1.Awareness rate about prevention and control of pulmonary tuberculosis among residents in Beijing

Objective To understand the awareness rate of pulmonary tuberculosis(TB) prevention and control among residents in Beijing, so as to provide basis for giving publicity on knowledge of pulmonary tuberculosis prevention and control.Methods Residents in Beijing were randomly selected for questionnaire survey, the main content of questionnaire included The 2006 national survey on knowledge, attitudes and practice about tuberculosis prevention and control, the core knowledge of pulmonary TB prevention and control, as well as the related symptoms of suspected tuberculosis were also included.Results A total of 520 questionnaires were collected, 503 of which were qualified, the effective rate was 96.73%.Among respondents, the total awareness rate of five core knowledge about pulmonary TB prevention and control was 40.87%, 78.93% of the respondents knew that pulmonary TB was a chronic respiratory infectious disease which could seriously damage people's health,48.51% knew that the majority of patients with pulmonary TB could be cured as long as they persisted in correct treatment, only 19.68% knew that examination and treatment of pulmonary TB in the county (district) level TB control institutions could enjoyed national free policy.Nearly 53.48% of respondents knew 4 or more suspected pulmonary TB symptoms, only 7.36% of respondents could recognize all eight symptoms of pulmonary TB.Conclusion Residents in Beijing have low awareness of pulmonary TB prevention and control knowledge.If patients in the early stage of pulmonary TB can not identify symptoms, it is not only delay treatment and increase the spread of tuberculosis, but also induce the transmission of multidrug-resistant Mycobacterium tuberculosis, so produce more challenges to pulmonary TB prevention and control.

4.Application of pre injection of low dose rocuronium in MECT

Objective To investigate the changes of vital signs, serum potassium and postoperative comfort of patients treated with MECT in the treatment of low-dose rocuronium.Methods 200 patients with MECT from June 2014 to June 2016 were selected and randomly divided into two groups, and the life signs and adverse reactions were analyzed and compared.Results There was no significant difference between the experimental group and the control group in the recovery time, the recovery time and the recovery time of the pharyngeal reflex.The recovery of spontaneous respiration in the experimental group was earlier than that in the control group, and the difference was statistically significant ( P<0.05 ) .There was no significant difference between the two groups (headache, drowsiness, postoperative irritability) compared with the control group, the incidence of muscle pain in the experimental group was significantly lower than that in the control group, the difference was statistically significant (P<0.05).Conclusion Pre-injection of rocuronium can reduce the level of serum potassium, shorten the recovery time of spontaneous breathing, and reduce the incidence of postoperative muscle soreness.Compared with the conventional MECT method, it can reduce the contraindication of MECT treatment and increase the comfort of patients.
5.Clinical study of postoperative three-dimensional conformal radiotherapy concurrent with paclitaxel and TS-1 chemotherapy in the treatment of advanced gastric carcinoma

Objective To investigate the clinical efficacy of postoperative three-dimensional conformal radiotherapy concurrent with paclitaxel +TS-1 chemotherapy in the treatment of advanced gastric carcinoma.Methods A total of 60 patients with advanced gastric carcinoma were randomly divided into treatment group and control group.In treatment group,30 patients received simultaneous three-dimensional conformal radiations therapy combined with paclitaxel +TS-1 chemotherapy.In control group,30 patients received paclitaxel +TS-1 chemotherapy.The long term survival rate,local recurrence rate and side reactions were observed in two groups.Results The 1,2-year survival rate in treatment group [93.1% (27/29),75.9 % (22/29)] and control group [72.4 % (21/29),48.3 % (14/29)] had significant difference (P < 0.05).The 1,2-year local recurrence rate in treatment group [6.9% (2/29),13.8% (4/29)] and control group [31.0% (9/29),44.8 % (13/29)] had significant difference (P < 0.05 or < 0.01).The side reactions in two groups were mainly manifested as leukopenia,thrombocytopenia,anemia,nausea,vomiting,liver function damage and oral mucositis.The incidence rate of nausea,vomiting in treatment group was significantly higher than that in control group [83.3% (25/30) vs.53.3% (16/30)] (P =0.012).Conclusions Postoperative threedimensional conformal radiotherapy concurrent with paclitaxel +TS-1 chemotherapy in the treatment of advanced gastric carcinoma may improve the long-term survival rate,lower local recurrence rate.But the adverse reactions are increased.
6.Investigation of the roles and functions of community nurses in Xi′an

