1.A method to optimize the selection of tube current for consistent image noise and dose control in 64-slice spiral helical CT cardiac imaging

Objective To establish and clinically evaluate a method to adjust tube current(mA)individually for obtaining images with consistent noise and optimized radiation dose in 64-slice spiral helical CT coronary angiography.Methods (1)80 patients,as a control group,were underwent CT coronary angiography(CTCA)using 64-muhislice spiral helical CT with fixed mA(650 mA).Two radiologists first assessed image quality of the fixed mA group by scoring from 1 to 5.A minimal dose requirement for meeting clinical diagnosis with the desired noise level(standard deviation SD0)were decided.By analyzing the images of the control group,we also established correlation between the image noises of timing bolus(TB)scans and cardiac(CA)scans and set up a formula to predict the required mA value for obtaining the desired cardiac image noise for individual patient.(2)Subsequently,another group of 80 patients(study group)wero scanned with individually adjusted mA using the established formula and the practicability of this method was then evaluated.The independent sample t test was used to compare the values of the two groups.Results (1)The noise of TB and CTCA scans was linearly correlated with r2=0.994. (2)A cardiac image noise level of 28 HU was found to be adequate for clinical diagnostic purpose based on the image quality analysis.(3)The average noise for the acquired cardiac images was found to be 28.97 HU for the study group with individually adapted mA using the established formula.The standard deviation of the image noisein study group was 1.93,which was much smaller than 5.14 in the fixed mA group,but the average image scores for the study group(4.27±0.68)and fixed group mA(4.03 4-0.93)were not statistically different.(4)In the study group,63 out of 80 patients required tube currents below the standard 650 mA,yielding an average CTDIvol of58.32 mGy,which was about 32.1%dose reduction than that of fixed mA(85.94 mGy).The lowest CTDIvol was 19.27 mGy with 160 mA which was 22.4%of the average CTDIvol in control group.Conclusion Adjusting mA individually based on timing bolus scan noise analysis is an accurate and practical method to obtain consistent image quality for different cardiac pailents and to minimize the radiation dose for individuals compared to the fixed mA.
2.Tracing transmembrane transport of amino-glucomannan

Objective To label the amino-glucomannan(AGM) with fluram and to explore the properties of the compound of fluram-AGM in transmembrane transport.Methods After AGM was labeled with fluram,the peripheral blood mononuclear cells(PBMC) and HepG2 were respectively cultured with Fluram-AGM,then observed under fluorescence microscope with violet light as exciting light.Results Both PBMC and HepG2 showed intracellular indigotic fluorescence.Conclusion AGM can be transported into cells across cell membrane.
3.Condensation mechanism of aminoglucomannan with plasmid DNA

Objective To study the condensation mechanism of aminoglucomannan(AGM)with plasmid DNA and some influencing factors in this process.Methods The polysaccharide-DNA complex was analyzed with agarose gel electrophoresis by observing the changes of DNA bands under various influencing conditions including different pH,ionic strength(NaCl concentration),temperature and plasmid DNA sorts.Results The new bands appeared in the slow migration positions after reaction between AGM and plasmid DNA,and the changes of DNA bands content were dependent on AGM concentrations.The condensation degree could be effected by pH,temperature,and ionic strength.Conclusion Our study suggests that the AGM can be condensed with plasmid DNA in proper conditions.
4.Experimental research of interstitially implanted ~(125)I seed on the transplanted bladder cancer in T_(739) mouse

