1.Factor analysis and treatment strategies of post-operative soft-tissue defect in leg trauma

Objective To analyze risk factors for soft-tissue defects after surgery on leg trauma,to discuss methods for prevention of soft-tissue defects and its treatment.Methods A retrospective analysis was conducted on 217 cases of soft-tissue defects with bone exposure and / or internal fixation exposure after surgery on leg trauma from January,1999 to December,2012.Soft-tissue defects with various flaps were used in 201 cases,including neurocutaneous flap(89 cases),random flap(21 cases),free flap(75 cases),cross-leg flap(16 cases),and by skin grafting in 16 cases.For the 117 internal plate-fixated fractures,96 removed the plate and 21 cases did not remove the plate.For the 13 intramedullary nail fixations,4 cases of intramedullary nails were removed,9 cases were not removed.For the 56 cases of external fixation of the fractures,39 patients had unadjusted external fixation,17 cases re-adjusted external fixations.Results Of the 217 cases of soft-tissue defect after surgery on legs,201 cases were conducted flap surgery.The flap was completely necrosis in one case,and partial distal necrosis were in 14 cases,of which 6 cases healed after changing the dressing,5 underwent debridement and skin grafting,3 cases underwent flap surgery again.In cases of fracture patients,177 cases healed within 1 year,delayed union 23 cases,nonunion eight cases.Nonunion healing after bone grafting and re-fixation surgery.In 9 cases of uncontrollable osteomyelitis,6 patients underwent bone removal and bone transport surgery,3 patients underwent amputation.Conclusion Soft-tissue defects after surgery on leg trauma is a serious complication whose prevalence is associated,in a certain degree,with incorrect surgical time and methods.It is crucial for early repair of soft-tissue defects by using various kinds of proper flaps whenever soft-tissue complication occurs.In patients with early stage internal fixation,flap surgery without internal fixation removing is feasible if little soft-tissue defect,little exposed internal fixation and mild infection,otherwise it is necessary to remove internal fixation and re-fix by external fixation before flap surgery.
2.Diagnostic value of echocardiography for cardiac tumors of 87 cases

Objective:To explore the diagnostic value of color Doppler echocardiography for cardiac tumors .Meth-ods:Clinical data of 87 patients with cardiac tumors were retrospectively analyzed ,including their diseased loca-tion ,clinical manif′estations and echocardiography feature .Results:All cardiac tumors obtain pathologic confirma-tion .There were 73 patients (83.9% ) with benign cardiac tumors ,including 66 patients (90.4% ) with myxoma and seven patients with other types ;there were 14 patients (16.1% ) with malignant cardiac tumors .Echocardiography indicated that myxoma was often located in left atrium (n=58) ,most cases possessed pedicell,its activity was large ;Other type benign cardiac tumors feature :The 6 case (86% ) were generated inside cardiac muscle without pedicell, its echo was more strong ;malignant tumors:its attachment surface was wide without pedicell,and it may show with globular or cauliflower ,and accompanied hydropericardium mostly .Conclusion:Echocardiography can dynamically observe the form ,size ,echo and hemodynamic changes of tumors ,its diagnostic accuracy is high .Myxoma is most frequent cardiac tumor .
3.Experimental study of the change of early regional myocardial function with diabetic using omni-directional Mmode echocardiography

ObjectiveTo evaluate the role of detecting early change of left ventricular regional myocardial function in the rabbit with type Ⅰ diabetes using omni-directional M-mode echocardiography (OME).MethodsEighty New Zealand rabbits were selected in this study..40 health rabbits were included in the control group,the other 40 with type Ⅰ diabetes were included in the experiment group.The OME was used to measure the peak time of myocardial motion during systole (Ts) and diastole (Td),myocardial velocity gradient during systole (MVGs) and diastole (MVGd) on the left ventricular short axis.These parameters were measured during the first,second,fourth,sixth and eighth week.ResultsOME was able to detect significant changes of Td during the second week,Ts and MVGd during the fourth week,and MVGs during sixth week after modeling ( P <0.05 ).ConclusionsOME is an important method to detect early changes of regional myocardial function in the patients with diabetes.
4. Protective mechanism of loganin on endoplasmic reticulum stress of mesangial cells induced by advanced glycation end products

