1.Correlation research on the MRI quantity of lumbar modic changes and low back pain.

OBJECTIVETo analyzed the relationship between lumbar endplate Modic area changes rate and low back pain by measuring MRI T2 sagittal image of lumbar endplate Modic area changes rate.
METHODSFrom December 2011 to June 2012,70 patients with low back pain in operation were evaluated on pain by VAS and function by JOA,and examined by MRI including 39 males and 31 females with an average age of (51.00 +/- 11.89) years ranging from 29 to 72 years old. Among them, 54 cases had lumbar endplate Modic changes involving 15 cases in types Modic I ,21 cases in type Modic II, 11 cases in type Modic III ,mixed type Modic in 7 cases (eliminated for too few cases). Modic area changes and corresponding vertebral area were measured on MRI T2 median sagittal. The areas of two ways were compared to yield the rate of changes for Modic, for multisegmental Modic changes to calculate the total ratios. A correlation was observed among JOA, VAS and the rate of Modic changes.
RESULTSThe correlation coefficient of change rate of Modic I with JOA score was r = -0.308, P = 0.048 < 0.05, there was a negative correlation;the correlation coefficient of change rate of Modic I with VAS scores was r = 0.428,P = 0.021 < 0.05, there was a positive correlation. The correlation coefficient of change rate of Modic II with JOA score was r = -0.375, P = 0.043 < 0.05, there was a negative correlation;the correlation coefficient of change rate of Modic II with VAS score was r = 0.352, P = 0.041 < 0.05, there was a positive correlation. The area change rate of Modic III had no significant correlation with low back pain degree (P > 0.05).
CONCLUSIONModic I and II area changes rate of of patients with low back pain is closely related to the degree of pain low back pain, Modic III area changes rate is not significant correlated to the degree of lower back pain.

3.Cardiac involvement of the Type Ⅰ mucopolysaccharidosis

Objective To investigate the manifestations of cardiac involvement in the patients with mucopolysacharidosis Ⅰ (MPS Ⅰ).Methods The clinical data of 10 MPS Ⅰ patients were collected.Electrocardiography (ECG) and echocardiography (Echo) were performed in all patients and then analyzed.Results Among the ten patients,seven were men.The onset age of MPS was (0.5 ~ 8.0) years old and the age of diagnosis was (1.8 ~ 20.0) years old.Two patients had grade 2 precordial systolic murmur.ECG was abnormal in three patients with right ventricular hypertrophy in two and right axis deviation in another one.Echo showed valvular thickening and insufficiency in nine patients,enlarged left atrium and ventricle in one patient,hapulmonary hypertension and right ventricular hypertrophy in two patients and abnormal left ventricular configuration in five patients.Conclusions Cardiac involvement is common in MPS Ⅰ patients and may present as valvular thickening with regurgitation,abnormal left ventricular configuration and pulmonary hypertension.The cardiac involvement progresses with age.ECG and Echo should be done regularly during follow-up of MPS Ⅰ patients.
4.EFFECTS OF LOW-DOSE MIFEPRISTONE ON HUMAN GRANULOSA CELLS

Objective To explore the effects of low-dose mifepristone on human granulosa cells and the molecular mechanism of its inhibiting or delaying ovulation.Methods Phase contrast microscopy and electron microscopy were conducted to observe the morphological and biochemical change of granulosa cells exposed to various doses of mifepristone for 24 hours(the final concentrations of mifepristone were respectively 2.5?mol/L,5?mol/L and 10?mol/L,Non-mifepristone as the control group).Immunocytochemistry was performed to detect the protein expressions of Bcl-2 and Bax in granulosa cells exposed to various doses of mifepristone for 24 hours.The mRNA levels of Bcl-2 and Bax in granulosa cells were detected by RT-PCR.Results Granulosa cells treated with various doses of mifepristone presented typical morphological characteristics of apoptosis such as condensation of chromatin and cytosolic,and shrink of cells.The expression of Bcl-2 protein might be down-regulated,while the expression of Bax might be up-regulated by mifepristone.The histological score(HSCORE) of Bcl-2 protein in granulosa cells,which were incubated with various doses of mifepristone(2.5?mol/L,5?mol/L,10?mol/L) for 24 hours,were respectively 2.15?0.16,1.88?0.13 and 1.64?0.16 respectively,Compared with the HSCORE of the control group(2.51?0.16),the difference was significant(P0.05).Conclusion Mifepristone could induce human granulosa cells apoptosis.The Bcl-2 protein and Bax protein may be involved in the apoptosis of granulosa cells induced by mifepristone.

