1.Meta-analysis of javanica oil emulsion injection combined with radiotherapy in the treatment of esophageal carcinoma

Objective To evaluate the advantage and clinical value of javanica oil emulsion injection combined with radiotherapy for esophageal carcinoma. Method Randomized controlled trials (RCTs) concerning javanica oil emulsion injection combined with radiotherapy for esophageal carcinoma were made on electronic databases of CBM, CNKI, Wangfang, VIP and PubMed from 1990 to February 28, 2015. Quality of the included RCTs was assessed by Jadad scoring , and Meta-analyses were performed by RevMan5.3 software. Results Seven RCTs involving 575 patients were included in Meta-analysis. All studies were in low quality. The results of Meta-analyses showed that Javanica oil emulsion injection combined with radiotherapy for esophageal carcinoma, compared with control groups, could increase the recent curative effect (P < 0.000 01) and improve the quality of live (P = 0.000 8), decrease the hematologic toxicities (P = 0.03) and the incidence rate of radioaction esophagitis (P = 0.02), but not enough evidence was found to prove that it can enhance survival rate (P = 0.25). Conclusions The results of Meta-analysis indicate that javanica oil emulsion injection may have therapeutic effects on the treatment of esophageal carcinoma. Yet the effects of javanica oil emulsion injection still need to be confirmed by large multi-center randomized controlled trials.

Objective To study the imaging characteristics of osteosarcoma.Methods 35 patients with osteosarcoma proved pathologically were examined by radiography,of them 25 cases were examined with CT scan or MRI,and 8 cases undergone contrast-enhanced MRI.The imaging features of osteosarcoma were analysed comparatively with that of pathology.Results The lesions localized at distal part of femur in 17 cases and proximal part of tibia in 8 cases,that were most location of this tumor.The imaging characteristics of osteosarcoma included bone destruction,neoplastic bone,periosteal reaction and soft-tissue mass,in some cases,epiphyseal plate and epiphysis were involved,even the joint face and the opposite osseous of joint(skip lesions).On enhanced MR scanning,the margin of tumor was enhanced in early and the center of tumor was delayed enhancement.CT could showed "sieve sign"and small neoplastic bone.Conclusion Osteosarcoma has certain imaging characteristics;X-ray examination is the first choice in diagnosing osteosarcoma,CT and MR could provide much more comprehensive imaging information for clinic.
4.The clinical evaluation of the bispectralindex of electroencephalogram in predicting movement during skin incision in patients under sevoflurane anesthesia

Objective: To evaluate the efficacy of the bispectral index (BI) in predicting patient movement on skin incision under sevoflurane anesthesia with the PK statistic. Method: Twenty-nine adult patients. scheduled for elective upper abdominal surgery, were selected. Anesthesia was induced with propofol and 4%-5% sevoflurane in oxygen and maintained with one of four randomized sevoflurane concentrations (0.6MAC, 1.0MAC. 1.5MAC and 2.0MAC) for fifteen minutes, then skin incision was performed by the surgeon at the planned site of the surgery, and each patient was observed carefully about 2 minutes to detect purposeful movement. The data of BI, 95% SEF, MAP, SABP and HR in one minute before skin incision were applied to statistically analysis. Result: The P_K was significantly greater than 0.5 for the sevoflurane level and the BI. The PK of 95% SEF. SABP and MAP were less than that of the level of sevoflu or Bl. but was significantly greater than 0.5 The P_K of HR was close to 0.5. Conclusion: These indicators of B, BP and 95% SEF may predict patient movement to skin incision under sevoflurane anesthesia. of which the BI is the most sensi tive.
5.Effects of recombinant adenovirus hypoxia-inducing factor-1? (HIF-1?) on the angiogenesis in a rat model of hindlimb ischemia