Objective: To investigate the protective effect and the mechanism of loganin (an active component in Cornus officinalis) on the endoplasmic reticulum (ER) stress of glomerular mesangial cells (GMCs) induced by advanced glycation end products (AGEs). Methods: Human GMCs were cultured in vitro and divided into control group, model group (AGEs group), loganin group, and amino guanidine group (set as positive control, 0.1, 1.0, and 10.0 μmol/L). After being incubated with loganin (final concentration of 0.1, 1.0, and 10.0 μmol/L) for 1 h, GMCs were stimulated by AGEs (200 mg/L) for 24 h. Then, the cell proliferation was measured of using MTS method. PGE2 was investigated by Elisa. Receptors of advanced glycation end products (RAGE), and ER stress-related protein like GRP78, IRE1, XBP1, and inflammatory factor NF-κB and COX-2 in GMCs were detected by Western blotting. Results: Loganin could suppress the proliferation of GMCs induced by AGEs, improve the subcellular injury of GMCs, down-regulate the expression of ER stress-related protein GRP78, IRE1, XBP1, and RAGE, reduce the inflammation-related protein NF-κB, COX-2, and the level of PGE2. Conclusion: Loganin could improve the ER stress of GMCs induced by AGEs, lessen the inflammation and subcellular injury of GMCs, its mechanism might be related to the decreased expression of RAGE and the inhibition of the IRE1 pathway.
5.Three-dimensional spherical culture of adipose-derived stem cells

BACKGROUND:Many types of mammalian cells aggregate and display three-dimensional multicellular spheroids when they are in normal physiological conditions. In order to observe and explore cellular natural states, many researchers try to use spherical cellculture in vitro, a common three-dimensional culture pattern.
OBJECTIVE:To use three different methods for spherical culture in vitro of adipose-derived stem cells and to observe their biological features.
METHODS:Adipose-derived stem cells were confirmed by the analysis of the markers for cellphenotypes as wel as adipogenic and osteogenic differentiation potential assays. Three different methods of sphere cultures were used as fol ows:(1) ultra low attachment culture;(2) hanging-drop culture and (3) Eppendorf tube culture. The sphere formation was compared among above three methods. We used Imagej to calculate mean areas of these spheres. And we used Viability/Cytotoxicity Assay Kit for Animal Live&Dead cells to detect their vitality.
RESULTS AND CONCLUSION:(1) Adipose-derived stem cells were confirmed by the analysis of the markers for cellphenotypes, CD29, CD44, CD59 were positive, as wel as adipogenic and osteogenic differentiation potential assays were positive. The conventional monolayer cultures of adipose-derived stem cells showed spindle and cloning growth within three passages. (2) Ultra low attachment culture, hanging-drop culture, Eppendorf tube culture al could elicit adipose-derived stem cells spherical growth. However, spherical size, shape and uniformity differed depending on cellnumbers, culture time and spherical culture methods. The ultra low attachment culture was comparatively difficult to control spherical shape and uniformity of adipose-derived stem cells. But hanging-drop culture and Eppendorf tube culture were able to form even cellspheres. (3) Spherical formation of adipose-derived stem cells using our three methods displayed good cellvitality.
6.Consistency of omnidirectional M-mode echocardiography and two-dimensional strain in assessment of left ventricular systolic asynchrony