6.Construction and expression of eukaryotic expression vector of PEA-15

Objective To construct the eukaryotic expression vector of pcDNA3.1-PEA-15 and express it in the human esophageal cancer (EC-109) cells,and to explore the effect of PEA-15 on EC-109 cells.Methods The PEA-15 gene was amplified from EC-109 by reverse transcriptase polymerase chain reaction (RT-PCR) and ligated to eukaryotic expression vector pcDNA3.1.After confirmation of recombinant plasmid was correctly by endonuc]eases digestion and DNA sequencing,the construct was transfected it into EC-109 through liposome inducing.The expression of PEA-15 in transfected EC-109 was detected by RT-PCR and Western blot.The cell growth inhibition ratio was evaluated by MTT assay.Results RT-PCR indicated that PEA-15 was highly expressed in EC-109 cells.The amplified fragment by RT-PCR was coincident with hypothesis enzyme digestion analysis and DNA sequencing confirmed that the pcDNA3.1-PEA-15 was constructed successfully.The expression of PEA-15 gene was increased obviously in the transfected EC-109 detected by RT-PCR and Western blot respectively (t =4.078,5.269,P < 0.05).The cell growth inhibition ratio in the group which transfected pcDNA3.1-PEA-15 was significantly lower compared with the pcDNA3.1 transfect group after 72 hours (t =6.163,P < 0.05).Conclusions The recombinant eukaryotic expression vector pcDNA3.1-PEA-15 is constructed successfully,and it can be expressed in EC-109.It also shows that PEA-15 has the function on cell growth which suggests that PEA-15 can inhibit the apoptosis of EC-109 cells and its expression involved in esophageal cancer development.
7.The impact of early surgery on long-term outcome of patients with left-sided infective endocarditis

Objective This study aims to evaluate the impact of early surgery on long-term outcome of patients with left-sided native valve infective endocarditis (IE).Methods Clinical data were retrospectively reviewed in 239 consecutive patients with left-sided native valve IE from 2002 to 2012 in Peking Union Medical College Hospital (PUMCH).Propensity score was used to match patients in the early operation and conventional treatment groups.Results Early surgery was performed in 70 (29.3%)patients and the conventional treatment strategy was applied in 169 (70.7%) patients.The median followup period was 2 years.IE-related mortality was lower in the early operation group than in the conventional treatment group (10.0% vs 23.1%,P =0.02).For 58 propensity score-matched pairs,the cumulative survival free from IE related death was significantly higher in the early operation group than in the conventional treatment group (P =0.027).Regression analysis of the matched cohorts revealed that early surgery was independently associated with decreased IE-related mortality (HR 0.286 ; 95% CI 0.092-0.893 ;P =0.031).While either cardiac function with NYHA Class Ⅲ-Ⅳ (HR 4.044; 95% CI 1.318-12.407;P =0.015) or uncontrolled infection (HR 52.064; 95% CI 10.996-247.194; P < 0.001) was associated with poor prognosis of increased mortality.Conclusions Early surgery improved long-term outcome in patients with left-sided native valve IE compared with conventional therapy.Risk factors related to increased mortality included heart failure and uncontrollable infection.
8.3D video-assisted thoracoscopic cardiac surgery, a report of 50 cases

