1.Evaluation of Pemberton osteotomy for the treatment of developmental dysplasia of the hip in older children

[Objective]To evaluate the outcome of the Pemberton osteotomy for the treatment of developmental dysplasia of the hip (DDH) in older children. [Method]There were 21 children (24 hips) older than 7 years in our study , who received the Pemberton osteotomy, and combined with femoral shortening, derotational or varus osteotomies according to the level of dislocation, anteversion and neck-shalf angle of the femur. The average age at operation were 9 years and 7 months (range: 7~13.5 years).[Result]The mean follow-up were 5 years and 5 months. Eighteen hips achieved the excellent or good clinical results according to the McKay's clinical criteria. All of the hips got improvements of the Acetabular Index, Sharp Acetabular Angle, Center-Edge Angle and Acetabular Head Index. The average results were in the normal range. Eighteen hips got ⅠorⅡclass of the Severin’s radiographic classification.However, 9 hips were limited in the range of movement. Four hips got subluxation.Six hips developed avascular necrosis of the femoral head. Three hips got improved osteoarthritis.[Conclusion]The Pemberton osteotomy for the treatment of developmental dysplasia of the hip in older children can mostly achieve well-used, contained or concentric reduction hips, meanwhile need to reduce the complications.
2.Determination and Stability of Nimesulide in Thermo-sensitive Hydrogels

Objective:To establish the method for determining nimesulide in thermo-sensitive hydrogels and study the stability of the preparation. Methods:An HPLC method was adopted with a Shim-Pack C18-ODS (150 mm × 4. 6 mm,5 μm) column, the mobile phase was composed of menthol-water-acetic acid (65∶35∶0. 8) with the flow rate of 1. 0 ml·min-1 at 30℃, the detection wavelength was at 299 nm, and the injection volume was 10 μl. Results:The separation of nimesulide, impurities and degradation products was good. The linear range of nimesulide was 2. 43-24. 37μg·ml-1(r=0. 999 8), the average recovery was 100. 02%(RSD=1. 12%, n=5). The lowest detectable limit and the lowest quantitation limit was 0. 098μg·ml-1 and 0. 25μg·ml-1, respectively. The stabil-ity results of hard-light exposure, destructive testing and long-term testing showed that the preparation was basically stable at room tem-perature, while under high temperature with hard light, strong basicity, acidicity or oxidation solution, the preparation was unstable. Conclusion:The method is accurate and reliable in the determination and stability study of nimesulide thermo-sensitive hydrogels. The hydrogels should be stored in shady, cool and dark place.
3.Effect of Ebosin on the inflammatory cytokines in THP-1 cells.

Ebosin is a novel exopolysaccharide produced by Streptomyces sp.139 with remarkable activity against rheumatic arthritis in vivo. In this paper, we reported effects of Ebosin on the inflammatory cytokines including interleukin-1beta (IL-1beta), interleukin-6 (IL-6) and tumor necrosis factor alpha (TNFalpha) in THP-1 cells. With the special fluorogenic peptide as substrates, the enzymatic activities of interleukin-1beta converting enzyme (ICE) and TNFalpha-converting enzyme (TACE) were inhibited by Ebosin separately. Using the real-time reverse transcription polymerase chain reaction (real-time PCR), the mRNA synthesis of the three cytokines were identified decline separately by Ebosin. The secretion quantum of three cytokines in THP-1 cells with Ebosin was lower than that of normal THP-1 cells determined by ELISA assay and Western blotting. All of these results showed that Ebosin has remarkably suppressed synthesis of the three cytokines in THP-1 cells through different pathways. The primary study of Ebosin on anti-inflammation mechanism was promoted developing the new drugs treating rheumatic arthritis.
4.Is closed reduction and interlocking intramedullary nailing fit for the treatment of fractures of humeral shaft with radial nerve injury?

