AIM This study was designed to investigate the effect of aminoguanidine, an inhibitor of the glycosylated proteins formation, on the impairment of endothelium dependent relaxation induced by advanced glycation end products (AGE) in isolated rat thoracic aorta and its possible mechanisms. METHODS Exogenous glycosylated bovine serum albumin (AGE BSA) was prepared. Aortic rings were exposed to AGE BSA for 60 min to induce the impairment of endothelium dependent vasodilatation. In the drug treated groups, aortic rings were incubated with drug for 15 min and then exposed to AGE BSA for another 60 min in the presence of the drug. Vasodilator responses to acetylcholine (ACh) or sodium nitroprusside (SNP) of aortic rings were measured by isometric tension recording after drug treatment. RESULTS AGE BSA significantly inhibited the endothelium dependent relaxation in response to ACh, but did not affect endothelium independent relaxation in response to SNP. Pre incubation of aortic rings with aminoguanidine(50~500 ?mol?L -1 ) for 15 min and co incubation of aortic rings with AGE BSA for another 60 min markedly attenuated the inhibition of endothelium dependenet relaxation induced by AGE BSA in a dose dependent manner. Superoxide diamutase (SOD, 2?10 5 U?L -1 ), a scavenger of superoxide anions, also prevented the inhibition of endothelium dependent relaxation, which is similar to the effect of 500 ?mol?L -1 aminoguanidine. Furthermore, aminoguanidine (500 ?mol?L -1 ) also reversed impairment of endothelium dependent relaxation of rat aortic ring induced by endogenous oxygen free radicals generated by diethyldithiocarbamate (DETC, 10 ?mol?L -1 ) via inhibiting intracellular SOD. CONCLUSION Aminoguanidine can protect rat aortic endothelium against damage due to AGE BSA, and the beneficial effect of aminoguanidine may relate to its antioxidant properties.

Objective To evaluate the curative effects of intravenous methylprednisolone (IVMP) during the conservative management for patients with indirect optic nerve injury (ONI),and to analyze the benefits of visual evoked potential (VEP) monitoring in prognosis assessment. Methods Among the 253 patients with cranial-facial injury,108 sufferred from optic nerve injury including 89 cases without optic nerve decompression treatment. These 89 patients were randomly divided into:group A,in which patients were given methylprednisolone intravenously for 7 days; and group B,in which patients without methylprednisolone treatment. The extent of and time required for eyesight recovery were recorded.VEP measurement was carried out in all cases. Results No differences in gender, age and BMI (Body Mass Index, all P > 0.05) were observed. The incidence of ONI was higher in patients with motorcycle accident than that in those with car accident (P <0.01).In the motorcycle accidents, casualties wearing a helmet had lower incidence of ONI (P < 0.01).Patients treated with methylprednisolone had a shorter recovery time (P < 0.01) from blindness, however there was no difference in long-term outcome between group A and B.In patients with visual improvement in the “ non-extinguished” group, the amplitude and latency of the P wave of VEP were markedly ameliorated after the treatments (P < 0.01).Conclusions Most of the ONI casualties in our study were resulted from motorcycle accidents, and wearing a helmet is an effective protection measure. The intravenous methylprednisolone (IVMP) showed no effect on improving the prognosis in patients with conservative treatment. The IVMP shortened the recovery time at the expense of increase in risk of complications.VEP examination is beneficial in prognosis assessment for non-operative ONI patients. The improvement of the amplitude and latency of P wave has close correlations with patients'visual recovery.

Chinese Medical Ethics 1994;0(06):-
Sleep deprivation due to extended work hours and circadian disruptionhas long been a concern in medicine.Clinicians who have been deprived of sleepfailed to ensure that patients are safe or that the quality of care they receive is high.So it can not be ignored to solve the tense relationship between the clinicians and the patients in China. As a result,to pay attention to the fatigue among clinicians and the safety of patients in our country has its practical significance.Take an active strategy to properly resolve the problem of fatigue among clinicians to ensure medical security, the establishment of safe,high-quality health care system, not only directly related to the treatment of patients, but also to the harmonious relations between clinicians and patients to ensure the safety of health care are very important.

Objective To explore reconstructive methods for defects uf bone and soft tissue in traumatic injury uf medial malleolus. Methods A vascular iliac bone with a piece of faacial strip is used together with a local island flap or a free flap to reconstruct the defects of bone, triangular ligament and skin in medial malleolus al one or two stages. Results Follow-ups of 11 to 24 months proved that the stability and walking and weight loading functions uf ankle joint of the patients were restored satisfactorily and their normal varus range without abnormal articular motion was restored.Slight varus occurred during walking in one case, and pain appeared after fatigue in two cases. The X-grams showed no obvious absorption or areosis in the free iliac bone implanted. Conclusion In repair of defects of medial malleolus, combined transplantation of iliac bone and flap is rceommendable because it can keep the essential integrity of malleolus and achieve good Stability and functional recovery of ankle joint.

OBJECTIVETo investigate the changes in the quantity of colonizing Streptococcus mutans(S. mutans) and Actinomyces on the root surface plaque before and after post-core crown restoration of the mandibular first molars in the elderly patients.
METHODSA total of 30 elderly patients, each with one post-core crown restoration of the mandibular first molar, were randomly chosen to participate in the studies. Patients with mandibular first molars with post-core crown restoration and those with healthy contralateral mandibular first molars were divided into the test and control groups, respectively. Root surface plaques of the two groups were collected before tooth preparation, 72 h after preparation, one week after preparation, and one month after restoration. S. mutans, Actinomyces naeslundii (A. naeslundii) and Actinomyces viscosus (A. viscosus), were identified using colony morphology, biochemical techniques, and polymerase chain reaction (PCR). Plaque count was measured using microbial colony count.
RESULTSThe number of S. mutans and A. viscosus and A. naeslundii in the test group, which was statistically significant (P<0.05), increased 72 h after preparation. The quantities of S. mutans, A. viscosus, and A. naeslundii one week after preparation were significantly different (P<0.05). The plaque count of S. mutans, A. viscosus, and A. naeslundii in the test group decreased one month after restoration (P<0.05).
CONCLUSIONThe quantities of S. mutans, A. viscosus and A. naeslundii increase one week after preparation but decrease one month after restoration. The finding suggests that dentists should educate patients about plaque control during the early period after tooth preparation.