Objective To evaluate safety and validity of 3D video-assisted thoracoscopic technology in cardiac surgery.Methods Between October 2013 and July 2014,50 patients underwent 3 D Video-assisted thoracoscopic cardiac surgery including:5 atrial septal defect closure,1 ventricular septal defect closure,repairment of 1 unroofed coronary sinus,1 partial atrioventricular septal defect and 1 cor triatriatum,28 mitral valve replacement (1 re-MVR),1 tricuspid valve replacement,9 mitral valvuloplasty,2 left atrial myoxoma resection,and 1 left internal mammary artery harvest + minimally invasive direct coronary artery bypass.All procedures were performed under extracorporeal circulation,using 30° thoracoscope and 3D high resolution monitor.Results There was no perioperative death.One patient was converted to median sternotomy because of pericardial adhesions.Mean operative time was(232.8 ± 54.7) min,mean cardiopulmonary bypass time was(128.0 ±42.5) min,mean aortic cross-clamp time was (74.7 ± 25.1) min.Mean ICU stay was (30.4 ± 22.0) h,mean duration of mechanical ventilation was(13.7 ± 11.9) h.Mean postoperative drainage time was (3.34 ± 1.91) d,mean hospital stay was (6.52 ± 3.06) days.There were 11 (22%) operative complications,including 4 intrathoracic bleeding,1 wound infection,3 hemothorax,1 brachial plexus injury,1 intractable hiccup and 1 mild paravalvular leakage.Compared with 20 mitral valve replacement by 2D thoracoscopy,the 3 D group was slightly less time consuming(P > 0.05).Conclusion 3 D video-assisted thoracoscopic cardiac surgery is feasible and safe,and it' s worthwhile to promote this promising technology.
9.Application value of pulse wave velocity in physical examination in Shenyang district and its risk factors analysis

Objective: To explore application value of brachial-ankle pulse wave velocity (baPWV) in physical examination in Shenyang district and its risk factors.Methods: A total of 1610 cases, who received regular physical examination in health management center of General Hospital of Shenyang Military Command and met enrollment standards, were selected.Bilateral baPWV and other indexes were measured in all subjects.Logistic regression analysis was used to assess relationship among age, body mass index (BMI), arterial blood pressure, blood biochemical indexes and baPWV.Results: Compared with women, there was significant rise in baPWV[(1361.03±244.72) cm/s vs.(1397.93±239.08) cm/s, P=0.003]in men.baPWV rose along with age increased despite male or female.Compared with normal baPWV group (<1400 cm/s), there were significant rise in age[(45.57±8.95) years vs.(57.07±10.50) years], BMI[(25.25±6.82) kg/m2 vs.(26.14±3.44) kg/m2], fasting blood glucose[FBG, (4.88±1.73) mmol/L vs.(5.43±2.02) mmol/L], low density lipoprotein cholesterol[LDL-C, (2.67±0.82) mmol/L vs.(2.84±0.77) mmol/L], total cholesterol[TC, (4.88±0.99) mmol/L vs.(5.13±0.98) mmol/L], triglyceride[TG, (1.73±1.65) mmol/L vs.(2.15±1.91) mmol/L], systolic blood pressure[SBP, (116.72±12.04) mmHg vs.(136.87±16.21) mmHg], diastolic blood pressure[DBP, (76.99±9.01) mmHg vs.(86.34±10.84) mmHg]and pulse pressure[PP, (39.72±8.32) mmHg vs.(50.53±14.17) mmHg]in elevated baPWV group (≥1400cm/s), P<0.01 all.Multivariable Logistic regression analysis indicated that age, FBG,TG, SBP, DBP and PP levels were independent risk factors for elevated baPWV (OR=1.549~5.943, P<0.05 or <0.01).Conclusion: In Shenyang district, baPWV is closely correlated with cardiovascular risk factors, and it possesses good application value in physical examination and early risk screening of cardiovascular diseases.
