1.Assessment of Therapeutic Effects of Neovascular Glaucoma in Treatment Period of 28 Cases

To summarize the etiological factor and to assess the therapeutic effect for neovascular glaucoma(NVG).Methods A retrospective study of 28 cases (29 eyes) with NVG was performed. The mean age, proportion of sex and primary ocular disease, incidence of symptom, sign and postoperative complication were analyzed. Results 78 6% of the patients were over 40 years old. 62% of the patients were secondary to the ocular ischemic disease such as central retina vein occlusion and proliferative diabetic retinopathy. The clinical characters included intraocular pressure (IOP) increasing, pain, mixed conjunctival congestion and rubeosis iridis. The mean IOP reduction was 19.5 mmHg in 29 eyes after antiglaucomatous drags treatment and 19.13mmHg in 23 eyes after Hunan aqueous humor drainage (HAD) implantation. The common postoperative complications were flat anterior chamber and choroidal detachment. The success rate of HAD implantation was 87%.Conclusions NVG was commonly secondary to ocular ischemic disease. Although prevention was the principal, reducing IOP and ameliorating ischemic situation were as the same important.
3.Application and effects of bone lengthening growth diary in self- management of patients after lower limb lengthening

Objective To explore the application and effects of bone lengthening growth diary in self management of patients after lower limb lengthening. Methods A total of 54 cases applying lower limb lengthening were divided into the experimental group (n=28) and the control group (n=26) with random digit table. The basic treatment was consistent, the control group was given routine guidance, the experi-mental group implemented self- management with bone lengthening growth diary. Self- recognition of disease was assessed and compared between the two groups 12 weeks after operation. Self- efficacy and self- manage ment were assessed and compared between the two groups 2 and 12 weeks after operation. Results The self- recognition of disease in the observation group was better than that of the control group, for patients with higher self- recognition degree (21 cases vs. 10 cases), χ2=8.64, P<0.05, the difference was significantly different. The total scores of self- efficacy and self- management in the observation group were higher than those of the control group,(48. 42±3.43) vs.(34.14±4.13),t=11.750, P<0.01;(55.35±2.69) vs.(42.15±3.67), t=15.130, P<0.01, the difference was significantly different. Conclusions Bone lengthening growth diary plays a beneficial role on self-management of patients with lower limb lengthening.
4.The comprehensive prevention strategy for occurrence and risk factors of haploidentical hematopoi-etic stem cell transplantation associated oral mucositisunder

Objective To investigate the comprehensive prevention strategy in the prevention of oral mucositis (OM) during allogeneic hematopoietic stem cell transplantation (HSCT). Methods A total of 721 patients who received HSCT from January 2010 to December 2011 at the Institute of Hematology, Peking University were enrolled. All the patients received the comprehensive prevention strategy of OM. Results A total of 315 patients (43.7 percents) suffered from OM during HSCT, the median time from HSCT to OM occurrence was 5 days. There were 173 (54.9 percents), 91(28.9 percents), 37(11.7 percents), and 14(4.4 percents) patients suffered from gradeⅠ,Ⅱ,Ⅲ, and IV OM, respectively. Tongue OM was most common. A total of 111 patients suffered from multiple OM (≥2 locations). Multiplicity showed that duration of agranulocytosis≥15 days was the only independent risk factor of OM in HSCT [ hazard ratio (HR) value 1.5, P<0.05 ]. Neutrophil engraftment time≥12 days was significantly associated with prolonged OM (≥8 days) (HR=2.1, P<0.05). Patient age ≥20 was independent risk factor for occurrance of grade Ⅲ-Ⅳ OM (HR=2.7, P<0.05). Conclusions Comprehensive prevention strategy can overcome the risk factors of OM and prevent the occurrence of OM during HSCT.
5.Individualized surgical treatment and prognosis of intraductal papillary mucinous neoplasms of the pancreas.

