1.Clinical application of esophageal stent loaded with ~(125)I seeds: recent progress in research

Esophageal cancer is a common malignant tumor. Most patients have already in the late stage of the disease when the diagnosis is confirmed and have lost the surgery chance. In recent years, esophageal stent has been widely applied in the treatment of esophageal stenosis caused by esophageal cancer. The clinical experience shows that the esophageal stent can effectively solve the problem of dysphagia. However, the ordinary esophageal stent has no therapeutic effect on the esophagus tumor. The esophageal stent loaded with ~(125)I seeds can not only solve the dysphagia problem but also can treat the primary esophagus cancer with its brachytherapy effect. This article aims to review the clinical application and the up-to-date research progress of the esophageal stent loaded with ~(125)I seeds.
2.Association between norepinephrine transporter gene polymorphism and decision-making processing in patients with cerebral infarction

Objective To investigate the association between norepinephrine transporter (NET)gene polymorphism and decision-making processing in patients with cerebral infarction.Methods A total of 145 patients with cerebral infarction and 188 normal controls were enrolled in our study.In all subjects,the polymerase chain reaction (PCR) for NET-T182C/G1287A polymorphism assay,gel electrophoresis,image analysis and enzymatic reaction,gene sequencing methods were used.The relationships of NET T182C/G1287A genotypes and alleles with decision-making processing were analyzed in patients with cerebral infarction.All participants completed six kinds of choice situational problems.Results There were significant differences in genotype and allele frequencies of T182C and G1287A polymorphism in NET between the patients with cerebral infarction and control group(for NET-T182C:genotype,x2 =4.437,P=0.049,allele frequency,x2=4.363,P=0.037,OR=0.625,95%CI:0.436-0.895;for NET-G1287A:genotype,x2=8.435,P=0.038,allele frequency,x2=2.765,P=0.036,OR=1.520,95%CI:1.053-2.193).The cerebral infarction patients with three NET-T182C genotypes and T/C alleles all completed six choice scenarios,and the scheme selection probability had no significant difference (all P>0.05).In high-risk and no-risk loss situation (scenario 4),the scheme selection probability had significant difference in cerebral infarction patients with NET-G1287 A genotypes and G/A alleles (P<0.05 and 0.05,OR=1.657,95%CI:1.149-2.390),and the patients with GG genotype tended to choose high-risk loss scheme,and the probability was obviously lower than that patients with other two genotypes,the patients with G allele tended to choose high-risk loss scheme,and the probability was obviously lower than that in patients with A allele (all P<0.05).In other five choice scenarios,the scheme selection probability had no significant difference between the patients (all P >0.05).Conclusions NET-G1287A polymorphism may be associated with decision-making processing in patients with cerebral infarction.In the high-risk and no-risk loss condition,patients with GG genotype and G allele have more loss risk aversion.
3.Relationship between nutritional risk and cognitive score in patients with acute stroke

Objective To investigate the relationship between nutritional risk and cognitive score in pa-tients with acute stroke .Methods From January 2015 to December 2015, 160 inpatients with acute stroke were observed and divided into two groups according to Nutritional Risk Screening 2002 (NRS 2002) scores.All the patients were on natural diets and did not receive enteral or parenteral nutrition support .Cognitive functions were evaluated using the Mini-Mental State Examination ( MMSE) within 24 hours and 6 months after the acute cere-bral artery diseases .Logistic regression analysis was performed to investigate the effects of nutritional risks on the cognition.Results Totally 145 patients entered the final analysis , among whom 72 had nutritional risks and 73 had no nutritional risks .The average ages of patients with nutritional risks were significantly older than those without nutritional risks [ (67.04 ±8.95) years vs.(63.10 ±9.12) years, P=0.027], along with significantly higher incidence of diabetes (75.0%vs.49.3%, P=0.001).The lesion locations showed no statistically signif-icant difference between two groups ( frontal lobe 24.5%vs.17.5%, P=0.670;parietal lobe and occipital lobe 40.8%vs.26.3%, P=0.398;basal ganglia region 51.0%vs.49.1%, P=0.490;brain stem and cerebellum 53.0%vs.42.1%, P=0.777).Compared to the patients without nutritional risks , the MMSE scores of the pa-tients with nutritional risks were lower within 24 hours, but without statistical significance (27.45 ±2.21 vs. 28.09 ±1.27, P=0.209);however, six-month follow-up showed that the MMSE scores were significantly differ-ent between two groups (24.70 ±2.16 vs.27.43 ±2.32, P=0.036).Univariate Logistic regression analysis was used to evaluate factors associated with cognitive impairment (MMSE<27).NRS 2002, ages, diabetes mellitus history, and body mass index entered in the Logistic regression analysis .In all patients, ages and NRS 2002 had a positive correlation with cognitive impairment (P=0.000, OR=1.156;P=0.004, OR=2.341).Conclusion The cognitive function in stroke patients with nutritional risks is worse than in patients without nutritional risks .
4.Role of placental growth factor in neovascularage-related macular degeneration

