1.Morphological observation on bone marrow megakaryocytes in patients with bacterial and fungal infection

Objective To investigate the morphological changes of bone marrow megakaryocytes in patients with bacterial and fungal infection.Methods Totally 76 patients with microorganism infection from the Second Affiliated Hospital,Zhejiang University School of Medicine from January 2008 to August 2009 were enrolled,including 56 bacteria infected patients and 20 fungal infected patients.All patients received bone marrow examinations,and were positive in microorganism culture.Thirty subjects without infection,hematological disease and other severe diseases were randomly selected as controls.The number and function of megakaryocytes were examined retrospectively, and the size, nuclear lobulation, and vacuolar degeneration of megakaryocytes were quantitative analyzed and compared among the groups.Results The size,nuclear lobulation,vacuolar degeneration,and Yat nuclear of megakaryocytes in bacterial infected group were 2.20 ±0.21,2.11 ±0.23,0.51 ±0.11 and 0.74 ±0.11 respectively,those in fungal infected group were 2.21 ±0.16,2.10 ±0.19,0.52 ±0.10 and 0.79 ±0.10 respectively;while those in control group were 1.40 ±0.10,1.36 ±0.12,0.28 ±0.06 and 0.54 ±0.09 respectively.The differences between bacterial infected group and control were of statistical significance(t values were 14.52,12.19,9.33 and 6.61 respectively,P < 0.05),and the differences between fungal infected group and control were of statistical significance(t values were 16.27,12.34,7.85 and 6.49 respectively,P < 0.05).The size,nuclear lobulation,and vacuoles of megakaryocytes in gram-negative(G-)bacteria group were 2.29 ±0.20,2.22 ±0.26 and 0.57 ±0.10,while those in the gram-positive(G+)bacteria group were 2.13 ±0.20,2.04 ±0.18 and 0.46 ±0.09,and the differences were also significant(t values were 2.07,3.03and 3.56 respectively,P < 0.05).The production of platelet by megakaryocytes in bacterial infected group,in fungal infected and the control were 31.4 ±7.6,32.4 ±6.4 and 41.3 ±5.5,and the differences between bacterial infected group and control,fungal infected group and control were significant(t values were 4.78and 3.98 respectively,P < 0.05).The production of platelet in G-bacteria group was 28.0 ± 6.7,while that in G + bacteria group was 34.4 ± 7.2,and the difference was also of statistical significance(t = 2.41,P <0.05). Conclusion Bacterial infected patients have increased megakaryocytes cell body,nuclear lobulation,obvious vacuolar degeneration,Yat nuclear and decreased platelet production function,which are more significant in G- bacteria infected group.
2.Application of time intensity curve of contrast enhanced ultrasound in the discrimination of benign and malignant soft tissue tumors

Objective To study the value of time intensity curve of contrast enhanced ultrasound in the discrimination of benign and malignant soft tissue tumors. Methods Thirty-seven cases of soft tissue tumors were carried out contrast-enhanced ultrasound examination. The region of interest in the dynamic images were chosen to carry out Gamma-Variate curve fitting through Q-LAB software and made a quantitative analysis. Then they were compared with pathological diagnosis to analyze the ultrasound imaging performance of benign and malignant tumors and their characteristics of micro-irrigation. Results Thirty-seven cases of soft tissue tumors were pathologically diagnosed 21 cases of benign and malignant in 16 cases. The perfusion of benign tumors were mainly uniformity (19/21) or potentialization (2/21). The perfusion of malignant tumors were mainly uniformity (10/16) or nonuniformity (6/16). There were no significant differences in time to peak(TTP) and arrival time of contrast agent(AT) between benign and malignant tumors( P >0.05). The upslope rate(A) and the derived peak intensity(DPI) in benign group were significantly less than malignant group ( P <0.05). Conclusions Time-intensity curve analysis of contrast-enhanced ultrasound showed that benign and malignant soft tissue tumors were different in perfusion,and it will be of a certain value for the clinical differential diagnosis.
3.Evaluation of therapy effect of 131Ⅰ treatment of nasopharyngeal carcinoma xenograft mediated by hNIS gene transfection in vivo using diffusion weighted MR