OBJECTIVETo investigate the clinical manifestation, individualized surgical treatment, and prognosis of intraductal papillary mucinous neoplasms (IPMN) of pancreas.
METHODSThe clinical data of 56 IPMN cases treated between January 2007 and December 2011 was retrospectively analyzed. Among the 56 patients (38 male and 18 female, mean age (61 ± 7) years), 26 were main-duct type, 18 were branch-duct type, 12 were mixed type. Pancreatectomy was performed on 48 cases, including pancreaticoduodenectomy on 29 patients, distal pancreatectomy on 17 patients, and total pancreatectomy on 2 patients.
RESULTSThe overall postoperative morbidity rate was 27.1% (13/48), there was no perioperative mortality. Pathology showed 31 cases of noninvasive IPMN, 17 cases of invasive IPMN, and 7 cases of lymph node metastasis. The rate of invasive tumors was 46.2% (12/26) in main duct type, 3/12 in mixed type, and 2/18 in branch duct type IPMN, the difference was statistically significant (χ(2) = 6.385, P = 0.041). The five-year survival rate for patients with noninvasive and invasive neoplasms was 100% and 24.6%, respectively. The prognosis of invasive cases with lymph node metastasis was significantly worse than those without lymph node metastasis (P = 0.017). A regular follow-up without surgical treatment was performed on 8 cases with asymptomatic side branch IPMN less than 3 cm in diameter, and no progression was found during the follow-up.
CONCLUSIONSIPMN has a relative good prognosis. Main duct type and mixed type IPMN have a higher malignant potential, and should receive a surgical treatment. Patients of branch duct type IPMN with a <3 cm diameter lesion and no clinical manifestations can be managed with close follow-up only.

6.Nature of the residue hard tissue after liposuction in lipoma and its role in lipoma recurrence.

OBJECTIVETo study the nature of the residue hard tissue after liposuction in lipoma and its role in lipoma recurrence.
METHODSThe residue hard tissue after liposuction was collected for histologic study, including HE-staining and immunohistochemistry with antibody of stem cell marker (CD29 and CD44). The average density and area of positive cells were analyzed statistically.
RESULTSThe residue hard tissue had a medium hardness. The center tissue was light yellow surrounding by soft, loose, irregular and cord-like tissue. The normal fat tissue is light yellow with a soft and unanimous texture. HE-staining showed abundant stromas in residue hard tissue, with lots of fibroblast-like cells locating among adipocytes. The cytoplasm was less with large and blue-staining nucleus. In normal fat tissue, the fat cells were uniform which were round or polygon. The stromas was less and the fibroblast-like cells were comparatively less. The slices were stained with antibody (CD29, CD44) of stem cell marker. The density and area of positive cells were much more in residue hard tissue than those in normal adipose tissue. Two Independence Samples T test were as follows: (t'CD29 = 9.931, P = 0.000; (t'CD44 =10.171, P = 0.000 for density; and (t'CD29 = 7.761, P = 0.000; tt'CDRR = 6.639, P = 0.000 for area. The difference was significant.
CONCLUSIONSAbundant fibroblast-like cells that are signed by the marker of stem cells exist in residue hard tissue. Those cells are proven to be lipoma derived stem cells (LDSCs) which may play an role in lipoma recurrence after lipoma liposuction.

7.Research on the osteogenic ability of 3-Ds interconnected porous titanium.

OBJECTIVETo evaluate the osteogenic ability of the three-dimensional interconnected porous titanium(TDIPT) coated with bonelike apatite, by the test of the culture of osteoblast in vitro.
METHODSThe pure TDIPT was prepared by the high temperature insostatic pressing (HIP) and was divided into two groups. One group of TDIPT was dipped into the 1.5 simulated body fluid (1.5 SBF) and developed the TDIPT coated with bone-like apatite (The test group). Another group was control group (pure TDIPT). The porous titanium of the two groups were all made into standard parts (5 mm x 5 mm x 4 mm) and placed into the 24-hole plates. The osteoblasts were extracted from SD rat. After the primary culture and subculture of the osteoblast in vitro, the osteoblasts were inoculated into the samples in the 24-pole plate and cultured continually. MTT Cell Proliferation Assay was done on the 1st, 3rd, 5th, 7th and 9th day respectively after inoculating. The ALP activity was tested on the 7th, 14th and 21st day, respectively. The collected data were analyzed by use of the student t test.
RESULTSThe MTT value and the ALP activity increased with the increasing of the culture time in the two group, but those of the test group was more significant than those of the control group (P < 0.05). Moreover, the osteoblast tachyauxesis and stable adhesion were observed in test group by SEM at the 7th day after culturing.
CONCLUSIONThe bioactivity of the TDIPT improves significantly after coating with the bone-like apatite onto the surface of the TDIPT.