Objective To investigate the consistency of omnidirectional M-mode echocardiography and two-dimensional strain in assessment of left ventricular (LV) radial systolic asynchrony. Methods Thirty patients with heart failure (HF) and 28 healthy volunteers underwent omnidirectional M-mode echocardiography and two-dimensional strain at the same time. The time to peak radial systolic movement (T_m) of LV in 12 segments were measured with omnidirectional M-mode echocardiography and its standard deviation (T_m-12-sd), and the maximum difference (T_m-12-dif) were calculated. The time to peak radial systolic strain (T_(SR)) of LV in 12 segments were measured with two-dimensional strain and its standard deviation (T_(SR)-12-sd), and the maximum difference (T_(SR)-12-dif) were calculated. The T_m-12-sd, T_m-12-dif, T_(SR)-12-sd and T_(SR)-12-dif were used as systolic asynchrony indicators. The value (-x)±2s in the control group was defined as the normal upper limit which represents 97.7% of the control group distribution. Any values above this limit in HF patients were classified as LV asynchrony. The results of two methods were analyzed with Kappa test. Results Compared with the controls, T_m-12-sd, T_m-12-dif, T_(SR)-12-sd and T_(SR)-12-dif were significantly higher in the HF group (P<0.001); there was consistency between T_m-12-sd and T_(SR)-12-sd, T_m-12-dif and T_(SR)-12-dif in detecting LV systolic asynchrony in HF group (Kappa=0.661, 0.733). Conclusion T_m-12-sd and T_m-12-dif of omnidirectional M-mode echocardiography have consistency with T_(SR)-12-sd and T_(SR)-12-dif of two-dimensional strain. These two technologies both have ability to evaluate LV radial systolic synchronicity.
7.Antiviral activity research of artesunate against human cytomegalovirus by fractionation dosage method in vitro

Objective To research the antiviral activity of artesunate (ART) in vitro fighting against both standard laboratory strains and ganciclovir(GCV)-resistance strains of human cytomegalovims(HCMV) and to explore whether fractionation dosage method can obviously enhance the antiviral effect of ART.Methods 1.Cytotoxicity assay to ART was performed by the use of 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) colorimetry.The 0% toxic concentration (TC0) were determined,and median cytotoxic concentration (TC50) was calculated with Probit regression method.2.Antiviral activity assays of ART against HCMV:human embryonic lung fibroblast cells (HELs) were infected with standard laboratory strains and GCV-resistance strains of HCMV,respectively,after which virus was removed and overlays of dulbecco's modified eagle medium(MEM) containing different antiviral drugs were added to the wells.All cells were cultured continuously at 37 ℃ in a 50 mL/L CO2 humidified atmosphere for 7-10 days and the cytopathic effect (CPE) was observed under a microscope.When the degree of CPE was clear (+ + +-+ + + +),the values of absorbency at 490 nm of all cell wells were measured by MTT colorimetry.The cell survival rate (CSR)and drug inhibitory rate (IR) for HCMV were calculated.By Probit regression method,the median inhibitory concentration (IC50) of 2 drugs was calculated respectively.3.To explore whether fractionation dosage method could obviously enhance the antiviral effect of ART against HCMV,the experiment was divided into 3 groups and compared with GCV group,respectively:Group 1:ART antiviral compounds were added to cell layers by one dosage.Group 2:Total drug dosage was divided into 3 parts,and each part was added to cell layers once a day for 3 days.Group 3:Total antiviral compounds were divided into 6 and delivery 2 times a day.The values of absorbency at 490 nm of all cell wells were measured by MTT colorimetry.The CSR and viral inhibitory rates were calculated.All data were statistically analyzed by One-Way ANOVA analyzing using SPSS 18.0 statistical software.P value of <0.05 was considered to indicate statistical significance.Results 1.Cytotoxicity assay showed that cytotoxicity was not found in the relevant range of ART concentrations under 62.5 μmol/L.TC0 and TC50 value of ART were 62.5 μmol/L and 171.7 μmol/L.2.In concentration of 5 μmol/L,15 μmol/L and 30 μmol/L,ART and GCV could obviously inhibit growth of HCMV AD169 strains.There was no significant difference between them.The value of GCV IC50 was 3.49μmol/L,and the value of ART IC50 was 2.17 μmol/L.Treatment index (TI) of ART was 28.8,and GCV was 716.3.ART could still obviously inhibit growth of HCMV resistant strains,but GCV couldn't.Differences between them were statistically significant.The value of GCV IC50 to HCMV resistant strains was 44.4 μmol/L,and the value of ART IC50 was 2.5 μmol/L.3.Fractionation dosage method (2 times a day) of ART could improve the inhibition rate of virus significantly compared to that used once a day and single dose method.Difference was statistically significant(P < 0.01).GCV delivered as the same method had little different changes in virus suppression ratio(P > 0.05).Conclusions 1.Cytotoxicity was not found in the relevant range of ART concentrations under 62.5 μmol/L.2.ART could obviously inhibit growth of HCMV resistant strains and standard laboratory strains.3.Fractionation dosage method (2 times a day) of ART could improve the inhibition rate of virus significantly compared to that used once a day and single dose method.4.Because the action mode of ART is different from other anti-HCMV drugs,and ART has a high biological activity and fewer side effects,it is expected to become a kind of new antiviral drugs for HCMV infections.
8.Comparison of four kinds of internal fixation for acetabular fracture of the lower anterior column: a finite element analysis