? Choroidal neovascularization is the primary pathogenesis of neovascularage - related macular degeneration ( nAMD ) , and the role of vascular endothelial growth factor ( VEGF ) in neovascularization has been widely recognized. Currently, drugs target different targets of VEGF have been widely used in the treatment of nAMD. As a subtype of VEGF, placental growth factor ( PlGF) has synergistic effects with VEGF-A on promoting angiogenesis, stimulating the migration of endothelial cell proliferation and mediating immune inflammatory response. There is no expression of PlGF in mature blood vessels so PlGF hashigh specificity. ln this paper, the role of PlGF in the pathogenesis and treatment of nAMD is reviewed.
5.The Effect of High-Flow Nasal Cannula Oxygen Therapy in Acute Respiratory Failure: A Systematic Review

Objective To assess the efficacy,comfort and tolerability of high-flow nasal cannula oxygen therapy in patients with acute respiratory failure (ARF).Methods The literature of RCTs concerning high-flow nasal cannula (HFNC) versus usual oxygen care (UOC) in ARF patients was searched using the national and international electronic databases.All relevant studies were screened according to inclusion and exclusion criteria,and then the included studies were evaluated,and finally,the extracted data were analyzed using Rev Man 5.3 software provided by the Cochrane Collaboration.Results eleven RCTs were included in 3 170 patients,of those 1 550 patients were in HFNC group,while 1 620 patients were in UOC group.There was no statistical difference in mortality among in HFNC and UOC group (OR =0.92,95% CI:0.70-1.2,P =0.53),neither was the incidence of nosocomial infection (OR =0.85,95% CI:0.63-1.15,P =0.3).The incidence of endotracheal intubation was lower in HFNC group than in UOC group.The comfort and tolerablity was greater in HFNC group than that in UOC group.Conclusions No difference in mortality or nosocomial infection was detected in patients with ARF treated with HFNC compared with UOC.However,HFNC seem well tolerated by patients and showed a lower rate of endotracheal intubation.
6.Investigation on Acaroid Mites in Jianghuai Region

Objective To investigate and report the catalogue and distribution of acaroid mites in region of Jianghuai.Methods The dust samples were collected from storage circumstances, working places and human dwellings from Jianghuai region(11 cities) in Anhui province from April to October, 2006.Acaroid mites were isolated by waterflotation and tullgren, then counted and identified.Results Thirty species of acaroid mites were identified, belonging to 20 genera, 7 families(4 subfamilies), the dominant species were Tyrophagus putrescentiae, Acarus siro, Lepidoglyphus destructor, Glycyphagus domesticus, Dermatophagoides farinae and Dermatophagoides pteronyssinus.Conclusion Acaroid mites breeding in differenai habitats of Jianghui region distributes widely and shows significant diversity.
7.The effect of intrathecal ropivacaine on spinal cord in dogs