8.MSC intervention on lung injury of diabetic rats with sepsis and its interaction with lung inflammation

Objective To investigate the effects of mesenchymal stem cells(MSCs)on the pathological structure and cytokine expression of lung tissue in septic model of diabetic rats. Methods Diabetic rats were ran-domly divided into control,sham-operation,sepsis and MSC-treated groups. The diabetic model was induced by high-sucrose and high-fat diet combined with streptozotocin and cecal ligation and puncture. The pathological changes of lung tissue were observed at 6,12,18 and 24 hours after operation respectively,and the expression of SP-D,TNF-α,IFN-γ and IL-10 in lungs were measured.Results The levels of SP-D,TNF-α and IFN-γ in lung tissue increased gradually with the elongation of time after CLP. The expression of IL-10 in lung tissue increased and then decreased. The trend of MSC intervention increased the content of SP-D,TNF-α and IL-10 in the lung tissue of non-diabetic septic rat model and reduced the content of IFN-γ.MSC intervention reduced the SP-D and TNF-α content.The intervention of MSC had no effect on the content of IFN-γ in the lung tissue of diabetic septic rats.However,it increased and then decreased the content of IL-10.Conclusions The model of sepsis in diabetic rats can be established by feeding combined high-sugar and high-fat diet and streptozotocin combined with cecal ligation and puncture. Mesenchymal stem cells affect sepsis inflammation and organ damage. But its specific role depends on the immune status and the timing of mesenchymal stem cell selection.
9.The expression of circulating microRNA-595 in patients with hepatitis B virus-related liver failure and its relationship with disease prognosis

Objective To measure the expression of circulating microRNA (miRNA)in patients with hepatitis B virus (HBV)-related liver failure and its relationship with disease prognosis.Methods The miRNA expressions in serum of 5 patients with HBV-related liver failure and 5 healthy control subjects were compared using Exiqon miRCURY LNATM miRNA microarray.The sera from 20 patients with chronic hepatitis B (CHB),20 patients hepatitis B related cirrhosis,50 patients with HBV-related liver failure and 40 healthy persons in Ruijin Hospital were collected.The relative expression of miRNA-595 was measured using quantitative real-time polymerase chain reaction (PCR).The relative expressions of miRNAs among groups were analyzed using student t test,the correlations were analyzed by Pearson and Spearman correlation.Results Microarray informed that 92 miRNAs changed significantly in patients with HBV-related liver failure,and miRNA-595 increased most significantly.The results of real-time PCR showed that the relative expressions of miRNA-595 ,miRNA-300 and miRNA-122 were 6.03 (t=3.134, P =0.003),3.12 (t=7.221 ,P <0.01)and 2.77 (t=2.671 ,P =0.021),which were higher compared to those in healthy control group.In the analysis of the relationship between miRNA-595 expression and disease prognosis in patients with HBV-related liver failure,the relative expressions of miRNA-595 in patients with CHB,hepatitis B related cirrhosis and HBV-related liver failure were 2.26 (t =3.780,P =0.001),3.32 (t = 6.111 ,P < 0.01)and 6.03 (t = 3.134,P = 0.003),respectively,which were all increased compared to that of the healthy control.The relative expression of miRNA-595 of patients with HBV-related liver failure was 2.66 times (t=2.450,P =0.043)higher than that of patients with CHB. When dividing patients according to prothrombin activity,miRNA-595 increased significantly in patients with early stage liver failure.When dividing patients according to model of end-stage liver disease (MELD) score,MELD score was positive correlated with the expression of miRNA-595 when MELD score was under 30 (r=0.673,P =0.004).The expression of serum miRNA-595 in survival group (11 .08,n=23) was higher than that in non-survival group (3.67,n = 27,t =4.309,P =0.041).Conclusions The expressions of miRNA595 ,miRNA-300 and miRNA-122 are all increased in patients with HBV-related liver failure,especially the expression of circulating miRNA-595 at early stage of the disease.The miRNA-595 may be used as a new serum biomarker for monitoring the severity of disease.
