Objective To evaluate the efficacy of laparoscopic ovarian drilling for treating clomiphene-resistant polycystic ovary syndrome(PCOS).Methods Laparoscopic ovarian drilling,with pelvic adhesion release and salpingoplasty,was performed in 38 cases of clomipheneresistant PCOS from March 2001 to May 2005 in this hospital.Serum concentrations of luteinizing hormone(LH),follicle-stimulating hormone(FSH),androgen(T),and estradiol(E2) were measured before and after operation.Ovarian follicular development,pregnancy rate,and abortion rate were observed after operation. Objective To study the effects of repetitive transcranial magnetic stimulation (rTMS) combined with hyperbaric oxygen (HBO) therapy on the recovery of neurological functional after cerebral infarction,and to discuss its possible mechanism.Methods Forty-five patients with cerebral infarction were randomly divided into a control group,an rTMS group and an rTMS plus HBO group,each group with 15 patients.The patients in the two treatment groups received 1 Hz rTMS to inhibit the unaffected hemisphere and 3 Hz rTMS to stimulate the affected hemisphere.One of the treatment groups also received HBO therapy,14 daily sessions.The National Institutes of Health stroke scale (NIHSS) and the Barthel index (BI) were evaluated on the 1st,14th and 30th day of treatment.Results The neurological function scores of those in the rTMS group and the rTMS & HBO group improved significantly.On the 1st and 14th day,no significant difference in NIHSS or BI scores was observed among the three groups,but by the 30th day the average neurological functional score in the rTMS & HBO group had improved significantly compared with the control group.The rTMS plus HBO group showed significantly better improvement than the rTMS group in terms of BI scores,but no significant difference in average NIHSS and BI scores was observed between the rTMS group and the control group.Fifteen days after the treatments had finished,the follow up results showed the improvements of the patients in the rTMS plus HBO group were significantly better than those in the other groups.Conclusion For patients with cerebral infarction,rTMS combined with HBO therapy can improve neurological function more significantly than rTMS therapy alone.

Objective To explore the experience of nursing patients with Crohn’s disease.Method The nursing histories of 80 patients with Crohn’s disease were retrospectively reviewed to analyze and summarize the measures for nursing them.Result After active treatment and effective nursing,all the patients were improved and discharged.Conclusion Effective nursing is of positive significance to improve the curative effect of Crohn's disease.

Objective To investigate the clinical features of acute coronary syndrome (ACS) combined with low T3 syndrome and its prognosis.Method Eighty-seven ACS patients with low free triiodothyronine (FT3) level (low FT3 group) and additional 64 ACS patients with normal FT3 level(control group) were selected in parallel; all the patients underwent coronary angiography.The contents of FT3,N-terminal pro-brain natriuretic peptide (NT-proBNP),cardiac troponin I (cTnI) and high sensitivity-C reactive protein (hs-CRP) were recorded,and the related factors were evaluated.The number of stenotic vessels was calculated.Results The contents of NT-proBNP,cTnI and hs-CRP in low FT3 group were elevated significantly compared with those in control group [(1242.14 ± 226.32) ng/L vs.(1126.36 ±195.55) ng/L,(0.92 ± 0.23) ng/L vs.(0.84 ± 0.19) ng/L,(6.46 ± 2.25) mg/L vs.(5.71 ± 1.78) mg/L],and there were significant differences (P ＜ 0.01 or ＜ 0.05).The contents of FT3 were negatively correlated with NT-proBNP,cTnI and hs-CRP in two groups (P ＜ 0.05).There was no significant difference in the number of stenotic vessels between two groups (x2 =4.512,P =0.105).Conclusion In patients with ACS,there is a negative correlation between contents of FT3 and the prognosis,but there is no correlation between contents of FT3 and the number of stenotic vessels.

[ ABSTRACT] AIM:To investigate the effect of hyperbaric oxygen ( HBO) on hypoxia-inducible factor-1α( HIF-1α) expression in rat experimental periodontitis with psychological stress.METHODS:Male special pathogen-free Wistar rats ( n=120) were randomly divided into 4 groups:normal control group;psychological stress stimulation group;experi-mental periodontitis group: the periodontitis model was induced by wrapping 3/0 silk ligature inoculated with Porphy-romonas gingivalis around the left maxillary second molar of the rats;periodontitis model with stress stimulation group.Psy-chological stress was removed at the 9th weeks after ligature, 6 rats from each experiment group were randomly chosen to HBO treatment.The rats were sacrificed at the 2nd, 4th, 8th and 10th weeks after ligature.Gingival index ( GI) and at-tachment loss (AL) were measured before sacrifice.The histological changes of periodontal tissues were observed under microscope with HE staining.The expression of HIF-1αwas observed by the method of immunohistochemistry.RE-SULTS:The sites of gingival attachment were normal in control group and psychological stress stimulation group.Periodon-tal pocket, and periodontal attachment loss were observed in experimental periodontitis group.The tissue damage was much serious in periodontitis model with stress stimulation group.No significant difference of GI and AL among psychological stress stimulation group and normal control group during the experiment was observed.GI and AL in periodonitis model with stress stimulating group were significantly higher than those in experimental periodontitis group at the 4th and 8th weeks (P<0.01).The levels of GI and AL were significantly lower at the 10th weeks after HBO treatmnt than those in untreated groups (P<0.05).No significant difference of HIF-1αexpression scores among psychological stress stimulation group and normal control group was found.HIF-1αexpression scores in periodonitis model with stress stimulating group was signifi-cantly higher than that in experimental periodontitis group at the 4th and 8th weeks (P<0.01).At the 10th weeks after HBO treatment the levels of HIF-1αwere significantly lower than that in untreated groups (P<0.01).CONCLUSION:Stress stimulation may aggravate periodontitis by decreasing tissue oxygenation in rats.HBO may represent a useful way in psychological stress periodontitis therapy.

