ObjectiveTo analyze the protective effects of heat-shock response on the retinae of the rats after retinal ischemic reperfusion injury. MethodTwenty Wistar rats (20 eyes) were divided into 4 groups: intracameral perfusion group (group P), intracameral perfusion after quercetin injection group (group P+Q), intracameral perfusion after heat shock group (group P+H), and intracameral perfusion after quercetin injection and heat shock group (group P+Q+H). According to the standard program established by International Society for Clinical Visual Electrophysiology, we recorded the results of the dark-adapted electroretinogram (D-ERG),oscillatory potentials (OPs),and light-adapted ERG (L-ERG) of the rats with intraocular hypertension after induced by heat shock response. The expressions of HSP 70 of the rats in all groups were observed by Western blotting.ResultsThe expression of HSP 70 of the rats in group P+H was the highest in all groups, but the expressions of HSP70 in group P+Q and P+Q+H were inhibited significantly. The amplitudes of a and b wave of ERG and O 2 wave of OPs decreased, and the delitescence of them were delayed significantly in rats after intracameral perfusion. Objective To explore the effect of surgical treatment of bronchioloalveolar carcinoma (BAC). Methods Data were collected from 94 patients with BAC (including 67 cases with pure bronchioloalveolar carcinoma, 16 cases with BAC by local infiltration and 11 cases with adenocarcinoma having BAC character). All patients were proved pathologically bronchioloalveolar carcinoma from January 1989 to December 2003. Clinical features, characteristics of radiology, methods of treatment and survival status were studied retrospectively. Surgical treatment and efficacy approaches according to different TNM stages and clinical patterns were also analyzed. Results Ninety-eight patients underwent complete resection and 4 underwent palliative operation and 2 underwent exploratory thoractomy. The 1-, 3-, 5-year survival rates were 75.5 %, 53.2 %, 41.5 %. According to international staging system of lung cancer, there were marked difference in stage Ⅰ group, stage Ⅱ group and stage Ⅲ group (P <0.01). According to pathological classification, there were marked difference in pure bronchioloalveolar carcinoma, BAC with local infiltration and adenocarcinoma having BAC character (P <0.01). Conclusion Bronchioalveoar carcinoma is a special form of lung cancer. It has its own characteristics on biology,radiology,clinic or pathology. Lobectomy is performed commonly in patients with bronchioloalveolar carcinoma. It may be concluded that the early diagnosis, early therapy are the key points for improving the survival rate of BAC.

Two hundred and fifty patients with subclinical hypothyroidism ( SCH) and 120 euthyroid volunteers were recruited for the study, SCH patients were stratified into 2 groups according to TSH levels(group A:TSH<10 mIU/ L; group B: TSH>10 mIU/ L). All subjects were examined for clinical characteristics, thyroid profile, lipid profile, and biomarkers of early atherosclerosis. Patients in group B received L-thyroxine replacement treatment to achieve euthyroidism. After 6 months of stable euthyroidism all measurements were repeated. SCH patients had higher levels of total cholesterol(TC), low-density lipoprotein-cholesterol(LDL-C), asymmetric dimethyl arginine (ADMA), carotid artery intima media thickness(CIMT), thromboxane B2(TXB2), and C-reactive protein(CRP) but with lower nitric oxide(NO) level compared with euthyroid subjects. Levels of TC, LDL-C, CIMT, TXB2, and ADMA decreased, and NO level increased significantly after 6 months of euthyroidism. TSH was positively correlated with TC, LDL-C, CIMT, ADMA, and TXB2; and negatively correlated with NO. Based on multivariate regression analysis, TSH was an independent influential factor of CIMT and NO. We conclude that raised TSH is an important risk factor of atherosclerosis in SCH.

Objective To evaluate the feasibility,safety,and efficacy of endovascular repair combined with second-stage surgical debridement for ruptured femoral pseudoaneurysms (RFP).Methods All patients who underwent emergency repair of an RFP in our institution between 2009 and 2013 were identified.Case notes were retrieved for clinical data.Results Seven patients were identified.All the patients were treated with emergent percutaneous stent-graft implantation and secondary surgical debridement.The procedure time was (55 ± 6.32) min,blood loss was (93.33 ± 25.03) ml,hospital stay was (9.5 ± 0.84) d,and no postoperative complications occurred.Conclusions stent graft implantation for the treatment of RFPs is safe and effective in short-or middle-term outcome.It is an alternative for RFPs and may be the better option,which makes the secondary surgical debridement feasible and viable.

