1.Analysis on the medication rules of state medical masterYan Zhenghua from the prescriptions containing Radix Paeoniae Alba based on data mining

Objective To explore the medication rules of State Medical MasterYan Zhenghua. Methods The prescriptions containing Radix Paeoniae Alba that prescribed by Pro. Yan were collected to build a database based on traditional Chinese Medicine(TCM)inheritance assist system. After analyzed by the statistical reports module and the data analysis module which were from TCM inheritance assist system, and the methods of data-mining that including association rules and apriori algorithm, the frequency of single medicine, the frequency of drug combination, the association rules between drugs and core drug combinations which all were including Radix Paeoniae Alba can be get from the database.Results The prescriptions including Radix Paeoniae Alba were commonly used to treat megrim, stomach-ache, acid regurgitation and other syndromes. The highest frequency used drugs were Radix PaeoniaeRubra, PericarpiumCitriReticulatae, Radix SalviaeMiltiorrhizae, Radix AngelicaeSinensis, Poria, and so on. The most frequency drug combinations were “RadxPaeoniae Alba, Radix PaeoniaeRubra”, “Radix SalviaeMiltiorrhizae, Radix Paeoniae Alba”, and “Radix Paeoniae Alba, PericarpiumCitriReticulatae”. The drug association rules that the confidence was more than once were “Radix PaeoniaeRubra - Radix Paeoniae Alba”, “OsDraconis - Concha Ostreae”, “Radix AngelicaeSinensis,RadixPaeoniaeRubra - Radix Paeoniae Alba”, and “Radix PaeoniaeRubra, HerbaTaxilli - Radix Paeoniae Alba”, and so on.Conclusions The drugs in the prescriptions containing Radix Paeoniae Alba that built by Pro. Yan mostly had the effects of enriching the blood and invigorating the circulation of blood, relieve uneasiness of mind and body tranquilization, which reflected the clearly thought when constructing prescriptions.
2.Diagnosis and treatment of cholesterol granuloma of the middle ear

OBJECTIVE To discuss the diagnosis and treatment of cholesterol granuloma of the middle ear and the choice and effect of surgical methods. METHODS A retrospective analysis was conducted on 18 cases of cholesterol granuloma of the middle ear confirmed by surgery and pathology. Different choices of surgical methods were made according to the lesion extension of this disease. RESULTS All patients were followed up for at least 6 months. Two cases with eustachian tube obstruction had ventilation tube inserted for a long time. One case who had recurrence and metastasis of nasopharyngeal carcinoma in the middle ear was transferred to oncology department for further treatment. Post-operative dry ears were obtained in the other cases whose pure tone thresholds were improved to different degrees. CONCLUSION Etiology of cholesterol granuloma of middle ear remains controversial. It's supposed to result from the bleeding, occlusion of ventilation and disturbance of drainage of the air cavity because of the middle ear inflammation, and bone marrow exposure. The middle ear and mastoid surgery can completely remove the disease tissues, and establish ventilation system of mastoid-tympani-eustachian, which is the key point of the middle ear surgery.
3.Review of hip joint center in total hip replacement

[Objective]To explore the effect of hip joint center on the stability of hip joint,the radiographic measurements were compared between preoperation and postoperation.[Method]A total of 155 hips from 120 patients with primary total hip replacement from resent years were studied,the hip joint center difference through radiographic measurements between preoperation and postoperation were compared.[Result]The coincident rate and the incoincident rate were 63.23%(group A,98 hips) and 36.77%(group B,57 hips) respectively.For loose,dislocation or low back pain,the rate of revision in group A and group B were 6.12% and 29.82% respectively.[Conclusion]The comeback of hip joint center was essential for stability of hip joint.
4.Expression of connective tissue growth factor is enhanced in human and rat liver fibrotic tissues

Objective To explore the role of connective tissue growth factor(CTGF)in the mechanism of liver fibrosis.Methods Liver fibrosis models of rats were made by subcutaneously injecting with CCl4.On the other hand,32 human liver cirrhosis and 12 normal liver tissues were collected too.Immunohistochemical staining was used to detect the expression of CTGF in these tissues.Results The expression of CTGF in rats after injection with CCl4 was found mainly in hepatic stellate cells(HSC) and hepatocytes are all significantly higher than those in normal rats.Moreover,it presents with progressive tendency for the expression of CTGF in rats respectively at 1st,4th and 8th week after injection of CCl4.The same tendency was found in human normal and fibrotic liver tissues(P
5.Dynamic relationship between macrophage and extra cellular matrix in the kidney of rats with experimental nephritis

AIM: To observe the changes in infiltrating macrophages (M?)and extracellular matrix (ECM) in the kidney in the progressive course of nephrotoxic nephritis (NTN). METHODS: NTN model was established with rabbit-anti-rat nephrotoxic serum. On day 3, 7, 15, 30 and 90, renal biopsies were performed. Renal histology was checked under light microscopy with HE and Masson's trichrome staining sections. M?, fibronectin (FN), type Ⅲ and Ⅳ collagen were examined with immunohistochemistry ABC method. RESULTS: Infiltration of M? appeared on day 3 of NTN and preceded changes of FN and collagen. On day 15 of NTN, all nephritic animals had significant proteinuria, increased serum creatinine, infiltration of M? and deposit of entracellular matrix. On day 90 of NTN, seven nephritic animals improved significantly, while other five developed renal scarring with diffuse infiltration of M? which positively paralleled to renal function and deposit of FN, type Ⅲ and Ⅳ collagen. CONCLUSION: M? infiltrating into renal tissue enhances deposit of ECM and therefore plays important roles in progression or improvement of NTN.
6.Analysis on the principle of Yang Boliang for the treatment for dysentery based on apriori and clustering algorithm