Objective: In T_(739) inbred line mice,bladder cancer model was established via tumor source transplant.In order to get insights of the biological effects of implanting ~(125)I seed to treat bladder cancer,the development and the pathological modality of tumor tissues were observed through the experiment of implanting ~(125) I seed.The experiment was to provide theoretical gist and practical guidance for clinical(utilization) of ~(125)I seed for brachytherapy of bladder cancer. Methods: 16 T_(739) inbred line mice were divided into two groups(A:experimental group,n=8;B:control group,n=8) in random;bladder cancer model was established via tumor source transplant.When the diameter of the transplanting tumor was about 0.7 cm,~(125)I seeds were implanted at the tumor of experimental group while the empty seed was penetrated into the control group at the same position as the experimental group and the experimental mice were executed after 24 days.The longest vertical diameters(a) and(b) of tumor body were observed every 3 days and the volume of tumor body(V=ab~(2)/2) was calculated.Combining with the weight of mouse(W) and the weight of tumor body after execution of mice(w),the growth curve of tumor was protracted and the colony redouble time and tumor suppression rate were calculated.After fixation of formaldehyde,the pathological modality of tumor tissues were observed with HE staining. Objective To explore the clinical symptoms and neuroimaging features of a patient with atypical hypomyelinating leukodystrophy with atrophy of the basal ganglia and cerebellum (H-ABC) caused by a novel TUBB4A mutation.Methods We analyzed the clinical data, imaging features and the result of genetic testing of a case diagnosed as atypical H-ABC.Results The initial symptoms were progressive spasticity, mild cerebellar ataxia and mild cognitive impairment.MRI showed regional blurring of slight high signal on T2-weight and FLAIR image in white matter of the bilateral midbrain ventral, internal capsule, posteior horn of lateral ventricle and centrum semiovale, with normal bilateral cerebellar and caudoputamen nucleus.Compared with normal subjects of the same age and gender, hypometabolism was found by 18F-FDG-PET in brainstem, cerebellar and caudoputamen nucleus in the patient.Genetic testing revealed a de novo pathogenic exome missense heterozygous mutations c.70G>A in TUBB4A, which was not reported in the human gene mutation database (HGMDpro) and was assessed to be a pathogenic mutation by pathogenic mutation prediction software.Conclusions The diversity of TUBB4A gene mutations may cause different functional and/or structural impairment in subcortical white matter, cerebellar and caudoputamen nucleus, leading to atypical symptoms and neuroimaging features.Genetic testing for pathogenic mutation in TUBB4A gene is a key for the diagnosis of H-ABC.
6.Characteristics of clinical presentation and neuropathology in idiopathic hypereosinophilic syndrome encephalopathy

Objective To discuss the mechanism and characteristics of clinic presentation and neuropathology in idiopathic hypereosinophilic syndrome (IHES) encephalopathy. Methods IHES encephalopathy was diagnosed by clinical presentations,lab examinations,neurologic images,marrow and brain biopsy,then treated with corticosteroids and continuous follow-up. Results The IHES patient presented progressive limbs weakness and cognitive deficit with elevated eosinophil count. Results of lab examinations and bone marrow biopsy ruled out secondary eosinophilia and clonal eosinophilia such as eosinophilic leukemia.Brain magnetic resonance imaging (MRI) study showed multiple lesions in right frontal lobe, bilateral parietal and occipital lobe, presenting hypointensity in T1 weighted images,hyperintensity in T2 and fluid-attenuated inversion recovery weighed images. Brain biopsy showed proliferation of vascular membrane,small vessels stenosis,ischemia-induced morphological change and necrosis of neurons in the lesions.The patient was continuously treated with corticosteroids,and the situation was stabilized with follow-up.Conclusions IHES encephalopathy should be paid more attention in clinical practice of neurologists for its rarity. Brain vascular damages caused by elevated eosinophil may be an important pathophysiological mechanism of IHES encephalopathy,and corticosteroids or hydroxycarbamide should be used for the treatment.
7.Immunogenicity study on recombinant fHBP of meningococcal group B