Objective To compare the biomechanical stability of 4 internal fixations in treatment of acetabular fracture of the lower anterior column through finite element analysis.Methods One normal adult male pelvis was subjected to 0.7mm thin-section CT scanning and 379 CT pictures were obtained.Finite element modeling software was used to establish internal fixation models for acetabular fracture of the lower anterior column,including lag screws (A),anterior column reconstruction plate (B),subcutaneous plate not crossing the pubic symphysis (C) and subcutaneous plate crossing the pubic symphysis (D).Finite element analysis was carried out to compare the biomechanical differences among the 4 internal fixation models which were subjected to the same loading conditions at both standing and sitting positions.Results At standing and sitting positions,the maximum displacement and the mean node displacement of fracture lines were the greatest in group A (0.558 mm and 0.462 ±0.092 mm at standing;0.634 mm and 0.473 ±0.108 mm at sitting),the smallest in group D (0.512 mm and 0.425 ±0.083 mm at standing;0.031 mm and 0.025 ± 0.004 mm at sitting),and in between in group B (0.513 mm and 0.432 ±0.085 mm at standing;0.630 mm and 0.466 ± 0.109 mm at sitting) and in group C (0.514 mm and 0.433 ± 0.085 mm at standing;0.627 mm and 0.464 ± 0.107 mm at sitting).At both standing and sitting positions,the maximum stress at the fracture line was the greatest in group D (10.519 MPa and 24.879 MPa),the smallest in group A (3.254 MPa and 8.954 MPa),and in between in group B (4.873 MPa and 9.431 MPa) and in group C (4.384 MPa and 10.128 MPa).Conclusions In treatment of acetabular fracture of the lower anterior column,subcutaneous plate crossing the pubic symphysis may result in the greatest biomechanical stability,lag screws the smallest biomechanical stability,and anterior column reconstruction plate and subcutaneous plate not crossing the pubic symphysis the moderate biomechanical stability.
9.Chemical constituents from safflower injection and their bioactivity.

The chemical constituents of Safflower injection were isolated and purified by polyamide, silica gel, Sephadex LH-20, ODS column chromatographies and preparative HPLC. As a result, sixteen compounds have been isolated. Based on the spectral data analysis, their structures were elucidated as scutellarin (1), kaempferol-3-O-β-rutinoside(2), hydroxysafflor yellow A(3), rutin (4), coumalic acid(5), adenosine(6), syringoside(7), (3E)-4-(4'-hydroxyphenyl)-3-buten-2-one(8), (8Z)-decaene-4, 6-diyne-1-Oβ-D-glucopyranoside(9), 4-hydroxybenzaldehyde (10), (2E, 8E) -tetradecadiene-4, 6-diyne-1, 12, 14-triol-1-O-β-D-glucopyranoside (11), kaem-pferol-3-O-β-sophorose (12), uridine (13), roseoside (14), cinnamic acid (15), and kaempferol (16). Compounds 1,2,7,9,11 and 12 were isolated from the Safflower injection for the first time. The anti-platelet aggregation activities of the isolated compounds were assayed. The results indicated all tested compounds exhibited potent activity except for 5, while 2, 3, 9 and 12 showed strong activity against platelet aggregation.
