The present of atlanto-axial pedicle screw fixation through posterior approach provide a new remedy for treating instability of pillow and cervical. A lot of researches have reported feasibility of atlanto-axial pedicle screw fixation, the results showed that it had advantages of easily exposure, less blood loss, shorter operative time, especially in treating as remedy fixation for atlanto-axial joint screw, atlas lateral mass screws and pedicle screw caused by injuries of tumor, inflammation and trauma. If not done properly, it can cause serious complications, such as iatrogenic fracture, injuries of vertebral artery and cervical spinal cord. Therefore, the safty and effectiveness of atlanto-axial pedicle screw fixation may be focus of research.

ABSTRACT:Objective To study the expressions of adrenomedullin (ADM)andβ-catenin in human osteosarcoma of different differentiation grading and their clinical significance. Methods We chose 33 cases of human osteosarcoma with different pathological grading (14 cases of high differentiation grading and 1 9 cases of low grading)and 10 cases of human osteochondroma to detect the expressions of ADM protein, total (T)β-catenin and phosphorylated (P)β-catenin using immunohistochemical method. Results The expression of ADM was detected in the cytoplasm. In osteosarcomas of higher and lower differentiation grading, the positive rate of ADM was 57.1% (8/14)and 100% (1 9/1 9),respectively,with significant differences (P =0.003<0.05).The expression of T-β-catenin was observed in both the cytoplasm and the nucleus in higher differentiation grading and in only cytoplasm in lower grading. In osteosarcomas of higher and lower differentiation grading, the positive rate of T-β-catenin was 85.7%(12/14)and 94.7% (18/1 9),with no significant difference (P =0.561>0.05).The expression of P-β-catenin was detected in the cytoplasm. In osteosarcomas of higher and lower differentiation grading, the positive rate of P-β-catenin was 35.7% (5/14)and 78.9% (1 5/1 9),with a significant difference (P =0.029 <0.05).There was a significant correlation between ADM and P-β-catenin (r =0.424,P =0.025<0.05),but no significant correlation between ADM and T-β-catenin (P = 0.078 > 0.05 )or between T-β-catenin and P-β-catenin (P = 0.052 > 0.05 ).Conclusion The expressions of ADM and P-β-catenin proteins and the different distributions of expression of T-β-catenin protein in cytoplasm and/or nucleus are important indicators for judging the pathological grading of osteo-sarcoma.

Objective To evaluate the preliminary clinical outcome of the screw fixation of minimally displaced scaphoid fracture assisted by computerized imaging reconstruction and preoperative planning system. Methods From March 2013 to Feburary 2014,Five screws on five patients were treated. We calculated and designed the position of scaphoid screw guide wire and ensured the guide wire centrally placed in the scaphoid by using the computerized imaging reconstruction and preoperative planning system software. The appropriate length of the screw was measured too in the software. We also identified the entry and exit points of the guide wire on the custom orthoses splinting the wrist joint.Five patients with scaphoid fractures were treated with percutaneous screw fixation under the guidance of a C-arm fluoroscopy and a guide wire were inserted based on the entry and exit points calculated preoperatively. All of the scaphoid fracture was nondisplaced or less than 1 mm.The modified MAYO wrist scoring system was used to evaluate the pain, function and range of motion for the wrist joint during follow up.Results Total surgical time was 28.8 min (25-39 min)and Fluoroscopy time was 18.4 s (11-23 s).The fracture healing was observed at routine 8 weeks follow up.No complication such as infection or fixation failure was reported.Modified MAYO wrist score was 83.8 (81-88)at average followup at 15 months (6-12 months). Conclusion Computer assisted percutaneous screw fixation is an effective method for the treatment of nondisplaced scaphoid fractures. It leads to significantly reduced guidewire placement time and radiological exposure time while provides accurate placement of the screw.

Through a series of chemical reactions, a kind of quaternary ammonium salt derivative of nanodiamond, ND-CO-NH-CH2-CH2-N ( CH3 ) 3+· I-( QAS-ND ) , was obtained , which was confirmed by FTIR, element analysis experiment and the electrochemistry measurements. Mixed myoglobin ( Mb) and QAS-ND solution was dropped on the surface of the glassy carbon ( GC ) electrode to prepare QAS-ND/Mb/GC modified electrode. In 0. 1 mol/L phosphate buffer solution (PBS) (pH 7. 0), Mb in the membrane exhibited direct electrochemical properties and showed good stability. The electrocatalytic property of the modified electrode toward H2 O2 was investigated, the results showed that the modified electrode could be used as the H2O2 biosensor to achieve fast, accurate detection of H2O2, with a detection limit of 3. 5 mmol/L (S/N=3).

BACKGROUND:The age, complication, injury to operation time, and pain level are important factors that affect patients with postoperative mortality, but many scholars disagree. Factors affecting death in elderly patients after femoral head replacement stil need further study. OBJECTIVE:To observe factors related to the survival status of elderly patients within 1 year after receiving femoral head replacement. METHODS:A total of 96 elderly patients receiving femoral head replacement at Department of Orthopedics, the First Affiliated Hospital of Xi’an Jiaotong University from January 2011 to January 2014 were selected. On admission, patients’ age, sex, time from injury to operation, injury to admission time, pain classification, admission hemoglobin, serum albumin content of admission, admission lymphocyte count, and the amount of blood transfusion and preoperative complications were recorded. Fisher test was used to analyze the factors affecting survival status within 1 year after replacement in patients with femoral head replacement using one-way analysis of variance. Multivariate Cox test was utilized to perform multi-factor analysis. RESULTS AND CONCLUSION: The injury to hospital time was > 5 days. Pain classification was grades III and IV. The hemoglobin content was < 120 g/L on admission. The volume of blood transfusion was > 1 000 mL. Serum albumin content was < 35 g/L, which was associated with the increased mortality (P < 0.05). The mortality was significantly higher in patients with more than 86 years old than in patients with 70-85 years old (P < 0.05). The mortality of patients with the time from injury to operation > 7 days was significantly higher than patients with the time from injury to operation < 7 days (P < 0.05). The serum albumin content < 35 g/L and grades III and IV of ASA classification were factors related to patient’s death (P < 0.05). Results confirm that the risk factors for survival status within 1 year of femoral head replacement contain intraoperative volume of blood transfusion, admission to operation time, hemoglobin content, and grading of pain.

Journal of China Medical University 2010;(1):47-50
Objective Evaluate the influence of medical monochrome liquid crystal display (LCD) with different resolutions on the detection of pulmonary nodules. Methods Ninety-three direct digital radiography (DR) chest images were selected on-line from picture archiving and communication system (PACS),including 38 positive,32 suspected and 23 normal cases. All of the images were confirmed by CT.These positive images were divided into group A and B according to the diameter of nodules.Three experienced radiologists blindly read the 93 images using three types of displays with different resolutions.Regarding the presence of nodule,five-point confidence system was used:definitely absent, probably absent, indefinite, probably present and definitely present. All observers marked their confidence levels of each image according to the presence of pulmonary nodule on different displays.Software SPSS 13.0 was employed for statistical analysis. Results In the group-A cases, the areas under ROC curves were 0.643,0.686 and 0.739,and in the group-B cases, those were 0.813,0.832 and 0.846 respectively. There was no difference in the detection efficacy among different radiodiagnostic systems. Conclusion Regardless of post-processing tools,it was equivalent to detect pulmonary nodules with different diameters of Group A or B among medical monochrome LCDs with different resolutions.
