Objective Discusstheinfluence of early diagnosis of myocardial damage and heart-type fatty acid binding protein(h-FABP)on patients with severe head injury after rehabilitation.Methods The sorum h-FABP was detected using enzyme-linked immunosorbent one-step quantitative detection,cTnI using solid-phase enzyme-linked immunosorbent assay(ELISA),CK-MB using the method of immunosuppressive.Results The sorum levels of h-FABP,cTnl and CK-MB in severe head injury patients levels were significantly higher than that of healthy control group(P<0.01),the positive serum h-FABP in severe head injury patients Was significantly higher than cTnI,CK-MB or ECG(P<0.01);8 cases died in47 cases of severe head injury patients,the mortalisty rate was 17.02%(8/47),the mortality rate of abnormal sentrum h-FABP,cTnI and CK-MB group was significantly higher than the normal group(P<0. 01);The serum h-FABP in predicting the death of severe head injury patients was high in sensitivity and negative predictive value,but lower in specificity and accuracy.Condusiom The serum h-FABP quantitative assay as a high-sensitive indicators to minor myocardial dsmage can be used an objective indicators forjudging severity ofacute severe head injury,evaluating incidence of accidents.

OBJECTIVE:To optimize the formula of piribedil hydrochloride orally disintegrating tablets and to investigate their related indexes.METHODS:Taking the contents of crospovidone(PVP XL)(disintegrating agent),amylum pregelatinisatum(loading agent) and Gum Acacia(glidant) as factors,the disintegrating time(td),the wet time(t) and the suspend stability(?A) as the evaluate indexes to carry on the orthogonal experimental to optimize the formula.The dissolution curve of the optimized formula was drawn.RESULTS:The optimized formula for the piribedil hydrochloride orally disintegrating tablets were as follows:the contents of crospovidone(PVP XL),amylum pregelatinisatum and Gum Acacia were 15%,20%,and 15% respectively.The prepared orally disintegrating tablets disintegrated completely within 30s and dissolved basically within 2min.CONCLUSION:The prepared preparation can meet the related standards specified of China Pharmacopeia.

OBJECTIVEThis study aims to determine the effect of fluoride concentration on the corrosion behavior of cobalt-chromium alloy fabricated by two different technology processes in a simulated oral environment.

METHODSA total of 15 specimens were employed with selective laser melting (SLM) and another 15 for traditional casting (Cast) in cobalt-chromium alloy powders and blocks with the same material composition. The corrosion behavior of the specimens was studied by potentiodynamic polarization test under different oral environments with varying solubilities of fluorine (0, 0.05%, and 0.20% for each) in acid artificial saliva (pH = 5.0). The specimens were soaked in fluorine for 24 h, and the surface microstructure was observed under a field emission scanning electron microscope after immersing the specimens in the test solution at constant temperature.

RESULTSThe corrosion potential (Ecorr) value of the cobalt-chromium alloy cast decreased with increasing fluoride concentration in acidic artificial saliva. The Ecorr, Icorr, and Rp values of the cobalt-chromium alloy fabricated by two different technology processes changed significantly when the fluoride concentration was 0.20% (P < 0.05). The Ecorr, Icorr, and Rp values of the cobalt-chromium alloy fabricated by two different technology processes exhibited a statistically significant difference. The Icorr value of the cobalt-chromium alloy cast was higher than that in the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20% (P < 0.05). The Ecorr, tRp alues of the cobalt-chromium alloy cast were lower htan those of the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20% (P< 0 .05).

CONCLUSIONFluoride ions adversely affected the corrosion resistance of the cobalt-chromium alloy fabricated by two different technology processes. The corrosion resistance of the cobalt-chromium alloy cast was worse than that of the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20%.

OBJECTIVETo explore the clinical effects of minor bone anchors and palmaris longus tendon graft in treating chronic mallet fingers deformity.