Objective To discusses the effect of different nursing methods on anastomotic healing after low colorectal surgery. Methods 60 patient with colorectal cancer undergoing low anastomosis were randomly divided into the experimental group and the control group with 30 cases in each.The experimental group adopted modified nursing care,while the control group received routine nursing care.The defecation times and incidence of complications were observed between the two groups.Results The defecation frequency of the experimental group was less than the control group,and the incidence of rectal stimulation sign,anastomotic fistula and obstruction was less in the experimental group.Conclusions Modifed nursing methods have certain effects on preventing postoperative complications of low colorectal anastomosis.

22 patients with acute myocardial infarction (AMI) under went early exercise testing (EET)(mean 12.50±4.39 days after MI) in our study. The positive rate of testing was 45.45%. The maximal ratepressure product (SBP×HR) and peak metabolic equivalent were 2782±776 bpm.kPa and 4.96 ±1.20METs. Following up 3 months to 2.5 years (1.33±0.84 years), we found that the occurrence of heart events in the positive EET group (EET-P) was more than in the negative EET group (EET-N) (X2=4.77, P<0.05).Time of return to work and housework in EET-P was earlier than EET-N. About time and ratio of return to work and housework, the EET group was higher and earlier than no EET(N-EET) group. On the other hand,sexual activity of EET was recovered to a certain degree and ma jority could do some physical training- The results would be showed that EET could improve the quality of life in the patients with AMI.

Objective To study whether the organs besides digestive system of musca domestica Ⅲ instar larvae have the capability of produceing musca β-glucosidase.Methods Tissues of malpighian tubules,trachea,epiploon and body wall of musca domestica Ⅲ instar lar-vae were dissected under anatomic microscope,and the expression of β-glucosidase gene in these dissected tissues were detected by reverse transcription PCR.And the tissue localization of β-glucosidase mRNA was further identified by in situ hybridization.Moreover,anti-cellulase was used to determinate the tissue distribution with immunohistochemical staining.The relative mRNA expression levels of musca domesticaβ-glucosidase gene in these organs were tested by real-time quantitative PCR.Results The reverse transcription PCR showed that the ampli-fication products of β-glucosidase gene were observed in tissues of malpighian tubules,trachea and body wall.β-glucosidase mRNA was shown in the epithelium cells of malpighian tubules,trachea and body wall by in situ hybridization,and it was almost the same in the results of im-munohistochemical staining.The real-time quantitative PCR showed that the relative expression quantity of β-glucosidase gene in malpighian tubules and body wall were higher than that in foregut,while it was lower in itrachea than that in foregut.And it was of statistical difference in gene expression level of β-glucosidase among these organs (P <0.05).Conclusion Malpighian tubules,trachea and body wall of musca domestica Ⅲ instar larvae have the function of secreting β-glucosidase.Combining with the characteristics of secreting β-glucosidase in most organs of digestive system,it may provide a new biological method for the prevention and treatment of human diseases transmitted by musca domestica with the use of taget gene β-glucosidase.

Objective To investigate the effects of early rehabilitative training program on patients with acute myocardial infarction(AMI).Methods One hundred and twenty-two patients with AMI were randomly divided into early rehabilitation group(n=62)and control group(n=60).In addition to routine treatment,patients in rehabilitation group received early rehabilitative training mainly by walking exercise for two weeks.Results There were no significant differences in ventricular arrhythmia(Lown≥Ⅲ), extension of infarction and heart rate variability(HRV)between the two groups(P＞0.05).Forty of 62 patients(64.5%)in rehabilitation group had their left ventricular ejection fraction(LVEF)more than or equal to 50% in the 3~(rd)～4~(th)week after admission,significantly higher than that in control group(45.0%, 27/60 ;P＜0.01 ).By the end of the 4~(th)week after admission,25.8% of the patients in rehabilitation group showed positive in treadmill test,significantly lower than that in control group(38.3%,P＜0.01). Occurrence of angina pectoris and reinfarction and fatality in rehabilitation group were significantly lower than those in control group(P＜0.05)during their hospitalization and follow-up period.Patients in rehabilitation group stayed at hospital for(16?3)days in average,significantly less than that in control group[(27?4) days],with statistically significant difference(P＜0.05).Conclusion Early rehabilitative training for patients with uncomplicated AMI is not only safe and feasible,but also useful in improvement for their prognosis and quality of life.
