clear images, more evident adminstrative levels of anatomise than general abdominal probe does. It is beneficial for diagnosing and differential diagnosing of these diseases.

5.Study of synergistic effect of hyperlipoproteinemiaand Aβ in processing of Alzheimer′s disease

Aim To research the synergistic effect of hyperlipoproteinemia and Aβ in the processing of Alzheimer′s disease.Methods Seventy SD rats were randomly divided into seven groups, and dealt with D-gal(hypodermic injection), hyperlipemia diet, microinjection into both side of CA1 section in hippocampus, independently.Morris water maze(MWM) test was used to evaluate the spatial memory impairments.Tau and tau(pThr181) pathology in the hippocampus were detected using Western blot and immunohistochemistry.Nissl′s staining was used to detect cell apoptosis.Results Aβ25-35-treated rats showed significant impairments of spatial memory in MWM test, especially in the group of D-gal+Aβ25-35+HLD(P<0.01).Furthermore, these rats treated with Aβ25-35, D-gal, and hyperlipemia diet, exhibited significantly increased phosphorylation of tau, particularly in the Thr181 site.Conclusion Hyperlipoproteinemia is the risk factor for older person, which could strengthen the toxic effect of Aβ, and promote phosphorylation of tau.
6.The assessment of ultrasonic measurement of superior vena cava blood flow for the volume responsiveness of patients with mechanical ventilation

Objective To approach the evaluative effect of respiratory variation of superior vena cava peak flow velocity measured using transthoracic echocardiography (TTE) on fluid responsiveness in patients with mechanical ventilation.Methods A prospective cohort study was conducted.All mechanical ventilated critically ill patients whose fluid therapy was planned due to hypovolemia in Department of Critical Care Medicine of Beijing Tongren Hospital of Capital Medical University from April 2011 to April 2013 were enrolled.Volume expansion was performed with 500 mL Linger solution within 30 minutes.Patients were classified as responders if pulse pressure variation (PPV) increased ≥ 13% before volume expansion.The respiratory variation in superior vena cava peak velocity was calculated as the difference between maximum and minimum values of velocity in peak A,peak S and peak D over a single respiratory circle,and their variations (ΔA,ΔS,ΔD) were also calculated.The receiver operating characteristic curve (ROC curve) was plotted to assess the evaluative effect of respiratory variation of superior vena cava peak velocity on fluid responsiveness.Results Twenty-seven patients were enrolled in this study.Volume expansion increased PPV ≥ 13% happened in 14 patients (responders).The velocity of superior vena cava in peak A,peak S,peak D was significantly increased after volume expansion compared with that before volume expansion in responders [peak A (cm/s):34.6 ± 2.2 vs.31.3 ±2.1,t=-2.493,P=0.027; peak S (cm/s):39.1 ± 1.3 vs.35.3 ±2.1,t=-2.564,P=0.024; peak D (cm/s):28.1 ± 1.2 vs.23.3 ± 1.4,t=-4.995,P=0.000],but there was no significant difference in ΔA,ΔS and ΔD between before and after volume expansion.The ΔA,ΔS and ΔD were positively correlated with PPV (r=0.040,P=0.854; r=0.350,P=0.074; r=0.749,P=0.000).The area under ROC curve (AUC) of peak S was 0.36 [95% confidence interval (95%CI):0.11-0.52],but the AUC of ΔS was 0.68 (95%CI 0.47-0.89),the AUC of peak D was 0.41 (95%CI 0.19-0.63),but the AUC of ΔD was 0.95 (95%CI 0.86-1.00),so the aberration rate of superior vena cava in respiration was better than the flow rate in superior vena cava.When the cut-off value of ΔS was 20.7% for predicting fluid responsiveness,the sensitivity was 78.6% and the specificity was 61.5%.When the cut-off value of ΔD was 12.7% for predicting fluid responsiveness,the sensitivity was 92.0% and the specificity was 92.3%.Conclusion Respiratory variations in superior vena cava peak velocity measured by TTE could assess fluid responsiveness in patients with mechanical ventilation.
7.Distribution of pathogen species and antibiotic resistance of pathogens from intravenous catheter-related bloodstream infections in pediatric intensive care unit

Objective To investigate the clinical characteristics,distribution and drug sensitivity of pathogens causing intravenous catheter-related bloodstream infections (CRBSIs) in pediatric intensive care unit (PICU) so as to use antibiotics reasonably.Methods All patients with CRBSIs in PICU of Guangdong General Hospital from September 2009 to September 2014 were investigated and the drug resistance profiles of pathogens causing CRBSIs were also analyzed retrospectively.Results Between 2009 and 2014,there were totally 10 834 catheter days and 23 episodes of CRBSIs with an incidence of 2.1 infections per 1 000 catheter days.Catheter indwell time < 7 days in 9 cases (39.1%),8 to 14 days in 10 cases (43.5%),14 to 21 days in 4 cases (17.4%).There were 13 strains (56.6%) of gram-positive bacteria,5 strains (21.7%) of gram-negative bacteria and 5 strains (21.7%) of fungi.The main pathogens causing CRBSIs were coagulase negative Staphylococci (7 strains,30.4%),Staphylococcus aureus (3 strains,13.0%),Candida albicans(3 strains,13.0%),Candida parapsilosis(2 strains,8.7%),and Enterobacter cloacae (2 strains,8.7 %).The susceptibility to Vancomycin,Linezolid and Teicoplanin of coagulase negative Staphylococ cus such as S.epidermidis and to Imipenem,Piperacillin/Tazobactam,Cefoperazone/ Sulbactam and Amikacin of gram-positive bacteria arrived at 100.0%,respectively.The candida were 100% susceptible to Amphotericin B,5-Flucytosine,Fluconazole and Voriconazole.Twenty-one cases (91.3%) received antibiotic treatment versus no antibiotic in 2 cases (8.7%).The average number of antibiotic kinds administered on the patients with fungal infection was 4.4,bacteria were 1.4.Ten cases (43.5%) treatment with 1 kind of antibiotic,4 cases (17.4%) with 2,4 cases (17.4%) with 3,5 cases (21.7%) with more than 3.Twenty-two cases (95.7%) cured and 1 case died (4.3%).Conclusions The major species of pathogen causing CRBSIs was coagulase negative staphylococci in PICU.It is critical for clinicians to guard against fungal infection because of prolonged catheter indwelling time and more antibiotics administered before indwelling catheter.It is effective way to prevent the CRBSIs by reasonably using antibiotics and shortening the time of catheter indwelling.Monitoring CRBSIs pathogenic bacteria distribution and drug susceptibility helps reasonable administration of antibiotics in the earlier time.
8.Application of the protein chip technology in pathology classification and prognosis of B-cell non-Hodgkin lymphoma