Objective To study whether closed reduction and interlocking intramedullary nailing will worsen the injured radial nerve associated with the fracture of humeral shaft.Methods Of the 353 patients with fracture of humeral shaft who received operation from January 2002 to January 2005,63 ones were complicated with preoperative injury to their radial nerve.A retrospective analysis was done of their physical examination records, operative records,X-ray films and results of the treatment.Eleven cases were treated with closed reduction and interlocking intramedullary nailing,and 52 cases with open reduction and internal fixation of plates and screws fol- lowed by exploration to the radial nerve.Chisquare test of PEMS 3.1 system was adopted to analyze the clinical data. Results The radial nerve was embedded by the fracture ends in nine cases(17.3%)of the 52 cases,and con- tused in the other 43 ones.In the 63 cases,The injured nerves recovered spontaneously 2 to 12 weeks postoperatively except in twn cases.All the eases got bony union 3 to 4 months after operation.Closed reduction and interlocking intramedullary nailing has hardly more harmful effects on the injured radial nerve assoeiated with the fracture of humeral shaft than open reduction and internal fixation of plates and screws followed by neural exploration.Con- clusion Closed reduction and interlocking intramedullary nailing is fit for the freatment of fractures of humeral shaft with radial nerve injury.
5.Quality standard for Guishao Soft Capsules

AIM: To establish the quality standard for Guishao Soft Capsules(Radix Paeoniae Alba,Radix Angelicae Sinensis,Rhizoma Chuanxiong, etc.) METHODS: Radix Paeoniae Alba,Rhizoma Atractylodis,Rhizoma Chuanxiong,Rhizoma Alismatis and Radix Angelicae Sinensis were identified by TLC.The contents of albiflorin and paeoniflorin in Guishao Soft Capsules were determined by HPLC.HPLC analytical method was carried out(using) a C_(18) column and a mixture containing 17 volume of acetonitrile and 83 volume of 0.05% acetic acid as the mobile phase.The detection wavelength was set at 230 nm. RESULTS: Spots obtained from the test solutions had the same color in reference solution and medical material in the same location,and the blank solution had no interfernece.The linear range of albiflorin was from 0.53 ?g to 1.06 ?g(r=0.999 8),the average recovery was(100.3%.) The linear range of paeoniflorin was from 0.29 ?g to 2.00 ?g(r=0.999 9),the average recovery was(99.49%.) CONCLUSION: In TLC,the spots are very clear and specific to identify the herbal medicine in Guishao Soft Capsules.It is simple,quick,high precise and accurate for HPLC to control the quality of Guishao Soft Capsules.
6.Preexcitation syndrome with tachycardia-bradycardia syndrome:Clinical analysis

Objective To investigate the association between syncope and transient suppression of sinoatrial node in patients with preexcitation syndrome and paroxysmal supraventricular tachycardia (PSVT).Methods Twelve patients (male 8,female 4;age range:19-49 years) were enrolled due to WPW(Wolff-Parkinson-White) syndrome and syncope when the tachycardia terminated.The function of sinoatrial node was evaluated with clinical electrophysiologic study,and then radiofrequency catheter ablation (RFCA) or antitachycardia pacemaker were optimized for the patients with a follow-up of 2 to 5 years.Results Electrocardiographic features of the 12 patients were: ventricular preexcitation (type A in 7 and type B in 5) and long pause when tachycardias ended up.Electrophysiologic study revealed the function of sinoatrial node was normal.Four patients received antitachycardia pacing therapy,5 underwent RFCA,and 3 received both.The 2 to 5 years follow-up found no relapse of PSVT associated syncope.Conclusion Transient suppression of sinoatrial node might explain the mechanism of syncope with preexcitation and PSVT.However,the reason why such a small group of patients differ from the majority of preexcited cases in clinical course remains to be unsettled.The first choice of prevention and treatment for these patients should be eliminating PSVT.Conventional pacemaker implantation for preventing syncope seems to be unnecessary.
7.Preoperative Utilization of Antibacterials in 300 Orthopaedic Patients

OBJECTIVE: To evaluate the preoperative utilization of antibacterial in orthopaedic patients in our hospital. METHODS: The preoperative use of antibacterial in 300 orthopaedic patients in our hospital in 2008 was reviewed. RESULTS: Of the 300 patients in our study, 100% of them were recieved preoperative antibacterial. Two of the most widely used antibacterials were clindamycin and cefuroxime sodium. The use of antibacterial in 228 patients were overtime after operation (76.00%), of which the use of antibacterial in 161 patients in type Ⅰ incision exceed 24 hour(70.61%), 56 patients in type Ⅱ incision exceed 2 days(24.56%) and 11 patients in type Ⅲ incision exceed 7 days (4.82%). CONCLUSION: The usage of preoperative antibacterial was inappropriate and the duration of administration is too long, which should be standardized.

9.Application of argon-fluoride excimer laser to corneal refractive surgery