Objective To analyze the experience of Yang Boliang for the treatment of dysentery. Methods The prescriptions for dysentery that used by Yang Boliang were collected to build a database, and analyzed by the unsupervised data mining methods, such as apriori algorithm, entropy Clustering complex systems. Results Based on the analysis of 35 prescriptions, the most frequently used drug, the core drug combinations and the new prescriptions were mined from the database. The most frequently used drugs were tuckahoe, rhizoma pinellinae praeparata, and roasted radix puerariae. The core drug combinations were“tuckahoe- radix scutellariae-moutan bark”, “plantago seed-rhizoma pinellinae praeparata-maticated leaven”, and “charred radix aucklandiae-charred radix etrhizoma rhei-waterlily leaf”, etc. The new prescriptions were such as “plantago seed-rhizoma pinellinae praeparata-maticated leaven-charred radix rehmanniae recen-processed rhizoma Cyperi”. Conclusion Yang Boliang was well experienced in treating dysentery by using the drugs with clearing heat, and drying dampness, and clearing dampness by promoting diuresis.
7.Rules of Property of Drugs Used by State Medical Master Yan Zhenghua Based on Data Mining

Objective To analyze the rules of property of the drugs used by State Medical Master Yan Zhenghua in clinical.Methods The prescriptions used by Pro. Yan were collected to build a database, which was based on traditional Chinese medicine inheritance assistant software. After analyzed by the software, such as using the module to analyze the prescriptions, the medication characters of the prescriptions can be got from the database.Results Drugs of warm nature were used with the highest frequency 7998 times, followed by the cool 7866 times, leveling 6763 times, cold 3942 times, and hot 95 times. From the property of five flavors, the most used flavor of drugs was bitter 15260 times, followed by sweet 10810 times, pungent 10453 times, sour 2794 times, salty 1651 times, mild 1203 times, and astringency 186 times. In the frequency of the channel tropism involved, the highest is of liver channel 14237 times, followed by lung 10452 times and spleen 10061 times.Conclusion Pro. Yan was accustomed to using the drugs that were of warm and cool natures, and sweet and pungent flavors, and also the drugs that have action on the collateral channels of liver, lung and spleen, which were the same as the experience from Pro. Yan.
8.Difference in time-course of relaxant effect of rocuronium between patients of Uygur and Han nationalities

Objective To compare the time-course of relaxant effect of rocuronium between patients of Uygur and Han nationalities.Methods Thirty ASA Ⅰ or Ⅱ patients (15 Uygur nationality,15 Han nationality) of both sexes,aged 20-55 yr weighing 45-73 kg were divided into 2 groups (n=15 each):group U Uygur nationality and group H Han nationality.Anesthesia was induced with midazolam,propofol,fentanyl and suxamethonium chloride.Tracheal intubation was performed and the patients were mechanically ventilated.Recuronium 0.6mg/kg was then given iv.Neuromuscular function was monitored with acceleromyograph using single stimulation of the ulna nerve.The onset time (time from injection of recuronium to maximal paralysis),nonresponse time (duration of maximal twitch depression),clinical duration (time required for the twitch to return to 25% of control height after injection of initial dose),75%recovery time (time for twitch to return to 75%of control height after injection of initial dose) and recovery index (time for twitch height to return from 25% to 75% of control height) were recorded.Results There was no significant difference in the onset time between the two nationalities.The time of nonresponse period,clinical duration and 75% recovery time were significantly shorter in Uygur patients than in Han patients.Recovery index was significantly shorter in Uygur patients.Conclusion There is a significant difference in the time-course of muscle relaxant effect of recuronium between Uygur and Han nationalities.The influence of nationality on recommended dosage of recuronimn should be considered.
9.Analysis on Menghe Physician Ma Peizhi’s Medication Rule in Prescriptions for Cough Based on Knowledge Discovery in Database

Objective To analyze the composing experience of Menghe physician Ma Peizhi for cough by TCM inheritance support system. Methods The prescriptions for cough of Ma Peizhi were collected, frequency and association of drugs were analyzed by using data mining methods such as revised mutual information, complex system entropy cluster and unsupervised hierarchical cluster. Results Based on the analysis of 57 prescriptions, the frequency of each herb and association rules among the herbs were computed, 18 core combinations and 9 new prescriptions were mined from the database. Conclusion Menghe physicians Ma Peizhi is well experienced in expelling wind and opening the inhibited lung, dissolving phlegm and relieving cough. TCM inheritance support system can be used to analyze clinical experience of old TCM doctor.
10.Operative treatment to spinal multi-level tuberculosis

ObjectiveTo investigate the effect of operation treating spinal multi-level tuberculosis.Methods45 patients with multi-level spine tuberculosis were treated with debridement completely and anterior or lateral-anterior intervertebrae autograft. Of them, 5 patients added to anterior instrumentation.Results45 patients had been followed up for 12 to 40 months.The back pain of 40 cases were relieved within 5 weeks after operation, erythrocyte sedimentation rate decreased 3 weeks after operation. 38 patients who involved in kyphosis decreased their kyphosis mostly and only one patient lost 3 degrees in follow-up.15 patients who involved in neurological deficits improved one or two grades (Frankel). Grafts fused in 44 patients and there were no recurrent in follow-up.Conclusions Operative treatment is efficacious to multi-level body spine tuberculosis.