METHODSFrom January 2008 to June 2013, 26 patients with chronic mallet fingers deformity were treated with minor bone anchors and palmaris longus tendon graft. There were 18 males and 8 females, aged from 18 to 52 years old with an average of (32.0±1.3) years. Among them, 8 cases caused by machine injury, 6 cases by fall injury, 6 cases by sprain from fight, 4 cases by tendon spontaneous rupture, 2 cases by knife trauma. There was no tendon attachment of extensor tendon check in 16 cases, and with 0.3 to 0.5 cm tendon attachment in 10 cases. All patients had the flexion deformity and the disability of dorsiflexion activity. During operation, the distal interphalangeal joint was fixed in 10° to 20° dorsiflexion by a Kirshner wire, the minor bone anchor was used to reconstruct the extensor tendon insertion, the palmaris longus tendon slice was transplanted the decayed area of extensor tendon insertion. Four weeks postoperatively, the Kirshner wire was removed and the plaster external fixation was used, and the patient began function exercises. Postoperative complications were observed and fingers functions were assessed according to Dargan standard.

RESULTSThe patients were followed up from 6 to 14 months with an average of (5.0±0.3) months. Wound superficial infection occurred in 2 cases, the skin pressure ulcer in 2 cases, joint activities disability in 1 case; these symptoms got improvement after symptomatic treatment. Traumatic arthritis occurred in 2 cases, 1 case was improved after treatment, and 1 case had chronic pain for a long time. No internal fixation loosening or breakage and tendon rupture were found. According to Dargan standard to evaluate the finger function, 17 cases got excellent results, 8 good, and 1 poor.

CONCLUSIONIt is an effective way to treat the chronic mallet finger deformity using minor bone anchors and palmaris longus tendon graft, and the method has advantages of reliable fixation, easy operation, satisfactory effect and less complication.

Objective To observe the curative effect of cervicogenic headache(CEH)treatment through the combination of pulse radio frequency(PRF)on the C2 dorsal rootganglion and continuous epidural space block.Methods Sixty patients with CEH in our hospital were randomly divided into groups A and B,30 cases in each group.The group A was treated with combination of PRF on cervical dorsal root ganglion and continuous epidural space block.The group B was treated with PRF on cervical dorsal root ganglion method.The pain VAS scores before treatment and at 1 week,3,6 months after treatment were compared between the two groups.Results Compared with pretreatment,the VSA scores at 1 week,3,6 wonths after treatment in the two groups had statistical difference(P＜0.05),moreover,the VAS score decrease in the group A was better than the group B.All the patients had no nerve and artery injury or infection complications.Conclusion It is safe and effective to treat cervical CEH through the combination of ganglion PRF on the cervical 2 dorsal root and continuous epidural space block.

Objective To discuss the intensive gastric cavitary lavage method in treatment of stress ulcer bleeding(SUB).Methods The therapeutic effects of 100 patients with severe SUB in intensive care unit (ICU)from July 2005 to February 2008 treated with intensive gastric cavity lavage method were observed and analyzed.Results In these 100 cases,the bleeding was stopped successfully in 97 cases,the therapeutic effective rate being 97%;there were 17 cases died of complicated pulmonary infection(17%)and 3 cases of bleeding(3%),the total fatality rate being 20%.All the other cases were cured.Conclusion Intensive gastrointestinal lavage therapy is an effective method as acid inhibitor for treatment of SUB,but it can decrease the inhabitant flora in the stomach to pass through gastric-pulmonary route,thus the incidence of pulmonary infection was reduced.