ObjectiveTo investigate the imaging characteristic of DW1,tumor cell apoptosis and proliferation of nasopharyngeal carcinoma (NPC) xenograft transfected hNIS gene after 131Ⅰ treatment.MethodsThe NPC xenograft models were established with CNE-2-hNIS cells(experimental group) and CNE-2 cells(control group) respectively.After i.p.injections 131Ⅰ in the experimental group and control group,the changes of xenograft tumor volume and ADC value were observed in 3,6,12,18,24 days by MR scan.The correlations of changes of ADC with pathological TUNEL,Caspase-3 immunohistochemistry of apoptosis,and Ki-67 proliferation detection were further investigated.Independent samples t-test were compared between the two groups and Pearson linear correlation analysis was used.ResultsThe tumor volume of the experimental group was significantly reduced compared with that of the control group after 131 Ⅰ treatment (t values:8.27-19.46,P <0.05 ).DW-MRI showed that in the 3th day after 131Ⅰ treatment,ADC values increased in the experimental group,and ADC values reached the peak in the 6th and 12th day after 131Ⅰ treatment,then ADC values were decreased. ADC values after treatment was positively correlated with apoptotic index ( r =0.72,P < 0.05 ) and caspase-3 positive rate ( r =0.65,P < 0.05) and there was a negative correlation with Ki-67 proliferation index ( r =- 0.71,P < 0.05 ).ConclusionADC values can reflect growth inhibition of NPC xenograft transfected hNIS gene after 131 I treatment.It is possible that DWMRI techniques can be used in early non-invasively monitor the therapy effect of 131Ⅰ treatment of NPC xenografts transfected hNIS gene.

5.Discussion on transmission mechanism of wild rodent plague to human in the northwestern area of Yunnan province

Objective To explore the relationship between wild rodent plague and human in wild rodent plague foci of the northwestern area in Yunnan to probe the possible transmission mechanism of wild rodent plague to human. Methods Data of component ratio of rodents and fleas was collected in different areas from 1985 - 1995. Activities and habits of residents regarding the way they keep cats and dogs and parasitic fleas and free fleas indoor were investigated, the dog serum was collected for detecting F1 antibody. Results Eothenomys miletus were main rodents in farmland and shrub, accounting for 48.00% (4753/9902) and 54.50% (4282/7857), Apodemus chevrieri were main rodents in garden, being 50.47% (1332/2639). The component ratio of Neopsylla specialis specialis was 13.31%(229/1720), 12.31%(1678/13 739) and 10.87%(957/8802) respectively in garden, farmland and shrub, higher than in indoor. The component ratio of Frantcpsylla spodix was 39.88% (686/1720), the highest in garden. Thirty-two per cent (32/100) of residents kept cats,in which 63% (20/32) with cat fleas, 68% (68/100) of villages kept dogs, in which 76%(52/68) with fleas. Eighteen parasitic fleas were caught from 43 dogs with a flea index of 0.119 and a rate for fleas of 11.63%, 7 pulex were collected from 17 indoor. Forty-three blood serum samples were obtained from dogs, among which 3 were positive blood serum. Conclusions Residents touch affected animals or media in different situations. The possibility of transmission for wild rodent plague to human exists in loci in a chain of wild rodent plague → fleas or predation → homebred animal plague (cats or dogs) →touching or respiratory → human.
6.Hepatitis C virus NS5A protein upregulates survivin gene expression.

OBJECTIVETo investigate whether the nonstructural protein 5A (NS5A) encoded by the human hepatitis C virus RNA genome could affect the expression of the survivin gene.
METHODSHCV NS5A expression plasmid (pCNS5A) was transfected into HepG2 cells using a lipofectin reagent and the HCV NS5A protein was detected using immuno-histochemistry staining. Survivin mRNA was evaluated by reverse transcriptase polymerase chain reaction (RT-PCR) and the expression of survivin protein was detected using Western blot.
RESULTSHCV NS5A protein was detected in the cytoplasm of the HepG2 cells transfected with pCNS5A, and the expressions of survivin mRNA and survivin protein were both up-regulated in the presence of the HCV NS5A protein.
CONCLUSIONSHCV NS5A protein can stimulate survivin protein expression, and this might be the result of the strengthening of the survivin gene transcription.

7.Effect of HCV NS5A on STAT1 phosphorylation and nuclear translocation induced by IFN alpha-2b.