Objective To explore the roles and functions of the community nurses in community health service centers and their main work content in Xi′an. Methods A total of 320 community nurses in Xi′an were investigated by General Information Questionnaire and revision of Community Nurses′Roles and Functions Questionnaire. Results The work often undertaken by community nurses were: health education, community prevention, providing direct care and chronic disease management, however, the hospice care, care for people with mental disorders, family planning guidance and rehabilitation nursing were seldom involved. The scores of each role from high to low were as follows:the organizers and managers, coordinators and partners, health educators, caregivers, observer and researcher and spokesman with(2.56±0.99),(2.43±0.97), (2.38±0.84),(2.34±0.87),(2.29±0.10),(2.07±0.99) points. Conclusions The job of community nurses in Xi′an includes the medical service, disease prevention, healthcare, rehabilitation and health education etc. However, the well done jobs only are limited to some of simple functions, such as health education and health guidance are done well. The work of hospice care and mental care which need more professional abilities and interpersonal cooperation need to be promoted.

8.Endovascular revascularization for the treatment of renal artery stenosis

Objective To evaluate percutaneous transluminal renal angioplasty ( PTRA) and stenting for the treatment of renal artery stenosis (RAS). Method From Feb 2003 through Jun 2005, 19 consecutive RAS patients received interventional therapy including PTRA and/or stents in 12 cases, aortorenal bypass procedures in 4 cases, nephrectomy in 1, and angiography in 2 cases. Among the 12 PTRA and/or stenting cases, stent was deployed in 10 cases, and 2 patients received PTRA only. Results All patients had hypertension preoperatively ( mean blood pressure 172/98 mmHg). Serum creatinine concentration was greater than 1.5 mg/dL in one patient. There was no perioperative mortality nor major complications. Technical success was achieved in 11 patients (91.7%). During follow-up, mean blood pressure was 156/88 mmHg. Hypertension was improved in 8 patients (66. 7% ). Renal function was stable except one patient with renal insufficiency. Restenosis was found in one patient (8.3%). Conclusion PTRA and/or stent is safe, effective for the treatment of RAS in appropriately selected patients.
9.Diagnosis and management of vascular graft infection: a report of 15 cases

Objective To evaluate diagnostic procedures and clinical outcomes of vascular prosthetic and stent graft infection. Methods Clinical data of 15 cases suffering from vascular graft infection between 1985 and 2005 were retrospectively analyzed. Results The rate of vascular graft infection was 1. 14% among our series of 1316 cases of revascularizations. Vascular graft infection occurred within 4 months after graft implantation in 13 cases (86. 7% ). Graft infection developed after 4 months postoperatively in the other 2 cases ( 13. 3% ). Clinical manifestations included wound infection with vascular graft exposure, inguinal swelling or fistula, fever or sepsis, anastomotic hemorrhage, pulse loss of vascular graft or distal lower extremity and gangrene of distal lower extremity. Surgical treatment included en bloc removal of the infected graft and surrounding infected tissue, drainage and local irrigation with antibiotic solution, graft en bloc removal and primary amputation; Graft en bloc removal, drainage and revascularization with a saphenous vein or a new prosthetic graft; Aggressive debridement and local irrigation with antibiotic solution. Four patients died and the others recovered successfully. Conclusion The infection of a vascular graft is a rare complication in vascular surgery. Early diagnosis and aggressive surgical management can improve its prognosis.
10.Expression of HIF-1? in rat's radiation-induced oral mucositis

AIM: To detect the mRNA and protein level of HIF-1 alpha in the tissues of rat's radiated mucosa. METHODS: The left buccal mucosa was irradiated and excised. The right buccal mucosa was excised to serve as own control tissue. The mRNA of HIF-1 alpha was determined by using the semi quantitative RT-PCR. SABC method was employed to immunostain and to elucidate the localization, intensity and distribution of HIF-1 alpha protein.RESULTS: A Sprague-Dawley rat's model of radiation-induced oral mucositis (ROM) was successfully established. The results of RT-PCR indicated that the left buccal mucosa expressed HIF-1 alpha mRNA while the right buccal mucosa did not or seldom expressed it. Immunohistochemical analysis of HIF-1 alpha demonstrated that the left side mucosa expressed HIF-1 alpha protein.CONCLUSION: The mucosa of ROM expresses the mRNA and protein of HIF-1 alpha. The expressions of HIF-1 alpha are correlated with the severity of ROM.