ve When a new drug is introduced for intrathecal (IT) administration, its effect on spinal cord should be studied for safety reason. The aim of this study was to determine the ultra-microstructure of spinal cord and Ca2+ content in spinal cord after IT administration of ropivacaine in dogs. Methods Eighteen mongrel dogs of either sex weighing about 10kg were randomly assigned to one of three groups according to the dose of ropivacaine administered IT: group A received normal saline 2ml IT and served as control; group B received 0.5% ropivacaine 2ml(10mg) IT; group C received 1% ropivacaine 2ml(20mg) IT. Anesthesia was induced with intramuscular ketamine 20mg.kg-1 and atropine 0.05mg?kg-1 and maintained with intermittent iv boluses of ketamine 3mg?kg-1 and fentanyl 5?g?kg-1 Left internal jugular artery was cannulated for intra-arterial pressure monitoring. An incision was made in the back at L3-4 and lumbar puncture was confirmed by aspiration of cerebral-spinal fluid (CSF). Normal saline or ropivacaine was then injected over 20 seconds. 3 hours after IT administration the animals were sacrificed and L1-2 segment of spinal cord and nerve roots were immediately removed for Ca content determination and electron microscopic examination. Results The Ca2+ content of spinal cord was significantly higher in group C than that in group A and B. Electron microscope revealed that in group A and B neurolemma of the nerve root and mitochondria and endoplasmic reticulum of the neurons in spinal cord were intact, while in group C neurolemma was stratified and partly disrupted and mitochondria and endoplasmic reticulum underwent swelling and there was vacuole degeneration. Conclusions Ropivacaine of high concentration or at high dose may be injurious to spinal cord.

Objective To study the antagonistic effect of organic extracts of Fruit of Chinese Wolfberry (FCW) on chromium (Ⅵ) _induced mutation. Methods Using unscheduled DNA synthesis (UDS) assay, the effects of 0.02 g/ml, 0.01 g/ml and 0.005 g/ml organic extracts of FCW on UDS induced by potassium bichromate [0.31 ?g/ml Chromuim (Ⅵ)] in human peripheral blood lymphocytes were determined. The effects of the order of adding organic extract of FCW and potassium bichromate into cultured lymphocyte suspension on UDS were observed at the same time. 9.STUDY ON PROLIFERATION AND APOPTOSIS IN DEVELOPING RENAL CORPUSCLES OF MOUSE

Objective To study cell proliferation and apoptosis of renal corpuscle in normal developing mouse.Methods PCNA immunohistochemisty method,terminal deoxynucleotidyl transferase mediated dUTP-biotin nick endlabeling(TUNEL staining method)and light,electron microscopy were used to observe proliferation and apoptosis within corpuscle at different stages of development.Results Nearly all cells in Ⅰ and Ⅱ stages had nuclear PCNA immunolabeling.The number of PCNA immunolabeling cells during Ⅲ and Ⅳ stages was gradually decreased,and to less in Ⅴ stage.TUNEL positive cells could be easily observed before birth and identified in Ⅲ and Ⅳ stages.Electron microscope revealed that the mitotic activity was observed in all cells of Ⅰ and Ⅱ stages renal corpuscles.The mitotic phases of endothelial cells and podocytes could be seen in Ⅲ stage,but only endothelial mitotic activities in Ⅳ stage of renal corpuscle.Apoptotic activities of endothelial cell and Bowman's parietal cell were noted,predominantly in Ⅲ and Ⅳ stages.Conclusion Proliferation and apoptosis exist universally in the course of developing renal corpuscle,and supervise and control the regular development of renal corpuscle cooperatively.
10.Percutaneous Nephrolithotomy Combined with Pneumatic and Ultrasonic Lithotripsy for Complex Renal Calculi:Report of 150 Cases

Objective To evaluate the clinical efficacy of percutaneous nephrolithotomy (PNL) in combination with pneumatic and ultrasonic lithotripsy in the treatment of complex renal calculi. Methods Under general anesthesia,an ureteral stent was placed into the patient by cystoscopy. With the prone position,percutaneous access was established by inserting an access needle into the intended calix under the guidance of ultrasonography or fluoroscopy. Afterwards,combined pneumatic and ultrasonic probes were used by nephroscopy for lithotripsy. Results A single tract was used in 137 patients,while two or three tracts were created in the other patients (11 and 2 respectively). One-step procedure to remove the stones was achieved in 119 patients,while a second intervention was performed in 27,and three procedures was carried out in 4. The mean time of the operations were (88?34) min. After the operation,one patient developed infectious shock,and then was cured by anti-shock therapy;two patients showed secondary hemorrhage in two weeks postoperation,and was cured by conservative therapy. In this series,the rate of stone clearance was 84.7% (127/150);23 cases had residual stones,10 of them received ESWL (the stone was removed completely in 7 of the 10 cases),and the other 13 were treated by oral medicines. Six months after the operation,follow-up examination showed a stone-free rate of 89.3% (134/150). Conclusion Combination of ultrasonic and pneumatic intracorporeal lithotripsy is effective and safe for complex renal calculi,which is intractable by open surgery.