Objective:To evaluate the immunogenicity of recombinant factor H binding protein(fHBP) by detecting serum antibody titer and serum bactericidal antibody test (SBA).Methods:fHBP sequence was selected and synthesized, connected to plasmid pET43.1a, transformed to Escherichia coli BL21(DE3), and expressed two recombinant fHBP proteins, included two subfamilies, fHBPA and fHBPB. After purification, the recombinant fHBP proteins were immunized to rabbits and mice. The immune antiserum titer and the bactericidal titer to epidemic strains of meningococcal bacteria group B were measured by ELISA and SBA respectively. Results:The antiserum titer of fHBP immunized rabbits was greater than 2.0×10 6, and that of immunized mice was not less than 1.0×10 6. fHBP immunized rabbit serum had bactericidal titer more than 1∶128 to 41 strains A subfamily and 20 strains B subfamily in the SBA against 69 endemic strains, and there was no cross-protection between the subfamily bacteria. The bactericidal titers of mouse serum immunized fHBPA to strains A subfamily such as Nm210902 Nm211009、Nm450522 were 1∶1 024, 1∶608、1∶861, to Nm510703、Nm311304、Nm431002 were 1∶234、1∶861、1∶430 respectively, and mouse serum immunized fHBP B to strains B subfamily Nm311302、Nm311304、Nm431002 were 1∶876、1∶274、1∶1858, all of three strains were positive in bactericidal titers. Conclusions:the titer of fHBP antiserum was higher than 1.0×10 6, the bactericidal titer was no less than 1∶128 to 61 epidemic strains, and it has a 94.2% protective effect on 69 meningococcal epidemic strains group B.

9.Studies on release behavior of sustained release tablets of extracts of Gardenia by antioxidant activity.

Using sustained release tablets of gardenia extract as model drug and DPPH radical scavenging capacity as antioxidant index, the feasibility of using pharmacodynamics index was explored to evaluate sustained release tablets. Applying the established quantifiable method of DPPH radical scavenging to the dissolved liquid of model drug, release profiles and biological effects profiles were drawn, and their correlation was discussed. A good correlation was observed by linear regression and f2 actor, suggesting that the indicator could be used to evaluate sustained release tabletsofextracts of gardenia in which iridoids were mainly involved.

10.Extremity-preserving efficacy of vascular prosthesis grafting in repairing arterial injuries of extremities

BACKGROUND: Vascular injuries of the extremitiesy are frequently concomitant with vascular defects and are usually repaired by autologous vessel grafting. However, the source of autologous vessels is limited and the preparation of them is traumatic.OBJECTIVE: To retrospectively evaluate the extremity-preserving efficacy of vascular prosthesis grafting in repairing major vascular injuries of the extremities in 29 cases.DESIGN: A retrospective study.SETTING: Department of Orthopedics and Traumatic Surgery, Center for Battle Wound and Trauma of Chinese PLA, Research Institute of Surgery,Daping Hospital, Third Military Medical University of Chinese PLA.PARTICIPANTS: We selected 29 patients with major vascular injuries of the extremities repaired by vascular prosthesis grafting who received the treatment at the Department of Orthopedics and Traumatic Surgery of the Center for Battle Wound and Trauma of the Chinese PLA of the Research Institute of Surgery of, Daping Hospital of the Third Military Medical University of Chinese PLA between January 1989 and December 2000. There were 23 males and 6 females. Injury sites: 9 cases were at subclavian artery, 6 at axillary artery, 2 at brachial artery, 10 at femoral artery, 1 at femoral vein and 1 at popliteal artery. 11 of these 29 cases (37.9%) were complicated with shock, 8 with fractures and dislocations, 5 with peripheral nerve injuries and 3 with infections.METHODS: The vascular prosthesis was anastomosed end to end with the trimmed culprit vessels. As for the 3 cases with infective arterial injuries,the vessels were placed away from the infected region and were bridged together in a non-inf1ammatory region and then muscles or musculo-cutaneous flaps were used to cover the infected regions.The functions of the extremities of the patients were evaluated according to MAS at week 2 and 1 year after the operation.MAIN OUTCOME MEASURES: The vascular patency rate and the extremity-preserving outcomes..RESULTS: All 29 patients entered the stage of result analysis. Extremitypreserving efficacy: All the patients who underwent vascular prosthesis grafting had their extremities preserved. Only 1 patient had the late sequelae of ulcers in the foot sole due to extended extremity ischemia and nerve damage. The rate of 2-week patency rate of the grafted vessels was 100% and the 1-year rate of patency was 96.5%, as revealed by Doppler blood stream scanning. The excellent and good rate of the function of extremity was 89%.CONCLUSION: Vascular prosthesis grafting is the one of the methods for repairing the major vascular injuries so as to preserve the extremities and their functions.