Objective To explore the expression of recombinant adenovirus hypoxia-inducing factor-1? (HIF-1?) and its effects on the angiogenesis in a rat model of acute hindlimb ischemia. Methods (1) Recombinant adenovirus HIF-1?-Al?564 (Ad-H564 for short) was amplified in large scale in HEK293A cells and concentration gradient centrifuged in CsCl, purified by dialysis and then its titer was determined by spectrophotometry. (2) A model of acute ischemia in rats' hind limb was reproduced, and 72 rats were randomly allocated into Ad-LacZ group, Ad-H0 group, Ad-H564 group and control group (18 rats for each group). After the HIF-1? mRNA expression was detected, six rats of control group were intramuscularly injected with normal saline (NS group) to serve as control. Angiography and vascular cast were carried out in another two groups with in each six rats to serve as sham operated controls. In to hind limb skeletal muscles of rats of Ad-LacZ, Ad-H0 and Ad-H564 groups Ad-LacZ, Ad-H0 and Ad-H564 were respectively injected, normal saline was given to the rats of NS group, and nothing was given to the rats in sham group. (3) HIF-1? mRNA expression was determined by RT-PCR on the 1st, 3rd, 5th and 7th day after injection. (4) Selective arteriography and vascular cast were performed to evaluate the vascular density. Results (1) PCR and gene sequencing suggested that correct recombinant adenovirus HIF-1? was obtained after being amplified and purified, and the titer of adenovirus was 1011OPU/ml. (2) RT-PCR showed that the highest expression of HIF-1? mRNA (0.544 7?0.141 2) was seen in Ad-H564 group, and the value was significantly different compared with that of normal group and Ad-LacZ group (P=0.000), but it did not show significant difference compared with that of Ad-H564 group and Ad-H0 group (0.533 0?0.041 6, P=0.368). The expression of HIF-1? mRNA reached the highest value on the 7th day in all the groups. (3) Arteriography on the 28th day after injection showed that the density of the collateral vessels was higher in Ad-H564 group than that in Ad-H0 group, but no significant difference was found. Vascular casts showed denser microvasculature in Ad-H564 group than that in the other groups. Conclusions The present study suggests that single site mutation may enhance the expression of human HIF-1? gene in the local muscles, facilitate angiogenesis in the ischemic tissue, but no significant difference was found between the wild-type and the mutated HIF-1? genes.

Objective To establish the cancer vaccine (EC109-DC) prepared by fusions of esophageal carcinoma cell line with dendritic cell derived from cord blood and study the biological characteristics and antitumor immunity.Methods CD 34 + hematopoietic stem cells were isolated from cord blood using CD 34 + Progenitor Cell Isolation Kit by magnetic cell sorting system (MACS);expended the cells as DC,and then fused with EC109 by PEG-3600;selected the fusion cells by MACS;observed the phenotypes and proliferation by flow cytometry and technique of cell culture in vitro. Lymphocytes proliferation reaction and the CTL cytotoxicity were examined with MTT.Results EC109-DC could proliferate slowly in vitro and highly expressed CD80,CD83 and CD86.Lymphocytes proliferation reaction and the specific cytotoxicity against EC109 induced by EC109-DC cells were significantly higher than the control groups (P

Objective To evaluate the health status ,dyspnea and exercise endurance in the patients with chronic obstructive pul-monary disease(COPD) by the CAT scale ,modified medical research council(mMRC) dyspnea scale and 6-min walk test(6MWT) and to analyze the correlation between their evaluation results with the percentage (FEV1% pred) of the forced expiratory volume at 1 second(FEV1) in the predict value and whether complicating pulmonary hypertension (PH) .Methods 70 patients with COPD were performed the examinations of the lung function ,CAT scale ,mMRC score and 6MWT .The correlation among the various e-valuation systems and between each evaluation system with the severity of the pulmonary function airway limitation were compre -hensively evaluated .The CAT scores ,mMRC scores and 6 min walking distance(6MWD) were compared between the COPD com-plicating PH group and the simple COPD group .Results The CAT scale was negatively correlated with 6MWD(r= -0 .623 ,P<0 .01) and had no correlation with FEV1% pred(P=0 .159) .The CAT scale was positively correlated with mMRC (r=0 .492 ,P=0 .000) .mMRC was negatively correlated with 6MWD(r= -0 .514 ,P= 0 .000) and had no correlation with FEV1% pred(P>0 .05) .The CAT scores had statistical difference between the COPD complicating PH group and the simple COPD group (P<0 .01) .6MWD had the highest accuracy for screening COPD whether complicating PH ,followed by CAT .The diagnostic cut-off point by adopting CAT for judging the risk of COPD complicating PH was 21 .Conclusion The CAT scale has good correlation with 6MWD and no correlation with FEV1% pred .The CAT scale is more suitable for the overall assessment of the total severity of COPD .Patients with the high CAT scores have higher risk of suffering from PH .

10.Enhanced Myocardial Protection with Ginsenosides in Cardioplegic Solution

Myocardial protection has been the subject of intensive investigation. Several studies have suggested that Ginsenosides may protect against ischemia/reperfusion injury. The isolated, working rat heart model was used in this study to evaluate the effects of St. Thomas' Hospital Cardioplegic Solution containing various concentrations of Ginsenosides on the enhancement of myocardial protection following hypothermic ischemic arrest (20 ℃, 120 min). The added concentration of Ginsenosides in cardioplegic solution was 0(control), 50, 100 150?g/ml, respectively. Recoveries of aortic flow, cardiac output, and minute work were markedly improved by adding Ginsenosides to cardioplegia. Dose-response study indicated that 100?g/ml in cardioplegia had the best protective effects, in which group the generation of thiobarbituric acid reactive materials (MDA) was markedly reduced. Our results indicated that Ginsenosides provides additional myocardial protection in St. Thomas' Hospital Cardioplegia. Addition of Ginsenosides to cardioplegia may protect the ischsmic myocardium from free-radical injury also.