Objective To test different expression protein markers of the serum from B-cell non-Hodgkin lymphoma(B-cell NHL) between diffuse large B-cell lymphoma(DLBCL) and follicular lymphoma(FL)patients using sudace enhanced laser desorption/ionization-time of flight-mass spectrometry(SELDI-TOF-MS)protein chip technology. Further, to test different expression of the protein markers of B-eell NHL patients after chemical therapy in order to discuss clinical significance. Methods Different expression of protein markers were analysed in serum between 54 B-cell NHL patients and 27 healthy volunteers by using SELDI-TOF-MS WCX-2 pertein chip. Meanwhile different expression of protein markers relative to pathology classification between 23 DLBCL patients and 12 FL patients were screened; and protein markers which affected prognosis of 23 DLBCL patients were screened. Results There were five specific marker proteins in 54 B-cell NHL patients and 27 healthy volunteers. Their relative molecular weights were 7974, 15 938, 3398,8564, and 8692. The protein markers of 7974 and 15 938 were at high level in patients and the protein markers of 3398, 8564 and 8692 were at low level in patients. There were two protein markers which affected the prognosis, with better outcome when the expression of 4795 and 4998 were increased. Conclusion SELDI-TOF-MS protein chip technology is a quick, easy and convenient method, with high-throughput analyzer which can screen several relatively specific protein markers from the serum of patients to diagnose B-cell NHL The relatively specific protein markers can be used to make pathology classification and to judge the prognosis of B-cell NHL, and have better clinical value.
9.Mechanisms underlying contraction of rat isolated coronary artery induced by acidosis

AIM:To explore the mechanisms underlying contraction induced by extracelluar acidosis (pHex6.8) in rat isolated coronary artery (RCA).METHODS:Using the microvessel tension recorder system, the effects of acid-base transporters on RCA contraction induced by pHex6.8 were explored by applying the selective pharmacological inhibitors of Na+-H+ exchanger 1 (NHE-1) and Na+-HCO-3 cotransporter (NBC), HOE-642 and S0859, respectively.The effects of chloride channel on RCA contraction induced by pHex6.8 were explored by applying the inhibitors of chloride channel (NPPB and NFA), and by replacing the extracellular NaCl with equimolar NaBr.RESULTS:pHex6.8 augmented the resting tension of RCA, and the maximum contraction was (3.90±0.95) mN.HOE-642 at 30 μmol/L and S0859 at 100 μmol/L both inhibited the contraction of RCA induced by pHex6.8 (P<0.01).NPPB and NFA both inhibited the contraction of RCA induced by pHex6.8 or KCl (60 mmol/L) in a concentration-dependent manner.NPPB and NFA (100 μmol/L) both inhibited the contraction of RCA induced by U46619 (1 μmol/L).Replacing the extracellular NaCl with equimolar NaBr almost completely inhibited RCA contraction induced by pHex6.8 (P<0.01), but had no obvious effect on the contraction induced by KCl (60 mmol/L) or U46619 (1 μmol/L).CONCLUSION:Extracellular acidosis-induced contraction in RCA may be related to the activated NHE-1 and NBC, and it may be also related to the enhanced chloride transport across the membrane.
10.Effects of dietary mixed probiotics on the growth performance, antioxidant activity and immune function in yellow-feathered broilers

The present study aimed to study the effects of applying mixed probiotics in the feed on growth performance,antioxidant activity and immune function in yellow-feathered broilers.Three hundred sixty yellow-feathered broilers at 10-day-old were divided into 4 groups.Group A was fed with basal diets,group B with basal diets added 50 g/t mixed probiotics,group C with basal diets added 100 g/t mixed probiotics and group D with basal diets added 300 g/t mixed probiotics.Each group included 9 replicates(10 birds per replicates),and the experiment lasted 50 days.Results showed that group D got the highest average daily gain(ADG) of all groups,and compared with the control group A,ADG was increased by 4.11% (P<0.05).Group B got the lowest feed-gain ratio of all groups,and the feed-gain ratio was decreased by 4.58%(P<0.05) compared with the control group A.Group D got significantly increased duodenal villous length,serum antioxidant index and antibody titer(P<0.05) compared with the control group A.The effects of mixed probiotics on the growth performance,antioxidant activity and immune function were distinct in yellow-feathered broilers,and 100 g/t mixed probiotics was recommended.