Objective To evaluate clinical use of adaptive statistical iterative reconstruction (ASiR) method in low-dose scan on children temporal bone by comparison of radiation dose delivered by GE Discovert HD CT 750 scan and GE Lightspeed VCT64 scan.Method Sixty patients with congenital deafness were divided into two groups according to gender,age (each N =30).Group a received low-dose CT scan on temporal bone by GE Discovery HDCT 750 (preset NI =12),and the original images were reconstructed with ASiR (weighted value =40％).Group B received CT scan by GE Lightspeed VCT 64 (preset NI =8),and the original images were reconstructed with filter back projection (FBP).Data were collected and analysed using the SPSS 18.0 software.Resultes The mean value of electric current of average tube and CTDIvol were significantly different between group A and B [(110.40 ± 21.72) mA vs.(168.56±24.36) mA,and (26.43 ±3.48) mGy vs.(39.66 ±4.17) mGy,respectively (t=-9.76,-3.31,P＜0.05)].The noise index was (33.13 ±2.68) in group A and (33.79 ±2.93) in group B respectively,and the results had no statistical significance (P ＞ 0.05).Subjective scores of the images were (4.06 ± 0.03) and (4.05 ± 0.03),with no statistical significance either (P ＞ 0.05).Conclusions On a condition of achieving same image quality,CT scan program using GE Discovery HD CT 750 with 40％ ASiR reconstruction and 4 units NI value enhancement could effectively reduce radiation dose on children's temporal bone compared to FBP method using GE Lightspeed VCT 64.

Objective Due to poor effects of sterilization,low efficiency and inconvenience of field emergency transportation for portable exhaust pressure steam sterilizer,we aimed to design and develop a portable pulse vacuum pressure steam sterilizer in order to effectively improve quality and efficiency of sterilization for sterile medical equipment in disaster and emergency medical treatment.Methods From the beginning of February 2013,we focused on the features of small size,light weight,reliable sterilizing effect and fast speed of sterilization,and conducted series of technology researches,and finally developed portable pulse vacuum pressure steam sterilizer which was suitable for individual carrying or air delivery.Results We performed outdoor sterilization tests for portable vacuum pressure steam sterilizer and portable exhaust pressure steam sterilizer under the same condition.Portable pulse vacuum pressure steam sterilizer achieved a 100％ qualification rate,average sterilization cycle was 28.6min per time,sterilization by weight was 48L/h,average water consumption was 0.4L/boiler,average power consumption was 0.7kw/h per time,and sterilization operation index,maneuverability and transportability were significantly better than those of portable exhaust pressure steam sterilizer.Conclusion The developed 4 portable pulse vacuum pressure steam sterilizer have higher cost performance,and have been successfully applied for medical service training and non-war medical treatment activities in our hospital.

Objective:To assess the effectiveness and safety of radiotherapy combined with surgery for gastric cancer compared with single surgery. Methods:We retrieved relevant randomized controlled trails in Cochrane Library (Issue 2, 2009), PubMed (Jan 1966-Jun 2009), EMBASE (Jan 1974-Jun 2009), Chinese Biomedical Literature Database (Jan 1978-Jun 2009), Chinese Science and Technology Periodicals Database (Jan 1989-Jun 2009) ,China National Knowledge Infrastructure (Jan 1994 to Jun 2009) and Wanfang database (Jan 1997-Jun 2009). Two researchers assessed the quality of included randomized controlled trials (RCT) and extracted data independently. The RevMan 5.0 software was used for Meta-analysis. Results:Nine RCTs including 1 548 patients were selected for Meta analysis. Five RCTs were related with comparison of preoperative radiotherapy plus surgery with single surgery. Two RCTs were about the comparison of surgery plus perioperative radiotherapy with single surgery. Two RCTs were the comparative studies between surgery plus postoperative radiotherapy and single surgery. The Meta analysis results showed that: (1)compared with surgery alone, preoperative radiotherapy combined with surgery can increase 3 years [OR = 1.78, 95% confidence interal (CI):1.14-2.78], 5 years (OR = 1.67, 95%CI :1.22-2.29),and 10 years (OR = 1.64, 95%CI 1.03-2.60) survival rate,and reduce the tumor recurrent rate(OR = 0.59, 95%CI :0.37-0.92) and metastasis rate (OR= 0.44, 95%CI :0.27-0.73); (2) The tumor recurrent rates (OR = 0.19,95%CI :0.03-1.14 )and tumor metastasis rates (OR = 0.09, 95%CI :0.00-1.77)had no difference between single surgery group and peri-operative radiotherapy plus surgery group;(3) Postoperative radiotherapy compared with surgery had no significant effects on 1 year (OR = 0.83, 95%CI :0.60-1.15) and 3 years (OR = 0.75, 95%CI :0.51-1.11) survival rate compared with single surgery, but the 5 years (OR = 0.57,95%CI :0.34-0.95) survival rate of the patients who received surgery alone was higher than those who received combined therapy. No difference of the tumor recurrence rate(OR=0.59,95%CI :0.33-1.05) and tumor metastasis rate (OR=0.90,95%CI:0.51-1.59) were observed between the two groups. Conclusion:Preoperative radiotherapy combined with surgery is more rational and effective than surgery alone in the treatment of gastric cancer. However in terms of the clinical effects of perioperative or postoperative radiotherapy combined with surgery, more RCTs with larger samples and higher quality need to be carried out to make further verification.