OBJECTIVETo study the effect of HCV NS5A on the Janus kinase (JAK)/signal transducer and activation of transcription (STAT1) phosphorylation and translocation induced by IFN alpha-2b and to understand the possible molecular mechanism of HCV interferon resistance.
METHODSHepatocellular carcinoma cell line Huh7 was transiently transfected with HCV NS5A protein expression plasmid pCNS5A and blank plasmid pRC/CMV respectively; the cells of the same cell line, without transfection, served as controls. Immunocytochemistry was used to prove the successful transfection. Immunofluorescence and Western blot were performed to observe the difference in STAT1 phosphorylation and nuclear translocation between HCV NS5A-expressed and non-HCV NS5A-expressed cells after 30 minutes of IFN alpha-2b induction.
RESULTSHCV NS5A protein was detected in the cytoplasm of Huh7 cells transfected with pCNS5A, indicating the successful transfection. In comparison to the blank plasmid-transfected and non-transfected group, STAT1 phosphorylation and sequential nuclear import were reduced in the presence of HCV NS5A protein.
CONCLUSIONHCV NS5A can, to some extent, inhibit phosphorylation and nuclear translocation of STAT1 in IFN alpha-2b-induced JAK/STAT pathway, which may be a possible mechanism of HCV interferon resistance.

UNLABELLEDTo observe the efficacy of adefovir dipivoxil(ADV) in combination with Anluohuaxian capsule in the treatment of chronic hepatitis B (CHB) patients.
METHODS72 cases with CHB were randomly divided into two groups. 36 cases of treatment group were given ADV combined with Anluohuaxian capsule for 48 weeks. 36 cases of control group were given ADV. The levels of serum ALT, AST, Alb, TBil, HA, LN, CIV, HBV DNA and hepatic tissue were compared before and after being treated.
RESULTSAfter 48 weeks treatment,the liver function, serum fibrosis index and histology of treatment group and control group all have improved. After treatment, the two groups in the levels of ALT(t=0.746, P=0.342), AST (t=0.369, P=0.713), TBil (t=0.146, P=0.684), Alb(t=0.148, P=0.883), liver tissue inflammation mobility scoring (t=1.666, P=0.100) and HBV DNA negative rate (x2=0.141, P=0.708) were no evident difference.The level of HA, LN, CIV were significantly lower in treatment group(101.58+/-30.11, 147.89+/-41.72, 38.75+/-9.50) compared with control group(182.25+/-117.59, 181.50+/-56.96, 74.92+/-31.14) (P less than 0.05). After the treatment, the liver tissue fibrosis scoring was significantly lower in treatment group (10.61+/-2.37) compared with before the treatment (12.28+/-3.16) (P less than 0.05).There was no difference found between after the treatment (11.36+/-2.93) and before the treatment (12.17+/-3.01) in control group (P more than 0.05).
CONCLUSIONSThe results show that the treatment with ADV in combination with Anluohuaxian capsule can play promoting antifibrotic effect and significant improved liver histology of chronic hepatitis B patients.

9.Factors influencing target volume motion in tangential whole breast irradiation for breast cancer

Objective To evaluate the impact of breathing motion on target volume and the factors influencing the set-up errors during tangential whole breast irradiation.Methods From Jan 2003 to Dec 2003,patients with early-stage breast cancer after breast conserving surgery,were selected to be eligible for the study.All patients were immobilized in treatment position by breast beard of Med-Tec 250.The motion of the breast treatment volume was observed on a fluoroscope in different directions under free breathing in 16 patients.The set-up errors in different dimensions during irradiation were measured by weekly portal films (PF) in comparison with digital reconstructed radiographs (DRR) in 11 patients.Results The central lung distance (CLD) variation during free breathing was (2.1?1.2) mm which is greater than the motion to- wards the other directions.By comparing the PF and DRR,the systemic error,random error and overall er- ror in the outer,inner and cranio-caudal directions was 1.9,1.6,2.5 and 2.4,1.7,3.1 and 2.6,2.3, 3.5 mm,respectively.In addition,the discrepancy of the treatment position in cranio-caudal direction and breast volume was most obvious at the beginning 2 weeks with the peak of breast volume at the second week. It decreased gradually during the following 3 weeks.Conclusions This study suggests that the mean value of the motion of the breast target volume during one breathing cycle is less than 2 mm.The set-up errors dur- ing irradiation is the greatest in cranio-caudal direction,suggesting that the fixing precision of the breast board should be further improved.The set-up error during irradiation are most obvious at the beginning two weeks,with the peak of the breast volume in the second week.