Objective To study the effects of sodium bicarbonate (SB) on resuscitation in swine model of asphyxiation cardiac arrest (CA).Methods Thirty-two healthy miniature pigs were randomized (ramdom number) into two groups,the SB group (n =16) and the control group (n =16).Animals in both groups underwent endotracheal tube clamping to induce CA.Once induced,CA left untreated for a period of 8 minutes.At the beginning of initiation of cardiopulmonary resuscitation (CPR),SB (1.0 mEq/kg,diluted to 40 mL) was injected in central venous in SB group and isopyknic normal saline solution was injected in control group.Two minutes following initiation of CPR,defibrillation was attempted until return of spontaneous circulation (ROSC) was achieved or animals died.To assess the SB effects on myocardial metabolism,positron emission tomography (PET) was performed at baseline and 6 after ROSC.To analyze 18F-FDG myocardial uptake identified in PET,the maximum standardized uptake value (SUV) was measured.Results ROSC was 10/16 successful in SB group and only 8/16 successful in control group.The average survival time of SB group was (3.63 ± 0.76) h and that of control group was (2.45 0.70) h.Myocardial metabolism imaging using PET demonstrated that myocardial metabolism injuries in control group were more severe than in SB group at 6 h after ROSC and SUVmax was more higher in SB group than in control group (6 hrs after ROSC:1.32 ± 0.20 vs.1.10 ± 0.14,P =0.035).Conclusions In swine model of asphyxiation CA,SB improve the myocardial metabolism and might raise the possibility of ROSC and have no hypernatronemia.

After analyzed by the statistical reports module and the data analysis module which were from TCM inheritance assist system, and the methods of data-mining that including association rules and apriori algorithm, the frequency of single medicine, the frequency of drug combination, the association rules between drugs and core drug combinations which all were including Radix Paeoniae Alba can be get from the database.Results The prescriptions including Radix Paeoniae Alba were commonly used to treat megrim, stomach-ache, acid regurgitation and other syndromes. The highest frequency used drugs were Radix PaeoniaeRubra, PericarpiumCitriReticulatae, Radix SalviaeMiltiorrhizae, Radix AngelicaeSinensis, Poria, and so on. The most frequency drug combinations were “RadxPaeoniae Alba, Radix PaeoniaeRubra”, “Radix SalviaeMiltiorrhizae, Radix Paeoniae Alba”, and “Radix Paeoniae Alba, PericarpiumCitriReticulatae”. Objective To evaluate the value of intraduetal ultrasonography (IDUS) in diagnosing biliary and pancreatic disorders. Methods The findings by endoscopic retrograde cholangiopancreatography (ERCP) and IDUS from 19 patients with suspected biliary and pancreatic disorders from July 2006 to August 2007 in our hospital were analyzed retrospectively. Results Of the 19 patients, 17 had obstructive jaundice (including 6 eases of cholangiocarcinoma, 2 pancreatic adenocareinoma, 2 gallbladder carcinoma, 2 chole-docholithiasis with bile duct stricture, 2 autoimmune pancreatitis, 1 papillary adenocarcinoma, 1 papillary adenoma, and 1 sclerosing cholangitis) and 2 intraduetal papillary mueinous tumor (IPMT). The diagnosis was confirmed by surgery and pathological findings in 11 patients. The diagnostic accuracy of ERCP and IDUS was 73. 7% (14/19) and 84. 2% (16/19), respectively, and that of ERCP combined with IDUS was 89. 5% (17/19). The sensitivity and specificity of ERCP to differentiate benign bile duct strictures from ma-lignant ones were 100. 0% (11/11) and 83.3% (5/6), respectively; and those of IDUS were 100. 0%(11/11) and 100. 0% (6/6), respectively. The sensitivity and specificity of ERCP in diagnosing cholan-gioeareinoma were 83.3% (5/6) and 60% (3/5), respectively; and those of IDUS were 100. 0% (6/6) and 40. 0% (2/5), respectively. Conclusion Combination of ERCP with IDUS can improve the diagnostic accuracy of pancreaticobiliary disorders. Additionally, IDUS shows higher sensitivity and specificity in differ-entiation between benign and malignant bile duct strictures, but it is still difficult to identify the etiologic factors of malignant bile duet strictures by IDUS.
